Macrovascular complication phenotypes in type 2 diabetic patients

Abstract: A diabetic outpatient cohort (n = 1199) was retrospectively studied. Demographic, clinical and laboratory parameters were included in analyses. A thorough cardiovascular history as documented by previous medical records (including medical and hospital records) and vascular laboratory studies (including standardised electrocardiogram, echocardiogram, provocative tests for cardiac ischaemia, ankle/brachial index, duplex ultrasonography of the carotid and lower limbs and, in selected cases, computed tomography angiography, carotid and peripheral arteriography and evaluation of transcutaneous oxygen pressure), was collected for all of the patients. Standardised procedures were used to assess microvascular complications as well as metabolic syndrome (Mets).The unadjusted MVD prevalence was 46.4% among the participants. The majority of patients with MVD were in the PVD group. In the multivariate analysis, age, male sex and diabetes duration were independent risk factors for PAD and PVD (P < 0.01). A low HDL-C value was an independent risk factor in the CAD and PVD groups (P = 0.03). Very high frequencies of MetS were observed in the PAD and PVD groups (94.9 and 95.7% respectively). The most MetS diagnostic criteria were recorded among members of the CAD group (all or all-1 criteria were present in 73% of patients). The average age in the CAD group (64.5 y) was comparable to that of the NMVD group. Microvascular complications were more frequent in the PAD and PVD patients.Phenotypic heterogeneity is associated with different macrovascular complications in T2DM patients. These findings might have clinical implications for developing diagnostic and therapeutic strategies targeting type 2 diabetes.Cardiovascular complications are the leading cause of morbidity and mortality among patients with type 2 diabetes, and cardiovascular disease (CVD) risk is 2- to 8-fold higher in the diabetic population than it is in non-diabetic individuals of a similar age, sex and ethnicity [1,2]

Abstract: L'amicizia tra i monaci di Camaldoli e di Vallombrosa nel medioevo può essere analizzata da vari punti di vista. C'è l'amicizia tra persone, che nella tradizione monastica prende spesso la forma del rapporto tra maestro e discepolo. L'amicizia dell'uomo con la natura e gli animali, particolarmente forte tra gli eremiti che vivono nelle foreste. L'amicizia tra monaci e laici, benefattori e quanti altri sono vicini alla spiritualità dei monaci. L'amicizia tra le istituzioni, che accolgono con cura particolare i monaci del monastero amico e li ricordano nelle loro preghiere. Infine, l'amicizia può essere un valore fondamentale per costruire una congregazione monastica: essa dà una prima forma ad una istituzione nascente. The friendship among the monks of Camaldoli and Vallombrosa in the Middle Ages can be a-nalyzed by various points of view. There is the friendship among people, that often takes the form of the relationship between teacher and disciple in the monastic tradition. The friendship of the man with the nature and the animals, spread exspecially among the hermits that live in the forests. The friendship between monks and laymen, benefactors and many others who are near to the spirituality of the monks. The friendship among the institutions, that admit with particular care the monks of the joint monastery and remember them in their prayers. Finally, friendship can be a fundamental value to build a monastic congregation: it gives a first form to a dawning institution.
Prophylaxis and therapy of viral infections in pediatric patients treated for malignancy

Abstract: Infections are still an important cause of mortality and morbidity in pediatric cancer patients. Most of the febrile episodes in immunocompromised patients are classified as a fever of unknown origin (FUO) while bacteria are the more frequent causes of documented infections. Viral infections are also feared during chemotherapy but less data are available on their incidence and morbidity. We reviewed the literature on incidence, morbidity, and mortality of viral infections in children undergoing chemotherapy and discussed the evidence concerning the prophylaxis and the therapy.
Application of Microwave Remote Sensing to Dynamic Testing of Stay-Cables

Abstract: Recent advances in radar techniques and systems have favoured the development of microwave interferometers, suitable for the non-contact vibration monitoring of large structures. The paper addresses the application of microwave remote sensing to the measurement of the vibration response in the stay-cables of cable-stayed bridges. The reliability and accuracy of the proposed technique were investigated by comparing the natural frequencies (and the cable tensions predicted from natural frequencies) identified from radar data and the corresponding quantities obtained using more conventional techniques. The investigation, carried out on the cables of two different cable-stayed bridges, clearly highlights: (a) the accuracy of the results provided by the microwave remote sensing; (b) the simplicity of use of the radar technique (especially when compared with conventional approaches) and its effectiveness to simultaneously measuring the dynamic response of all the stay-cables of an array.
