Abstract: Objetivo: Describir la frecuencia de infecciones oportunistas y determinar cual es la más frecuente en pacientes VIH+ del Hospital Vargas de Caracas durante el período 2005 - 2006. Dise o: Estudio descriptivo, transversal, retrospectivo y observacional. Lugar: Hospital "José María Vargas" de Caracas, Venezuela. Participantes: Pacientes ingresados con historia de VIH+ e infección oportunista. Intervención: Revisión de Historias Clínicas de archivo entre los a os 2005-2006. Principales medidas de Resultados : Porcentajes y Medidas de Tendencia Central, tabuladas en una base de datos con ayuda del programa Microsoft Excel 2003 para Windows . Resultados : Se revisaron 120 historias; 83 cumplían con los criterios de inclusión. El promedio de edad fue 36,87 a os; 59,04% correspondían al sexo masculino y 40,96% al sexo femenino. El 44,4% fueron infecciones respiratorias, 31,88% infecciones del SNC. Las infecciones más frecuentes: 24,64% Mycobacterium tuberculosis, 19,87% Toxoplasma gondii, 15,94% Candida albicans y 10,14% Pneumocystis jiroveci. En el 63,86% de los casos se encontraron dos o más infecciones simultáneas. El 64% de estos pacientes recibía tratamiento y el 46% era heterosexual no promiscuo. Conclusiones : Las alteraciones respiratorias constituyen la infección oportunista más frecuente en pacientes VIH+.
Gram-Negative Enterobacteria Induce Tolerogenic Maturation in Dexamethasone Conditioned Dendritic Cells

Abstract: Dendritic cells have been investigated in clinical trials, predominantly with the aim of stimulating immune responses against tumours or infectious diseases. Thus far, however, no clinical studies have taken advantage of their specific immunosuppressive potential. Tolerogenic DCs may represent a new therapeutic strategy for human immune-based diseases, such as Crohn’s disease, where the perturbations of the finely tuned balance between the immune system and the microflora result in disease. In the present report, we describe the generation of tolerogenic DCs from healthy donors and Crohn’s disease patients using clinical-grade reagents in combination with dexamethasone as immunosuppressive agent and characterize their response to maturation stimuli. Interestingly, we found out that dexamethasone-conditioned DCs keep their tolerogenic properties to Gram-negative bacteria. Other findings included in this study demonstrate that the combination of dexamethasone with a specific cytokine cocktail yielded clinical-grade DCs with the following characteristics: a semi-mature phenotype, a pronounced shift towards anti-inflammatory versus inflammatory cytokine production and low T-cell stimulatory properties. Importantly, in regard to their clinical application, the tolerogenic phenotype of DCs remained stable after the elimination of dexamethasone and after a second stimulation with LPS or bacteria. All these properties make this cell product suitable to be tested in clinical trials of inflammatory conditions including Crohn’s disease.
Estimación de información faltante, imputación y estadísticos de prueba en modelos mixtos a dos vías de clasificación

Abstract: we propose a methodology to estimate missing information in mixed cell means models. this methodology improves on that melo & melo (2005), which is based on the methods of maximum likelihood estimation and covariate proposed by bartlett (1937), and reduces the correlation between the observed and estimated information. once the imputation of the missing information is done, we suggest a way to perform the analysis of variance in models without interaction, by generating a weighted test for the fixed and random effects involved in the model.

Abstract: objective: the objective of this paper is to critically identify and analyze the many concepts that professors of the subject nutrition education have of practice and how they understand the theory-practice relationship. methods: the study was done in universities and colleges of the state of s?o paulo, brazil, that have been registered at the ministry of education since 1999. with this information, professors of the subject nutrition education were interviewed in all these institutions totaling 11 individuals. the results were analyzed qualitatively, using the hermeneutic-dialectic method. results: the professors had similar opinions regarding the importance of practice; however what practice meant in nutrition education varied. some consider that practical activities are enough for the formation of the student and do not search theoretical fundamentals in social and human sciences. we also observed that practice was overvalued in relation to theory. all these aspects contribute to compromise the effectiveness of the theory-practice relationship. conclusion: it is up to the graduation courses and the government departments that surveil education to discuss the education of the professors, the concepts behind practical activities and more effective ways to work the dialectics between practice and theory. it is necessary to stimulate, from the beginning of the graduation course, the articulation between theory and practice in order to allow the students to contextualize the acquisition of knowledge and form social and ethical compromises.

