Abstract: purpose: to determine the association between maternal complications and type of delivery in women with heart disease and to identify the possible clinical and obstetrical factors implicated in the determination of the route of delivery. methods: this was a retrospective and descriptive study of the medical records of pregnant women with heart disease admitted to a tertiary reference hospital in the municipality of fortaleza, ceará, from 2006 to 2007. the study population included all pregnant women with an antepartum diagnosis of heart disease admitted for delivery, while women who received a diagnosis of heart disease after delivery were excluded, regardless of age and gestational week. a semi-structured questionnaire regarding sociodemographic, clinical and obstetrical variables was used. a descriptive analysis was first performed based on simple frequencies and proportions of the sociodemographic variables. next, possible associations between clinical and obstetrical aspects and type of delivery were analyzed, with the verification of association between maternal complications and type of delivery. the fisher exact test was applied for this analysis, with the level of significance set at p<0.05. the collected data were processed and analyzed using the epi-infotm software version 6.04 (atlanta, usa). results: seventy-three pregnant women with heart disease were included in the study. interatrial communication was the condition most frequently observed among congenital diseases (11.0%) and mitral calcification among the acquired ones (24.6%). the proportion of cesarean deliveries was higher than the proportion of vaginal deliveries, except for women with acquired heart disease. an association was detected between type of heart disease and type of delivery (p=0.01). there were 13 cases of maternal complications (17.8%). among them, ten (76.9%) occurred during cesarean section and three during vaginal delivery. no association mas detected between maternal complicati

Abstract: The objective of this work was to evaluate the phosphorous requirements (Pd) when monoammonium phosphate (MAP) is used as a phosphorous (P) source for broilers in the starter phase (1 to 21 days). We used 800 male chicks at one day old, in a 5x2 factorial arrangement, with five Pd levels (0.28, 0.34, 0.40, 0.46 and 0.52%) and two Ca levels (1.00 and 0.60), totaling ten treatments and eight replicates. Significant interaction was observed between the phosphorus and calcium levels from the diet, in the performances traits, bone ashes and P retention and excretion. Utilizing the level of 1.0% Ca in the diet, the best results of performance and bone ashes were founded when the levels of 0.42% and 0.439% Pd were used, respectively. Better contents of bone ashes were observed with the use of 0.60% Ca in the diet only when the lowest level of P was used. When the MAP was utilized as a source of P in the diet for broilers in the starter phase, the use of the 0.43% of Pd and 1.00% of Ca is enough to promote better performance and bone mineralization.

Abstract: the enzymatic extraction of proteins from a commercial rice flour was studied in this work. in order to increase the protein extraction yield (pey), the following parameters were evaluated: enzyme type (alkaline and neutral protease); temperature (40, 50 and 60 °c); ph (9.5, 10.5 and 11.0); physical treatment of the sample (no treatment; ultra-turrax at 16.000 rpm and ultrasound at 120 w, both for 5, 10 and 15 minutes); enzyme:substrate ratio (e:s) of 5:100 and 10:100 and initial concentration of raw material (1:3, 1:5 and 1:10 w/v). the pey was calculated using the protein contents of rice flour and the extraction residues. the results showed that the best condition for protein extraction, which gave the highest pey (63.4%), was that using an initial concentration of raw material of 1:10 (w/v), no physical treatment, ph 10.5, the alkaline protease, an e:s of 10:100, at 50 °c.

