Brain SPECT in mesial temporal lobe epilepsy: comparison between visual analysis and SPM

Abstract: objective: to compare the accuracy of spm and visual analysis of brain spect in patients with mesial temporal lobe epilepsy (mtle). method: interictal and ictal spects of 22 patients with mtle were performed. visual analysis were performed in interictal (visual(inter)) and ictal (visual(ictal/inter)) studies. spm analysis consisted of comparing interictal (spm(inter)) and ictal spects (spm(ictal)) of each patient to control group and by comparing perfusion of temporal lobes in ictal and interictal studies among themselves (spm(ictal/inter)). results: for detection of the epileptogenic focus, the sensitivities were as follows: visual(inter)=68%; visual(ictal/inter)=100%; spm(inter)=45%; spm(ictal)=64% and spm(ictal/inter)=77%. spm was able to detect more areas of hyperperfusion and hypoperfusion. conclusion: spm did not improve the sensitivity to detect epileptogenic focus. however, spm detected different regions of hypoperfusion and hyperperfusion and is therefore a helpful tool for better understand pathophysiology of seizures in mtle.
Functional electrical stimulation improves brain perfusion in cranial trauma patients

Abstract: objective: demonstrate brain perfusion changes due to neuronal activation after functional electrical stimulation (fes). method: it was studied 14 patients with hemiplegia who were submitted to a program with fes during fourteen weeks. brain perfusion spect was performed before and after fes therapy. these patients were further separated into 2 groups according to the hemiplegia cause: cranial trauma and major vascular insults. all spect images were analyzed using spm. results: there was a significant statistical difference between the two groups related to patient's ages and extent of hypoperfusion in the spect. patients with cranial trauma had a reduction in the hypoperfused area and patients with major vascular insult had an increase in the hypoperfused area after fes therapy. conclusion: fes therapy can result in brain perfusion improvement in patients with brain lesions due to cranial trauma but probably not in patients with major vascular insults with large infarct area.
Low sensitivity of three-phase bone scintigraphy for the diagnosis of repetitive strain injury

Abstract: context and objective: the diagnosis of repetitive strain injury (rsi) is subjective and solely based on clinical signs and physical examination. the aim of this paper was to assess the usefulness of three-phase bone scintigraphy (tpbs) in diagnosing rsi. design and setting: prospective study at the division of nuclear medicine, department of radiology, school of medical sciences, universidade estadual de campinas (unicamp). methods: seventy-three patients (mean age 31.2 years; 47 males) with clinical suspicion of rsi in the upper limbs were studied. a total of 127 joints with suspicion of rsi were studied. the shoulders, elbows and wrists were analyzed semi-quantitatively, using the shafts of the humeri and ulnae as references. the results were compared with a control group of 40 normal individuals. the patients? signs and symptoms were used as the "gold standard" for calculating the probabilities. results: from visual analysis, abnormalities were observed in the flow phase for four joints, in the blood pool phase for 11 joints and in the delayed images for 26 joints. visual analysis of the joints of the control group did not show any abnormalities. semi-quantitative analysis showed that most of the patients? joint ratios were normal. the exceptions were the wrists of patients with left-sided rsi (p = 0.0216). however, the sensitivity (9%) and accuracy (41%) were very low. conclusion: tpbs with semi-quantitative analysis has very low sensitivity and accuracy in the detection of rsi abnormalities in the upper limbs.

