Abstract: introduction: the standard care for these lower extremity injuries with exposed bone is vascularized tissue transfer; however, this option is often not available or is technically difficult. the advent of dermal regeneration template, a bioengineered skin substitute, provides a remarkable alternative to the traditional reconstructive choices. cases report: the authors report two patients from a total of 246 patients who had burns involving their lower extremities, admitted to s. josé hospital burn unit, between january 2007 and december 2008. both patients had bilateral deep burn injuries with exposed bone affecting the anterior surface of lower leg and were treated with tangential excision of the burn eschar and placement of the dermal regeneration template. four weeks later, after engraftment of the matrix, the thin outer layer of silicone was removed and the epidermal coverage was restored with a split thin skin graft. stable wound cover was achieved and no further surgical procedures were required to maintain wound closure. conclusion: the use of a dermal replacement matrix expands reconstructive options for difficult burn (and non burn) wounds, preventing prolonged, staged procedures, allowing successful coverage of vital structures without the use of a flap and, as in these particular cases, the salvage of the extremity.

Abstract: study that proposes to describe the historical and social circumstances of the creation of the dom epaminondas nursing school in the vale do paraíba (s?o paulo brazil), and to analyze the strategies used for its implantation. the primary sources, found in the heritage of the school's support institution and in the city archive, and the secondary sources, from the library of the city and of the espa?o cultural mario covas, treated by individual recorded and grouping for similarity and relevancy, had resulted in four thematic groupings: the role of the city in the treatment of the tuberculosis; the influence of the catholic church in the health, economy and education; the role of the nursing in the combat to the tuberculosis; and the creation of the school. it was concluded that the school was created to take care of to the necessities of nursing staff qualification to face the changes in the therapeutical treatment of the tuberculosis and, following the anglo-american model of formation, to enlist and to keep man power of the pupils in the sanatoriums of the support entity of the school.
Screening Test for Antibiotics in Medicinal Plants (STAMP): Using Powdered Plant Materials Instead of Extracts  [PDF]

Abstract:

Plants are a rich source of antibiotics, but screening all the existing plant species for biological activity using current methods can be time and resource consuming. The present study is to investigate whether powdered plant materials would perform as well as plant extracts in the screening of plants with antimicrobial activity. In the new method proposed (STAMP), we compared in vitro antimicrobial activity of powdered plant materials from 12 species against bacteria and fungi. We confirmed these results with their corresponding aqueous (wet) and hydro-alcoholic extracts and one species testing the antimicrobial activity of two isolated compounds. Compared with hydro-alcoholic extracts, screening using the powdered plant materials correctly identified the majority of the species with antimicrobial activity against Candida albicans (sensitivity 91%, specificity 86%), C. parapsilosis (sensitivity 100%, specificity 67%), and Staphylococcus aureus (sensitivity 64%, specificity 86%). For bacteria, the method performed better in a pH of 9.0. The antimicrobial activity of two compounds isolated from one species (maytenin and netzahualcoyone) confirmed the results. In conclusion, the use of powdered plant materials for screening plants with antimicrobial properties is a cheap, widely available, technically easy, time sparing, reproducible, and sensitive method and can significantly shorten the time and money spent during drug development.

This text aims at bringing about discussions, analyses and propositions regarding the teaching of Psychology in the medical field. It starts presenting some problems in teaching in the medical schools at the present time in the public policies of education and health in Brazil, a main challenge being the process of transformation of the graduation courses in the medical field into the incorporation of the broadened concept of health, and the emphasis on the integrity and care in the process of professional training, as well as learning how to work within a multi-professional team, and the discussion of the conditions brought by the educational and assistance plans and projects.In this context, the question is asked what for and to whom the teaching of Psychology is directed which is able to articulate this care, integrity, social control, humanization and ethics, without putting into second place the psychological approach. The senses and subjects of the teaching of Psychology in the health area are being decoded and recreated, thus demanding a careful analysis of the social-political surroundings, and the ethical projects we want to establish within the proposals of education for health professionals. This introductory text is to put into context the topic of “Teachers education and knowledge about psychology” aiming at provoking questions and reflections for the discussions of the relevant Work Group. As the topic suggests that the knowledge of psychology constitutes the necessary background for teaching, we have placed the ongoing trend at teaching professionals in education and based on research of the knowledge which is the basis of the teaching profession. Pulse oximetry: instrumental alternative in the clinical evaluation by the bed for the dysphagia

