Ocular Findings of Cryptococcal Meningitis in HIV-AIDS Patients  [PDF]

Abstract:
Purpose: To describe the ophthalmological characteristics, clinical course and visual prognosis of HIV-AIDS patients with cryptococcal meningitis. Methods: Retrospective study conducted in HIV-AIDS diagnosed outpatients treated at Dos de Mayo Hospital, from 2004 to 2014. Descriptive statistics were obtained for age, gender, associated diseases, CD4 T cell counts, antiretroviral therapy (ART), cryptococcal meningitis symptoms, relapses and visual complaints data. Diagnosis was based in CSF analysis, Chinese dye and culture. CSF opening pressure was recorded. Descriptive statistics were performed. Results: 18 cases were studied. 16 men and 2 women. The range of age was 24 - 51 and median 33 years. Three patients had been treated with ART. Tuberculosis was the most frequent associated disease IN 5 cases (27.8%). Cryptoccocal meningitis relapses were present in 5 (27.8%). CD4 count below 50 was the most prevalent in 11 (61%). Thirteen had ocular symptoms, low visual acuity was present in the half of cases and diplopia in 2 (11.1%). 13 (72.2%) coursed with headache. In eleven cases, the CSF opening pressures had been reported, and the range was 18 - 350 mm H2O, mean 67-6. CSF culture was obtained in 11 patients, it was positive in 7 (38.9%). Indian ink was positive in 16 (88.9%). Conclusion: There was no blindness related with cryptococcal meningitis despite the higher values of CSF opening pressure reported.
Inorganic Chemical Composition of Swimming Pools in Amman-Jordan

Abstract: Monitoring was carried out during summer 2011 in three types of swimming pools in Amman-Jordan. Thirty six water samples, collected from three users type of swimming pools (adults, family and infants), were examined for its major ionic composition (HCO3-, Cl-, NO3-, SO4=, Ca+2, Mg+2, Na+, K+ and PO4+4) in addition to its BOD and COD content. All of the examined samples from the swimming pools water were acceptable according to the local and WHO standards. The type and number of users as well as the maintenance of the swimming pool water influenced the water quality. The results showed that there was a noticeable increase in NO3, PO4 and Cl than other ions. All of the examined samples from the swimming pools water were acceptable according to the local and WHO standards and although the water of the infant’s users changed frequently, it showed the highest concentration of most of the parameters and Adults’ pool showed the lowest.

Abstract: the article aims at discussing some aspects about how hegel and heidegger use the term "phenomenology". the author examines as a first step heidegger's reading of hegel's texts, and then as a second step indentifies the differences. therefore it becomes necessary to give a brief introduction about hegel's use of "phenomenology", in order to put it in contrast with heidegger's idea of phenomenology, especially starting from his first lectures in freiburg. in this way we will reveal some elements in order to distinguish the dialectic meaning and the her-meneutic meaning which the term "phenomenology" may bear.

Abstract: the narrative work of luis rafael sánchez starts in the 60s with the issue of en cuerpo de camisa, an anthology of short stories. later, he published two novels, la guaracha del macho camacho (1976) and la importancia de llamarse daniel santos (1989). in each work he departs from the totalizing criteria of puerto rican literature that was considered hegemonic until the 60s, and initiates a praxis that subverts the established order in its cultural field. the narrator?s creative project becomes realized with the construction of puerto rican and caribbean identity. in this corpus we have found discursive ruptures, the incorporation of subaltern genres and marginal characters, as well as a constant problematization of puerto rican literature from irony and humor. all of this leads the work of sánchez through a path of hybridization, the same path that configures puerto rican and caribbean identity.

Abstract: this article focuses on gender studies as an analytical theory that allows revealing the representations, ideas, behaviors that have been built socially as "natural" and usually thought of being related to biological diff erences between sexes. in the text, the patriarchal figure and those who break conventional patterns, known as subordinates, are opposed. the pink panther icon acts as an openness symbol that at the same time constitutes the antithesis of a supposed heterosexual supremacy. the text deals with the need of tolerating human diversity tolerance and at the same time calls into question the traditional order. the idea of "subordinatedness" embraces the peripheral status of latin american people relative to those from developed countries. also this reflection deals with how democratization fosters human pluralism, although diversity leads to tensions between characters that the novel describes as stereotyped. the unconventional character who embodies the narrator of efraim medina is the opposite of the pater familia, a sort of enemy of the catholic church who does not have any kind moral preoccupation.

Abstract: democracy, without doubt, is one of the concepts most used in works related to political theory. there is practically no speech, text or conversation associated to politics that doesn?t mention it. nevertheless, it is pertinent to establish its meaning in the present world, given the constant transformation of the political reality and, with it, the demand to review, modify and even create the concepts that we, as society, use to describe ourselves. though the bibliography on this matter is vast, it is not our intention here to exhaust all visions and arguments on this aspect, but only those on which the mexican reformatory elite leaned on to democratize the country.

