Labial Fusion Causing Coital and Voiding Difficulty in a Young Woman  [PDF]

Adhesions of the labia are extremely rare in the reproductive population with only a few cases described in the literature. We report a young female patient with coital and voiding difficulty. The patient was a 22-year-old woman presented with voiding difficulty and inability to achieve sexual intercourse. Clinical examination revealed normally developed labia majora, clitoris, and vaginal opening, but fused labia minora and pinhole external urethral orifice. Intraoperatively, labial fusion was dissected with sharp dissection. Urethral dilatation was performed with dittel dilators. Local estrogen ointment was administered postoperatively for four weeks resulting with complete resolution of coital and voiding symptoms.

Gynecologic laparoscopy in patients with prior renal transplantation: 3 cases

Abstract: The number of patients with end stage renal disease whounderwent renal transplantation are getting higher everyyear. And of course, this situation results in an increasein this group, facing a surgery indicated problem. Laparoscopicmanagement of benign and even malign gynecologicpathologies offers a number of clearly defined benefitswhen compared with laparotomy. Randomized andretrospective studies suggest that laparoscopy provideslower estimated blood loss, shorter hospital stay, andfewer postoperative complications with an improved qualityof life and faster return of normal functioning. Abstract: Placenta percreta is one of the life-threatening conditions in modern obstetrics. The rising caesarean section rate means rising placenta percreta rate. Treatment strategies range from a caesarean hysterectomy to leaving the placenta in situ with or without internal iliac artery ligation/uterine artery embolisation and/or methotrexate therapy. We describe a case of placenta previa percreta which we managed successfully with conservative modalities.
Control of the menstrual cycle: the impact of nutrition on reproductive function

Abstract: The menstrual cycle is a complex series of events, especially prone to outside factors during the adolescent period due to the incomplete maturation of hypothalamo-hypophysio-ovarian axis. Both over- and undernutrition may adversely affect the menstrual cycle. Anorexia nervosa, bulimia nervosa and the female athlete triad are the main causes of undernutrition. Menstrual cycle abnormalities and ovulatory problems are common. Overnutrition presents with obesity, becoming more and more prevalent throughout the world, and oligo or anovulation. Present knowledge suggests that energy balance and the amount of readily available energy are more important than “critical weight” in determining anovulation and menstrual cessation. Treatment of these adolescents by a multidiciplinary team increases their reproductive capacity and normolizes their menstrual cycle. (Turk Arch Ped 2011; 46 Suppl: 107-10)
De-Dollarization and European Energy Policy: Testing Brent and WTI  [PDF]

Abstract: Historically most of the oil trade has been made with US Dollars. This has important practical implications. For example, US Dollar is always a reserve currency since it has to be obtained to trade oil. There were previous attempts to trade in Euros such as Iraq however recently there is more support from resource countries such as Russia, China and Iran. A dedollarization campaign against petrodollar has already been started. This can be done by exporting oil, gas and gold with currencies other than US Dollar, such as Ruble, Yuan or Rial or most importantly an international reserve currency which could be Euros as a strong alternative to US Dollar. This is possible due to monetary policy practice of European Central Bank and support from European Union. This increases the value of Euro, decreases interest rates and helps European economy with increased interest on European assets. This paper investigates Brent and WTI for the changes in the value of major currencies. Brent is traditionally a European oil index, and oil is produced in North Sea whereas, WTI (West Texas Intermediate) is a Texas based US oil index. Both indices are used as international benchmarks of oil. The data for this research is daily between February 2011 and September 2014. The WTI and Brent are represented by variables from NYSE exchange traded funds namely Teucrium WTI Crude Oil ETF (WTI), and United States Brent Oil ETF(BRENT). The currencies analyzed for the study are EUR/USD, USD/CHF, USD/JPY, USD/RUB, USD/SAR and USD/ ZAR. The analysis includes unit root tests, vector autoregression (VAR), vector error correction model (VECM), cointegration and Granger Causality. Finally European Energy Policy implications, and opportunities and challenges of oil trade in Euros are discussed.