BENEFíCIOS DO MANEJO DAS TRILHAS PARA USO PúBLICO NO PARQUE ESTADUAL DO JARAGUá

Abstract: O estudo analisou os benefícios gerados após o manejo das trilhas no Parque Estadual do Jaraguá (PEJ) e se os impactos negativos gerados pelo fluxo de visitantes foram minimizados. Para o desenvolvimento da pesquisa utilizou-se de bibliografias referente às Unidades de Conserva o (UC) e manejo de trilhas, observa o in loco e acompanhamento de grupos antes e após o manejo das trilhas pelo pesquisador. O PEJ é uma UC de Prote o Integral, permitindo apenas pesquisas científicas, atividades de educa o e interpreta o ambiental, recrea o e ecoturismo. Com 491,98 ha de Mata Atlantica, o parque se caracteriza como uma das principais áreas verdes do município de S o Paulo e a principal área de lazer para a popula o de seu entorno. é perceptível que a visita o pública nessas áreas cresce, muitas pessoas buscam o prazer que o contato com a natureza proporciona e experiências que o homem urbano n o está acostumado, como por exemplo: observar sua fauna e flora, beber água da nascente e respirar ar menos poluído e mais fresco. Para observar a natureza ou acessar um atrativo, os visitantes percorrem trilhas, ou seja, caminhos abertos na mata para acessar um determinado ponto, além disso, s o utilizadas para patrulhamento, acesso rápido em caso de incêndios, resgate, atividades de educa o e interpreta o ambiental e ecoturismo. Contudo, existem muitos impactos negativos que a abertura de uma trilha pode causar, tais como: compacta o do solo, eros o, pisoteamento das plantas e das mudas em desenvolvimento, altera o nas rotas de deslocamento dos animais, entre outros. O PEJ possui três trilhas abertas á visita o: Trilha da Bica, Silêncio e Pai Zé. Desde 2006, o parque intensificou o manejo de suas trilhas. A primeira foi a do Silêncio, que foi totalmente adaptada para o atendimento de visitantes com necessidades especiais, com a constru o de rampas, corrim os, disposi o de bancos, fechamento de áreas para regenera o da vegeta o e conten es de eros o. O manejo da trilha da Bica come ou com o fechamento em partes do seu trajeto, que originalmente levava os visitantes próximos a nascente, além disso, o uso constante para banho causou expans o da área, compacta o do solo e pisoteamento das plantas. Na readequa o foi construída uma escadaria na entrada, corrim os, proibi o do banho, uma plataforma para diminuir o impacto nas margens do curso d’água e conten es. Abstract: à semelhan a da Argentina, El Salvador, Guatemala, entre outros países, o Brasil, e mais especificamente o Rio de Janeiro, tem assistido à mobiliza o de familiares de vítimas de violência armada, em particular de massacres levados a cabo por agentes do Estado. Tais grupos, mais ou menos organizados, têm conduzido pesquisas, apresentado queixas, acompanhado a evolu o dos inquéritos policiais e dos procedimentos judiciais. O presente artigo procede a um levantamento das dificuldades e das perspectivas de familiares das vítimas de violência armada perante os desafios que se apresentam nas suas trajectórias, tanto do ponto de vista individual quanto colectivo. Através da análise de uma experiência – o desenvolvimento do projecto de apoio a familiares de vítimas de chacinas –, pretende se discutir a natureza, os riscos, os limites, as tens es e as possibilidades da ac o colectiva, a partir da história de um grupo com cerca de 30 mulheres na sua luta por justi a e repara o. Like Argentina, El Salvador and Guatemala, amongst other countries, Brazil, and more specifically Rio de Janeiro, has witnessed the mobilisation of families of victims of armed violence, in particular massacres carried out by agents of the state. Such groups, which are, to a certain extent, organised, have carried out investigation, presented complaints, and monitored the progress of police inquiries and judicial proceedings. This article provides an overview of the difficulties and perspectives of the families of the victims of armed violence in the face of the challenges presented during the course of their trajectories, both from an individual and a collective point of view. Quantification of the Expression of HIF-1alpha by Real-Time PCR in Rat Hepatocytes Cultures Invaded by Shigella flexneri under Normoxic and Hypoxic Conditions  [PDF]

Abstract: Aim: Shigella flexneri (S. flexneri) is a gram-negative enterobacterium responsible for severe intestinal end systemic infection in humans. The bacteria can reach the liver due to degeneration of the colonic epithelium. Hypoxia is present in many human diseases and can induce the expression of the transcription factor HIF-1alpha that may have a cell protective role. The influence of hypoxia and HIF-1alpha on bacterial infection, studied in this work, is unclear. Hypoxia inducible factor-1alpha (HIF-1alpha) is a transcription factor that acts as a master regulator of gene expression induced by hypoxia. Methods: We compared the ability of S. flexneri to invade rat hepatocytes in primary culture both in normoxic and hypoxic conditions. We evaluated TNF-alpha released by hepatocytes, apoptosis rate and HIF-1alpha expression by confocal microscopy as well as real time PCR technique. Results: We showed that S. flexneri invaded less hepatocytes previously submitted to 24 h hypoxia (6.5% O2) than those cultivated in normoxia (21% O2). S. flexneri also induced HIF-1α expression in hepatocytes, TNF-α secretion and apoptosis. Conclusion: a) Hypoxia alone was not a stimulus to TNF-α secretion, but induced cell apoptosis and HIF-1α expression; b) S. flexneri was able to invade rat hepatocytes and hypoxia apparently influenced significantly bacterial cell invasiveness; c)

Abstract: brain spect imaging (bsi) with statistical parametric mapping (spm) offers a clear and accurate vision of perfusion changes. objective: to study brain perfusion abnormalities in adolescents' dependent of multiple drugs. method: sixteen male patients (15.1±2.1 years) were submitted to 99mtc-hmpao bsi with spm. results: cortical hypoperfusion occurred in 7/16 patients (44%). there was a significant inverse correlation between the number of hypoperfused regions and the patient's age (p= -0.6737; p=0.004) and with the age when the drug dependence began (p= -0.5616; p=0.023). there was also a tendency towards an inverse correlation between regions of hypoperfusion and the duration of the drug dependence. conclusion: bsi with spm can help detect hypoperfusion in adolescents dependent on multiple drugs. the younger the patients, the more regions of hypoperfusion are noted. probably, the neuronal plasticity has an important role in this phenomenon because the highest neural activity occurs in childhood.
