Dilata??o mediada por fluxo no diagnóstico diferencial da síndrome de pré-eclampsia

Abstract: background: the preeclampsia syndrome is associated with endothelial dysfunction and the differential diagnosis between pure preeclampsia (pe) and superimposed preeclampsia (spe) can be only be attained 12 weeks after delivery. objective: to compare the assessment of endothelial function through flow-mediated dilatation in pregnant women with pure preeclampsia and superimposed preeclampsia. methods: the flow-mediated dilatation of the brachial artery was carried out according to the recommendations of the international brachial artery reactivity task force in pregnant women with preeclampsia syndrome. pe (n=14) and spe (n=13) were diagnosed in the postpartum period according to the definitions of the national high blood pressure education program working group on high blood pressure in pregnancy. results: the median of the flow-mediated dilatation (fmd) in spe (6.0%; 1.9-10.3) was decreased in comparison with the pe (13.6%;4.4-17.1), an apparently relevant difference , but not statistically significant (p = 0.08). the fmd < 10% was detected in 30.8% of the pe cases and in 69.2% of the spe cases (p = 0.057). significant differences could not be detected in the morphology of the uterine arteries between the pe and spe cases through the doppler spectrum. conclusion: the fmd of the brachial artery of patients with preeclampsia syndrome was not capable of differentiating between pe and spe. however, the data suggest that spe is associated with worse endothelial function i comparison to pe.
Preload and torque removal evaluation of three different abutment screws for single standing implant restorations

Abstract: several authors still consider the mechanical problems of fracture and component loosening as the main causes of failure of implant-supported restorations. the purpose of this in vitro study was to compare the preload of three types of screw for transmucosal abutment attachment used in single implant-supported prosthesis through strain gauge and removal torque measurements. three external hex fixtures were used, and each received a transmucosal abutment (cera one?), which was fixed to the implant with its respective screw: group a- gold screw, group b- titanium screw and group c- surface-treated titanium screw (ti-tite?). ten screws of each type were attached applying a 30.07±0.28 ncm torque force and maintained in position for 5 minutes. after this, the preload values were measured using strain gauges and a measurement cell. gold screws presented higher preload values (131.72±8.98 n), followed by surface-treated titanium screws (97.78±4.68 n) and titanium screws (37.03±5.69 n). anova (p<0.05) and tukey's test (p<0.05) were applied. statistically significant differences were found among the groups for both preload and removal torque values. in conclusion, gold screws may be indicated to achieve superior longevity of the abutment-implant connection and, consequently, prosthetic restoration due to greater preload values yielded.
Workplace stress in nursing workers from an emergency hospital: Job Stress Scale analysis

Abstract: this study identifies workplace stress according to the job stress scale and associates it with socio-demographic and occupational variables of nursing workers from an emergency hospital. this is a cross-sectional study and data were collected through a questionnaire applied to 388 nursing professionals. descriptive statistics were applied; univariate and multivariate analyses were performed. the results indicate there is a significant association with being a nursing technician or auxiliary, working in the position for more than 15 years, and having low social support, with 3.84, 2.25 and 4.79 times more chances of being placed in the ‘high strain job' quadrant. the study reveals that aspects related to the workplace should be monitored by competent agencies in order to improve the quality of life of nursing workers.
Cardiac tumours: clinical, echocardiographic and pathological features

Abstract: objective: the purpose of our paper was clinical and echocardiographic characterisation of patients with cardiac tumours and the evaluation of the surgery and tumour recurrence. method: the first part of research was a retrospective investigation to identify cardiac tumours identification and to collect data from patients' records of the messejana hospital and walter cantídio university hospital, from 1981 to 2001. in this period, 19 cases of cardiac tumours were discovered. the patients were submitted to clinical, electrocardiographic and echocardiographic re-evaluation from march to july 2002. results: congestive symptoms and thoracic pain were the most frequent symptoms. the location in which most of the tumours were identified was left atrium. among patients submitted to surgery, all of them had the tumour extracted from atrium. the most common histopathologic type was myxoma (78%). surgical mortality was 14%. tumour recurrence occurred in one patient. conclusion: cardiac tumours were more frequent in left atrium, benign tumours were more frequent than malignant tumours, and the most frequent histopathologic type was myxoma.

