Factores asociados a presentar parada respiratoria en las sobredosis por opiáceos atendidas en un escenario abierto de consumo de drogas en Barcelona

Abstract: background: to determine the factors associated with respiratory arrest in opiate overdoses (coma, pupillary miosis, respiratory depression, and response to naloxone) among injecting drug users in the can tunis quarter of barcelona. methods: we ran a transversal observational study where all overdoses assisted between march, 2001 and june, 2002. after overdose treatment, data were collected using a standard questionnaire, including: patients' sociodemographic data, opiate and other substances' use prior to overdose, clinical signs and symptoms presented, and medical intervention received, by ways of a standardised questionnaire. logistic regression was used as a tool for analysis. results: of 222 opiate overdose cases, 60.8% showed respiratory arrest. of all risk factors tested, only prior abstinence heroin abstinence for 2 weeks or longer days (or= 1.893; p=0.04), and no previous consumption of benzodiazepines (or:0,462; p=0,017), proved to have a statistically significant association with suffering a respiratory arrest. concomitant use of alcohol, cocaine or methadone appeared not associated with suffering respiratory arrest in opiate overdose. conclusions: the main risk factor for respiratory arrest in opiate overdoses was a prior abstinence period of more than 2 weeks. benzodiazepines use was associated with absence of respiratory arrest in overdose cases. alcohol or methadone use, as well as the use of larger quantities of heroin, was not associated with suffering respiratory arrest in opiate overdoses. a study of other factors, not included in this study, and that could interfere with our results, should be considered for their possible relationship to benzodiazepine use as well as to absence of respiratory arrest in overdose cases.

Abstract: The aim of this paper is to analyze the effect of the accessibility to employment offices on local unemployment rates according to the distribution of three different types of municipalities: large urban, small urban and non-urban. We built a new accessibility measure taking into account the number of employment offices together with the distance and size of their catchment area. We propose an empirical model with spatial regimes that allows including simultaneously spatial heterogeneity and spatial autocorrelation. The results suggest that the accessibility to employment offices is especially important in non-urban areas where employment opportunities are limited. Employment services are important because bridge the gap between unemployed workers and employers where job opportunities are unclear.

Abstract: metastatic tumors involve the oral cavity, and the most common primary sites are the breast and lung. most cases affect the mandible and maxilla in that order, although some of them can be located in the soft perioral tissues. we report the case of a 62-year-old male who had been diagnosed with sigmoid adenocarcinoma with nodal and liver metastasis, who presented 6 months later with a gingival polypoid tumor, at first considered as a primary neoplasm of gingiva, that was diagnosed in a biopsy as metastatic intestinal adenocarcinoma. the histological evaluation is essential to separate adenocarcinoma from the commoner in this site squamous cell carcinoma, and the immunohistochemical techniques are useful to distinguish metastatic tumor versus primary adenocarcinoma from the minor salivary glands of the area. the intraoral spread of a disseminated neoplasm is generally a sign of bad prognosis, although a longer survival can be expected if a radical surgical treatment of a solitary metastasis is carried out.
Cribaje de microfilariasis sanguínea (Loa Loa) en la población inmigrante de zonas endémicas

Abstract: background: given the increasing flux of immigrant population from high-risk areas to our country, the need of screening for loasis arises, despite systematic screening being a debated and poorly evaluated practice to wich there is no protocol. this study is aimed at identifying the population to which loasis screening would be most appropriate, by drawing a comparison among four alternatives. methods: case and control group study, involving 30 cases (all those who came to our unit for treatment and who tested positive for loa loa in a microfilariae in the blood detection test) and 90 control cases (three controls per case from among the subjects having tested negative for microfilariae in the blood chosen at random without any pairing criteria). results: of the 1,638 subjects on whom the microfilariae blood test was performed, 30 tested positive (1.8%; 1.2-2.6%). of these 30 cases of loasis, 76.7% (23;57.7-90.1%) had eosinophylia (or 8.8; 3.3-23.1; p<0.0001) and 30.0% (9;14.7-49.4%) compatible clinical symptoms (or 2.8; 1.0-7.5; p=0.04). if we were to apply the screening test to the entire immigrant population coming from endemic areas, we would have to perform 54.6 tests to detect one case. it we were to perform the test on patients showing eosinophylia and/or compatible clinical symptoms, we would have to perform a smaller number of tests for every case detected (nns=29; ic=21-48), but there would be 16.7% (5;5.7-34.7%) false negatives. conclusions: conducting a screening test with determination of microfilariae in the blood on the immigrant population coming from central and west africa, independently of the presence of eosinophylia or compatible clinical symptoms, would be indicated, provided that the necessary resources are available.
Safety of intramuscular influenza vaccine in patients receiving oral anticoagulation therapy: a single blinded multi-centre randomized controlled clinical trial

