Breastfeeding Recommendations. Pediatricians Mexicans Survey  [PDF]

Objective: To know recommendations on breastfeeding by Mexican pedia-tricians. Methods: Cross-sectional, descriptive study conducted through a survey of Mexican pediatricians. Results: Two hundred and fifty-four pedi-atricians (range 27 to 72 years) were surveyed. Most of them worked in public hospital or private practice or both. Among them 94.5% recom-mended newborn early contact with mother and 54.3% rooming. Exclusive breastfeeding was suggested by 89.8%, out of them 51.9% for 6 months. Breastfeeding on demand was counseled by 88.2% and 84.6% without fixed schedules. Pacifier use was rejected by 86.6%. 51.2% used iron supplements and 59.5% used other vitamins. Stopping breastfeeding in special situations was suggested by 87.4%; of them, 86.6% in case of infection with AIDS; 74.4% if the mother used drugs; 74.4% by infection with hepatitis B virus; 49.6% in case of mastitis; 17.3% in newborns with jaundice; and 18.5% in other situations. Finally, 93.3% of pediatricians surveyed reported interest in breastfeeding; 54.7% obtained information in medical journals; 54.7% through medical courses, and 31.9% in books. 58.3% of respondents oriented on breastfeeding according to WHO recommendations; 31.5% did so regularly and 10.2% did not. Conclusions: Pediatricians play a major role in breastfeeding promotion, but their formation is scarce and they are not prepared to counsel patients. Governments of developing countries should promote breastfeeding to ensure the health and proper development of infants.

Abstract: laboratory tests are essential for the diagnosis and follow-up of autoimmune and neoplastic thyroid diseases. the most important test for the etiologic diagnosis of hashimoto's thyroiditis, a disease with high prevalence in the population, is the determination of anti-thyroperoxidase antibodies (a-tpo), which is also useful for the diagnosis of graves' disease, for predicting risk of hypothyroidism in sub-clinical hypothyroidism and for predicting postpartum thyroiditis in at-risk women. another very useful test in autoimmune diseases is the determination of anti-tsh receptor antibodies (trab) in the diagnosis of rare forms of graves' disease, as patients with few symptoms or multi-nodular goiters. in thyroid cancer, thyroglobulin is essential in the follow-up of patients with differentiated thyroid cancer (papillary and follicular), whereas calcitonin is necessary for the diagnosis and follow-up of medullary thyroid carcinoma.

Abstract: O laboratório é fundamental no diagnóstico das doen as auto-imunes e neoplásicas da tiróide. O teste mais importante para o diagnóstico etiológico da tiroidite de Hashimoto, doen a de alta prevalência, é a determina o do anticorpo anti-tiroperoxidase (A-TPO), dosagem que pode também ser útil no diagnóstico de doen a de Graves, do risco de hipotiroidismo em pacientes com hipotiroidismo sub-clínico e do risco de gestantes apresentarem tiroidite pós-parto. Outro teste útil nas doen as autoimunes é a determina o dos anticorpos anti-receptor de TSH (TRAb) naqueles casos de doen a de Graves pouco sintomáticos ou quando a mesma ocorre em pacientes com bócio multi-nodular. Nas doen as neoplásicas da tiróide, a tiroglobulina (Tg) é fundamental e de alta sensibilidade no seguimento dos pacientes com cancer diferenciado da tiróide (papilífero e folicular), enquanto que a dosagem de calcitonina é obrigatória para o diagnóstico e seguimento do cancer medular da tiróide.

Abstract: the author reviews the literature on the new assays for serum thyroglobulin (stg) presenting lower functional sensitivity and demonstrates that its use, whilst the patients are taking l-t4, presents better results than stg following tsh stimulation in the follow-up of patients with differentiated thyroid carcinoma. therefore, he suggests a revision on the guidelines for the follow-up of these patients (developed when the available assays present a sensitivity of 1 ng/ml), proposing the use of stg assays with functional sensitivity of 0.1-0.2 ng/ml with the patients on l-t4 treatment instead of stg stimulated by tsh.

Abstract: in this review we present practical aspects of thyroid molecular tumorigenesis. the most important issue is the molecular diagnosis of affected patients with ret mutations in families with medullary thyroid carcinoma and multiple endocrine neoplasia type 2. in addition, we discuss the molecular pathologenesis of differentiated thyroid carcinoma, like the data on ret/ptc rearrangements in papillary carcinoma and pax8-pparg1 rearrangement in follicular carcinoma. subsequently, we present gain of function mutations in thyroid nodules and finally we conclude discussing the use of messenger rna of thyroglobulin in the follow-up of patients with differentiated thyroid cancer.

