Abstract: this research focused on opinions of mothers in londrina whose infant children had died in 1997, attempting to demonstrate the feasibility and utility of studies on user satisfaction with health services. opinions were analyzed according to type (positive, negative, or both positive and negative) and comments. the majority of mothers had positive opinions of the health services, especially prenatal care (75.6%), while fewer (52.6%) gave a positive assessment towards the hospital care provided to their infants who had died before they could be discharged from hospital. most of the low opinions (21.1%) concerned intrapartum care. analysis of comments revealed satisfaction but also some problems in the relationship between the physician staff and patients; others related to the organization of services and medical techniques or guidance. this type of study can be useful for a more comprehensive evaluation of health care provision.

Abstract: Este estudo teve como objetivos conhecer as opini es de m es de crian as menores de um ano que morreram em 1997, em Londrina, sobre os servi os de saúde utilizados, e demonstrar a viabilidade e utilidade dos estudos de satisfa o de usuários de servi os de saúde. As opini es foram analisadas segundo o tipo (positiva, negativa ou mista) e quanto ao teor dos comentários. Para todos os servi os prevaleceram as opini es positivas, com maior propor o para a aten o pré-natal (75,6%), e menor para o atendimento hospitalar às crian as que n o chegaram a ter alta do hospital após o nascimento (52,6%). O atendimento ao parto recebeu a maior propor o de opini es negativas (21,1%). O teor dos comentários revelou satisfa o e alguns problemas na rela o profissional/equipe e usuários, na organiza o de servi os e com rela o a técnicas e orienta es. Conclui-se que estudos deste tipo fornecem elementos importantes para uma avalia o mais global da assistência à saúde.

Abstract: the objective of this study was to identify risk factors associated with pre-term birth among live births in a hospital in londrina, paraná state, brazil. cases consisted of 328 pre-term births, and controls were 369 births with gestational age greater than 37 weeks. a multiple hierarchical logistic regression analysis was carried out. there was a significant correlation (p < 0.05) between pre-term birth and the following variables: socioeconomic status (slum residence); low age of head-of-family; maternal bmi < 19 or > 30 kg/m2; history of prematurity; history of assisted reproduction; mothers in a relationship for less than two years; maternal stress; weekly maternal alcohol consumption; insufficient prenatal care; gestational complications such as bleeding, genital tract infection, altered amniotic fluid volume, high blood pressure, and hospital admission; and twin pregnancy. regular walks during pregnancy were a protective factor. improved quality of prenatal care and the identification of the above factors during pregnancy may help reduce premature birth.

Abstract: As reflex es sobre a profissionalidade docente têm remetido para a análise de dois conceitos constitutivos de sua condi o, que s o autonomia e autoridade. Para se constituir em uma profiss o, o magistério precisa incorporar elementos que d o contornos próprios ao seu fazer e, especialmente, ser reconhecido como um campo portador de conhecimentos e saberes que identifiquem a profiss o. Nesse contexto, os conceitos de autoridade e autonomia assumem propor es significativas porque est o ligados ao reconhecimento e valoriza o social que se atribui aos professores, numa inser o política e social. O texto realiza um diálogo com a teoria, procurando, em autores contemporaneos, vis es e reflex es sobre quest es relacionadas com os conceitos de autoridade e autonomia na profiss o docente. Abstract: The effects of including defatted corn-germ flour (DCGF), a source of phytic acid, as an ingredient in pig feed for different periods in the growing-finishing phase were evaluated with respect to feed performance, carcass characteristics and meat quality. The test group consisted of 24 male pigs from the same genetic background, with an average initial weight of 75.41±4.41 kg and an average age of 123 days. The treatments consisted of adding 50% DCGF to the feed at 0, 7, 14 and 21 days before slaughter. The zootechnical performance, carcass characteristics, meat quality and antioxidant effects of phytic acid were evaluated through the analysis of lipid oxidation in the meat of the slaughtered animals. There were no effects (P>0.05) for any period of DCGF feed supplementation on the zootechnical performance or carcass characteristics. Refrigerated meat showed less oxidation as the length of the DCGF-inclusion period in the diet was increased. O experimento avaliou os efeitos de diferentes períodos de inclus o do farelo de gérmen de milho desengordurado (FGMD), como ingrediente e fonte de ácido fítico em ra es de suínos em fase de termina o, sobre o desempenho, características de carca a e qualidade da carne. Foram utilizados 24 suínos machos de mesma genética comercial com peso médio inicial de 75,41 ± 4,41 kg e idade média de 123 dias. Os tratamentos corresponderam a inclus o de 50% de FGMD na ra o nos períodos de 0, 7, 14 e 21 dias antes ao abate. Foram avaliados o desempenho zootécnico, as características de carca a e de qualidade da carne e o efeito antioxidante do ácido fítico, através da análise de oxida o lipídica na carne. O delineamento experimental utilizado foi em blocos (peso inicial dos animais) ao acaso, com 4 tratamentos e 6 repeti es por tratamento. N o houve efeito (P>0,05) do tempo de inclus o do FGMD na dieta no desempenho zootécnico e nas características de carca a. Na qualidade de carne houve efeito linear decrescente (P?0.05) na oxida o lipídica da carne mantida em refrigera o durante 7 dias. A carne refrigerada apresentou menor oxida o com o aumentou do período de inclus o de FGMD nas ra es.

