Abstract: objective: to analyze the clinical results after proximal humerus fracture fixation using blocked plates and screws. material and methods: in the period from november 2003 to january 2008, the authors treated 33 patients with 34 fractures of the proximal humerus. the mean age was 57 years old (33 to 86 years), 14 males and 19 females. all patients were operated in the acute phase, within the first week after the trauma. the patients were treated with open reduction and internal fixation using locking plates. the surgical technique, implants, period of immobilization and rehabilitation protocol were the same for all the patients. results: the mean follow-up period was 24 months (8 to 60 months). the functional evaluation was based in the ucla score. good and excellent results were present in 31 (91.2%) of the patients and 3 (8.8%) of them presented poor results. conclusions: as a conclusion, the locking plate system of fixation was considered efficient for the treatment of proximal humerus fractures.

Abstract: OBJETIVO: Avaliar a rela o existente entre os casos de tendinopatia calcárea de ombro e doen as metabólicas sintomáticas como litíase renal, biliar e gota. MéTODO: A tendinopatia calcárea de ombro foi diagnosticada em 63 pacientes no período compreendido entre maio de 2007 e setembro de 2011. Todos os pacientes foram tratados pelo mesmo médico ortopedista e entrevistados para coleta dos seguintes dados: idade ao diagnóstico, sexo, lado acometido, lado dominante, índice de massa corpórea (IMC), tabagismo e história pregressa de litíase renal, biliar ou gota. Para análise estatística, um grupo controle de 63 pacientes com características demográficas semelhantes foi utilizado. RESULTADOS: Dos 63 pacientes que apresentavam tendinopatia calcárea do ombro, 35 (56%) eram do sexo masculino. O lado direito foi afetado em 38 (60%) pacientes, a média de idade foi de 48,2 anos. Trinta e um (49%) pacientes apresentaram história prévia de alguma das doen as metabólicas questionadas, sendo que 20 pacientes (32%) relataram litíase renal, seis (9,5%) litíase biliar, quatro (6,3%) gota e um (2%) apresentava diagnóstico concomitante de litíase renal e gota. No grupo controle, observamos que 11 (17%) pacientes apresentaram história prévia de alguma das doen as metabólicas, sendo que seis pacientes (9,5%) relataram litíase renal, quatro (6,3%) litíase biliar e um (1,6%), gota. CONCLUS ES: A elevada frequência de litíase renal em pacientes diagnosticados com tendinopatia calcárea do ombro no presente estudo sugere que existam mecanismos em comum na fisiopatologia desses distúrbios. O melhor entendimento destas doen as pode possibilitar a melhoria dos seus diagnósticos e tratamentos. OBJECTIVE: To evaluate the relationship between cases of calcifying tendinopathy of the shoulder and symptomatic metabolic diseases such as kidney stones, gallstones and gout. METHODS: Calcifying tendinopathy of the shoulder was diagnosed in 63 patients between May 2007 and September 2011. All the patients were treated by the same orthopedic surgeon and were interviewed to gather the following data: age at diagnosis, sex, affected side, dominant side, body mass index (BMI), smoking status and previous histories of kidney stones, gallstones or gout. For statistical analysis, a control group of 63 patients with similar demographic characteristics was used. RESULTS: Among the 63 patients with calcifying tendinopathy of the shoulder, 35 (56%) were male. The right side was affected in 38 patients (60%) and the average age was 48.2 years. Abstract: objective: to evaluate the long-term results of seventy patients who underwent hip arthrodesis using the original technique described by davis. methods: we carried out a retrospective study involving seventy patients submitted to hip arthrodesis between 1982 and 1995. the presence of symptoms involving the lumbar spine, ipsilateral knee, and contralateral hip was noted, as well as the success of the arthrodesis fusion, its positioning, and the need for conversion surgery to total hip replacement. results: the mean follow-up time was 21.6 years. a satisfactory hip fusion was found in 48 patients (85.7%). lumbar spine pain was reported by 11 patients (19.6%) and ipsilateral knee osteoarthritis was found in four patients (7.1%). discussion: two works, with a longer follow-up time (35 and 38 years) present more prevalent symptoms involving the lumbar spine (57% and 62% ) and ipsilateral knee joint (45% and 57%), and higher conversion surgery rates (17% e 28%). conclusion: hip arthrodesis by the davis technique presents satisfactory results until the 20th year after surgery, however lumbar spine disease seems to become more prevalent over the years, and the pain in the homolateral knee suggests an association with the initial position in abduction of the hip submitted to arthrodesis. level of evidence: level iii clinical study.

Abstract: scapulothoracic arthroscopy is a procedure presenting restricted indications, for resecting free bodies, benign tumors, bursitis, and snaping scapula. the authors report four cases of scapulothoracic joint arthroscopy; in the first case, only a benign tumor (osteochondroma) could be visualized; in the second case, arthroscopic resection of an osteochondroma was found; in the third case, arthroscopic bursectomy due to scapulothoracic bursitis, and; in the fourth case, bursectomy and partial superomedial arthroscopic scapulectomy due to snaping scapula.

