EFECTO DE LA INFLAMACIóN SUBCLíNICA SOBRE EL ESTADO NUTRICIONAL DE HIERRO, COBRE Y ZINC EN ADULTOS EFFECT OF SUBCLINIC INFLAMMATION ON IRON, COPPER AND ZINC NUTRITION STATUS IN ADULTS

Abstract: Chile se encuentra en la etapa de transición epidemiológica, en la que coexisten las deficiencias de micronutrientes con las enfermedades crónicas no transmisibles. El objetivo de este estudio fue medir el efecto de infecciones leves sobre la prevalencia de la deficiencia de hierro, cobre y zinc en adultos. Participaron 377 adultos aparentemente sanos (20-55 a os), provenientes del área sur de la comuna de Pe alolén de Santiago de Chile. Se midió hemoglobina, VCM, protoporfirina libre eritrocitaria, ferritina sérica, niveles séricos de cobre y zinc, transaminasas, recuento de leucocitos y proteína C reactiva. Como resultado se obtuvo que solo el 3.9% de las mujeres y el 0,6% de los hombres presentaron anemia, todos los casos por déficit de hierro y un 20% de las mujeres mostraron depleción de depósitos de este mineral. El 8,6% de los hombres y el 5.9% de las mujeres tuvieron bajos niveles de cobre y en ambos grupos se encontraron altos porcentajes de deficiencia de zinc (25.1 % en mujeres y 24,7% en hombres). El estado de inflamación sub-clínica no alteró la prevalencia de anemia o el estado nutricional de hierro, cobre y zinc en esta población. Chile is undergoing an epidemiological transition, where micronutrient deficiencies coexist with chronic diseases. The aim of this study was to determine the effect ofmild infections on the prevalence ofiron, copper and zinc deflciencies in apparently healthy adults. A venous blood sample was obtained from 377 apparently healthy adults, 20-55 years of age,from the south area of Pe alolén (Santiago, Chile), to measure white blood cell count, MCV and hemoglobin, free erythrocyte protoporphyrin, serum ferritin, serum copper and zinc, transaminases, and C reactive protein concentrations. The prevalence of anemia was 3.9% in women and 0.6% in men. All cases of anemia were due to iron deficiency and 20% of women presented depleted iron stores. Furthermore, 8.6% of men and 5.9 % ofwomen had low serum copper levels and 25.1% ofwomen and 24.7 % of men had low serum zinc concentrations. The presence of a subclinical inflammation neither modified the prevalence of anemia or the status of iron, zinc and copper in this population.

Abstract: in order to design effective health promotion interventions, nutritional status and the stages of change related to the consumption of fruit and vegetables, physical activity, and weight control were determined in 955 students of both genders at the university of bio-bio, chile. the sample was randomly selected by campus, faculty, and career, with a level of confidence of 95% and a maximum error of 3%. beside the descriptive analysis, to evaluate the association among nutritional status, fruit and vegetables consumption, physical activity and weight control, chi2 test was applied. nutritional status was determined by body mass index and who reference standards for adults. a questionnaire previously validated by inta was applied to evaluate the stages of change. the prevalence of overweight and obesity reached 48.2% in men and 25.5% in women (p<0.0001). only 3.1% of men and 7.3% of women consumed the 5 recommended daily servings of fruit and vegetables. the majority consumed 1 or 2 daily servings (69.5% men and 54.3% women), and the rest 3 to 4 servings (18.2% and 34.7%, respectively) (p<0.0001). 58% of men and 43% of women carried out physical activity regularly (p<0.0001). 73.1% of men and 79.3% of women were trying to reduce or not to increase their weight (p<0.0002). it is concluded that students require specific interventions and appropriate conditions to increase their fruit and vegetables consumption, and physical activity.
Prevalencia de Caries en Alumnos de Educación Básica y su Asociación con el Estado Nutricional Prevalence of Dental Decay in Elementary School Children and Association with Nutritional Status