Abstract: Finite Element Method (FEM) is a well known technique extensively studiedfor spatial and temporal modeling of environmental processes, weather predictioncomputations, and intelligent signal processing for wireless sensors. The need for hugecomputational power arising in such applications to simulate physical phenomenoncorrectly mandates the use of massively parallel computers to distribute the workloadevenly. In this study, a novel heuristic algorithm called Line Graph Bisection whichpartitions a graph via vertex separators so as to balance the workload amongst theprocessors and to minimize the communication overhead is proposed. The proposedalgorithm is proved to be computationally feasible and makes cost-effective parallelimplementations possible to speed up the solution process.
Interleukin-6 and chronic inflammation

Abstract: Inflammation is a complex defence mechanism in which leucocytes migrate from the vasculature into damaged tissues to destroy the agents that potentially can cause tissue injury. Acute inflammation is a limited beneficial response, particularly during infectious challenge, whereas chronic inflammation is a persistent phenomenon that can lead to tissue damage. One hallmark of acute inflammation is that initially the leucocyte infiltrate is mostly neutrophilic, but after 24 to 48 hours monocytic cells predominate [1-3]. In contrast, chronic inflammation is histologically associated with the presence of mononuclear cells, such as macrophages and lymphocytes [1,2].Although several explanations have been suggested, the mechanisms that control the transition from neutrophil to monocyte recruitment during the transformation from acute to chronic inflammation are poorly understood. It is possible that the IL-6/soluble IL-6 receptor α (sIL-6Rα) complex plays an important role in this transition.IL-6 has a dual effect; at some levels it acts as a defence mechanism but in chronic inflammation it is rather proinflammatory.The acute phase response includes changes in the concentrations of many plasma proteins, which are known as the acute phase proteins (Figure 1), as well as numerous behavioural, physiological, biochemical and nutritional changes [4]. The acute phase proteins have been defined as a set of plasma proteins with concentrations that increase (positive acute phase proteins) or decrease (negative acute phase proteins) by at least 25% in inflammatory disorders [4,5] (Figure 1). The changes in concentrations of acute phase proteins are largely due to changes in their production by hepatocytes.The cytokines that are produced during inflammatory processes, and that participate in them, are stimulators of the production of acute phase proteins. Abstract: Witchcraft, namely putting a spell on someone in a general meaning is a traditionally ritual of early pagan societies. In the middle ages, thousands of people was sentenced to torture or death by accusing witchcraft and wondering and harming people around. As we look at the Ottoman Empire, we will see that there were some people who were accused by being a witch and exiled, or burned after their deaths by opening their graves. This study discusses the place of witchcraft and witches in the Western tradition and examines the trials of witchcraft both in Western and Ottoman societies. Lorsqu’on regarde l’Empire ottoman, on constate également qu’il existe des gens accusés de sorcellerie et, de ce fait, ils sont envoyés en exil ou on a ouvert leurs tombes et brulé leurs corps. Dans ce travail, on va étudier d’abord la sorcellerie et la place des sorciers dans la tradition de l’Occident et les affaires judiciaires concernant ce sujet. Puis on va établir un cadre général au sujet de la sorcellerie dans l’Empire ottoman et les attitudes de l’Etat envers les sorciers ; ainsi on va effectuer une étude comparative.

Abstract: Rapid growth in the air transportation industry raises serious concerns regarding operational and training capacity of airports, the key components of the system. Airport capacity development, therefore, is a critical issue not only for the sustainable growth of the air transportation industry but also for providing sufficientnumber of trained pilots to airlines. This study aims to develop a practical discrete-event simulation model to analyze the traffic flow and capacity in maneuvering areas of flight training airports under various traffic scenarios. Total traveling times, average airborne and ground delays are selected as for the analysis of baselineand alternative configurations. The simulation model has been developed using SIMMOD for Anadolu University Airport as the sample study. The model can contribute to the efficient use and development of runway capacity as well as effective planning of flight training at airports.
The Translabyrinthine Approach of Tos and Thomsen for the Treatment of Acoustic Neuroma and Traumatic Facial Nerve Paralysis: Case Reports

Abstract: The translabyrinthine approach recommended by Tos and Thomsen has several different aspects compared to the standard method of the translabyrinthine approach recommended by House. Especially, the dissection of facial nerve in the meatus and porus is safer in the Tos and Thomsen's technique. In this report, results of the translabyrinthine approach of Tos and Thomsen in a 50-year-old patient with a 30 mm acoustic tumor (SRT: 70dB) and a 19-year-old patient with traumatic total facial nerve paralysis and hearing loss are presented. The examination over two years of follow-up revealed that both of the patients had no complications without any restrictions in their life. Both of the patients have normal facial nerve function and there is no recurrence in the MRI scan of the acoustic neuroma patient.