Abstract: with the aim of obtaining wheat flour hydrolysates with low-phenylalanine (phe) content, protein extracts were prepared using an enzymatic method through the action of a protease from bacillus licheniformis. next, these protein extracts were hydrolyzed by a crude enzymatic extract (cee) obtained from pineapple peel followed by a commercial enzyme (pancreatin). some parameters, such as temperature (30, 35, 40, 50 and 70 °c), time (1 hour and 30 minutes, 2 hours and 30 minutes, 3 hours and 30 minutes), and ph of the reaction (6.0, 7.0, 8.0 and 9.0) were evaluated. activated carbon (ac) was used for removing phe and the efficiency of this process was evaluated by second derivative spectrophotometry, measuring phe content in wheat flour as well as in its hydrolysates after ac treatment. varied effects were observed for the three parameters studied, and the best results were found using successive association of cee (e: s 10:100, 1 hour and 30 minutes) with pancreatin (e:s 4:100, 3 hour and 30 minutes) ph 7.0, at 50°c having reached a final phe content of 522.44 mg.100 g-1 of hydrolysate.

Abstract: OBJETIVO: Avaliar os efeitos de um protocolo de mobiliza o precoce na musculatura periférica e respiratória de pacientes críticos. MéTODOS: Ensaio clínico, controlado e randomizado realizado em 59 pacientes de ambos os gêneros, em ventila o mecanica. Os pacientes foram divididos em grupo fisioterapia convencional - grupo controle, n=14, que realizou a fisioterapia do setor, e grupo mobiliza o precoce, n=14, que recebeu um protocolo sistemático de mobiliza o precoce. A for a muscular periférica foi avaliada por meio do Medical Research Council e a for a muscular respiratória (dada por press o inspiratória máxima e press o expiratória máxima) foi mensurada pelo manovacu metro com uma válvula unidirecional. A mobiliza o precoce sistemática foi realizada em cinco níveis. RESULTADOS: Para os valores de press o inspiratória máxima e do Medical Research Council, foram encontrados ganhos significativos no grupo mobiliza o precoce. Entretanto, a press o expiratória máxima e o tempo de ventila o mecanica (dias), tempo de internamento na unidade de terapia intensiva (dias), e tempo de internamento hospitalar (dias) n o apresentaram significancia estatística. CONCLUS O: Houve ganho da for a muscular inspiratória e periférica para a popula o estudada quando submetida a um protocolo de mobiliza o precoce e sistematizado. OBJECTIVE:To evaluate the effects of an early mobilization protocol on respiratory and peripheral muscles in critically ill patients. METHODS: A randomized controlled clinical trial was conducted with 59 male and female patients on mechanical ventilation. The patients were divided into a conventional physical therapy group (control group, n=14) that received the sector's standard physical therapy program and an early mobilization group (n=14) that received a systematic early mobilization protocol. Peripheral muscle strength was assessed with the Medical Research Council score, and respiratory muscle strength (determined by the maximal inspiratory and expiratory pressures) was measured using a vacuum manometer with a unidirectional valve. Systematic early mobilization was performed on five levels. RESULTS: Significant increases were observed for values for maximal inspiratory pressure and the Medical Research Council score in the early mobilization group. However, no statistically significant improvement was observed for maximal expiratory pressure or MV duration (days), length of stay in the intensive care unit (days), and length of hospital stay (days). Abstract: objective: to describe the effects of chest physiotherapy in preventing respiratory complications in a child with acquired immunodeficiency syndrome and a giant pneumatocele. case description: an eight-year-old female child was sent to the physiotherapy service of a university hospital due to pulmonary hypersecretion, repeated pneumonias and presence of a giant pneumatocele. the physiotherapy team selected active airway clearence techniques and expiratory flow techniques to promote pulmonary hygiene and to reduce risks for respiratory complications. the follow-up around was accomplished by a multiprofissional team with the objective of avoiding recurrent pulmonary infections, pneumatocele's rupture and need for long-term hospitalization. the child's pulmonary condition remained stable and no changes in the size of the pneumatocele were seen in repeated x-rays. comments: the role of chest physiotherapy in patients with giant pneumatoceles is not well defined in the literature. there is no evidence of beneficial effects of this therapy and there are no guidelines that address the most efficient and safest techniques. currently, much is discussed about the risks and complications arising from chest physiotherapy. in this case, regular physiotherapy intervention was important to maintain child's pulmonary integrity, but studies are needed to evaluate techiques and clinical relevant effects.

