Compara??o entre comissurotomia e valvoplastia por cateter-bal?o na estenose mitral: cinco anos de acompanhamento

Abstract: objective: to compare clinical and laboratory data in patients with mitral stenosis undergoing open commissurotomy or balloon valvuloplasty, who were followed up for 5 years. methods: eighty-one patients were prospectively assessed prior to the procedure (pre) and immediately after the procedure, in the immediate postoperative period (ipo), and followed up yearly for 5 years (po12m, po24m, po36m, po48m, and po60m). they were randomized into the following 2 groups: gc (group undergoing open commissurotomy): 37 patients (32.4 ± 7.2 years; 89.2% females); and gv (group undergoing balloon valvuloplasty): 44 patients (32.9 ± 9.5 years; 90.9% females). the patients' assessment comprised the following items: functional class, occurrence of events, electrocardiography, and doppler echocardiography. results: a significant improvement in functional class occurred in most patients. three patients in gc and in gv were in functional class iii in po60m. no difference in the mitral gradient was observed between the groups. a difference in the mean mitral valve areas was observed between the groups during the entire evaluation. no patients died. in regard to the ipo of gc, 3 patients had moderate mitral insufficiency (mi), and 3 had bleeding (1 was reoperated upon). in the ipo of gv, 4 patients had moderate mi, 1 had severe mi, 2 had cardiac tamponade, and 1 patient required surgery due to severe mi. over 60 months, 9 gv patients evolved to moderate or severe mi, while 6 gc patients evolved to moderate or severe mi, and 2 other gc patients required surgery due to double mitral dysfunction. conclusion: the rate of success in open mitral commissurotomy and balloon mitral valvuloplasty was 100%, and the rate of complications was low. during follow-up, a mild elevation in mitral gradient and a drop in mitral valve area were observed in both groups.

Abstract: OBJETIVO: Comparar resultados imediatos e após 12 meses de seguimento entre valvoplastia por bal o e comissurotomia na estenose mitral. MéTODOS: Oitenta e oito portadores de estenose mitral sintomáticos com anatomia favorável foram randomizados nos dois grupos. Todos os pacientes submeteram-se a avalia o clínica e Doppler ecocardiográfica antes, imediatamente após e 12 meses depois do procedimento. RESULTADOS: Gradiente médio mitral (mmHg) diminuiu (p<0,001) de 12,18±5,84 para 5,80±2,66 no grupo comissurotomia (GC) e de 11,66±6,13 para 4,98±2,40 (p<0,001) no grupo valvuloplastia (GV). área valvar mitral (cm2) aumentou de 0,98±0,21 para 2,52±0,46 no GC e de 1,05±0,25 para 2,18±0,40 no GV (p<0,001). Em ambos os grupos ocorreu uma diminui o discreta na área valvar mitral ao final do acompanhamento. N o ocorreram óbitos. Um paciente do GV apresentou insuficiência mitral significativa e necessitou tratamento cirúrgico. Aos 12 meses de acompanhamento todos os pacientes do GC e 97,7% dos pacientes do GV estavam em classe funcional I ou II (NYHA). CONCLUS O: Os procedimentos foram seguros e mostraram melhora semelhante no gradiente mitral e classe funcional. A área valvar mitral aumentou expressivamente mais no GC, e apresentou, também, uma redu o mais significativa após 12 meses. A maioria dos pacientes n o alterou a classe funcional no acompanhamento.

Abstract: this research work shows the impact of bauxite residue (br) calcination on the rheological and hardened properties of portland cement suspensions. these suspensions were evaluated with water-to-cement ratio of 0.5 and variations of residue content from 5% to 20% in partial substitution of cement. both properties had influence of br content, but had a negligible influence of calcination. therefore, it can be inferred that the utilization of br, in nature or calcinated, in formulations with portland cement, may reduce the consumption of cement, being an alternative to use large amounts of this type of residue.
Perianal squamous cell carcinoma with high-grade anal intraepithelial neoplasia in an HIV-positive patient using highly active antiretroviral therapy: case report

Abstract: context: highly active antiretroviral therapy (haart) has turned human immunodeficiency virus (hiv) infection into a chronic condition, and this has led to increased incidence of anal dysplasia among hiv-positive patients. routine anal evaluation including the anal canal and perianal area is recommended for this population, especially for patients infected by oncogenic human papillomavirus (hpv) types. case report: a 54-year-old homosexual hiv-positive man presented with a six-year history of recurrent perianal and anal warts. he had previously undergone incomplete surgical excision and fulguration in another institution on two occasions. he had been using haart over the past two years. he presented some condylomatous spreading lesions occupying part of the anal canal and the perianal skin, and also a well-demarcated slightly painful perianal plaque of dimensions 1.0 x 1.0 cm. both anal canal pap smears and biopsies guided by high-resolution anoscopy revealed high-grade squamous intraepithelial lesion. biopsies of the border of the perianal plaque also revealed high-grade squamous intraepithelial lesion. hpv dna testing of the anus detected the presence of hpv-16 type. the patient underwent local full-thickness excision of the lesion. histological analysis on the excised tissue revealed high-grade squamous intraepithelial lesion with one focus of microinvasive squamous cell cancer measuring 1 mm. no lymph vessel or perineural invasion was detected. the patient showed pathological evidence of recurrent anal and perianal high-grade squamous intraepithelial lesions at the sixth-month follow-up and required further ablation of those lesions. however no invasive squamous cell carcinoma recurrence has been detected so far.

