How to Reduce Unnecessary Invasive Angiograms When Patients Are Initially Evaluated by Coronary Computed Tomography?  [PDF]

Abstract: Purpose:When Coronary Computed Tomography (CCT) detects coronary obstruction, patients are regularly referred to invasive angiogram. With higher sensitivity than specificity, CCT might induce unnecessary angiograms (“false positive CCT”). We sought to determine the patients and CCT findings associated with false positive CCT. Methods: Patients were prospectively referred to CCT with a 64-slice CT scan for suspected CAD. Inclusion criteria were: 1) clinically suspected angina pectoris; or 2) suspected silent ischemia on resting EKG. Exclusion criteria were acute coronary syndrome and non sinus rhythm. Invasive coronary angiogram (ICA) was performed on the basis of CCT findings (stenosis >50%). Analysis was performed on a per patient basis. Results: Out of 702 patients, 228 had suspected significant stenosis by CCT and ICA was performed in 176 patients. Coronary stenosis >50% was not confirmed by ICA in 44 (25%). In multivariate analysis, we observed that atypical angina (OR 3.63,CI 1.43-9.66), silent ischemia (OR 5.11, CI 1.89-14.6) and number of suspected stenosed arteries by CCT (OR 1.81, CI 1.15-2.94) were independently predictive of false positive CCT (p < 0.05). Lesion location and coronary plaque characteristics did not impact on CCT accuracy. Conclusions:Performing CCT for atypical angina or silent ischemia is associated with higher rate of unnecessary invasive coronary angiograms. We failed to identify lesions characteristics prone to be “false positive” of CCT.

Abstract: The objective of this study was to investigate the effects of many citrus flavanones, such as neoeriocitrin, naringin and neohesperidin, in cartilage degradation. Degenerative joint disease involved degradation of joints, including articular cartilage and subchondral bone. When bone surfaces become less well protected by cartilage, bone may be exposed and damaged. The degradation cartilage is mediated by alteration of the balance between anabolic and catabolic processes, changes in proteolytic enzyme activity, mechanical disruption of the cartilage extracellular matrix (ECM), or a combination of these processes. We examine the capability of neoeriocitrin, naringin and neohesperidin, to inhibit metalloproteinase (MMP)-13, collagenase involved in degradation of cartilage matrix components. Also, we assay the flavonoids effect on reducing of Glycosaminoglycans (GAGs) release, and restore Nitric oxide (NO) levels in explant of human articular cartilage. Our results suggest that neoeriocitrin, naringin and neohesperidin are a potential therapeutic agent to protect cartilage tissue.
Strategies, Performances and Profiling of a Sample of U.S. Universities in 2012  [PDF]

Abstract: The global economic crisis is affecting performances of not-for-profits. At the same time donors are targeted by a pressing good-cause related marketing, so that the competition for philanthropy is particularly keen. U.S. universities can be public, not-for-profit and for-profit. U.S. not-for-profit universities are confronted with different marketing, fundraising and revenue diversification. Above all, marketing concerns customers and their segmentation and their purchasing-power exploitation; fundraising aims to gain the trustworthiness of donors, instead. The aim of this paper is the analysis of the revenue diversification of a sample of 100 U.S. not-for-profit universities according to IRS (Internal Revenue Service) Forms. These 100 U.S. universities had the highest 2012’s revenues for the Guidestar ranking (www.guidestar.org). The cluster analysis gives evidence that the highest gain and the highest solvency are both connected with the implementation of revenue diversification for one profile. The most crowded cluster is the Marketing Expert with the second highest gain.

Abstract: this study reflects upon some episthemological and ethical dilemma we find in the psychological knowledge production facing the hegemonic rationality that establishes a model of production, application and evaluation of knowledge to education and to post-graduation programmes, above all in psychology. in order to understand the present condition of psychology, we revisited its past, searching for indication of its struggle and resistence history, searching marks of permanence and ruptures, so we could think prospectively about our condition and our becoming. in the effort to systematize some points of the discussion, we initially approach the specificities of psychology knowledge production; then, we emphasize the hegemonic rationality crisis and the emergence of a new pradigm in science, and we conclude with the discussion on evaluation.

Abstract: the following article discusses the constitution of the subject and the subjectivity on the socio-historical approach, making evident the question of the language - production of meaning in vygotskian contemporary authors. this theoretical study contextualizes the conceptual elaboration of vygotsky's work, marked by complexity and incompleteness that generate diversity in/of interpretations among authors. it attempts to undercover and understand the different notions of subject and production of meaning between them. to this end, gonzález rey's contributions support the idea of subjective dimension of personal meaning and individual and social subjectivity. such ideas resume the vygotskian thinking, considering bakhtin as an interlocutor, dialoging with authors based in the historical and dialectical materialism, which makes possible to understand the subject constituted by the other and constituted by language. such features enables the focus on drama, subjectivity and meaning production in interconstitutive aspects of multiple dimensions singular/collective, subjective/objective, biological/cultural, historical/dialectical.
The rehabilitation of a reservoir: A new methodological approach for calculating the sustainable useful storage capacity  [PDF]

Abstract:

Present work introduces the sustainable useful storage capacity as the minimum storage capacity able to satisfy the water demand for drinkable, industrial and irrigational purposes and necessary in order to overcome water deficit situations which, at least in Central Southern Italy, occur in the summer, when agricultural demand is really high. Sediment volumes to be removed from the reservoir bottom will be calculated as the difference between the current and the sustainable useful storage capacities of the reservoir in study. The calculation methodology of the useful sustainable storage capacity, based on the reservoir water balance between inflows at the reservoir and water demand, has been applied to the Camastra reservoir (Basilicata, Southern Italy), for which numerous reliable data including more than 40 years of inflows and water supplied volumes and data relative to 7 bathymetric surveys are available. Result analysis shows that this methodology, at least in the study case, enables sediment quantities to be removed more sustainably from a technical, economical and environmental point of view.

Abstract: some questionings about the subject of education in universitary education were launched in our studies cycle in education. along them, we ask "who's the subject of education? "; "the ways for teaching and learning have changed if compared to the time past". this paper is a qualitative explorer research, according to a bibliographical rising. my research shows that today's student doesn't learn nor coexist or is connect to others in the same way as he did in the past and it says that adults are responsible for organization and psychological signification of non-adults lives. concluding, it's possible to say: the questioning about "who is the subject of education" is indispensable for all educator. today's definition to a space in the classroom sees the student as the core and the teacher as a mediator.