Abstract: The platelet indices Mean Platelet Volume (MPV), Platelet Distribution Width (PDW) and Plate-letcrit (PCT) provided by automated hematology analysis are rarely used in clinical practice. Platelets play a central role in the pathogenesis of acute coronary syndromes (ACS) and high MPV has been associated to more reactive platelets and regarded as an independent risk factor for myocardial infarction. In this study, platelet indices were evaluated in 39 patients with ACS presenting two altered biochemical parameters, C reactive protein (CRP) and creatine kinase fraction MB (CK-MB). The results obtained showed significantly higher MPV and PDW values in the group with ACS in comparison to the control group. Also observed was a weak but significant positive correlation between MPV and CK-MB. In view of findings of previous studies, which have associated macroplatelets with higher thrombotic potential our results suggest that the use of the MPV and PDW indices as additional and complementary markers may contribute to the investigation and follow-up of thrombotic risk in patients with ACS.

Abstract: The investigation of electrical properties in alexandrite (BeAl2O4:Cr3+) in synthetic and natural forms is presented in this paper. Alexandrite is a rare and precious mineral that changes color according to the light incident on it. In the synthetic form, it is used technologically as an active laser medium. The electrical characterization was obtained using the Thermally Stimulated Depolarization Current (TSDC) technique, an interesting tool to study the behavior of impurities in insulators. Alexandrite presented the electric dipole relaxation phenomenon, both in natural and in synthetic samples. It was possible to observe TSDC bands for the synthetic sample at around 170 K, and at around 175 K for the natural sample. Besides, photo-induced TSDC measurements were performed through the excitement of the samples by using a continuous wave argon laser. In addition, photoluminescence measurements were performed to verify in advance whether the laser light would be absorbed by the sample, and in order to complement the photo-induced TSDC measurements analysis. The results of photo-induced TSDC experiments have contributed to the understanding of the TSDC bands behavior: the results obtained with the technique suggest that there is an effective participation of Cr3+ ions in the formation of TSDC bands because they were more intense when the sample was exposed to the argon laser beam.
Impact of HIV Status, CD4 Count and Antiretroviral Treatment on Tuberculosis Treatment Outcomes in a Low-Burden Country  [PDF]

Abstract: Background: Few data is available in low-burden TB-HIV countries on TB treatment outcomes. We evaluated TB patients (pts) characteristics by HIV status and compared treatment outcomes in relation to HIV status, CD4 count and antiretroviral therapy (ART). Methods: We performed a retrospective analysis of 447 adult TB pts who registered and initiated treatment in our center from 2014 to 2015. Results: Pts were categorized as HIV-negative (n = 399, 89.3%), HIV-positive on ART (n = 42, 9.4%) and HIV-positive not on ART (n = 6, 1.3%). The proportion of pts with extended TB therapy was higher among HIV-positive pts (p = 0.03). Increased age was associated with higher death rates regarding treatment success (OR 1.08; 95% CI 1.01 - 1.17, p = 0.03). Statistically significant higher mortality was found among HIV-positive pts on ART (OR 9.93; 95% CI 1.36 - 72.37, p = 0.03) and HIV-positive pts not on ART (OR 397.00; 95% CI 44.27 to 3559.91, p ≤ 0.0001) compared to HIV-negative pts. When multivariate analyses were restricted to HIV-positive pts, being not on ART was associated with higher mortality (OR 40.0; 95% CI 4.37 - 365.78, p = 0.001). Conclusions: There was significant difference in death rates between HIV-positive and HIV-negative TB pts. HIV-positive pts not on ART had a significantly higher mortality.

Abstract: under natural condition and without water deficit, assimilation of co2 (a) and transpiration (e) rates, stomatal conductance (g) and photosyntetic efficiency of water use (e/a), were monitored daily on maize (c4), rice (c3) and wheat (c3). in all species, the shape of response curves of a in function of irradiance (i), was a retangular hyperbole. however, luminic saturation was not observed in maize. stomatal conductance response curve in function of i was also a retangular hyperbole, while e was linear in all species. due to differential response of a and e, as a function of i as well as g, the ratio e/a was increased with the increase of i. the c3 species (rice and wheat) showed higher values of e/a than the c4 specie (maize), in all levels of i and g, showing the better c4 adaptation when stomatal limitation aperture occurs.
