Hipertens?o arterial e diabetes na Estratégia de Saúde da Família: uma análise da frequência de acompanhamento pelas equipes de Saúde da Família

Abstract: the aim of this article is to evaluate the diabetes and hypertension records and the follow-up care by the health professionals of the 13th regional health precinct teams (13th crs). it is a descriptive-exploratory. the student's t test was used to check the differences between the hypertension and diabetes records in the municipalities. significant statistical difference was ascertained only for p<0.05. the pearson test was used to check the relation between the hypertension and diabetics records. the monitoring of hypertension and of diabetes cases by the health professionals do not correspond to the annual average of register in the different municipalities of the 13th crs. the municipalities of the 13th crs showed a smaller level of annual monitoring by the health professionals regarding the mean of hypertension register (register: 37.3 ± 2.4 versus monitoring: 28.0 ± 2.1; p<0.05) and diabetes (register: 7.2 ± 0.6 versus monitoring: 5.7 ± 0.5; p<0.05). the municipality of venancio aires showed the lowest rates of follow-up care by the health teams, reaching 50% in cases of hypertension and 55% in diabetes, as compared to the other municipalities. the health teams were found to provide very little follow-up care to hypertension and diabetes patients in the municipalities comprised by the 13th crs.

Abstract: Esta investigación objetiva describir al perfil de las embarazadas en la 13a Coordenaduría Regional de Salud (2003) en las dimensiones del acompa amiento pre-natal, inmunizaciones y gestantes con menos de 20 a os. El estudio es de tipo descriptivo-exploratorio. De entre las mujeres embarazadas menores de 20 a os, se destacan las ciudades de Candelária (53,3%), Herveiras (50,0%), Venancio Aires (30%) y Santa Cruz do Sul (28,9%). En cuanto al porcentaje sobre la realización de las inmunizaciones, excepto Santa Cruz do Sul (74,7%) y Venancio Aires (72,5%), los otros fueron superiores al 85%. Santa Cruz do Sul y Venancio Aires también, presentaron los más bajos porcentajes pare el pre natal en el 1° trimestre, excepto Mato Leit o y Herveiras (94,1% y 94,6%, respectivamente), los otros municipios presentaron porcentajes abajo del 77%. Estos datos sugieren acciones más eficaces en el campo de la salud preventiva, lo cual podría atenuar los niveles elevados de gestantes menores de 20 a os en esta región.

Abstract: the present study's objective was to evaluate health indicators in the local authority covered by the 13th crs/rs-2003 in the dimensions of accompaniment of pre-natal, immunization, and pregnant women younger than 20 years (p<20). this is a descriptive-exploratory type study. among the p<20, candelária (53.3%), herveiras (50.0%), venancio aires (30%) and santa cruz do sul (28.9%) stood out. in relation the percentages of immunizations achieved, except santa cruz do sul (74.7%) and venancio aires (72.5%), all those covered were higher than 85%. santa cruz do sul and venancio aires also showed the lowest percentages for pre-natal in comparison with other local authorities. the realization of pre-natal in the 1st trimester, except in mato leit?o and herveiras (94.1% and 94.6%, respectively), all those covered showed percentages lower than 77%. this data suggests that more effective actions be taken in the field of preventative health care, which could match the elevated levels of p<20 in the local region covered by the 13th crs.

Abstract: intestinal parasites in hiv infected patients are a compelling issue in epidemiological settings with high prevalence of these parasites. the present study reviewed frequencies of diagnosis for common intestinal parasites in hiv infected patients in association with sex, age, hiv risk behavior and cd4 lymphocyte count. patients with confirmed hiv diagnosis and positive routine stool examination between january and december 2000 were selected. diagnostic methods for cyclospora cayetanensis and microsporidia were not included in the routine stool examination. a total of 146 patients had one or more positive samples (median 1.5) with age ranging between 2-75 years (mean: 34). most of them were male (70.5%) and 53.6% had a cd4 count > 200 cells/mm3 (median: 158.5 cells/mm3). endolimax nana (27.4%), entamoeba coli (22.6%), strongyloides stercoralis (21.9%), giardia lamblia (13.0%), isospora belli (12.3%), ascaris lumbricoides (8.2%), cryptosporidium parvum (6.8%) were the most frequent parasites diagnosed in this group. g. lamblia was more common in children with vertically transmitted hiv and men who have sex with men. no significant associations were found in relation to sex and age. patients with cd4 count < 200 cells/mm3 presented lower prevalence of non-pathogenic parasites (e.g. e. nana or 0.46 and e. coli or 0.33) but had more pathogenic parasites diagnosed such as s. stercoralis (or 3,9) and i. belli (or 3,5) in relation to those hiv-infected individuals with cd4 count e?200 cells/mm3. these results underscore the importance of s. stercoralis in aids patients; the decrease in c. parvum prevalence in relation to previous reports from our institution before the routine use of haart; local differences in parasite profiles in comparison to other epidemiological settings as well as changes in the proportion of diagnosis for pathogenic and non-pathogenic parasites according to the immune compromise due to hiv infection

