Prevalência de transtornos mentais comuns e fatores associados em uma popula??o assistida por equipes do Programa Saúde da Família

Abstract: objective: investigate the prevalence of suspected cases of common mental disorders in a population assisted by one professional team of the brazilian family health program as well as to evaluate its associated factors. methods: a cross-sectional survey study was carried out using a randomized systematic sampling procedure for selecting 277 subjects from the target population. for data collection, two instruments were administered to the sample, the self report questionnaire (srq-20) as well as a sociodemographic questionnaire. results: the global prevalence of cases suspected of presenting common mental disorders in the target population was of 43.70%. females had a significantly higher prevalence rate (48.37%), when compared to males (34.41%). the variable "family income" had an inverse relation to the common mental disorders prevalence. the logistic regression analysis indicated that only two variables - "use of medication" and "family income" - were significantly associated to common mental disorders. conclusions: the results indicated a higher prevalence of common mental disorders in this target-population, compared to data from other similar studies, indicating the need for a special attention to be rendered by the primary health care system to detect and treat those cases in families attended by the brazilian family health program, specially those characterized by the associated factors detected.
Intensive care unit professionals' knowledge and behavior related to the adoption of contact precautions

Abstract: this study aimed to assess the knowledge and behavior of professionals working in intensive care units (icu) related to the adoption of contact precautions for the control of hospital infections (hi). this cross-sectional study used a semi-structured questionnaire to collect data. descriptive and multivariate analyses were carried out, including logistic regression and decision tree using chaid algorithm. a total of 102 professionals participated in the study, of whom 36.3% presented appropriate knowledge and 51% appropriate behavior in relation to hi control measures. nursing professionals had almost four times more chance (or = 3.58, ci 1.48-8.68) of presenting appropriate behavior than the remaining professionals. the multivariate analysis did not reveal variables associated with knowledge. no statistically significant association was found between knowledge and behavior (p = 0.196). these results suggest the need to implement educational activities so as to permit a balance between theory and professionals' practice concerning hi preventive measures, aiming to improve knowledge and behavior.

Abstract: the objective of this study was to identify facilitating and limiting factors for professionals' compliance with contact precautions in an intensive care unit of a general hospital. this cross-sectional study was performed from may to october 2007, using a semi-structured questionnaire for data collection. participants were 102 professionals, as follows: nursing technician (54.9%), nurse (12.7%), preceptor physician (10.8%), apprentice physiotherapist (8.8%), preceptor physiotherapist (7.8%) and resident physician (4.9%). the limiting factors for compliance with hand cleansing were forgetting, lack of knowledge, distance from sink, skin irritation, and lack of materials. the use of scrubs presented the most difficulty (45%) because they were not available at the shower box, were inappropriately stored, and due to the heat and collective use. glove use was the practice most easily conducted in everyday practice. results show the need to implement precaution measures to minimize the dissemination of resistant microorganisms.
Physiological Conditioning of Alibertia edulis (Rich) Seeds  [PDF]

Abstract: Physiological conditioning of seeds has been shown to increase the uniformity of seedlings; thus, it has been useful for propagating native tree species from the Brazilian Cerrado which, otherwise, are difficult to propagate successfully. The objective of this work was to evaluate the effect of physiological conditioning of Alibertia edulis seeds by soaking for 5 or 10 days in solutions of varying osmotic potential. After conditioning, seeds were dried down to original water content, sown on sheets of germitest paper inside gerbox plastic boxes, and incubated at 25°C. We evaluated the effect of conditioning by studying seed germination and vigor. Seed conditioning by osmotic pretreatment showed positive effects; however, germination and growth of seedlings from seeds conditioned at osmotic potentials of -0.3 to -0.7 MPa were reduced. Osmoconditioning for 10 days at -0.7 MPa resulted in increased percent germination, indicating that the longest imbibition period in the osmotic solution of the lowest osmotic potential (-1.3 MPa) favored the seed germination process. A. edulis seeds did not require conditioning to attain high germination rates; nonetheless, osmotic conditioning reduced average seed-germination time.
Ordinal logistic regression models: application in quality of life studies

Abstract: quality of life has been increasingly emphasized in public health research in recent years. typically, the results of quality of life are measured by means of ordinal scales. in these situations, specific statistical methods are necessary because procedures such as either dichotomization or misinformation on the distribution of the outcome variable may complicate the inferential process. ordinal logistic regression models are appropriate in many of these situations. this article presents a review of the proportional odds model, partial proportional odds model, continuation ratio model, and stereotype model. the fit, statistical inference, and comparisons between models are illustrated with data from a study on quality of life in 273 patients with schizophrenia. all tested models showed good fit, but the proportional odds or partial proportional odds models proved to be the best choice due to the nature of the data and ease of interpretation of the results. ordinal logistic models perform differently depending on categorization of outcome, adequacy in relation to assumptions, goodness-of-fit, and parsimony.

