Revista Brasileira de Cirurgia Cardiovascular , 2009, DOI: 10.1590/S0102-76382009000200011
Abstract: objective: to assess, by scintillography, the effect of using bilateral internal thoracic arteries (bitas) - prepared by two different techniques - on the sternal perfusion. methods: 35 patients undergone coronary artery bypass grafting (cabg) were divided into two groups: group a (18) had both ita's dissected using skeletonization technique and group b (17) as pedicle preparation. there was no difference in the two groups relating gender, age and demographic characteristics. on the 7th postoperative day the patients underwent bone scintillography. the statistical analysis was performed using the student's t test with 95% significance. results: group a (skeletonized ita) showed higher perfusion (11.5%) of the sternum as a mean, than group b (pedicled ita) patients; however this was not statistically significant (p = 0.127). on the other hand, comparing the diabetic population, seven in each group, there was a marked 47.4% higher perfusion of the sternum in group a patients (skeletonized ita) comparing to group b (pedicled ita) and this difference reached statistical significance (p = 0.004). conclusion: 1- sternal perfusion is not affected significantly apart from the dissection technique used for both internal thoracic arteries in the general population when assessed by bone scintillography. 2 - in the diabetic subgroup, a significant preservation of the sternal perfusion was observed in patients undergone skeletonized dissection of the internal thoracic arteries. although these findings should be confirmed in a greater number of cases, diabetic patients should have the internal thoracic arteries dissected using skeletonization techinque.

Arquivos de Neuro-Psiquiatria , 2006,
Abstract: Cerebral infarctions of cardiac etiology are observed in around 20% of patients with ischemic stroke. Cerebral ischemia is the first clinical manifestation in 1/3 of cases of atrial myxomas. Although almost half of patients with atrial myxoma show changes at neurological exam, non-hemorrhagic cerebral infarction is seen in computed tomography in practically all cases. We present the case of a 40 year-old woman whose first clinical manifestation of atrial myxoma was an ischemic stroke. We point out to the possibility of silent cerebral infarction in atrial myxoma patients.

Abstract: Diabetes is now considered one of the main threats to human health in the 21st century and many researchers are dedicated to investigate the physiopathology of the disease, with further insights on the managements of its major complications. Since understanding the pathophysiology of the major complications of diabetes and their underlying processes is mandatory, experimental models of the disease may be useful as they allow the recognition of the early mechanisms involved in the long-term complications of diabetes. Peripheral nerve involvement is highly frequent in diabetes mellitus and it has been documented that one third of diabetic patients have peripheral neuropathy. The true prevalence is not known and reports vary from 10% to 90% in diabetic patients, depending on the criteria and methods used to define neuropathy. In this review, the most common experimental models of diabetes are presented and the pathological findings on major peripheral nerves are discussed. Also, the insights brought by morphometry to the diabetic neuropathy research are highlighted.

Brazilian Journal of Otorhinolaryngology , 2012, DOI: 10.1590/s1808-86942012000400009
Knowledge on the anatomy of the sphenopalatine artery (SPA) and its branches is fundamental for the success of the endoscopic treatment of posterior epistaxis. However, the complex anatomical variations seen in the irrigation of the nasal cavity poses a significant surgical challenge. OBJECTIVE: This paper aims to describe the endoscopic anatomy of the SPA in human cadavers. MATERIALS AND METHODS: This is a contemporary cross-sectional cohort study carried out between April 2010 and August 2011. The presence of the ethmoidal crest on the lamina perpendicular to the palatine bone and the location of the principal sphenopalatine foramen (PSF) and the accessory sphenopalatine foramen (ASF) were analyzed in 28 cadavers, and the branches emerging from the foramens were counted. RESULTS: Fifty-six nasal fossae were analyzed. The ethmoidal crest was present in 96% of the cases and was located anteriorly to the PSF in most cases. The PSF was located in the transition area between the middle and the superior meatus in all cases. The ASF was seen in 12 cases. Most nasal fossae (n = 12) presented a single bilateral arterial trunk emerging from the PSF. In other cases, three (n = 8) or two (n = 5) arterial trunks em

Abstract: it is widely described in the literature that diabetic patients present hearing impairment. despite the histological alterations of the internal ear structures in these patients as well as in experimental models of diabetes, to the best of our knowledge, an histological evaluation of the vestibulocochlear nerve have not been performed. in the present study, ultrastructural alterations are described and compared between a spinal nerves and a cranial nerve in rats with chronic induced diabetes. male wistar rats (n = 12), fed with standard diet from the animal care facility at 42 days of age were used. induced diabetic animals (n=6) were fasted for 12 hours prior to being injected intraperitoneally with streptozotocin (stz - 60mg/kg) in a single dose. control animals (n=6) received (0.01 mol/l citrate buffer, ph 4.5) vehicle alone. ten weeks after stz injection the animals were perfused intracardially with karnovsky solution. right and left vestibulocochlear nerves were dissected and histologically processed for epoxy resin embedding. samples were imaged with the transmission electron microscope. large myelinated fibers with morphological signs of axonal atrophy in the vestibulocochlear nerves were readily observed. these results suggest that chronic stz-induced diabetes in rats caused alterations in the myelinated fibers and schwann cells, compatible to the classic diabetes signs and symptoms. morphological alterations of the vestibulocochlear nerve in diabetes is described for the first time and contributes information for a better understanding of why there are changes in hearing observed in diabetic patients.