Abstract: in order for the adoption of advanced manufacturing technology (amt) to provide competitive advantages to the companies, it is necessary to select these technologies according to criteria that consider strategic aspects. the organizational characteristics of a company influence particularly the process of amt adoption, in such a way that it is necessary to develop recommendations for the review of the organizational project. the present work proposes a process for the generation of these recommendations, which through a methodology called 'successive refinements' produces a set of recommendations for the review of the organizational project, based on requirements established by the amt adoption process. these recommendations are a set of decisions that may be taken about the organizational project to adapt them to demands of the new technology to be introduced, in other words, the amt implementation.

Abstract: Para que a ado o de Tecnologia Avan ada da Manufatura (AMT) possa, de fato, proporcionar vantagens competitivas às empresas, faz-se necessário que essas tecnologias sejam selecionadas segundo critérios que considerem aspectos estratégicos. As características organizacionais de uma empresa influenciam sobremaneira o processo de ado o de AMT, de tal forma que se faz necessário desenvolver recomenda es para a revis o do projeto organizacional. O presente trabalho prop e um processo para a gera o de tais recomenda es, através de uma metodologia denominada de "refinamentos sucessivos". Como resultado deste processo de "refinamentos sucessivos" obtém-se um conjunto de recomenda es para a revis o do projeto organizacional, baseados em requisitos estabelecidos pelo processo de ado o de AMT. Essas recomenda es constituem um conjunto de decis es a serem tomadas acerca do projeto organizacional para adequá-lo às demandas de uma nova tecnologia a ser introduzida, ou seja, a implementa o de AMT. Abstract: interstitial lung diseases (ilds) are heterogeneous disorders, involving a large number of conditions, the approach to which continues to pose an enormous challenge for pulmonologists. the 2012 brazilian thoracic association ild guidelines were established in order to provide brazilian pulmonologists with an instrument that can facilitate the management of patients with ilds, standardizing the criteria used for the diagnosis of different conditions and offering guidance on the best treatment in various situations. the objective of this article was to briefly describe the highlights of those guidelines.

Abstract: the organisational competences that an enterprise uses in its operations potentially define its capacity for implementing its business strategy. the present paper proposes a process for the analysis of investment projects based on competences. the study is focused on small enterprises, and it is applied to capacity expansion projects. the developed approach is based on a theoretical framework refined through a round of interviews conducted with experienced professionals. three investment analysts and three academics that work in the strategic operations management and economic production management areas participated in the interviews. the developed process was tested in simulation studies using the results of three investment projects analyzed by a development bank in the period of march 2002 to september 2003. the cases are related to manufacturing, commerce, and service sectors. the result of this work is an investment analysis process that uses the enterprise competences concept to evaluate its capability to generate results. the process relates resources and competences to the adopted business strategy. the study nature is exploratory and, in this sense, it does not allow generalizations. it is necessary to increase the number of simulations and to test the process using different investment analysts. the major practical implication is related to the fact that a structured procedure was proposed for the analysis of investment projects for small companies due to the difficulty of obtaining accurate and reliable financial and economic information about those companies. this study contributes to the development of methodologies for the analysis of investment projects based on a strategic approach.

Abstract: understanding value creation mechanisms for producing goods and services occupies a special place in enterprises' agenda. the renaissance of resource based theory is a sign of this research intensification, which is focused on organizational resources, capabilities and the role of competences in operations systems value creation. this work develops an integration model based on the intellectual capital concept, to articulate resources, capabilities and competences. the model is built using the resources, capabilities, competences and intellectual capital variables definition, addressing them in their different levels of analysis. the structure of the integration model construction used is based on the skandia navigator model, taking into account their performance dimensions for developing this study. the result is a model that explains the integrative dynamics of resources, capabilities and competences for assessing value creation from the perspective of intellectual capital.

Abstract: organisations are developing an in-dept change process on how they design and implement their business strategies. a special attention is given to knowledge management, as they recognize the potential strategic value that this resource could have. this paper presents the results of a research project, which main goal is to identify and represent the positioning of a set of cooperative organizations related to the adoption of knowledge practices. the results of five case studies are presented; they are defined in a universe of twenty two cooperatives business units in parana south east region. the study presents the development and test of an analysis procedure named 'adoption matrix'. based on results, it is identified the knowledge management functions and practices presence, however, they are incomplete in the sense of a knowledge management model implementation.