Abstract: this study aimed to assess the nurses' perception on the nursing care provided to women facing the diagnosis of fetal death. research with qualitative approach carried out in a maternity school in fortaleza-ce , brazil, from september to october 2009. the sample was composed of nine nurses who worked in the service of high-risk pregnancy.the results showed that the focus of assistance offered was the provision of psychological support, through the permission given to women or family members to see the dead fetus. the difficulties most frequently mentioned were associated with the structural issue of motherhood that did not have a specific ward for mothers with a diagnosis of fetal death. they also mentioned the heavy workload, once they had functions that were not part of the procedures of praxis. thus, we emphasize the need for a different assistance to women facing fetal death covering, in addition to human resources, structural issues of health services.

Abstract: paracoccidioidomycosis is a systemic disease caused by inhalation of conidia of paracoccidioides brasiliensis, a dimorphic fungus that initially affects the airway, spreading by lymphatic and hematogenous routes to various organs and systems, and that can be fatal if the diagnosis and treatment are not correct. the authors describe a patient who presents small vessel vasculitis as an initial manifestation of the disease, which is an atypical clinical form with a single description in the literature.

Abstract: backgroud: chronic liver diseases in childhood often cause undernutrition and growth failure. to our knowledge, growth parameters in infants with neonatal cholestasis are not available aim: to evaluate the nutritional status and growth pattern in infants with intrahepatic cholestasis and extrahepatic cholestasis. patients and methods: one hundred forty-four patients with neonatal cholestasis were followed up at the pediatric gastroenterology service of the teaching hospital, state university of campinas, campinas, sp, brazil, in a 23-year period, from 1980 to 2003. the records of these patients were reviewed and patients were classified into two groups, according to their anatomical diagnosis: patients with intrahepatic cholestasis - group 1, and patients with extrahepatic cholestasis - group 2. records of weight and height measurements were collected at 4 age stages of growth, in the first year of life: 1) from the time of the first medical visit to the age of 4 months (t1); 2) from the 5th to the 7th month (t2); 3) from the 8th to the 10th month (t3); and 4) from the 11th to the 13th month (t4). the weight-by-age and height-by-age z-scores were calculated for each patient at each stage. in order for the patient to be included in the study it was necessary to have the weight and/or height measurements at the 4 stages. analyses of variance and tukey's tests were used for statistical analysis. repeated measurement analyses of variance of the weight-by-age z-score were performed in a 60-patient sample, including 29 patients from group 1 and 31 patients from group 2. the height-by-age data of 33 patients were recorded, 15 from group 1 and 18 from group 2 results: the mean weight-by-age z-scores of group 1 patients at the 4 age stages were: t1=-1.54; t2=-1.40; t3=-0.94; t4=-0.78. there was a significant difference between t2 x t3 and t1 x t4. the weight-by-age z-scores for group 2 patients were :t1=-1.04; t2=-1.67; t3=-1.93 and t4=-1.77, with a significant difference be

Abstract: the aim of this study was to evaluate the subcutaneous biocompatibility of: epiphany, ah plus, pulp canal sealer and sealapex root canal sealers. sixty rats were randomly assigned to 4 groups, according to the sealer. polyethylene tubes containing the tested materials were inserted into the connective tissue. the implants were removed after 7, 15 and 30 days, and the tissue samples were processed, stained and examined by light microscopy. the descriptive analysis considered: thickness of the fibrous capsule, severity of the inflammatory reaction, and presence of giant cells. after 7 days, all sealers induced moderate to severe inflammatory reaction. after 15 days, epiphany and ah plus sealers showed a moderate inflammatory reaction, while pulp canal sealer and sealapex induced severe and mild inflammatory reactions, respectively. after 30 days, mild inflammatory reactions were observed for epiphany, sealapex and ah plus. sealapex induced the lowest inflammatory response at all evaluation periods, and only pulp canal sealer did not show a decreased in the inflammatory reaction over time.