Abstract: Introduction: The clinical evaluation by the bed is the clinical procedure utilized by the speech therapy as a screening test for established if the individual presents or not a clinical picture of dysphagia. Its achievement count with the use of the pulse oximetry that is the not-invasive measure of the peripheral saturation of O2. When it related to the dysphagia, the saturation can decline in the moment of the swallowing, suggesting a compromise of the respiratory system. Such decline is observed in individuals that aspire with sustenances of liquid and solid consistency, before during and after food, when compared to the individuals that do not aspire. Objective: To verify the possibility of the utilization of the pulse oximetry as an instrumental alternative in the clinical evaluation for the dysphagia. Method: Revision of literature, based in articles indexed in the bases Lilacs and Medline having as a reference the descriptors: saturation of oxygen, pulse oximetry and swallowing disorders. Results: The association of the clinical evaluation to the pulse oximetry shows up with high sensibility in the detection of the penetration and aspiration laringotraqueal and with bass false-positive index, considering it desaturation, the indices that pierced with bigger values than 2%. Some data found are divergent as the aspiration detection possibility. Final Comments: The studies analyzed confer to oximetria of wrist the character of practically and possibility of use in the actions by the bed, being utilized ally to the clinical evaluation for dysphagia, although certain disagreement between the same is found as regards the utilization of its results as indicator of aspiration laringotraqueal, suggesting need of subsequent researches.

Abstract: Introdu o: A avalia o clínica junto ao leito é o procedimento clínico utilizado pela fonoaudiologia como um screening teste para se estabelecer se o indivíduo apresenta ou n o um quadro clínico de disfagia. Sua realiza o conta com o uso da oximetria de pulso que é a medida n o-invasiva da satura o periférica de O2. Quando relacionada à disfagia, a satura o pode declinar no momento da degluti o, sugerindo um comprometimento do sistema respiratório. Tal declínio é observado em indivíduos que aspiram com alimentos de consistência sólida e líquida, antes durante e após a alimenta o, quando comparados aos indivíduos que n o aspiram. Objetivo: Verificar a possibilidade da utiliza o da oximetria de pulso como alternativa instrumental na avalia o clínica para a disfagia. Método: Revis o de literatura, baseando-se em artigos indexados nas bases Lilacs e Medline tendo como referência os descritores: satura o de oxigênio, oximetria de pulso e transtorno de degluti o. Resultados: A associa o da avalia o clínica à oximetria de pulso evidencia-se com alta sensibilidade na detec o da penetra o e aspira o laringotraqueal e com baixo índice falso-positivo, considerando-se dessatura o, os índices que variam com valores maiores que 2%. Alguns dados encontrados s o divergentes quanto à possibilidade detec o de aspira o. Considera es Finais: Os estudos analisados conferem a oximetria de pulso o caráter de praticidade e possibilidade de uso nas a es junto ao leito, sendo utilizada aliada a avalia o clínica para disfagia, embora se encontre certa discordancia entre os mesmos quanto à utiliza o dos seus resultados como indicadores de aspira o laringotraqueal, sugerindo necessidade de pesquisas posteriores.

Abstract: the purpose of this paper is to present a critical reflection on the knowledge produced on the topics: the impact of the mental illness in the family, the impact of the family environment in the evolution of the eating and the experience family with the mental illness. the measures of overload and express emotion are useful to guide public politics and the practice professional, however in they do not give a multidimensional picture to them of the experience of the care. research on the familiar experience contribute to the understanding of the present suffering in this experience and of the forms of confrontation adopted by the families, also contributing to perceive the differences of the cultural contexts in the confrontation of the families.
Chemical Characterization and Application of the Essential Oils from Chenopodium ambrosioides and Philodendron bipinnatifidum in the Control of Diabrotica speciosa (Coleoptera: Chrysomelidae)  [PDF]

Abstract: The compositions of essential oils from Chenopodium ambrosioides L. and Philodendron bipinnatifidum Schott were determined, and the their potential effects on the nutrition and mortality of Diabrotica speciosa were studied. The extraction of the oils was performed by hydrodistillation (2 h) using a modified Clevenger apparatus and the oils were subsequently subjected to analysis by gas chromatography/flame ionization detector (CG/FID) and gas chromatography/mass spectrometry (GC-MS). A completely randomized design with five treatments and four replications was adopted. The bean plants were sprayed with solutions of the oils dissolved in aqueous Tween 80 solutions at concentrations of 0 (water + Tween 80), 0.5%, 1.0%, 1.5% and 2.0% and then furnished to the insects with no choice available. Seven days after the application, the percentage of leaves with injury, degree of defoliation, the preference index for consumption and the percent of mortality of insects were evaluated. Neither of the essential oils caused a reduction in foliar injury, but antifeeding activity was observed, causing reduced feeding and increasing the mortality of adult D. speciosa insects.
