Candidemia in a brazilian tertiary hospital: microbiological and clinical features over a six-year period

Abstract: yeasts are becoming a common cause of nosocomial fungal infections in immunocompromised patients. such infections often develop into sepsis with high mortality rates. the aim of this study was to evaluate some of the numerous factors associated with the development of candidemia. medical records were retrospectively analyzed of 98 candida spp. patients. results showed that the most prevalent risk factors for developing candidemia were: antibiotics and antifungal agents (93.9% and 79.6%, respectively); the use of central venous catheter (93.9%); mechanical ventilation (73.5%); and parenteral nutrition (60.2%). the main species of candida found were: c. parapsilosis (37.76%), c. albicans (33.67%); and others (28.57%). c. glabrata showed the highest mortality rate (75%), followed by c. tropicalis (57.1%) and c. albicans (54.5%). the elevated mortality rate found in this study indicates that preventive measures against candidemia must be emphasized in hospitals.

Abstract: yeasts are becoming a common cause of nosocomial fungal infections that affect immunocompromised patients. such infections can evolve into sepsis, whose mortality rate is high. this study aimed to evaluate the viability of candida species identification by the automated system vitek-biomerieux (durham, usa). ninety-eight medical charts referencing the candida spp. samples available for the study were retrospectively analyzed. the system vitek-biomerieux with candida identification card is recommended for laboratory routine use and presents 80.6% agreement with the reference method. by separate analysis of species, 13.5% of c. parapsilosis samples differed from the reference method, while the vitek system wrongly identified them as c. tropicalis, c. lusitaneae or as candida albicans. c. glabrata presented a discrepancy of only one sample (25%), and was identified by vitek as c. parapsilosis. c. guilliermondii also differed in only one sample (33.3%), being identified as candida spp. all c. albicans, c. tropicalis and c. lusitaneae samples were identified correctly.
Vascular adaptive responses to physical exercise and to stress are affected differently by nandrolone administration

Abstract: androgenic anabolic steroid, physical exercise and stress induce cardiovascular adaptations including increased endothelial function. the present study investigated the effects of these conditions alone and in combination on the vascular responses of male wistar rats. exercise was started at 8 weeks of life (60-min swimming sessions 5 days per week for 8 weeks, while carrying a 5% body-weight load). one group received nandrolone (5 mg/kg, twice per week for 8 weeks, im). acute immobilization stress (2 h) was induced immediately before the experimental protocol. curves for noradrenaline were obtained for thoracic aorta, with and without endothelium from sedentary and trained rats, submitted or not to stress, treated or not with nandrolone. none of the procedures altered the vascular reactivity to noradrenaline in denuded aorta. in intact aorta, stress and exercise produced vascular adaptive responses characterized by endothelium-dependent hyporeactivity to noradrenaline. these conditions in combination did not potentiate the vascular adaptive response. exercise-induced vascular adaptive response was abolished by nandrolone. in contrast, the aortal reactivity to noradrenaline of sedentary rats and the vascular adaptive response to stress of sedentary and trained rats were not affected by nandrolone. maximum response for 7-10 rats/group (g): sedentary 3.8 ± 0.2 vs trained 3.0 ± 0.2*; sedentary/stress 2.7 ± 0.2 vs trained/stress 3.1 ± 0.1*; sedentary/nandrolone 3.6 ± 0.1 vs trained/nandrolone 3.8 ± 0.1; sedentary/stress/nandrolone 3.2 ± 0.1 vs trained/stress/nandrolone 2.5 ± 0.1*; *p < 0.05 compared to its respective control. stress and physical exercise determine similar vascular adaptive response involving distinct mechanisms as indicated by the observation that only the physical exercise-induced adaptive response was abolished by nandrolone.
Species distribution and susceptibility profile of Candida species in a Brazilian public tertiary hospital

Abstract: Candida albicans represented 33% of the isolates, Candida parapsilosis 31.1%, Candida tropicalis 17.9%,Candida glabrata 11.8%, and others species 6.2%. In blood culture, C. parapsilosis was the most frequently encountered species (48%). The resistance levels to the antifungal azoles were relatively low for the several species, except for C. tropicalis and C. glabrata. Amphotericin B resistance was observed in 1 isolate of C. parapsilosis.The species distribution and antifungal susceptibility herein observed presented several epidemiological features common to other tertiary hospitals in Latin American countries. It also exhibited some peculiarity, such as a very high frequency of C. parapsilosis both in bloodstream infections and dialysis-associated peritonitis. C. albicans also occurred in an important number of case infections, in all evaluated clinical sources. C. glabrata presented a high proportion of resistant isolates. The data emphasize the necessity to carry out the correct species identification accompanied by the susceptibility tests in all tertiary hospitals.Infections caused by opportunistic pathogens, such as yeasts, are becoming important causes of morbidity and mortality in many patients, because of alterations in the immune system and invasive hospital procedures [1]. Abstract: phenotypic and genotypic spm and imp metallo-β-lactamases (mbl) detection and also the determination of minimal inhibitory concentrations (mic) to imipenem, meropenem and ceftazidime were evaluated in 47 multidrug-resistant pseudomonas aeruginosa isolates from clinical specimens. polymerase chain reaction detected 14 positive samples to either blaspm or blaimp genes, while the best phenotypic assay (ceftazidime substrate and mercaptopropionic acid inhibitor) detected 13 of these samples. imipenem, meropenem and ceftazidime mics were higher for mbl positive compared to mbl negative isolates. we describe here the spm and imp mbl findings in clinical specimens of p. aeruginosa from the university hospital of botucatu medical school, s?o paulo, brazil, that reinforce local studies showing the high spreading of blaspm and blaimp genes among brazilian clinical isolates.

Abstract: this work is an application of a clinic strategy based on the alienation concept - as defined by jacques lacan - to handle a supposed psychotic child and her mother. this strategy, proposed by jussara falek brauer, prescribes individual assistance through weekly sessions with daughter and mother. it considers the family structure and intends to deal with the structural determinations on the subject constitution. two moments are especially focused - when the work begins and a later one - that exemplify the changing in the positioning of the child as a result from the therapeutic process in course. the theoretical concepts that support this approach - alienation, the lacanian topology about the subject constitution, significant, unconscious, castration, fetish, perversion, separation - are presented and related to the clinical material. some hypotheses are presented about the mother?s psychic structure, withdrawn from the changing of positions and the evolution observed in both mother and daughter. these lead to the establishment of a diagnostic hypothesis for the daughter.

Abstract: This paper has been retracted. The authors have discovered a possible problem in applications of the dephasing terminal approach to shot noise calculations in interference situations: Eq. (7) correctly re-introduces the anticorrelations between the currents in the two arms, but due to its semiclassical nature probably fails to describe the consequential diminishing of exchange effects at the second beamsplitter.

Abstract: We study quantum systems with broken symmetry that can be modelled as cyclic three-level atoms with coexisting one- and two-photon transitions. They can be selectively optically excited to any state. As an example, we show that left- and right-handed chiral molecules starting in the same initial states can evolve into different final states by a purely dynamic transfer process. That means, left- and right-handed molecules can be distinguished purely dynamically.
