Sildenafil no tratamento da hipertens?o arterial pulmonar idiopática em crian?as e adolescentes

Abstract: objective: this study aims to provide data on the use of oral sildenafil in patients in new york heart association functional class iii or iv with severe idiopathic pulmonary arterial hypertension unresponsive to conventional therapy. method: in this series, six patients with idiopathic pulmonary arterial hypertension were prospectively treated with 2 to 8 mg of oral sildenafil in four to six doses a day. all pa-tients were submitted to physical examination, electrocardiogram and echocardiogram, chest computed tomography, ventilation and pulmonary perfusion scintigraphy, coagulation studies, and tests for collagen vascular disease, acquired immune deficiency syndrome and schisto-somiasis in order to rule out secondary causes of pulmonary arterial hypertension. all patients underwent cardiac catheterization for vasoreactivity tests using nitric oxide, o2 at 100% and oral nifedipine, and a 6-minute walking test was performed in those patients who were con-sidered able to exercise. results: all patients achieved a good therapeutic response, with improvement by at least one functional class, and presented an increase in systemic arterial oxygen saturation. five patients showed a decrease in the pulmonary systolic pressure to systemic systolic pres-sure ratio and improvement in the six-minute walking test. no major side effects were ob-served at 4 to 36 months of follow-up. one patient had sudden death after sildenafil had been withdrawn by mistake. conclusions: these data suggest that sildenafil may be useful in the manage-ment of idiopathic pulmonary arterial hypertension. patients should be advised against the withdrawal of sidenafil without medical supervision.

Abstract: this study addresses patient-centered care and the importance of teaching and learning it in medical schools. changes in disease prevalence and available information about diseases and their treatments require shared patient and physician decision-making. the use of different scales for measuring patient-centeredness was reviewed, particularly the "patient-practitioner orientation scale" (ppos). higher ppos scores are associated with more patient-centered attitudes. the study presents data on the attitude with respect to patient-centered care collected from 738 students in six different years of the medical course of the federal university of minas gerais using the patient-practitioner orientation scale (ppos) and compares these data with scores obtained from american medical students. the ppos scores of the brazilian students were higher than those of the american students. however, although the caring scores were higher among brazilian students, their sharing scores tended to be lower.

Abstract: the aim of this article is to identify what are the characteristics of the firm that might influence the difficulty of meeting technical requirements. it was conducted a survey with 117 firms and the data were analyzed using multivariate classification trees. it was verified that the most influential characteristic on the difficulty to meet technical requirements is the type of export product and the firms that export "fresh and processed meat and fish" are those that realize the greatest difficulty. the size of the firm, the adoption of international standards and exporting experience also influence the difficulty of adjustment. micro and small firms that adopt international standards have more difficulties to adjust their products and process to technical requirements than larger firms. the origin of the capital is not relevant to explain the difficulty of adjustment.
Case-control study of factors associated with chronic Chagas heart disease in patients over 50 years of age

Abstract: a case-control study on chronic chagas heart disease (cchd) was carried out between 1997 and 2005. ninety patients over 50 years of age were examined for factors related to (cchd). fourty-six patients (51.1%) with chagas heart disease (anomalous ecg) were assigned to the case group and 44 (48.9%) were included in the control group as carriers of undetermined forms of chronic disease. social, demographic (age, gender, skin color, area of origin), epidemiological (permanence within an endemic zone, family history of chagas heart disease or sudden death, physical strain, alcoholism, and smoking), and clinical (systemic hypertension) variables were analyzed. the data set was assessed through single-variable and multivariate analysis. the two factors independently associated with heart disease were age - presence of heart disease being three times higher in patients over 60 years of age (odds ratio, or: 2.89; confidence interval of 95%: 1.09-7.61) - and family history of chagas heart disease (or: 2.833, ci 95%: 1.11-7.23). systemic hypertension and gender did not prove to hold any association with heart disease, as neither did skin color, but this variable showed low statistical power due to reduced sample size.

Abstract: the objective of this study was to evaluate the performance of castor bean 'iac 80' grown in double rows spaced at 4.0 x (1.0 x 1.0 m) in no-tillage in the white oat straw, intercropped with food crops. the cereal was used for grain production (1375 kg ha-1) and soil coverage (1593 kg ha-1 ms; cycling of 35 kg k ha-1). the associated cultivation was double seeded between the lines of castor beans (ma): ma + corn, beans + ma, ma + beans and maize corn ma + pumpkin. the cv. iac 80, though susceptible to gray mold, produced 1060 kg ha-1 grain; corn ears were harvested with 22.542 ha-1 (5.333 kg ha-1), meaning an input of 3.15 t ha-1 ms (39 kg n ha-1, 58 kg k ha-1). pumpkin and bean crops did not yield commercial crops. the system containing oats, castor beans, and corn yielded satisfactory and contributed to the production of bioenergy with food security and environmental conservation.