Abstract: Design: Randomised, controlled, single blinded, multi-centre clinical trial. Setting: 4 primary care practices in Barcelona, Spain. Participants: 229 patients on oral anticoagulation therapy eligible for influenza vaccine during the 2003–2004 season. Interventions: intramuscular administration of influenza vaccine in the experimental group (129 patients) compared to subcutaneous administration in the control group (100 patients). Primary outcome: change in the circumference of the arm at the site of injection at 24 hours. Secondary outcomes: appearance of local reactions and pain at 24 hours and at 10 days; change in INR (International Normalized Ratio) at 24 hours and at 10 days. Analysis was by intention to treat using the 95% confidence intervals of the proportions or mean differences.Baseline variables in the two groups were similar. No major side effects or major haemorrhage during the follow-up period were reported. No significant differences were observed in the primary outcome between the two groups. The appearance of local adverse reactions was more frequent in the subcutaneous administration group (37,4% vs. 17,4%, 95% confidence interval of the difference 8,2% to 31,8%).This study shows that the intramuscular administration route of influenza vaccine in patients on anticoagulant therapy does not have more side effects than the subcutaneous administration route.NCT00137579 at clinicaltrials.govInfluenza vaccines are recommended for administration by the intramuscular route [1]. Abstract: objective: to identify the components of the primary health care (phc) product defined by health professionals and users in order to establish indicators for evaluation. methods: qualitative methodology was used with group techniques: a nominal group (health professionals) and focus groups (users). the study was performed in phc centers in catalonia (spain). there were 7 groups: a) family physicians and pediatricians; b) nurses and social workers; c) staff from admissions units and customer services; d) other medical specialists; e) users; f) managers, pharmacists, pharmacologists, and technicians. participants responded to the question: ?which features should be evaluated in the services that should be provided by phc??. a content analysis was performed. textual data were broken down into units and then grouped into categories, following analogy criteria. the interpretative context of the research team was taken into account. results: health professionals and users identified 4 dimensions of the phc product, coinciding with its basic attributes: a) access to services; b) coordination and continuity of the phc teams with other levels of healthcare; c) relationship between health professionals and users, and d) scientific-technical quality of the phc teams and the portfolio of services. equity, satisfaction and efficiency appeared as keystones in all the components of the product identified. conclusion: there was broad agreement in the product definition among health professionals and users. the relationship between health professionals and patients was a key element in all groups. the four dimensions should be included in the evaluation of phc teams.

Abstract: this essay focuses on the andamios magazine, created in 1925 by the i asociación general de profesores, intended to disseminate the first expressions of the chilean avant-garde. the magazine, as an object of study, will be studied through the analysis of its layout, as well as its writing and visual content. this will allow an articulation, from a variety of angles, of the group's ideological and aesthetical proposal. this essay will describe and contextualize the contribution of andamios to the independent development of the chilean's artistic and literary field. it will also give account of the positioning of the avant-garde in chile in the twenties.

Abstract: The edition of handbooks adapted to the self-learning paradigm and using the advantages of the information and communication technologies (ICT) is basic in the educational innovation processes’ success that the Spanish university is involved. This article describes the characteristics that a good electronic book should have. It proposes a methodology for the development of electronic books, shows that this method can be successfully implemented, and presents an example of that: the e-book titled Programación matemática para la economía y la empresa. La elaboración de manuales adaptados al enfoque de ense anza-aprendizaje que hagan uso de las ventajas didácticas de las tecnologías de la información y de las comunicaciones es un requisito fundamental para el éxito del proceso de innovación educativa en el que está comprometida la universidad espa ola. Este artículo realiza una reflexión sobre las características que debe poseer un buen manual multimedia para cumplir con este reto, presenta un método para la elaboración de libros multimedia y demuestra que ese método se puede implementar con éxito presentando el libro multimedia “Programación matemática para la economía y la empresa”