Abstract: bone marrow has been proposed as a potential source of stem cells for regenerative medicine. in the eye, degeneration of neural cells in the retina is a hallmark of such widespread ocular diseases as age-related macular degeneration (amd) and retinitis pigmentosa. bone marrow is an ideal tissue for studying stem cells mainly because of its accessibility. furthermore, there are a number of well-defined mouse models and cell surface markers that allow effective study of hematopoiesis in healthy and injured mice. because of these characteristics and the experience of bone marrow transplantation in the treatment of hematological disease such as leukemia, bone marrow-derived stem cells have also become a major tool in regenerative medicine. those cells may be able to restore the retina function through different mechanisms: a) cellular differentiation, b) paracrine effect, and c) retinal pigment epithelium repair. in this review, we described these possible mechanisms of recovery of retinal function with the use of cell therapy with bone marrow-derived stem cells.

Abstract: purpose: to compare immediate and late (12 months) follow up of clinical and doppler echocardiographic results between percutaneous mitral balloon valvuloplasty and open commissurotomy in a prospective and randomized trial. methods: eighty eight symptomatic patients with severe mitral stenosis and favorable anatomy were randomized in a prospective trial comparing the two procedures. all patients were submitted to clinical and doppler echocardiographic evaluation before the procedures and immediate and twelve months thereafter. results: mean mitral gradient (mmhg) decreased from 12.2±5.8 to 5.80±2.7 (p<0.001) in commissurotomy group (cg) and from 11.7±6.1 to 5.0±2.4 (p<0.001) in the balloon valvuloplasty group (vg). mitral valve area (cm2) increased from 0.98±0.21 to 2.52±0.46 in cg and from 1.05±0.25 to 2.18±0.40 in vg (p<0.001). in both groups there was a slight decrease in mitral valve area at 12 month follow-up. there was no death in either group. one patient in the vg had moderate mitral regurgitation and underwent surgery. at the 12 month follow-up, all patients in cg and 97.7% of patients in vg were in new york heart association functional class i or ii. conclusion: both procedures were safe and showed similar immediate improvement in mitral gradient and functional class. mitral valve area had a greater increase immediately after commissurotomy, however there was a significantly greater reduction in the cg after 12 months of follow-up, when compared to balloon valvuloplasty. in both groups, mitral gradient remained reduced and most patients did not change functional class during the follow-up.

Abstract: globalization and international trade are having an increasingly evident impact on the day-to-day duties of the health sector, and the phenomenon has aroused a great deal of interest among governments, nongovernmental organizations, international organizations, and the mass media. up to this point the heated and polemical debate on the subject has seriously hindered objective discourse on the health implications of globalization and international trade. this piece examines the possible impact of the two processes on health in the region of the americas, in order to foster policies for equity that are adopted within the framework of public health in the americas. the piece considers the relationships among globalization, trade, and health in general and then focuses on the special case of trade in health goods and services. the piece looks at the possible impact on health equity of the agreements for integration and free trade that are being negotiated in the americas. the piece concludes with a summary of the activities that the pan american health organization has been carrying out in this area.

The piece considers the relationships among globalization, trade, and health in general and then focuses on the special case of trade in health goods and services. The piece looks at the possible impact on health equity of the agreements for integration and free trade that are being negotiated in the Americas. The piece concludes with a summary of the activities that the Pan American Health Organization has been carrying out in this area.
Inhibiting AKT Phosphorylation Employing Non-Cytotoxic Anthraquinones Ameliorates TH2 Mediated Allergic Airways Disease and Rhinovirus Exacerbation

Abstract: Background Severe asthma is associated with T helper (TH) 2 and 17 cell activation, airway neutrophilia and phosphoinositide-3-kinase (PI3K) activation. Asthma exacerbations are commonly caused by rhinovirus (RV) and also associated with PI3K-driven inflammation. Anthraquinone derivatives have been shown to reduce PI3K-mediated AKT phosphorylation in-vitro. Objective To determine the anti-inflammatory potential of anthraquinones in-vivo. Methods BALB/c mice were sensitized and challenged with crude house dust mite extract to induce allergic airways disease and treated with mitoxantrone and a novel non-cytotoxic anthraquinone derivative. Allergic mice were also infected with RV1B to induce an exacerbation. Results Anthraquinone treatment reduced AKT phosphorylation, hypoxia-inducible factor-1α and vascular endothelial growth factor expression, and ameliorated allergen- and RV-induced airways hyprereactivity, neutrophilic and eosinophilic inflammation, cytokine/chemokine expression, mucus hypersecretion, and expression of TH2 proteins in the airways. Anthraquinones also boosted type 1 interferon responses and limited RV replication in the lung. Conclusion Non-cytotoxic anthraquinone derivatives may be of therapeutic benefit for the treatment of severe and RV-induced asthma by blocking pro-inflammatory pathways regulated by PI3K/AKT.
