Open Journal of Ophthalmology (OJOph) , 2017, DOI: 10.4236/ojoph.2017.73023
Abstract: Introduction: Descemet Stripping Automated Endothelial Keratoplasty (DSAEK) is currently considered the procedure of choice for the treatment of endothelial dysfunctions. Despite DSAEK being a promising procedure to improve visual acuity (VA), the ideal graft thickness donor still remains under discussion. Purpose: To analyze the influence of graft thickness on postoperative VA and complications in DSAEK surgery. Methods: Retrospective study of 112 eyes that underwent DSAEK surgery. Donor grafts were obtained by dissection with automatic microkeratome, using different head sizes. Central corneal thickness (CCT) of donor corneas before dissection was measured by ultrasound pachymetry. The choice of the cutting depth was based on CCT measurements and the final desired residual corneal thickness, corresponding to the estimated donor graft thickness (EDGT). The eyes were divided into 3 groups, depending on the EDGT: <180 μm (Group 1), 180 - 200 μm (Group 2) and >200 μm (Group 3). Intraoperative, postoperative complications and the best corrected visual acuity (BCVA) at 6 months postoperatively were evaluated, comparing these 3 groups. Results: The average EDGT was 164 ± 10 μm in Group 1188 ± 7 μm in Group 2 and 234 ± 30 μm in Group 3. At 6 months postoperative the average BCVA was significantly better in Group 1 (BCVA: 0.74 ± 0.20), compared to Group 2 (BCVA: 0.54 ± 0.20) and Group 3 (BCVA: 0.39 ± 0.18). Intraoperative and postoperative complications included: graft perforation during cutting (0.9% of cases), graft rejection (3.6%), primary graft failure (4.5%), and graft detachment (3.6%). Its distribution was uniform between the 3 groups. Conclusion: The study proves that DSAEK is an effective surgical technique for the treatment of endothelial dysfunction, demonstrating that the use of thinner grafts (EDGT < 180 μm) can improve the final visual outcome, with a significant improvement on BCVA, without increasing the risk of complications.

Advances in Infectious Diseases (AID) , 2013, DOI: 10.4236/aid.2013.34036

Bacillus Calmette-Guérin (BCG) is a live attenuated form of Mycobacterium bovis, initially used in medicine as a vaccination agent only. The discovery of its antineoplastic effects in bladder cancer has led to the widespread recognition of BCG intravesical instillation as a therapeutic option. Although sepsis following BCG intravesical instillation is rare, it is nonetheless a dreadful and potentially fatal complication. Therapy usually relies on antituberculous therapy and steroids, alongside with intensive care unit admission. The authors report a case of a 67-year-old male patient who developed septic shock with multiple organ dysfunction after intravesical BCG instillation and review the currently available knowledge concerning the risk factors, diagnosis, management and prevention of BCG sepsis.

Abstract: Critical issues have taken place involving potential interactions between sunscreen components and the components of antiaging active (AA) agents. Critical issues have taken place involving potential interactions between sunscreen components and the components of antiaging active (AA) agents. The aim was to evaluate compounded sunscreens containing different AA agents with or without fragrance, which were submitted to macroscopic analysis, centrifugation, pH determination and in vitro sun protection factor (SPF). All preparations presented physical stability. However, formulations containing Deanol exhibited pH significantly lower than the others independent to the fragrance, indicating that the stability of these formulations may be lower. As regards in vitro SPF variation no significantly difference was found after adding to AA agent into compounded sunscreens. However, sunscreens containing Enteline presented SPF significantly higher in formulations with no fragrance. In conclusion, the preparations studied showed physical stability and the pH range of stability presented in accordance with the need for compounded sunscreens containing Longevicell , Enteline or Iris Iso as AA agents. Nevertheless, formulations containing Deanol should be adjusted to pH range (6.0 – 7.0), regardless of the fragrance. Furthermore, the fragrance used in compounded sunscreens containing Enteline assessed in this work can significantly decrease the SPF in the preparation and should be avoided in order to ensure the safety and efficacy of sunscreens associated with AA agents.

International Journal of Morphology , 2008, DOI: 10.4067/S0717-95022008000400037
Abstract: nicotine is the more abundant component in cigarette smoke. because nicotine is first metabolized in the liver, our aim was to investígate the effects of nicotine on this organ by biochemical and stereological methods. male wistar rats were treated with oral nicotine (on) diluted in drinking water during 32 days. the control group was treated with drinking water in the same period. rats were sacrificed 24 hours after last day, the blood was collected and the liver was removed. lipidogram was performed by enzymatic method and collagen fibers, fat globules and hepatocytes were count in the liver by stereological methods. we observed in control group preserved hepatocytes, with no presence of inflammatory cells. however in the on group was possible to observe varied size of hepatocytes with cloudy cellular limits and histoarchitecture loss. capillaries were fully of red blood cells. we observed also an increase of fat globules with small size. we observed in leucogram a reduction of leukocytes number (lymphocytes, neutrophils and monocytes) in the on group in comparison to control group. the lipidogram showed an increase of triglycerides and total cholesterol for on group when compared to control group. moreover, a reduction of hdl- cholesterol was observed in on group when compared to control group. numerical density of hepatocytes was lesser in on group when compared to control group. we suggest harmful effects of oral nicotine in liver induced by toxic mechanism with reduction of hepatocytes number and disturbance in lipid metabolism.
