The Relation of Externalizing Behavior and Central Auditory Processing Deficits in 4-Year-Old Children  [PDF]

Abstract: The integrity of the central auditory system is a fundamental condition for language development. Good language development is related to good academic performance and adaptive behavior. Therefore, the evaluation of auditory processing along with behavior as well as the verification of a possible relationship between them can indicate possible ways on how to deal with both problem behavior and difficulties in language. 187 parents of preschoolers (mean age 3.8 years) were interviewed individually and asked to fill in the CBCL (Child Behavior Checklist), for ages 1(1/2) to 5, providing a behavioral profile concerning externalizing and internalizing behavior. Children were assessed regarding central auditory processing skills through the Simplified Assessment of the Auditory Processing—SAAP (Pereira & Schochat, 1997) including Sound Source Localization (SSL); Non-Verbal Sound Sequence Memory (NVSSM); Verbal Sounds Sequence Memory (VSSM). To investigate a possible association between central auditory processing skills and behavioral problems, all the scores in CBCL and the total scores of Simplified Auditory Processing Assessment were correlated with functional parameters using the Spearman rank correlation. The behaviors “acts too young for age”; “constantly seeks help”; “does not eat well”; “does not seem to feel guilty after misbehaving”; “easily frustrated”; “nervous movements or twitching”; “nervous, highstrung, or tense”; “poorly coordinated or clumsy”; “repeatedly rocks head or body”; “stares into space and seems preoccupied”, “sulks a lot” and “wanders away” were correlated with poor auditory processing skills. This indicates that the school staff along with health professionals should provide informational counseling regarding, not only the communicative difficulties associated with a poor performance on central auditory processing skills but also the psychosocial difficulties that these children may be facing.

Abstract: health care service consumption is related not only to need and individuals' behavior, but also to factors associated to the supply side of health care market. the new brazilian constitution (1988) established the unified health care system (sus) which is based on universal access to health care services. the system was implemented in 1990. the principle of equity can be identified in the health sector legislation and can be translated as equal opportunity of access to equal needs. this study aimed at evaluating equity in the use of health care services considering two dimensions: geographical and social dimensions. data came from two general household surveys carried out in 1989 and 1996/1997 by the brazilian census bureau (ibge). standardized utilization rates controlled by morbidity and insurance coverage were used to analyze geographical variation. private expenditure with health insurance and drugs was also compared. logistic regression was used to test for social inequalities. results of the study suggest small reduction in inequalities between 1989 and 1996/1997, indicating that brazilian health care system remains highly unequal.

Abstract: this study aimed at analyzing the perception of pre-term babies' parents in experiencing the kangaroo mother method. it is a descriptive study with a qualitative approach. data were collected from semi-structured interviews with 10 parents, held in the first 60 days after the discharge of the pre-term baby from the kangaroo method, in a philanthropic hospital in the interior of s?o paulo state. the parents' perception concerning the experience was grouped in four thematic units: making maternal stay in the kangaroo method flexible; favoring the mother-child and family relationship; completing the pre-term newborn's growth and development and developing skills in caring for the child. the obtained data provided information for the organization of care in the kangaroo mother method, from the institutional perspective of the health care team as well as the relationship with clients, which enabled understanding about the difficulties and meanings attributed to the experience and optimization of nursing care.
Percep o dos pais sobre a vivência no método m e-canguru

Abstract: O presente estudo tem como objetivo analisar a percep o de pais de bebês prematuros sobre a vivência no Método M e-Canguru. Trata-se de estudo descritivo, inserido na abordagem qualitativa. Os dados foram coletados a partir de entrevista semi-estruturada com 10 pais, realizada em até 60 dias após a alta do prematuro do Canguru, realizado em um hospital filantrópico do interior do Estado de S o Paulo. A percep o dos pais sobre a vivência foi agrupada em quatro núcleos temáticos: flexibilizando a permanência materna no Método M e-Canguru; favorecendo a rela o m e-filho e família; completando o crescimento e desenvolvimento do prematuro e desenvolvimento de habilidades para o cuidado com o filho. Os dados obtidos fornecem subsídios para a organiza o da assistência no Método M e-Canguru, tanto sob a perspectiva institucional da equipe atuante como da rela o com a clientela, possibilitando a compreens o das dificuldades e significados atribuídos à vivência e otimiza o do cuidado de enfermagem.