Abstract: the objective of this work was to estimate the ideal number of trichogramma pretiosum riley to be released in field for the tomato leafminer control tuta absoluta (meyrick). an experiment was carried out in a greenhouse, in which tomato plants with 60 days, after the transplantation, were infested with 200 eggs of anagasta kuehniella (zeller), an alternative host, in the subsequent ratios: 70% of eggs were put on the top of the plants, 24% on the medium height and 6% on the very bottom of the plants. females of t. pretiosum were released following the ratios of 1, 2, 4, 8, 16 and 32 parasitoids per host egg. independently of the three distinct plant parts (top, medium or bottom), the ratio of 16 parasitoids per host egg delivered the best results. this ratio value was closer to the ideal number for field releasing in commercial crop production of stalked tomato, aiming the tomato leafminer control.
Atitude do estudante de medicina a respeito da rela??o médico-paciente x modelo pedagógico

Abstract: this study focused on medical students' attitudes towards the physician-patient relationship, comparing two private brazilian medical schools, one with a traditional curriculum (tc) and the other with a problem-based learning (pbl) curriculum. attitudes were measured with the patient-practitioner orientation scale (ppos). 132 students from the pbl school and 142 from the tc school completed data collection. total ppos scores were 4.62 and 4.45 for the pbl and tc medical schools, respectively (p=0.002), meaning more patient-centered attitudes among the pbl medical students. among male students enrolled in the 10th semester, the total ppos score was significantly higher in the pbl school. in both schools, as compared to men, women showed more decision-sharing attitudes with patients. multivariate regression analysis showed that ppos results were only explained by the school variable. this study found more patient-centered attitudes in the pbl school, which could be attributed to the curriculum model, since this was the only basic difference between the two schools.

Abstract: three patients presented respiratory abnormalities following crotalus durissus snakebite. these abnormalities appeared in the first 48 h after the snake bite and consisted of dyspnea, tachypnea, use of accessory muscles of respiration (cases 1 and 2) and flaring of the nostrils (case 2). cases 1 and 2 developed acute respiratory failure. case 2, 24 h after the snakebite presented difficult breathing and periods of apnea. he was intubated in the emergency room and transferred to the intensive case unit where he arrive with spontaneous breathing. his respiratory pattern worsened and measurement of arterial ph and blood gases showed metabolic and respiratory acidosis with partial carbon dioxide pressure increasing from 40 to 50,3 mmhg compatible with acute ventilatory failure. both patients needed mechanical ventilation. weaning from the ventilator was accomplished after 33 days in case 1 and after 15 days in case 2. both patients also presented acute renal failure treated with peritoneal dialysis with full recovery of the renal function. measurements of forced vital capacity (fvc) and forced expiratory volume in the first second (fev 1.0) was carried out 58 hours after the snakebite in case 3. both fvc and fev 1.0 were reduced in relation to the predicted values (60 and 67% respectively) but the ratio fev 1.0/fvc was in the normal range. these findings were compatible with a restrictive pattern of ventilatory failure. serial measurements showed progressive increase of both fvc and fev 1.0 reaching 72 and 79% of the predicted values, respectively, in the 10 th day after the snakebite. case 1 developed deep coma attributed to cerebral hypoxia suffered during several episodes of seizure and died of pulmonary infection 10 months after the snakebite. cases 2 and 3 recovered completely. these respiratory abnormalities were attributed to the action of crotoxin in the respiratory muscles since the patients did not present complications such as severe uremia, atelectasis, pulm

Abstract: O objetivo desse trabalho foi avaliar a influência de dois níveis de aduba o nitrogenada em cobertura, associados a duas densidades de semeadura, sobre características agron micas e a severidade de doen as foliares em híbridos comerciais de milho recomendados para regi o Centro-sul do Paraná. O experimento foi conduzido no campo experimental do Departamento de Agronomia da Universidade Estadual do Centro-Oeste, em Guarapuava-PR. O delineamento utilizado foi de blocos ao acaso, com três repeti es, em esquema fatorial 4x2x2, correspondente a 4 híbridos de milho (P30P34, P30R50, FORMULA e NK7G27), 2 níveis de aduba es nitrogenada em cobertura (90 kg ha-1 e 120 kg ha-1) e duas densidades de plantas (65.000 e 75.000 plantas ha-1), totalizando 16 tratamentos. A maior produtividade foi obtida na densidade 65.000 plantas ha-1, com 120 kg ha-1 N. A aduba o nitrogenada propiciou maior peso de gr os para o híbrido P30R50 com 120 kg ha-1 de N. O híbrido P30R50 quando cultivado na densidade de 75.000 plantas ha-1, obteve menor área abaixo da curva do progresso da doen a (AACPD) para Puccinia sorghi.
When students become teachers: training period, research and teaching practice in the formation of indigenous teachers

Abstract: The main traineeship theme in the Indigenous Teachers Training Course at the Federal University of Amazonas is one of the most important challenges in the college training of indigenous teachers. Since indigenous university students are already teachers in their villages, current investigation endeavors to answer the following questions: What kind of activities will characterize this training and which will compose the teachers' formation? How may research and teaching practice be articulated so that efforts and results may contribute towards teacher training and indigenous schools? Current research focuses on the experience of the Mura Class which is in its final year and finishing its traineeship. Traineeship has been a space-time opportunity for reflection and activities on the school and on the teaching practice to highlight teachers' training, qualify their professional insertions and, consequently, the work of the indigenous school.
