Preliminary study of ventilation with 4 ml/kg tidal volume in acute respiratory distress syndrome: feasibility and effects on cyclic recruitment - derecruitment and hyperinflation

Abstract: Patients with pulmonary acute respiratory distress syndrome (ARDS) were included in a cross-over study with two Vt levels: 6 and 4 ml/kg. The protocol had two parts: one bedside and other at the CT room. To avoid severe hypercapnia in the 4 ml/kg arm, we replaced the heat and moisture exchange filter by a heated humidifier, and respiratory rate was increased to keep minute ventilation constant. Data on lung mechanics and gas exchange were taken at baseline and after 30 minutes at each Vt (bedside). Thereafter, a dynamic CT (4 images/sec for 8 sec) was taken at each Vt at a fix transverse region between the middle and lower third of the lungs. Afterwards, CT images were analyzed and cyclic recruitment-derecruitment was determined as non aerated tissue variation between inspiration and expiration, and hyperinflation as maximal hyperinflated tissue at end-inspiration, expressed as % of lung tissue weight.We analyzed 10 patients. Decreasing Vt from 6 to 4 ml/kg consistently decreased cyclic recruitment-derecruitment from 3.6 (2.5-5.7) % to 2.9 (0.9-4.7) % (p<0.01) and end-inspiratory hyperinflation from 0.7 (0.3 - 2.2) to 0.6 (0.2 - 1.7) % (p=0.01). No patient developed severe respiratory acidosis or severe hypercapnia when decreasing Vt to 4 ml/kg (pH 7.29 (7.21-7.46); PaCO2 48 (26 - 51) mmHg).Decreasing Vt from 6 to 4 ml/kg reduces cyclic recruitment-derecruitment and hyperinflation. Severe respiratory acidosis may be effectively prevented by decreasing instrumental dead space and by increasing respiratory rate.

Abstract: echinococcal disease remains a major problem within some endemic areas. we report a case of a single primary echinococcal cyst located in the retroperitoneal space. a 54-year-old woman, born in a rural area of southern chile, was admitted with a 3-month history of right hip pain and painful swelling of the gluteal region. hidatid disease was confirmed with serologic test, radiological examinations and histo-pathology. there were no cysts in any other location. a percutaneous drainage was performed and antihelminthics were administered for 12 weeks and is now being closely followed up, with good response to therapy. especially in the endemic areas hydatid cyst should be considered when evaluating retroperitoneal cystic masses.

Abstract: small bowel neoplasms are rare, accounting for approximately 3-6% of all priman/ malignancies of the gastrointestinal tract. mdct-e is an imaging modality that combines the advantages of enteroclysis and helical ct, allowing the detection of small bowel diseases wherever they are located (intraluminal, intramural, or extramural). the aim of our study was to assess the reliability of mdct-e in the detection of small bowel neoplasms. in our institution, mdct-e was used to study 600 patients admitted for suspicion of small bowel diseases, between january2005 and may 2008. a total of 22 (3, 6 %) small bowel neoplasms were identified, all confirmed by histological examination. the neoplasms found in this series were adenocarci-noma (n: 4), lymphoma (n: 3), carcinoid tumor (n: 3), benign gist (n: 2), benign hamartomatous polyps (n: 2) and secondary small bowel tumors (n: 8). mdct-e appears to be a reliable method in the detection of small bowel neoplasms, allowing tumor staging tobe determined during the same procedure.

Abstract: Los tumores de intestino delgado (ID) son raros, representando aproximadamente 3-6% de las neoplasias del tubo digestivo. La E-TC es una técnica que combina las ventajas de la enteroclisis con las de la TC multicorte, con el potencial de demostrar alteraciones intraluminales, murales y extraintesti-nales. El objetivo de nuestro estudio fue evaluar el potencial diagnóstico de la E-TC en la detección de tumores de ID. En nuestra institución se realizaron 600 E-TC desde Enero del 2005 a Mayo del 2008. Un total de 22 pacientes (3,6 %) presentaron hallazgos tomográficos compatibles con tumores de ID, todos confirmados histológicamente. Los tumores encontrados en esta serie fueron; adenocarcinoma (n:4), linfoma (n:3), tumor carcinoide (n:3), tumor estromal gastrointestinal benigno (n:2), pólipos hamartomatosos (n: 2) y compromiso neoplásico secundario (n:8). Esta serie corrobora el valor de la E-TC como técnica de imagen de elección en la detección de tumores de ID, con la posibilidad de etapificación en el mismo procedimiento. Utilidad y complicaciones de la biopsia percutánea esplénica con aguja tru-cut guiada por imágenes: Review of 13 procedures Ultrasound guided needle biopsy of the spleen