Abstract: Intestinal parasites in HIV infected patients are a compelling issue in epidemiological settings with high prevalence of these parasites. Endolimax nana (27.4%), Entamoeba coli (22.6%), Strongyloides stercoralis (21.9%), Giardia lamblia (13.0%), Isospora belli (12.3%), Ascaris lumbricoides (8.2%), Cryptosporidium parvum (6.8%) were the most frequent parasites diagnosed in this group. G. lamblia was more common in children with vertically transmitted HIV and men who have sex with men. No significant associations were found in relation to sex and age. Patients with CD4 count < 200 cells/mm3 presented lower prevalence of non-pathogenic parasites (e.g. E. nana OR 0.46 and E. coli OR 0.33) but had more pathogenic parasites diagnosed such as S. stercoralis (OR 3,9) and I. belli (OR 3,5) in relation to those HIV-infected individuals with CD4 count e 200 cells/mm3. These results underscore the importance of S. stercoralis in AIDS patients; the decrease in C. parvum prevalence in relation to previous reports from our institution before the routine use of HAART; local differences in parasite profiles in comparison to other epidemiological settings as well as changes in the proportion of diagnosis for pathogenic and non-pathogenic parasites according to the immune compromise due to HIV infection

Abstract: this article discuss, in prospect historic, the public administration classics models and the burocratic phenomenon in brazil. on the one hand, it shall presented the bureaucratic phenomenon into a wide perspective, linking it to a social changes processus, like the industrialization and the urbanization. on the other hand, it shall discussing the bureaucratization phenomenon into a perspective denominated micro, to detachable it into a administrative politic dimension. the focus is the decision process and state management that are the substance of the administrative reforms made from the 1930' until the vargas management in 1945. this period coincides with a series of political administrative reforms than it shifted, definitely, the relationship between state and society, as well as the acquaintanceship into the brazilian state.

Abstract: the article discusses the professional experience of samuel pessoa (1898-1976), parasitologist and physician, within the framework of the getúlio vargas government's political and administrative reforms. pessoa's trajectory shows that growing bureaucratization of all state sectors, presented new challenges to enforcers of public health policies. nevertheless, this trend did not imply a radical sterilization of the public health campaign approach endorsed by the medical elite in the 1920s. an examination of pessoa's career reveals how brazil's various 'sanitary schools' did not necessarily espouse antagonistic views; rather, pessoa himself proved a recognized proponent of 'developmentalism', influenced by a more technicist public health model, yet did not turn his back on the 1920s discussions, particularly when it came to his interest in sanitary initiatives in rural brazil.
A saúde pública em tempos de burocratiza??o: o caso do médico Noel Nutels

Abstract: this case study of the trajectory of sanitarian noel nutels argues that starting in the 1930s and particularly during the long vargas period, the history of brazilian public health became characterized by a process of bureaucratization. although the chief consequence was an increase in problems in implementing public policies, these difficulties do not appear to have prompted any immediate ideological demobilization within the sanitation movement, as occurred under the old republic, having instead engendered conflicts between those directly responsible for designing and implementing public policies, on the one hand, and the growing state bureaucracy, on the other. the historical period initiated in 1930 thus marked a broad divergence between the conduct of 'men of science' and 'men of state'. if the period preceding the revolution was characterized by a certain harmony between these two social sectors, won in the late nineteenth century, the process of bureaucratization altered this balance, dislodging public health agents and authorities from the special roles they had enjoyed within the brazilian state machinery.

Abstract: this article discusses some meanings associated with the concept of solidarity. the themes of social support and social justice are particularly focused, as the intellectuals of the same current of thought turned against social exclusion and towards democracy and social solidarity. it is proposed that the absence of social solidarity is linked to the decline of public support and social assistance systems and institutions in modern western societies. from the conjunction of ideas of solidarity in time and space, the possibility of strong solidarity ties in social life can be developed.
A saúde pública em tempos de burocratiza o: o caso do médico Noel Nutels

Abstract: Este artigo discute a trajetória do médico sanitarista Noel Nutels como um estudo de caso. Meu argumento central é que a história da saúde pública, a partir dos anos 1930, sobretudo durante o longo período getulista, é marcada por um processo de burocratiza o, cujo principal resultado é uma crescente dificuldade na implementa o das políticas públicas durante o período. Esta dificuldade indica um gradual conflito nas rela es entre aqueles diretamente responsáveis pela elabora o e implementa o das políticas públicas e a crescente burocracia estatal. O período histórico que se constitui a partir de 1930 assinala uma ampla divergência entre os padr es de comportamento dos 'homens de ciência' e dos 'homens de estado'. Se o período anterior à revolu o tem como marca uma certa sintonia entre esses dois segmentos sociais, a burocratiza o altera rápida e gradualmente o antigo equilíbrio, conquistado ainda no final do século XIX, e promove um deslocamento do papel de destaque que até ent o desfrutavam os agentes e autoridades da saúde pública no interior dos aparelhos do Estado.