Abstract: objective: to investigate the quality of life (qol) in patients diagnosed with schizophrenia that are under involuntary commitment. methods: a cross-sectional epidemiological study was carried out on patients from a penitentiary hospital. the survey was conductid by a social-demographic questionnaire and the quality of life scale (qls-br), specific for patients diagnosed with schizophrenia. a descriptive and multivariate analysis was performed. the association between the scores of qol and the clinical, social-demographic and felony characteristics was obtained by multivariate analysis, using a resource the decision tree, by means of the chi-squared automatic interaction detector (chaid) algorithm. results: 54 people participated in this study, of which 90.7% were male, with an average age of 40 years old. all the domains of quality of life, including the global one, presented scores compatible with a very low qol. the occupational domain showed to be the most affected one. long lasting disease, having commited homicide and being over 40 years old were the variables associated to a low quality of life in the multivariate analysis. conclusion: a low qol was noticed among the patients that are under involuntary commitment, a reality that needs to be modified by making possible changes in brazilian policies.

Abstract: satisfaction is an important measure of quality care, of adherence to the treatment and adequate use of health services. the objective here is to build two scales which evaluates team' and patients' satisfaction with cardiovascular disease treatment provided through a distance telecardiology project. the procedure followed international standards for development of measure instruments, including operational definition of satisfaction contents and its area for evaluation; item definition; pre-test and pilot study. the literature review, focal groups and discussion with specialists had delimited the domains to be included in the scales and the elaboration of its items. the cardiosatis-team included 15 items and the cardiosatis-patient included 11. satisfaction was measured through a five-point likert scale. the scales' items comprised satisfaction with physical structure, human resources, capacity of resolution, attention and care offered by the service and the satisfaction with the received/given care. the scales also included open questions. cardiosatis scales have showed to be an easy and accessible instrument very well accepted by medical team and patients. preliminary results presented good characteristics of validity and reliability.

Abstract: objective: to evaluate psychometric validity and reliability properties of the cardiosatis-team scale and measure physician satisfaction before and after the implementation of the project. this scale was designed to evaluate physician satisfaction with the healthcare delivered for cardiovascular diseases in cities that participate in a telemedicine system. methods: the scale was applied in 82 cities of minas gerais, before and after the implementation of a telecardiology system. the analysis of the psychometric properties of cardiosatis-team scale included: construct validity using factorial analysis; internal consistency reliability using cronbach's alpha; pearson′s correlation between items; spearman′s correlation between domains and global scale and; discriminant analysis. results: the factor analysis of the principal components extracted two factors that explained 66.5% of the variance in satisfaction scores: healthcare delivery and diagnosis structure and satisfaction with the care delivered. cronbach′s alfa for internal consistency reliability of the scale showed values of 0.92 for the global scale and up to 0.84 for factors. inter-factor and inter-scale correlations were adequate, showing values of 0.59 to 0.89 and 0.73 to 0.85, respectively. the scale was able to distinguish the two moments of its application (before and after intervention), showing high satisfaction for most items measured afterwards (p < 0.05). conclusions: we can conclude based on the present study that the telemedicine system improved provider satisfaction and that the cardiosatis-team scale is a good tool to evaluate provider satisfaction with care delivered for cvd. its appropriate characteristics in terms of validity and internal consistency reliability justify its utilization in other studies.

Abstract: Este trabalho se trata de uma revis o narrativa da literatura para conhecer as informa es históricas e teóricas disponíveis sobre as rela es entre crime e loucura. As modernas concep es de crime e de loucura constituíram espa os privilegiados de interven o e controle social nos últimos dois séculos. Os manic mios judiciários e as medidas de seguran a funcionam como instrumentos de controle social. Em nome da defesa social, tornou-se possível a aplica o de métodos de corre o, tratamento e ressocializa o dos indivíduos, e o nível de periculosidade pode ser extinto pelos mecanismos de controle e vigilancia. A cria o de uma categoria social “louco-criminoso” desenvolveu um complexo aparelho jurídico-institucional voltado para o tratamento clínico e para a conten o de comportamentos ditos anormais. Conclui-se que a realidade social dos manic mios judiciários e das medidas de seguran a necessita ser compreendida, assim como as atuais concep es e formas de atua o frente à cria o da rela o entre o crime e a loucura.