Abstract: Objetivo: Reportar la experiencia de biopsias percutáneas esplénicas con aguja tru-cut guiadas por imágenes. Materiales y Métodos: Revisión retrospectiva de biopsias esplénicas con aguja tru-cut guiadas por ultrasonido (US) y tomografía computada (TC) realizadas en nuestro hospital desde Enero de 2005 a Abril de 2009. Resultados: Se identificaron un total de 13 procedimientos. La biopsia percutánea logró un diagnóstico específi co en 9 (69%) de las 13 intervenciones. Los diagnósticos fueron linfoma (n = 4), melanoma (n = 2), sarcoma (n = 1), hematopoyesis extramedular (n = 1) y criptococosis esplénica (n = 1). De las biopsias no diagnósticas 3 casos correspondieron a patología neoplásica y uno a patología benigna. Se reportaron 2 complicaciones (15%). Discusión: La biopsia esplénica percutánea guiada por imágenes con aguja tru-cut es un procedimiento útil y seguro, capaz de determinar el diagnóstico definitivo en la mayoría de los pacientes y evitar la mayoría de las esplenectomías diagnósticas. Background: Needle biopsies of the spleen were avoided due to the fear of bleeding in a highly vas-cularized organ. However their safety, even using 18 gauge needles, has been demonstrated. Aim: To report the experience with ultrasound guided needle biopsies of the spleen. Material and Methods: Retrospective review of records of ultrasound guided biopsies of the spleen using Tru-cut needles, performed between 2005 and 2009. Results: Thirteen procedures performed in 12 patients were identified. A specific diagnosis was achieved in nine (69%) procedures (lymphoma in four, melanoma in 2, sarcoma in 1, extremedullary erythropoiesis in one and splenic cryptococcosis in one. Two patients with negative results were subjected to a new biopsy, which yielded the diagnosis of lymphoma. A third patient was studied elsewhere, finding a malignant tumor. Two patients had complications, one had a vagal reaction and other had a perisplenic hematoma without clinical repercussion. Conclusions: Ultrasound guided needle biopsy of the spleen is a safe and useful procedure.

Abstract: El hemangioma rectal es una entidad infrecuente, con menos de 200 casos publicados en la literatura mundial. Se presenta un caso clínico, que debuta con rectorragia y es documentado con estudios endoscópicos e imagenológicos, siendo resuelto quirúrgicamente. We report a 41 years old male with a history of hematochezia since childhood. A colonoscopy showed a highly vascularized submucosal lesion extending from the pectinate line to the distal sigmoid colon. Magnetic resonance showed a thickening of rectal wall with multiple vascular structures and phleboliths. The lesion was excised surgically. The postoperative period was uneventful. The pathological report disclosed a large rectal hemangioma.

Abstract: La hidatidosis sigue siendo un problema prevalente en áreas endémicas. Presentamos el caso de un quiste hidatídico primario ubicado en el espacio retro-peritoneal. Mujer de 54 a os, procedente de área rural del sur de Chile, hospitalizada por cuadro de coxalgia derecha y aumento de volumen glúteo, con tres meses de evolución. Se realizó el diagnóstico de hidatidosis complicada por medio de exámenes de imágenes, tests serológicos y anatomía patológica. No se demostraron quistes hidatídicos en otras ubicaciones. Se realizó un drenaje percutáneo del quiste infectado y se administraron antihelmínticos durante 12 semanas y actualmente permanece en seguimiento cercano con buena respuesta al tratamiento. El quiste hidatídico debe ser considerado en el diagnóstico diferencial de masas quísticas retro-peritoneales, especialmente en áreas endémicas.