Abstract: Este artículo centra su interés en los estudios de género como teoría analítica, que permite desvelar las representaciones, ideas, comportamientos, que se han ido construyendo socialmente como "naturales" y atribuidos a las diferencias biológicas de los sexos. Se opone en el texto la figura patriarcal a las de aquellos que rompen con los patrones convencionales, y que son definidos como subalternos. El ícono de la pantera rosa funge como símbolo de apertura a la vez que constituye la antítesis de una supuesta supremacía heterosexual. El texto aborda la necesidad de tolerar la diversidad humana al tiempo que cuestiona el orden tradicional. La noción de subalternidad engloba el origen periférico de los latinoamericanos en relación a los sujetos nacionales de países desarrollados. Asimismo, esta reflexión alude a la democratización que da cabida al pluralismo humano, no obstante la diversidad crea tensiones entre personajes que el texto literario describe como estereotipadas. El sujeto anticonvencional que personifica el narrador de Efraim Medina es el opuesto al pater familia, una especie de enemigo de la iglesia católica, que no tiene ningún tipo de preocupación moral. Abstract: El artículo se propone tematizar algunos aspectos en torno al uso del término "fenomenología" por parte de Hegel y Heidegger. Así, el autor revisa en primer lugar la lectura que Heidegger hizo de Hegel para, en segundo lugar, matizar las diferencias. En este sentido se hace necesaria una breve introducción al uso que Hegel hace de "fenomenología" para así contrastarlo con la idea de la fenomenología en Heidegger, especialmente a partir de sus primeras lecciones en Friburgo. Con ello se presentan algunos elementos para diferenciar el sentido dialéctico y el sentido hermenéutico que puede tener la "fenomenología".
Criptococosis meníngea: características clínicas y de laboratorio

Abstract: introduction. the incidence of infections caused by cryptococcus neoformans has increased over the past 20 years as a result of the human immunodeficiency virus epidemic and the raise of immunosuppressive therapies. a clinical and epidemiological study in colombia found a mean annual incidence rate of cryptococcosis in the general population of 2.4/1’000000 inhabitants, central nervous system cryptococcosis in 891 (95.7%) of the subjects, and hiv infection in 78.1% of the patients. objetive. to describe the demographics, clinic presentation and laboratory results of patients with a first episode of cryptococcal meningitis. the frequency of positive cerebrospinal fluid cultures after administration of 500 mg and 1 g of amphotericin b, respectively, was also assessed. materials y methods. case series study. the subjects were eighteen patients with first episode of cryptococcal meningitis confirmed by positive cerebrospinal fluid culture. demographic, clinical, laboratory and post treatment changes were described. results. 77.7% of the subjects were male. all the patients presented headache as the main symptom. hiv infection was found in 83.3%. initial cerebrospinal fluid analysis revealed an average of 24.8 white blood cells/ul, and average values for protein and glucose were 117mg/dl and 32.1 mg/dl, respectively. after treatment with 1 gram of amphotericin b, the average values cerebrospinal fluid values for white blood cells, protein and glucose were 20.2 ul, 85.7 mg/dl, and 42.3 mg/dl, respectively. the frequency of positive cryptococcus culture after administration of 500 mg of amphotericin b was 23%, and 0% after 1 gr. india ink stain remained positive after the treatment in 53.8% of subjects and the latex agglutination test was reactive in 84.6%. conclusions. almost all the subjects were found to have hiv infection and other bad prognostic features. the analysis of initial vs. post treatment cerebrospinal fluid analysis did not reveal statistically significant d

Abstract: introduction: psychotic syndromes are frequently seen in the course of epilepsy, either during the seizures or during the period following them. this is especially associated with poor symptomatic control, refractivity, and change or discontinuation of the anticonvulsive treatment. it has also been described after temporal lobectomy prescribed to control it. objective: to describe general aspects, classification, clinical features, and treatment of psychosis associated with epilepsy. method: relevant data research in several data bases. results and conclusions: epileptic psychosis produces greater morbidity in epileptic patients that might lead to psychiatric hospitalizations, lengthening the time of neurological internment and a more complex use of the pharmacological treatment. there are ictal, interictal and postictal psychosis, along with forced normalization and post-surgery psychosis, the latter two with a lower prevalence. epileptic psychosis might share some aspects of psychiatric disorders such as mania and schizophrenia. in this case, the clinician must make a better assessment of the findings in relation to the seizures to ensure an adequate diagnosis and treatment that generally requires the intervention of both the fields of psychiatry and neurology.