Abstract: Introducción: Se ha postulado que la obesidad constituiría un factor de riesgo para el desarrollo de caries en ni os, sin embargo, la literatura publicada muestra información discordante respecto de esta asociación. Objetivo: Conocer la prevalencia de caries en escolares de clase media baja de región metropolitana de Santiago, Chile y determinar su asociación con el estado nutricional de los mismos, entre los a os 2006 y 2007. Materiales y Métodos: Se ejecutó un estudio de corte transversal en 8 colegios de la Sociedad de Instrucción Primaria (SIP). Se seleccionó, mediante un muestreo aleatorio estratificado por género y curso, una muestra de 1190 escolares de 5 a 15 a os. Mediante examen bucal se consignó índice COPD. Se determinó el estado nutricional mediante la utilización de la referencia CDC 2000. Para los cálculos y estimaciones estadísticas se utilizó el programa Stata 9,0. Resultados: La prevalencia de caries en la población total fue de 79,5%. La prevalencia de caries en los ni os eutróficos, con sobrepeso y obesos fue de 80,0%, 78,1% y 79,9% respectivamente. Conclusión: La población evaluada presenta una alta prevalencia de caries, sobretodo en el grupo clasificado como normal. En esta muestra no se encontró una asociación estadísticamente significativa entre la prevalencia de caries y el estado nutricional. Introduction: It is thought that obesity is a risk factor in the development of dental decay in children. However, there is no agreement in the literature in this area. Objective: To define the prevalence of caries among lower class school children in the metropolitan region in Santiago, Chile, years 2006-2007, and observe its association with the children's nutritional status. Subjects and Methods: A random sample of children 5-15 years of age, in eight primary schools provided a stratified population of 1190 individuals. Oral examination provided a health index, nutritional status was determined using CDC 2000. Statistical analyses were performed utilizing Stata 9,0. Results: Prevalence of dental caries in the total population was 79,5%. Prevalence of decay among eutrophic, overweight and obese children was 80,0%, 78,1% y 79,9% respectively. Conclusion: This population presents a very high prevalence of dental caries, especially among a group classified a "normal". In this sample, there was no statistically significant association between dental decay and nutritional status.

Abstract: Background: There is concern about the consequences that hypertension has on the well being and mortality of elderly people. Aim: To compare the morbidity and mortality of young and old treated hypertensive subjects. Material and methods: Retrospective analysis of hypertensive subjects treated and followed for a period of 10 ± 9 years. Subjects that started treatment at an age of less than 65 years and those that started treatment after 65 years of age, were analyzed separately. The antihypertensive drugs used for treatment and blood pressure reduction were similar in both groups. Results: Subjects that started treatment after 65 years of age (157 females and 133 males) had lower serum cholesterol levels and a higher prevalence of end organ damage than their younger counterparts. Women over 65 had a higher body mass index than women of less than 65. Males over 65 years had a higher prevalence of diabetes than younger males. Mortality and complications of hypertension were higher in subjects that started treatment after 65 years of age. Conclusions: The higher mortality of subjects that start hypertensive treatment after 65 years of age, is indicative of the need to detect and treat hypertension at the earliest possible moment. Abstract: along the years, worldwide industrial development has causing a growing generation of residues, bringing potentials environmental problems. a study of the characteristics of these wastes, as well as the development of techniques for their use in new processes becomes indispensable for the environment preservation. the main purpose of this work is to evaluate the possible use of two important industrial residues from the amazon region for zeolite synthesis: (a) the fly ash (particle size < 100 μm) that comes from burning of mineral coal in boiler; and (b) the microsilica, a by-product of the reaction between quartz and coal in the production of metallic silicon and alloys iron-silicon.the following chemical, physical and mineralogical characterization methods were carried out: x-ray diffractometry, x-ray fluorescence, scanning electron microscopy, granulometric analysis, differential thermal and thermogravimetric analysis (dta-tg). the analyses were carried out at the following conditions: 60, 100, 150 and 190 °c, na2o/al2o3 molar ratio of 5 and si/al molar ratio ranging from 2.12 to 15, and reaction time of 24 h. the results of the fly characterization demonstrate its enormous potential as raw material for the zeolite synthesis. sio2 and al2o3 represent more than 50% of its composition, mineralogical phases defined, low humidity content, low particle size (d90 < 10 μm), among others. mineralogical analyses of the synthesized products showed the formation of some zeolite types, as follow: analcime, phillipsite, sodalite, zeolite p and tobermorite. the results show that the mixture fly ash-microsilica in these reaction conditions point to a promising material for zeolite synthesis.