Se propone una metodología para la estimación de información faltante en modelos mixtos de medias de celda que permite la disminución de la correlación entre la información observada y la información estimada, basada en el método propuesto por Melo & Melo (2005). éste se fundamenta en los métodos de estimación vía máxima verosimilitud, expuesto en Searle (1971), y de covariable, propuesto por Bartlett (1937). Después de realizar la imputación de la información, se plantea una manera de llevar a cabo el análisis de varianza en modelos sin interacción, mediante pruebas ponderadas para los efectos fijos y aleatorios involucrados en el modelo.
Crosstalk between Immune Cells and Adipocytes Requires Both Paracrine Factors and Cell Contact to Modify Cytokine Secretion

Abstract: Increased adiposity results in a heightened infiltration of immune cells into fat depots, which in turn generates a pro-inflammatory phenotype in obese individuals. To better understand the causal factors that establish this pro-inflammatory profile, we examined events leading to crosstalk between adipocytes and immune cells. Using isolated spleen-derived immune cells, stimulated with LPS, together with cultured adipocytes, we differentiated the effects of paracrine factors and cell-cell contact on TNFα, IL-6 and MCP-1 secretion levels and secretion profiles. When splenocytes and adipocytes were co-cultured without direct contact, permitting only paracrine communication, secretion of IL-6 and MCP-1 were increased by 3- and 2.5-fold, respectively, over what was secreted by individual cultures, whereas TNFα secretion was reduced by 55%. When cells were co-cultured with direct cell-cell contact, IL-6 and MCP-1 secretion were increased by an additional 36% and 38%, respectively, over that measured from just paracrine stimulation alone, indicating that cell contact provides a synergistic signal that amplifies elevated cytokine secretion stimulated by paracrine signals. Using splenocytes from TNFα-/- mice showed that the absence of TNFα has little effect on paracrine stimulation of cytokine secretion, but attenuates cell contact-mediated enhancement of IL-6 and MCP-1 secretion. Furthermore, TNFα supports cell contact-mediated signaling in part, but not exclusively, through Nuclear Factor-κB activation. These findings indicate that engagement of cell contact between immune cells and adipocytes, in conjunction with locally secreted paracrine factors, activates a unique signaling pathway that mediates crosstalk between these cell types leading to marked effects on cytokine secretion and profile.
NFκB in the Development of Endothelial Activation and Damage in Uremia: An In Vitro Approach

Abstract: Impaired hemostasis coexists with accelerated atherosclerosis in patients with chronic kidney disease (CKD). The elevated frequency of atherothrombotic events has been associated with endothelial dysfunction. The relative contribution of the uremic state and the impact of the renal replacement therapies have been often disregarded. Plasma markers of endothelial activation and damage were evaluated in three groups of patients with CKD: under conservative treatment (predialysis), on hemodialysis, and on peritoneal dialysis. Activation of p38 MAPK and the transcription factor NFκB was assessed in endothelial cell (EC) cultures exposed to pooled sera from each group of patients. Most of the markers evaluated (VCAM-1, ICAM-1, VWF, circulating endothelial cells) were significantly higher in CDK patients than in controls, being significantly more increased in the group of peritoneal dialysis patients. These results correlated with the activation of both p38 MAPK and NFκB in EC cells exposed to the same sera samples, and also to the peritoneal dialysis fluids. Hemodialysis did not further contribute to the endothelial damage induced by the uremic state observed in predialysis patients, probably due to the improved biocompatibility of the hemodialysis technique in recent years, resulting in lower cellular activation. However, peritoneal dialysis seemed to exert a significant proinflammatory effect on the endothelium that could be related to the high glucose concentrations and glucose degradation products present in the dialysis fluid. Although peritoneal dialysis has been traditionally considered a more physiological technique, our results raise some doubts with respect to inflammation and EC damage.

Abstract: the following review article focuses on chemoprevention clinical trials of breast cancer. to date, serms (selective estrogen receptor modulators) have been the most studied drugs. four randomized trials with tamoxifen vs. placebo have been performed and two with raloxifene are being carried out. two tamoxifen trials showed between 30 and 50% reduction in breast cancer incidence. however, two other studies showed no statistical differences. moreover, the real impact on mortality that these therapies could have is still unknown. this article includes a revision of trials that evaluated the relationship between daily vitamin intake and breast cancer. a follow up of these trials will give us answers about which patients will benefit from chemoprevention therapies.
