Estimula??o elétrica nervosa transcutanea no pós-operatório de cirurgia torácica: revis?o sistemática e metanálise de estudos randomizados

Abstract: objectives: to evaluate the effects of transcutaneous electric nerve stimulation (tens) on pain and pulmonary function during the postoperative period after thoracic surgery by performing a systematic review and meta-analysis of randomized trials. methods: the search strategy included medline, pedro, cochrane central, embase and lilacs, besides a manual search, from inception to august, 2011. randomized trials were included, comparing tens associated or not with pharmacological analgesia vs. placebo tens associated or not with pharmacological analgesia or vs. pharmacological analgesia alone to assess pain (visual analog scale - vas) and/or pulmonary function represented by forced vital capacity (fvc) in postoperative thoracic surgery patients (pulmonary or cardiac with approach by thoracotomy or sternotomy). results: of the 2.489 articles identified, 11 studies were included. in the approach by thoracotomy, tens associated with pharmacological analgesia reduced pain compared to the placebo tens associated with pharmacological analgesia (vas -1.29; ci95%: -1.94 to - 0.65). in the approach by sternotomy, tens associated with pharmacological analgesia also reduced pain compared to the placebo tens associated with pharmacological analgesia (vas -1.33; 95%ci: -1.89 to 0.77) and compared to pharmacological analgesia alone (vas -1.23; 95%ci: -1.79 to -0.67). there was no significant improvement in fvc (0.12 l; 95%ci: -0.27 to 0.51). conclusion: tens associated with pharmacological analgesia provides pain relief compared to the placebo tens in postoperative thoracic surgery patients both approached by thoracotomy and sternotomy. in the sternotomy it also provides more effective pain relief compared to pharmacological analgesia alone, but has no significant effect on pulmonary function.

Abstract: Objective: To describe women’s perception of and satisfaction with vaginal examinations performed during labor in public hospitals in Ciudad de Panama. Methods: Cross-sectional study of 346 women in labor at public hospitals in Ciudad de Panama between April-August 2008. Prticipants were given a questionnaire with 20 statements about their perception of vaginal examinations and the performance of health professionals; answers were scored with a Likert scale ranging from 1 - 10 points. Results: On average women received 5.7 ± 3.5 vaginal examinations. The notional satisfaction index was 72%. The 97.1% of patients were very satisfied or satisfied with the experience of labor, while 2.9% mentioned being less than satisfied (p<0.001). The perception of the medical performance in the vaginal examinations during labor averaged 8.4 ± 1.9 (on a maximum of 10 points). We found an association between satisfaction and perception of medical performance OR = 12.8 (95% CI 3.3-50.2). Conclusion: The 97.1% of women were satisfied with the experience of vaginal examination during labor, although vaginal examination was performed more than recommended, confirming that women accept the procedure as long as it is necessary, explained, and performed with respect for their dignity.

Abstract: construction industry stands for a very important part of the economy in developed and developing countries. nevertheless, investment in research, development and innovation (r&d&i) in the construction industry is less than in other economic sectors. this paper addresses r&d&i management in construction as another business process, taking into account the possibility of systematizing r&d&i activities using the set of standards une 166000. the literature review delivers an affinity diagram that shows the main ideas regarding innovation in the construction industry. taking into account these previous concepts, a general model of competitiveness focused in innovation is proposed; finally, a model of r&d&i management for construction companies is also displayed. this model states the need to ease the information flow inside the organization, thus the knowledge generated by innovation in construction sites allows a substantial raise in the business competitiveness. the implementation of a systematic innovation process in construction companies presumes the need to build new organizational hierarchies that differ from the current ones