Abstract: the objective of this study was to discuss the complexity of preparing the monthly nursing schedule during the nursing administration course, in an inpatient unit of a public teaching hospital, in s?o paulo. we started from the position that the preparation of a monthly schedule consisted of a series of coordinated actions which required: recognition of staff, collection of data needed to characterize the clinical reality, comparison of those data with the literature, and development of a proposal for action, which we would discuss with the nurses in the unit. this experience provided the possibility to reflect on empowerment and the understanding of these professionals about the factors involved in the implementation of this management activity. it allowed us, as well, to demonstrate the importance of diagnosing the needs of the unit and comparing it with the institutional reality, with a view toward possible actions to resolve the identified problems

Abstract: background and objectives: the use of pneumoperitoneum during videolaparoscopic surgeries activates neuroendocrine mechanisms, and causes cardiovascular and hormonal changes. the aim of this study was to evaluate the effects of the addition of nitrous oxide, to maintain adequate anesthetic levels, on the cardiovascular sympathetic response and in the expired concentration of sevoflurane (ecsevo) during pneumoperitoneum, by evaluating hemodynamic parameters, bis, and sef95%, during videolaparoscopic cholecystectomies. methods: thirty-one patients, physical status asa i and ii, ages 19 to 76 years, were included in the study. anesthetic induction was accomplished with sufentanyl 0.3 μg.kg-1), propofol (2.5 mg.kg-1), and cisatracurium (0.15 mg.kg-1). during the maintenance phase of anesthesia, the dose of ecsevo was adjusted to maintain bis between 40 and 60. the administration of increasing concentrations of nitrous oxide, 20%, 40%, and 60%, was initiated after inducing pneumoperitoneum. the bispectral index, sef95%, systolic blood pressure (sbp), diastolic blood pressure (dbp), mean arterial pressure (map), and heart rate (hr) were evaluated at m1 - 5 minutes after the pneumoperitoneum; m2 - when the expired concentration of n2o = 20% (ecn2o = 20%); m3 - ecn2o = 40%; m4 - ecn2o = 60%. results: there were no significant differences among sbp, dbp, map, and hr in the moments studied. there was a significant difference in ecsevo in those same moments, with a 35% reduction when m1 and m4 were compared. conclusions: when nitrous oxide is administered in association with sevoflurane during pneumoperitoneum in videolaparoscopic cholecystectomy, it provides for hemodynamic stability and reduces the amount of sevoflurane needed while, at the same time, maintaining electroencephalographic parameters compatible with adequate anesthesia.

Abstract: background and objectives: anesthetic induction with inhalational agents using a facemask has gained attention since the introduction of sevoflurane. at the same time, the influence of adding nitrous oxide on the pattern of induction and recuperation deserves attention, especially regarding recovery of the cognitive function. the objective of this study was to evaluate the cognitive effects of inhalational anesthetic induction with sevoflurane alone or associated with nitrous oxide in adults. methods: twenty adult volunteers, asa physical state i, without a history of psychiatric disorders or prior use of benzodiazepines, were enrolled in the study. after answering the mini-mental state examination (mmse), group i received 5% sevoflurane until a bispectral index (bis) of 60 was achieved. group ii received nitrous oxide in increments of 10% until it achieved 50% followed by the administration of sevoflurane until a bis of 60 was achieved. arterial blood pressure, heart rate, pulse oxymetry, sef 95% (spectral edge frequency), induction time, and anesthetic recovery time were evaluated. results: time of induction showed no differences between both groups. group ii showed greater hemodynamic stability and smaller sef 95% values when bis achieved 60. the time for recovery of cognitive function was similar in both groups, as well as the incidence of nausea and vomiting. there were no cases of apnea. group ii demonstrated greater incidence of psychomotor agitation during induction. conclusions: sevoflurane alone or in association with nitrous oxide is an adequate option for inhalational anesthetic induction or outpatient sedation in adults, maintaining spontaneous ventilation, and providing a fast recuperation of the cognitive function.

Abstract: this experience report had as purpose to describe the evaluation process in curriculum clerckship of nursing administration in the nursing course of the school of nursing at the university of s?o paulo, in the light of dialectical methodology. in this perspective, knowledge is constructed in three stages: síncrese, analysis and synthesis, in which the pedagogical task of the teacher is mediating the steps of mobilization, construction and synthesis of knowledge through the doubt and criticism. it was concluded that assessment of learning valued interpersonal relations and participation of the subjects involved, through the host and establishment of links in the process, encouraging the development of knowledge from the practice of creating problem situations constructive and meaningful, developing multiple competences.

Abstract: this paper aims at analyzing the occurrence of nursing diagnosis anxiety, in puerperal women immediately after and in the later period, within a community context. this is a transversal descriptive quantitative study. the no probabilistic samples consisted of 40 puerperal women. during data collection, interview procedures were used, in addition to physical examination and home observation. anxiety was present in 80% of puerperal women. the anxiety-related factors predominating in puerperal women were stress (62.5%) and maturational crisis (34.4%); mainly evidenced by the behavioral defining characteristic: worries expressed due to changes in life events (46.9%), and insomnia (43.8%). in addition to concerns about the child's well-being and an increased demands in burden of activities. the application of nursing diagnoses provides the identification of priorities in the assistance and enables the adequate implementation of early intervention can be planned.
