Abstract: recent studies have suggested an association between pulmonary arterial hypertension (pah) and thyroid diseases (hypothyroidism and hyperthyroidism). this combination has a good prognosis, because the increase in the pulmonary artery pressure is usually slight and reverses after the treatment of the thyroid disease. although the exact mechanism involved in the pathogenesis of this combination has not yet been established, it has been hypothesized that thyroid hormones and autoimmunity have a direct influence. due to the high prevalence of thyroid disease in patients with pah, thyroid function tests should be considered in the investigation of every patient with pah. in this review, we describe the prevalence of pah in patients with thyroid diseases and the prevalence of thyroid disease in patients with pah, as well as addressing the principal effects that thyroid diseases have on the respiratory system. in addition, we report the treatment effects in patients with these diseases.

Abstract: this paper aims to report an experience carried out by nursing students from the undergraduate school of health sciences, federal district, during their first year of study. the objective was to implement a training for 22 community health workers, in order to clarify them about their role in community work and to elevate their self esteem. a methodology of problematization and various educational techniques were used. the themes were: duties and rights, responsibilities, communication, ethics, team work, children′s health, care for the elderly and domestic violence. the result was considered positive for both sides: the community workers acquired new knowledge/capabilities and better feeling of self worth and the students were able to integrate theory and practice and become active participants in the learning process.

Das análises de Joaquim Nabuco, Oliveira Vianna e Francisco Campos ser o extraídos conceitos tocquevillianos que, aplicados à singular forma o do Estado brasileiro, nos conduzir o à composi o do bin mio liberalismo-democracia. Oral Alendronate Treatment for Severe Polyostotic Fibrous Dysplasia due to McCune-Albright Syndrome in a Child: A Case Report

Abstract: McCune-Albright Syndrome (MAS) is a rare disease presenting with the classical triad of polyostotic fibrous dysplasia (FD), skin hyperpigmentation (café-au-lait spots) and endocrine dysfunction, usually seen as precocious puberty in females. The mosaicism of mutations in the GNAS1 gene results in hyperactive Gsα protein which causes activation of hormonal receptors and bone dysplasia [1–3]. There is increased osteoclastic activity, which can be detected by the elevation of biochemical markers of bone turnover, and normal bone marrow architecture is progressively replaced by abnormal fibrous tissue [4, 5].bisphosphonates, antiresorption agents, largely used in the treatment of osteoporosis have been tried as an option for improvement of FD symptoms in MAS [6–9]. Most patients are treated with intravenous pamidronate, in a hospital regimen, or as outpatients in clinic settings. Alendronate (ALN) is a second-generation bisphosphonate that has, as an advantage, oral administration. It has been used for treatment of several disorders related to increased bone resorption; however, it is rarely used by the pediatric population.Since we could not find reports on the use of oral ALN in children with FD of MAS, although we are aware of an ongoing, as yet unpublished, study with children [4], we present the case of a girl with MAS treated during a one-year period with oral ALN.A 10.5-year-old white girl was admitted with complaints of severe weekly headache, diplopia, and nasal obstruction. She also had daily bone leg pains which caused frequent school absenteeism. She was in outpatient followup since she was 20 months old when it was noticed café-au-lait spots. By the age of 3 years she presented with left exophthalmos, and a severe form of FD of skull, which leads to the diagnosis of MAS. Abstract: municipal health policy planning can include exemplary practices for expanding the population's access to health services. the current study seeks to analyze the limits and possibilities of planning practices for policy implementation by a municipal health department. the empirical data were analyzed based on a concept of planning practices and the theoretical link between the three spheres of government and the postulate of coherence. the use of unstructured strategic planning practices by the management team allowed a significant expansion in the supply of services to the population, but lacked the scope to overcome the limits imposed on the organization's governability by the health services funding model, characterized by strong induction from the federal level. the project was the most robust vertex in the government triangle and led to the development of initial expertise by the municipal management team, thereby ensuring a certain level of governability over the health project. the goals of increasing the supply of health services were constrained by the method's weakness and the organization's institutional insipience.

O presente relato descreve um paciente com catarata e DMP que foi tratado com facoemulsifica o e implante de lente intraocular tórica com recupera o da acuidade visual em ambos os olhos. Pellucid marginal degeneration (PMD) is a rare corneal ectasia and its progression leads to irregular astigmatism and low vision that can not have spectacles or contact lens correction. This report describes a patient with low vision due to cataract and PMD that was treated with phacoemulsification and implantation of a toric intraocular lens with a satisfactory visual acuity outcome.

Abstract: we aim to analyze the economic and political determinations which have instigated the movement towards a class consciousness among an unemployed population mediated by the formation of a social movement, the unemployed workers movement (mtd). we use the marxist debate about social classes (sc) and class consciousness using the studies of iasi (2006). the research method used in the present work is characteristic of the dialectical-materialist approach, which is based in three assumptions: contradiction, totality and historicity. the condition of the unemployed which fostered the movement for group fusion (sartre's discussion), was the result of the new organization of production that has caused both to the working class and the movement of regressivity in the advanced progressivity as regards the possibilities of organization beyond the trade unions. the sc are imposed at the same time as they are imposed by the concrete action of their subjects, in a way which is required by capitalism, but only take place while practical mediations, while contingent mediations.

Abstract: Objetivamos analisar as determina es econ micas e políticas que tensionaram o movimento de constitui o de uma consciência de classe em um estrato da popula o desempregada, mediada pela forma o do Movimento dos Trabalhadores Desempregados [MTD]. Valemos-nos do debate marxista sobre classes sociais (CS) e consciência de classe, utilizando os estudos de Iasi (2006). A metodologia utilizada é própria da abordagem dialético-materialista, que se assenta em três pressupostos: a contradi o, a totalidade e a historicidade. Práticas de aten??o à saúde no contexto familiar: um estudo comparativo de casos

Abstract: this paper examined everyday health care practices focusing on families with different structures and socioeconomic status, following an ethnographical study with ten low-income families and six middle-income families. four families (single-mother versus new marital arrangement) were selected to the present analysis. the comparative analysis described the resources and strategies utilized and the social network available for each pair of families. the results suggested that, in all families, the mother is the most important caretaker when it comes to health problems. the expression institutionalized helplessness was employed to describe the insufficient resources available to poor families. the middle-income families showed consistently more preventive behaviors. the overworked single-mothers were characterized at the same time as caretakers and providers. this characteristic is presented as a risk to the family. the results were helped the understanding of mechanisms of risk-protection to health and to health promotion in family context.

Abstract: objectives: to analyze the determining factors underlying the extent to which positron emission tomography (pet) is used in a selected medical services provider covered by private health insurance plans in brazil. methods: a qualitative case study was undertaken with data collected by means of in-depth interviews (n=9) with representatives of four private health service providers and three health insurance plan operators. results: the decisions to acquire the technology under study are taken by managers of hospitals and clinics that adopt a strategy based on excellence in technology. the factors that influence the decision are: a history of being pioneers in the introduction of new technology; pressure from the institution's clinical staff; the prestige of the physician or the area covered by the unit; the financial resources available; ease of access to technology; competition among health service providers; scientific evidence; and the expected return on the investment. conclusions: the policy instruments currently used to manage the use of medical technology in the brazilian health system has little influence on the decisions made by large-scale private service providers, whose activities are not necessarily related to the health needs of the population or the priorities of public health policy.