Abstract: context: there are different causes for shoulder pain in athletes. structures such as tendons and ligaments are frequently affected by injuries. in young athletes with immature skull, bone injuries must be ruled as a cause of pain. case report: the authors present a case of proximal humeral epiphysiolysis in a gymnast and review literature addressing this topic. conclusion: injuries on proximal humeral physeal plates of athletes are uncommon. the early diagnosis and correct treatment can avoid deformity and functional deficits.

Abstract: objective: to report on the management of five cases of pyomyositis in athletes after the use of anabolic steroids. method: over the past 10 years, five cases of athletes who developed pyomyositis after using anabolic steroids were attended at the sports trauma center (cete), epm-unifesp. results: all the patients were diagnosed clinically and through laboratory and imaging tests. surgical treatment was carried out (with collection of material for culturing) and antibiotic therapy was administered. in four cases, the injection sites were in the upper limbs and in one case, in the gluteus muscles bilaterally as well as in the upper limbs. in all five cases, occurrences of leukocytosis and neutrophilia were observed in the hemogram. after debridement, the germs of normal skin (s. aureus and s. viridans) were found in cultures on the secretions. demarcation of the abscess and examination of the muscle plane in which the abscess was located were performed using ultrasound and magnetic resonance imaging. all the patients responded to broad-spectrum antibiotic therapy. two cases required more than one surgical procedure because of the appearance of more than one abscess site with different evolution times. conclusion: the use of anabolic steroids by some athletes may have grave consequences. rapid, energetic and multidisciplinary intervention is necessary in such cases in order to avoid undesirable results. the right treatment healed the athletes completely, and they returned to their sports at the same level.

Abstract: objective: to establish the anatomic relationship of the suprascapular nerve (ssn) located in the suprascapular notch to the medial border of the base of the coracoid process, the medial acromial surface of the acromioclavicular joint and the anterolateral edge of the acromion. methods: we dissected 16 shoulders of 16 cadavers, 9 males and 7 females. the distance from the suprascapular nerve (in its course beneath the transverse ligament) to certain fixed points in the medial base of the coracoid process was measured with the aid of a caliper, as well as to the articular surface of the acromion and the acromioclavicular joint on the anterolateral edge of the acromion. these measures were correlated with age and sex. the specimens with signs of previous surgery were excluded. results: measuring the suprascapular nerve in its notch to the medial border of the coracoid process, the base of the coracoid process averaged 3.9 cm (ranging from 3.1 cm to 5.2 cm), the acromioclavicular joint averaged 4.7 cm (ranging from 3.9 cm to 5.2 cm,) and the anterolateral border of the acromion averaged 6.1 cm (ranging from 5.7 cm to 6.8 cm). conclusion: accurate knowledge of the anatomy of nerves of the scapulohumeral region is essential in order to avoid iatrogenic injuries and to achieve satisfactory results in surgical treatment (open or arthroscopic) of shoulder pathologies.

Abstract: objectives: our primary aim was to evaluate the occurrence of dislocation of non-cemented total hip arthroplasty, when using the posterior and the direct lateral approaches. methods: we performed a comparative retrospective study with 232 patients submitted to non-cemented total hip arthroplasty, due to the diagnosis of primary or secondary osteoarthritis. the posterior approach was used in 105 patients while direct lateral approach was used in 127 patients. there was only one prosthesis model and the same rehabilitation program and post-operative care was used for all patients. we checked the occurrence of dislocation, the acetabular positioning and also the size of the components. results: there was only one case of dislocation, treated with closed reduction successfully. this was a 47 year-old female, submitted to direct lateral approach. the mean follow-up time for both groups was 23.7 months, ranging from six to 42 months. conclusion: the authors conclude that the prevalence of total hip arthroplasty dislocation is similar for both approaches, and educational measures besides the use of a higher femoral offset seem to reduce the risk of this complication.
El castillo de Penna Cadiella en la sierra de Benicadell

Abstract: In this article we try to locate the place where the castle of Penna Cadiella, mentioned in the Poem of Mio Cid could had been built. To this we reintroduce the hypothesis expressed in its time by P. Guichard regarding the castle's location on top of the Benicadell range of mountains (Valencia-Alicante) and we provide new documentary and archaeological data. Likewise, we analyse the 371st chapter of Jaime I′s Crónica in which the battle fought for the possession of the above-mentioned castle by the troops of the Aragonese king against al-Azraq in the thirteenth century is narrated. En este trabajo se intenta localizar el lugar en el que pudo asentarse el castillo de Penna Cadiella mencionado en el Poema de Mío Cid. Para ello, se retoma la hipótesis que en su día planteara P. Guichard sobre su ubicación en la cima de la Sierra de Benicadell (Valencia-Alicante) y se aportan nuevos datos documentales y arqueológicos. Asimismo, se analiza el capítulo 371 de la Crónica de Jaime I en el cual se relata la batalla que, por su posesión, libraron las tropas del rey aragonés contra al-Azraq a mediados del siglo XIII.