Abstract: Esta revis o apresenta aspectos de utilidade prática da tumorigênese tiroideana. O mais importante é a possibilidade de se fazer o diagnóstico genético precoce dos indivíduos portadores de muta es do gene RET em famílias com carcinoma medular de tiróide e neoplasia endócrina múltipla tipo 2. Além disso, discutem-se dados relativos à patogênese molecular dos carcinomas diferenciados da tiróide, relacionados aos rearranjos entre RET e outros genes no caso do carcinoma papilífero (RET/PTC) e entre PAX-8 e PPARg1 no carcinoma folicular da tiróide. A seguir, analisam-se as muta es que causam ganho de fun o no receptor de TSH, causadoras dos nódulos aut nomos. Finalmente, apresenta-se o emprego do RNA mensageiro da tiroglobulina no seguimento de pacientes com cancer de tiróide.

Abstract: autoimmune thyroid disease (aitd) is the most common organ-specific autoimmune disorder affecting 2% to 5% of the population in western countries. clinical presentation varies from hyperthyroidism in graves' disease to hypothyroidism in hashimoto's thyroiditis. while the exact etiology of thyroid autoimmunity is not known, interaction between genetic susceptibility and environmental factors appears to be of fundamental importance to initiate the process of thyroid autoimmunity. it has been postulated that 79% of the susceptibility to develop aitd is attributed to genetic factors, while environmental factors contribute to 21%. the identified aitd susceptibility genes include immune-modulating genes, such as the major histocompatibility complex (mhc), cytotoxic t lymphocyte antigen-4 (ctla-4), cd40 molecule, and protein tyrosine phosphatase-22 (ptpn22), and thyroid-specific genes, including tsh receptor (tshr) and thyroglobulin (tg). the exact nature of the role environmental factors play in aitd is still not well known, but the involvement of several factors such as iodine diet content, stress, drugs and infections has been reported. however, there is no clear evidence of causality and the mechanisms by which environmental factors trigger thyroid autoimmunity in genetically predisposed individuals remain not fully understood. knowledge of the precise mechanisms of interaction between environmental factors and genes in inducing thyroid autoimmunity could result in the development of new strategies for prevention and treatment.

An intelligent and biodegradable material pack- aging was developed based on a natural and thermal-sensitive pigment. Anthocyanin (ATH, 0.50 g/100g) was incorporated into chitosan matrix films (2 g/100g) forming a chitosan intelligent film (C-ATH). The system is able to indicate the variation of temperature during distribution and storage chain of industrial products. The novelty of this work was an alternative packaging material that it is biodegradable and could inform any temperature variations on the range of 40?C - 70?C, by irreversible visual colour changes. The effects of temperature (10?C, 30?C and 50?C) and luminosity (0, 500 and 1000 l×) were analyzed on C-ATH using an experimental design of 2 variables, measuring the colour parameters (L*, a*, b*) and mechanical properties (tensile strength, elongation break and Young’s modulus) as responses. C-ATH suspensions were applied as a coating on cardpaper surface forming a temperature indicator prototype (TIP). C-ATH darkened after being exposed to temper- atures above 50?C and luminosity of 1000 lx for 72 hours. TIP was obtained, without bubbles or defects, with reduced water absorption capacity. Irreversible visual colour change was verified on TIP exposed at 40?C independently of luminosity, turned gradually yellow. Chitosan suspensions containing ATH and applied as a coated on card paper sheets could be an alternative of biode-gradable material for packaging system that indicates efficiently temperature changes. This indicator system has potential application temperature range of 40?C to 70?C, such as food, pharmaceuticals, biological, agricultural and others products, that are highly dependent of storage temperature conditions.

Thermal shot noise in top-gated single carbon nanotube field effect transistors

Abstract: The high-frequency transconductance and current noise of top-gated single carbon nanotube transistors have been measured and used to investigate hot electron effects in one-dimensional transistors. Results are in good agreement with a theory of 1-dimensional nano-transistor. In particular the prediction of a large transconductance correction to the Johnson-Nyquist thermal noise formula is confirmed experimentally. Experiment shows that nanotube transistors can be used as fast charge detectors for quantum coherent electronics with a resolution of $13\mathrm{\mu e/\sqrt{Hz}}$ in the 0.2-$0.8 \mathrm{GHz}$ band.