Abstract: El sector de la construcción representa una parte muy importante de la economía en los países desarrollados y en vías de desarrollo. No obstante, la inversión en investigación, desarrollo e innovación (I+D+i) en el sector de la construcción es inferior a la de otros sectores económicos. El presente artículo plantea enfocar la gestión de la I+D+i en la construcción como otro proceso empresarial, contemplando también la posibilidad de sistematizar la I+D+i utilizando la serie de normas UNE 166000. La revisión bibliográfica realizada se concreta en un diagrama de afinidad que muestra las principales ideas relativas a la innovación en el sector de la construcción. Teniendo en cuenta todo lo anterior, se plantea un modelo de competitividad general focalizado en la innovación que se particulariza en una propuesta de modelo de gestión de I+D+i para empresas constructoras. El modelo expone la necesidad de facilitar los flujos de información dentro de la organización, de forma que el conocimiento generado por la incorporación de la innovación en las obras permita un aumento sustancial de su competitividad. Abstract: Desde principios de la década de los noventa se dio origen a lo que hoy se conoce como Medicina Basada en la Evidencia (MBE), que ya se extiende no sólo a la atención de pacientes individuales sino a la atención de las poblaciones. Con este nuevo movimiento surgieron ciertas metodologías que tienen como fin la evaluación, la sistematización, la síntesis y la sinopsis de la información. Una revisión sistemática debe: tener objetivos claros y establecidos, o sea que la pregunta clínica que se desea responder esté claramente especificada en cuanto a población de interés, intervención o exposición que se desea evaluar y los resultados que se desean medir. Las guías clínicas basadas en la evidencia, siguen un proceso sistemático en su desarrollo y tienen el objetivo de asistir a los profesionales de salud y a los pacientes en la toma de decisiones sobre el adecuado cuidado de su salud en circunstancias específicas. Esta corta revisión tiene como objetivo orientar al estudiante en conocimientos básicos sobre estas dos metodologías parte de la MBE.

Abstract: Un tercio de la población mundial está infectada por tuberculosis. Cada a o en el mundo se reportan de 8 a 10 millones de casos nuevos de tuberculosis, y cerca de 3 millones fallecen a causa de la enfermedad. Suele afectar los pulmones, pero en 33 % de los casos afecta otros órganos. La tuberculosis vertebral ocurre en menos de 1% de los pacientes, es más frecuente en ni os que adultos. Los datos clínicos más comunes son: dolor, limitación funcional, y contractura muscular. En etapas avanzadas presenta una giba dorsal, y alteraciones neurológicas. Presentamos el caso de una paciente femenina de 41 a os con dolor lumbar de dos a os de evolución. Estudios imagenológicos revelan fractura de L3 con desplazamiento posterior de L2 más abscesos que rodean la fractura. El cultivo microbiológico informó presencia del Bacilo ácido Alcohol Resistente Mycobacterium tuberculosis. Recibió tratamiento médico quirúrgico con adecuada respuesta clínica.

Abstract:

We separate the rentier portion of the budget constraint of the representative agent from the income-plus-distributed profits segment. The former’s wealth consists exclusively of returns on government bonds, the latter’s wealth is wage income from working for firms plus the distributed profits of the latter. The non-neoclassical element is the non-imposition of the market-clearing assumption. The Barro-Ricardo theorem only applies to rentiers. The level of activity is shown to depend on the level of employment along with a set of parameters that capture the imperfect competition of the model.

Grapevine Yield and Leaf Area Estimation Using Supervised Classification Methodology on RGB Images Taken under Field Conditions

Abstract: The aim of this research was to implement a methodology through the generation of a supervised classifier based on the Mahalanobis distance to characterize the grapevine canopy and assess leaf area and yield using RGB images. The method automatically processes sets of images, and calculates the areas (number of pixels) corresponding to seven different classes (Grapes, Wood, Background, and four classes of Leaf, of increasing leaf age). Each one is initialized by the user, who selects a set of representative pixels for every class in order to induce the clustering around them. The proposed methodology was evaluated with 70 grapevine (V. vinifera L. cv. Tempranillo) images, acquired in a commercial vineyard located in La Rioja (Spain), after several defoliation and de-fruiting events on 10 vines, with a conventional RGB camera and no artificial illumination. The segmentation results showed a performance of 92% for leaves and 98% for clusters, and allowed to assess the grapevine’s leaf area and yield with R2 values of 0.81 (p < 0.001) and 0.73 (p = 0.002), respectively. This methodology, which operates with a simple image acquisition setup and guarantees the right number and kind of pixel classes, has shown to be suitable and robust enough to provide valuable information for vineyard management.