Abstract: zeolite a and hydroxysodalite were synthesized after kaolin wastes from kaolin processing industries for paper coating, located in the amazon region. the waste is mainly composed by kaolinite, which has the same sio2/al2o3 ratio requested for those two zeolites. thereby, synthesis process may occur in two steps: 1) the kaolin was submitted to thermal treatment, through calcination process at 700 °c for 2 h, in order to obtain the metakaolinite phase. the metakaolinite obtained reacted with naoh solution, in hydrothermal conditions, using autoclaves, at the following temperatures: 80, 110 and 150 °c for 24 h. the starting materials were identified and characterized by means of x-ray diffraction (xrd) and scanning electron microscopy (sem) analysis and synthesis products were characterized by means of xrd, sem, thermal methods (dta-dtg) and fourier transformed infrared spectroscopy (ftir). a mixture of zeolite a and hydroxysodalite was obtained through this process at 150°c. in the temperature of 110 oc there was only the zeolite a formation. nevertheless, in the lowest synthesis temperature (80°c) there was no crystalline material formation for the established conditions.
Influencia de virus respiratorios, frío y contaminación aérea en la infección respiratoria aguda baja del lactante

Abstract: background: all winters, there is an increase in the number of pediatric consultations, associated to three factors: cold weather, air pollution and respiratory virus epidemics. aim: to study the influence of these three factors in the demand for pediatric consultations between march and september, in an area of metropolitan santiago. patients and methods: the number of consultations between march and september 1998 in the emergency room and the number of hospital discharges due to lower respiratory tract infections, were registered in a public pediatric hospital of santiago. a respiratory virus surveillance (respiratory syncytial virus, adenovirus, influenza and parainfluenza virus) was done among children admitted for lower respiratory infections. atmospheric temperature values and air pollution, measured as the number of particles of 10 μm or more per m3 (mp 10), were obtained from local health services. results: two elevation waves of outpatient consultations were detected at weeks 19 and 26, that coincided with the periods of maximal detection of influenza and syncytial respiratory virus, respectively. the epidemics of respiratory syncytial virus coincided with the maximal number of hospital admissions for lower respiratory tract infections at week 27. there was no correlation between air pollution and the number of pediatric consultations. the lower ambient temperatures coincided with the higher detection of respiratory syncytial virus at week 28, moment in which the demand for consultations or hospital admissions was descending. conclusions: there is a direct relationship between respiratory virus epidemics and the demand for pediatric consultations. there is also a minor influence of ambient temperature.
Trasplante alogénico de precursores hematopoyéticos en pacientes con síndrome de Wiskott-Aldrich

Abstract: background: wiskott-aldrich syndrome (was) is an x linked congenital disease that presents as eczema, thrombocytopenia and immune deficiency. the only curative procedure for this illness is hematopoietic stem cell transplant (hsct), preferably from a healthy hla identical sibling donor. cord blood is becoming an excellent alternative as stem cell source from unrelated donors. aim: to report our experience with hsct in children with was. patients and methods: six boys with was diagnosed at 1 to 6 months of age were transplanted at our institution. all of them developed eczema and thrombocytopenia. two had episodes of severe bleeding and three had repetitive infections (two with recurrent pulmonary infections and one a recurrent otitis). three patients had a positive family history. two received hsct from sibling donors and four from unrelated cord blood donors at 7 months to 4 years of age. results: ah 6 patients had full hematopoietic engraftment after transplantation. three had mild chronic graft-versus- host disease which responded to immune suppressive therapy. one patient died of cytomegalovirus related pneumonia 111 days after grafting. the other 5 patients are alive and healthy 11 to 104 months after transplantation. conclusions: hsct is an effective treatment for patients with was. the procedure should be done as soon as diagnosis is confirmed and before life threatening infections occur

Síndrome de hiper-IgM asociado a colangitis esclerosante y neoplasia vesicular: Caso clínico X-linked hyper-IGM syndrome associated to sclerosing cholangitis and gallbladder cancer

Abstract: We report a 11 years old male diagnosed as a X-linked hyper-IgM syndrome that presented with recurrent infections and sclerosing cholangitis and later developed a gallbladder cancer. Immunological evaluation showed decreased levels of serum IgG and IgA with elevated levels of IgM. Study of CD40 ligand expression on mitogen activated peripheral blood mononuclear cells revealed total absence of this marker on T lymphocytes. Molecular analysis detected, in the patient and his mother, a nonsense mutation in exon 1 of the transmembrane segment of the CD40 ligand. He also presented elevation of alkaline phosphatases and mild elevation of liver enzymes. Liver biopsy demonstrated the presence of idiopathic sclerosing cholangitis. The patient was started on monthly IVIG therapy at 400 mg/kg, as well as ursodeoxycholic acid and vitamin E, with normalization of his IgG and IgM levels a decrease in the incidence of infections and normalization of liver function. Three years after diagnosis, we detected the presence of polyps inside the gallbladder that were reported at biopsy as adenocarcinoma. He underwent hepatic bisegmentectomy (VI B-V) and local lymphadenectomy (Rev Méd Chile 2003; 131: 303-8).
