Abstract: background: obesity is a chronic and multifactorial disease, associated with increased cardiovascular risk, especially diastolic heart failure. objective: to evaluate left ventricular diastolic function in morbidly obese patients in the pre-operative for bariatric surgery, correlating it with cardiovascular risk factors and heart structure. methods: this is a cross-sectional study with 132 patients eligible for bariatric surgery submitted to transthoracic echocardiography assessment and of cardiovascular risk factors, as follows: 97 women (73.5%), mean age 38.5 ± 10.5 years and bmi of 43.7 ± 7.2 kg / m2. patients were divided into three groups: 61 with normal diastolic function, 24 with mild diastolic dysfunction and 47 with moderate/severe diastolic dysfunction, of which 41 with moderate diastolic dysfunction (pseudonormal pattern) and six with severe diastolic dysfunction (restrictive pattern). results: hypertension, age and gender were different in the groups with diastolic dysfunction. groups with dysfunction had higher left atrial diameter, left ventricular diameter, left atrial volume in four and two chambers, left atrial volume index and left ventricular mass index corrected for body surface area and height. conclusion: the high frequency of left ventricular diastolic dysfunction in the preclinical phase in morbidly obese patients justifies the need for careful echocardiographic assessment, aiming at identifying individuals at higher risk, so that early intervention measures can be carried out.

Abstract: background: chronotropic incompetence (cti) is frequent in elderly patients and may limit the role of the exercise test in the identification of coronary artery disease (cad) in this population. objective: to assess the value of cti in an elderly population in the diagnosis of cad. methods: a total of 3,308 patients were studied, 804 were elderly individuals (age > 65 years) who underwent exercise stress echocardiography (ese). based on the heart rate (hr) reached during the exercise test, were divided into two groups: g1 150 patients who did not reach 85% of the age-predicted hr, and g2 654 patients who did. the groups were compared to clinical characteristics, segmental left ventricular contractility rate (wmsi) and coronary angiography (cag). results: clinical characteristics were similar between the groups. wmsi was higher in g1 than in g2, both at rest (1.09 ± 0.21 versus 1.04 ± 0.15) and after exercise (1.15 ± 0.29 versus 1.08 ± 0.2) (p < 0.001). abnormalities in wall contractility were more frequent in g1 than in g2 (55% versus 37%; p < 0.05), thus suggesting that elderly with cti have a higher frequency of cad. cag was performed in 69% ese positive for myocardial ischemia. in the g1 group, 91% of the ese were true positive versus 84.5% in g2, that is, presence of obstructive coronary artery disease (> 50%). conclusion: cti is associated with a higher frequency of contractile alterations in the elderly population and adds a positive predictive value to ese in the identification of patients with obstructive cad.

Abstract: this article aims to contribute to the study of the production of academic texts and research projects by post-graduate students. it relies on the constructivist conception of knowledge, whereby learning is seen as a continuous process of recomposition of cognitive structures, with contributions from humberto maturana on the central role of emotion and language in coming-to-be. it is suggested that the metaphor of embryogenesis is an apt one for the evolution of a research project. the research project begins with a template and takes shape during the course of successive phases, like an embryo, attributing its components to undifferentiated cells, which simultaneously differentiate themselves, giving rise to the project as a whole. the article also presents a series of criteria for evaluation of writing and reflection based on flows of emotion and attitudes, as a way of overcoming the challenge posed by the blank page or screen, which can help us to use our creative process in a more productive, consistent and pleasurable manner.
Occurrence of Staphylococcus aureus in raw milk produced in dairy farms in S?o Paulo state, Brazil

Abstract: the objective of the present study was to evaluate the occurrence of staphylococcus aureus in milk produced in 37 farms located in the regions of ribeir?o preto and s?o carlos, state of s?o paulo, brazil. two-hundred and eight samples of milk from individual cows showing subclinical mastitis, and 37 samples of bulk tank milk were analyzed. s. aureus strains were detected in 18 (7.3%) milk samples: 14 (6.7%) from samples of individual cows, and 4 (10.8%) from bulk tank milk. two individual milk samples (14.3%) and two bulk milk samples contained enterotoxigenic s. aureus. pfge analysis revealed the genetic heterogeneity of the strains isolated from raw milk, which presented to 13 s. aureus patterns. results confirmed the potential transmission of staphylococcal food poisoning to consumers via milk of cows affected by subclinical mastitis, mainly when raw milk is ingested.

Abstract: objective: evaluate the effects of optimized atrial stimulation - oas (dual-site atrial pacing, heart rate above the intrinsic rate, and specific functional algorithm), and the use of atenolol in preventing recurrent atrial fibrillation (af). primary endpoint: to quantify the rate of af episodes. secondary endpoints: assessment of quality of life, specific cardiovascular symptoms, rate of hospital admissions, rate of electrical and pharmacological cardioversions, and adverse cardiac events. methods: twenty-five patients with recurrent episodes of paroxysmal af and sinus node disease had dual-site atrial and ventricular pacemakers implanted, and were started on atenolol, 100 mg/day. next, they were randomized to two groups: group i: first three months with oas and the specific pacing algorithm (dao) turned on, and three more months with the algorithm off. group ii: the inverse sequence to group i. the pacing mode chosen was dddr, and after three months patients underwent clinical and electronic evaluations of the stimulation system by: automatic mode switch (ams), 24-hour holter monitoring, doppler echocardiogram, and sf-36 questionnaire. following, a crossover comparison took place, and a new assessment was performed six months later. results: when compared to the group with the algorithm turned off, oas patients had lower rates of: af/week (p < 0.001); ams activations (p < 0.01); hospitalizations (p < 0.001); cardioversions (p < 0.001), and higher scores on the physical and mental components of quality of life. conclusion: the hybrid therapy adopted, oas associated with the use of atenolol, reduced the rate of recurrent af and improved the clinical-functional status of patients with symptomatic bradyarrhythmias.

