Epidemiological changes and financial consequences of hypertension in Latin America: implications for the health system and patients in Mexico

Abstract: the aim of this study was to assess the costs and financial consequences of epidemiological changes in hypertension in méxico. the cost evaluation method to estimate costs was based on instrumentation techniques. to estimate the epidemiological changes and expected cases of hypertension in 2010-2012, three probabilistic models were constructed according to the box-jenkins technique. comparing the economic impact, from 2010 to 2012 there will be a 24% increase in financial requirements (p < 0.05). the total cost of hypertension in 2011 will be us$ 5,733,350,291, including us$ 2,718,280,941 in direct costs and us$ 3,015,069,350 in indirect costs. if the risk factors and various healthcare models remain unaltered in the institutions analyzed here, the financial consequences will have a major impact on users' pockets, followed by social security providers and public healthcare providers. the authors suggest a revision in the planning, organization, and allocation of resources, particularly programs for health promotion and prevention of hypertension.
Economic burden of expected epidemiological changes in diseases related to tobacco, Mexico

Abstract: objective: to determine health care costs and economic burden of epidemiological changes in diseases related to tobacco consumption. methods: a time-series analysis in mexico (1994-2005) was carried out on seven health interventions: chronic obstructive pulmonary diseases, lung cancer with and without surgical intervention, asthma in smokers and non-smokers, full treatment course with nicotine gum, and full treatment course with nicotine patch. according with box-jenkins methodology, probabilistic models were developed to forecast the expected changes in the epidemiologic profile and the expected changes in health care services required for selected interventions. health care costs were estimated following the instrumentation methods and validated with consensus technique. results: a comparison of the economic impact in 2006 vs. 2008 showed 20-90% increase in expected cases depending on the disease (p<0.05), and 25-93% increase in financial requirements (p<0.01). the study data suggest that changes in the demand for health services for patients with respiratory diseases related to tobacco consumption will continue showing an increasing trend. conclusions: in economic terms, the growing number of cases expected during the study period indicates a process of internal competition and adds an element of intrinsic competition in the management of preventive and curative interventions. the study results support the assumption that if preventive programs remain unchanged, the increasing demands for curative health care may cause great financial and management challenges to the health care system of middle-income countries like mexico.

Abstract: Background: Splenic abscesses are uncommon, appear in subjects with predisposing factors such as systemic infections and have high mortality rates. Aim: To report seven patients with splenic abscesses. Material and Methods: Retrospective review of medical records of patients with a splenic abscess treated between 1987 and 2005. Results: The records of four women and three males aged 20 to 74 years, were reviewed. The most common presenting symptoms were fever and abdominal pain and all had predisposing factors. Six patients had a leukocyte count of 19,500 x mm3. Mean erythrocyte sedimentation rate and C reactive protein values were 75 mm/h and 13.5 mg/dl. Diagnosis was made with ultrasound in two patients and CT scan in five. Six patients had an unique abscess and one patient had multiple lesions. A splenectomy was done in three patients as the first choice treatment and in one, due to medical treatment failure. In two patients, a CT guided percutaneous drainage was performed and one patient was subjected to medical treatment only. Abscess cultures were positive in 50% of patients subjected to percutaneous drainage and in 50% of splenectomized patients. No patient died and no complications were observed in the early or ate postoperative period. Conclusion: Splenic abscesses are associated to predisposing conditions. The first choice is surgical treatment, but percutaneous drainage is also a therapeutic option (Rev Méd Chile 2008; 136: 38-43)

Abstract: El colirio de fenilefrina 10 % y tropicamida 1 % se emplea en la práctica médica como antihistamínico, analgésico, midriáticos y ciclopléjico. El objetivo del presente trabajo consistió en desarrollar una formulación de fenilefrina 10 % y tropicamida 1 % que cumpliera con las especificaciones de calidad establecidas por el fabricante, que fuera estable física, química y microbiológicamente, para lo cual se realizó un dise o y los estudios de preformulación. Se estudió además, las especificaciones de calidad de la formulación seleccionada, la estabilidad del producto y el tiempo de vigencia de este. Se desarrolló y validó un método analítico para el control de la calidad y el estudio de estabilidad del producto terminado por cromatografía líquida de alta resolución. Se realizaron los resultados analíticos del estudio de estabilidad acelerado y por vida de estante, para lo cual se emplearon 3 lotes del producto a escala piloto. El colirio resultó estable física, química y microbiológicamente envasado en frascos de polietileno de baja densidad, por un tiempo de 12 meses almacenados a temperatura ambiente. The aim of present paper was to develop a 10% Phenylephrine and 1 % Tropicamide formula fulfilling the quality specifications established by manufacturer physically, chemically and microbiologically stable with the performing of a design and pre-formula studies. Also, we studied the quality specifications of selected formula, the product stability and its expiry time. An analytical method was developed and validated to quality control and stability study of the finished product by high-performance liquid chromatography. Analytical results of accelerated stability study and by shelf life using 3 batches of product at pilot scale. Eyedrops was physically, chemically and microbiologically stable when it was bottling in low density polyethylene flasks during 12 months of storage at room temperature.

Abstract: colonic intussusception is uncommon among adults. in 20 to 50% of cases, it is secondary to malignant tumors. however, occasionally, it can be secondary to a lipoma. we report a 46 years old female and a 76 years old male with a colonic intussusception. the diagnosis was made in the female with an abdominal cat scan that showed the lesion in the transverse colon, associated to an image with the density of fat. in the male, the diagnosis was made with a colonoscopy that showed a polypoid lesion that partially obstructed the ileocecal valve. both patients were operated and the lesions were excised. the postoperative period was uneventful and they were discharged at the tenth and eighth postoperative day, respectively. the pathological study of the surgical piece confirmed the diagnosis of lipoma

Abstract: this paper reports a cognitive study of a qualitative character in relation to the learning of the two variable functions and partial derivative concepts in the engineering context. it defends the fact that didactic settings in context can be conducive to a logical meaningful learning for the student with sense in the future professional area. this motivated us to research what happened at cognitive levels for students in this type of didactic environment. the theoretical framework is described by the study of cognitive functioning in a mental act of learning (the process of resolving problem). the analysis reports the experience which results in the implementation of didactic setting designs with one group of engineering students.

Abstract: In this article we estimate a two-factor model for the risk-free term structure yield in Chile. These factors are the short rate and the central tendency that are not directly estimable. Both factors follow an Ito stochastic process. In the solution of the model we follow the Balduzzi et al. (1996) specification, which provides an estimation procedure that do not depend on the parametric specification of the second factor and an "exponential affine" solution type that allows to estimate the model by mean of only one equation. The data used in this study is the average weekly yields of the bonds "Bonos de Reconocimiento" (BR) of the Chilean stock exchange during May 1993 and December 1997. The results show that when we use a stochastic level for the long term rates the yields adjust better than the case when this level is constant. Also, the speed of the reversion process increases due to the better performance of the short term rate. In addition, using an ARCH specification for the rate volatility we found additional evidence that the variance of the rates.