Abstract: background: the literature lacks studies about the prognostic value of exercise stress echocardiography (ese) in patients with complete left bundle branch block (lbbb) of the bundle of his. objective: to assess the prognostic value of ese in patients with lbbb. methods: this is a retrospective cohort that evaluated 135 patients with lbbb, from january 2001 to october 2009, of which 37.8% were men, mean age 63.6 ± 11.5 years submitted to ese according to bruce protocol on a treadmill. cox regression was used, considering these outcomes: death from all causes and from cardiac events, defined as acute myocardial infarction (ami), percutaneous angioplasty (pa), coronary artery bypass grafting (cabg) and death from cardiac causes. results: positive ese was observed in 42 patients (31%). the mean follow-up was 45.8 ± 4.7 months. during this period, there were 9 deaths from all causes and 9 cardiac events (3 deaths from cardiac causes, 3 myocardial infarctions, 2 pa and one cabg). the mortality rate from all causes during five years was 16.1% in the group with positive ese and 2.5% in the group with negative test (p = 0.171), whereas the rate of cardiac events in the same period was 15.1% for the positive ese and 1.6% in the group with negative test (p = 0.009). conclusion: ese showed to be a predictor of cardiac events in patients with lbbb.

Abstract: background: despite abundant evidence of increased morbidity and mortality, chronotropic incompetence (ci) is not a routine diagnosis well defined in protocols of cardiac evaluation and its clinical importance is still underestimated. objective: to evaluate the clinical and echocardiographic parameters associated with hf in non-elderly patients submitted to stress echocardiography (se). methods: one thousand seven hundred ninety-eight patients with a mean age of 48.4 ± 7.5 years, who underwent se between january/2000 and august/2009 were evaluated. patients with chronotropic index smaller than 0.8 were considered chronotropic incompetent as compared to competent patients as to clinical and echocardiographic characteristics. results: the duration of the exercise was 9.3 ± 2.4 minutes on average. two hundred and seventy (15%) patients were chronotropic incompetent. the chronotropic index of this group was 0.7 ± 0.1 vs. 1.0 ± 0.1 for competent patients. multivariate logistic regression analysis identified the following parameters as independently associated with hf: dyspnea on examination [odds ratio (or) = 4.27, p <0.0001], previous chest pain on medical history (or = 1.51; p = 0.0111), higher left ventricular mass rate in incompetent patients (lvmi) (or = 1.16, p = 0.0001), metabolic equivalents (mets) (or = 0.70, p = 0 , 0001), st segment depression (or = 0.58, p = 0.0003) and high systolic blood pressure (δsbp) (or = 0.87, p = 0.0011). myocardial ischemia was not associated with hf. conclusion: hf is associated with functional parameters, such as dyspnea on exertion, history of chest pain and lower mets. it is also associated with structural benchmark index of left ventricular mass. in addition, chronotropic incompetence does not appear to increase the chance of myocardial ischemia in non-elderly patients. Abstract: the ornamental species hemerocallis hybrida hort. is a perennial herbaceous plant originated in asia. the objective of the present work to study the artificial methods of hybridization? floral biology and reproductive system of daylily plants to support genetic and breeding programs of this species. the technique of covering the stigma of daylily's flowers with aluminum paper instead of covering the whole flower after the artificial crossing, provides higher setting and maturation of seed capsules (p < 0,001; test 2 contingency 2 x 2; correction of yates). blooming is the best phenophase for controlled crossings with estigmatic reception of 100%, and amount of viable pollen grains of 45% higher then the phase of the senescence. the studied species is hermaphrodite, essentially allogamous, with self-incompatibility and autogamy indexes equal to zero.

Abstract: This research evaluated the physical and chemical quality of 72 samples of crude milk from 9 farms in the Gameleira region, municipality of Sacramento, MG, Brazil, and 12 milk samples from the same farms collected from a community milk tank. Values for titratable acidity, density, cryoscopy, fat, total dry extract, defatted dry extract were determined (BRASIL, 1981). Results showed that 10 (13.9%) samples did not comply with at least one physical and chemical item required by Norm 51. The 12 milk samples from the community tank analyzed during the assay complied with Norm 51 for physical and chemical requirements. The use of community tank actually provides satisfactory quality milk with regard to its physical and chemical characteristics.

Abstract: We present numerical studies on the switching characteristics of a fiber Bragg grating (FBG) with modulation in the third order nonlinear index of refraction along it’s length. The FBG is operating in a continuous wave regime (CW). This study was done taking into account the possible asymmetry brought by the non harmonic modulation of the nonlinearity, leading to different reflection and transmission characteristics, that depend on the direction of propagation along the modulated nonlinear FBG. This phenomenon may be useful for applications like an optical isolator. It was found that for a set of values of the modulation parameter, the FBG can exhibit multistable states. The numerical studies were obtained starting from the coupled-mode equations solved from the coupled-mode theory and simulated using the fourth-order Runge-Kutta method.
