Abstract: background: indications for the implantation of sirolimusand paclitaxel-eluting stents are expanding and include complex lesions and subsets of patients with clinical and demographic characteristics very different from those of early pivotal randomized trials. the desire registry was planned to monitor the safety of both cypher? and taxus? stent available in brazil since 2002. methods and results: from may 2002 through march 2007, 2043 patients treated with one or more than one stent (either cypher? or taxus?) at hospital do cora??o da associa??o do sanatório sírio were included in this registry. mean age was 63.8 (11.3) years; 76.6% were male and 28.4% had diabetes. a total of 2,415 lesions were treated and 2,983 stents were implanted: 2,608 cypher? and 375 taxus?. acute and subacute (<30 days); late thrombosis (31 days - 360 days) and very late thrombosis (>360 days) occurred in 0.34%, 0.73% and 0.34% of the patients, adding up to a 1.42% overall rate. likewise, the major adverse cardiac event rate was low and added up to 8.6% (154 patients), including: 45 (2.51%) cardiac deaths; 50 (2.8%) myocardial infarctions and 59 (3.3%) of additional revascularizations. conclusions: this long-term analysis of the desire registry shows the high efficacy and safety profiles of drug-eluting stents, with a low thrombosis rate, which is similar to those of randomized trials.

Abstract: objective: to assess the clinical efficacy and safety of the amplatzer occluder for percutaneous closure of secundum atrial septal defect. methods: forty-nine patients underwent the procedure guided by transesophageal echocardiography (tee) while under general anesthesia, afterwards being clinically followed up for 12 months. results: the defect presented as a single orifice in 91.8% of the cases. the means of the longitudinal and transverse diameters were 14.3±5.0 mm and 14.4±4.9 mm, respectively. the means of the stretched diameters and prostheses were 19.3±5.1 mm and 20.3±4.9 mm, respectively. technical success occurred in 97.9% of the cases. immediate residual flow was observed in 54.1% of the patients, being minimum or small in 45.8% and moderate in 8.3%. after 24 hours, residual flow was observed in 25% of the patients (p = 0.0002). at the end of 13.1±1.3 months, the incidence of the overall residual flow decreased to 14.6%, only 4.1% being moderate. a significant reduction in the right ventricular diastolic diameter was observed in the different phases of clinical follow-up (p < 0.001). conclusion: implantation of the amplatzer prosthesis proved to be effective and safe, constituting an option for the treatment of the secundum atrial septal defect in selected cases.
Comparative study of the use of diltiazem as an antispasmodic drug in coronary angiography via the transradial approach

Abstract: objective: to evaluate the impact of the use, prior to the procedure, of injectable diltiazem to prevent complications. methods: between september 2000 and july 2001, 50 patients underwent transradial coronary angiography and were randomized to receive placebo (gi) or diltiazem (gii) through a catheter inserted into the radial artery. all patients received isosorbide mononitrate. ultrasound analyses of the radial artery were performed before examination, 30 minutes afterwards, and 7 days afterwards to evaluate the flow, the diameter, and the artery output. results: the radial artery diameter of gi was 2.4± 0.5 mm before the procedure and 2.3±0.5 mm after 30 minutes (ns), whereas in gii the diameter was 2.2±0.3 mm before the examination and 2.5±0.4 mm 30 minutes after it (p<0.001). radial artery output in group 1 was 7.3±5.l2 ml/min before the examination and 6.1±3.5 ml/min 30 minutes after the examination (ns), and gii had an increase of 5.9±2.5 ml/min before examination to 9.05± 7.78 ml/min after the examination (p=0.04). complications (spasm, occlusion, and partial obstruction) occurred in 4 patients (17.4%) in gi and did not occur in gii (p=0.04). conclusion: the study suggests a decrease in vascular complications through the transradial access for coronary angiography with the use of diltiazem as an antispasmodic drug, resulting in the significant increase in the diameter of the radial artery and radial artery output.

Abstract: the incidence of diabetes mellitus has continuously risen over recent years. patients with this chronic condition have a higher risk of developing coronary artery disease. unfortunately, due to the frequent diffuse involvement of multiple vessels, the long term outcomes after percutaneous intervention, whether by balloon catheter or bare-metal stent, has always been poor and frequently inferior to cardiac surgery for this subset of patients. recently, the introduction of drug-eluting stents (des) has brought new hope to treat patients with more complex diseases percutaneously. in this paper we report a successful percutaneous approach of a 63-year-old insulin dependent diabetic patient treated with nine des (taxustm, boston scientific).

Abstract: objective: to assess whether atrial stimulation with pulsatile continuous electrical current induces atrial fibrillation and to evaluate its effects on atrial electrophysiological properties as well as the atrial histological alterations. methods: twenty-two dogs underwent right lateral thoracotomy and implantation of pacemaker electrodes in the sulcus terminalis (st), in the right atrial appendix (rab), and in the posteroinferior region of the left atrium (la). a pair of electrodes was sutured to the right atrial appendix for stimulation with a 9-volt alkaline battery connected to a system (lm 555) that transforms the linear continuous energy of the battery into a pulsatile continuous current for 60 minutes. atrial epicardial biopsy was performed before and after atrial stimulation. results: no differences were observed in the durations of the effective atrial refractory periods. the times of intra-atrial and interatrial conduction, as well as that of atrial extrastimuli, were prolonged. the duration of atrial electrograms was prolonged during sinus rhythm and programmed atrial stimulation; in 68% of the dogs, atrial fibrillation was induced and was sustained. interstitial edema and bands of cellular contraction were observed in the subepicardium on optical microscopy, as were intense myofibrillar disorganization and an increase in the size of the mitochondria on electron microscopy. conclusion: that technique of atrial stimulation induces atrial fibrillation and causes atrial changes that increase atrial vulnerability to the appearance of atrial fibrillation.
Creatine supplementation in exercised rats: Effects on the aerobic capacity  [PDF]

Abstract: This study aimed to analyze the possible metabolic disturbances caused by creatine supplementation on aerobic capacity of rats, inferred by the maximal lactate steady state. Forty male Wistar rats (90 days old) were distributed into two groups for eight weeks: trained group (T): rats that were submitted to a training protocol, and supplemented-trained group (TCr): rats that were submitted to a training protocol and received balanced diet supplemented with 2% creatine. The blood lactate concentrations equivalent to maximal lactate steady state during treadmill running were analyzed at the beginning and also at the end of the experiment. At the end of the experiment were done comparing the test results MLSS between the two groups. At the beginning of the experiment, prior to groups division, the majority of animals obtained MLSS at a speed of26 m/min, blood lactate concentration of 3.79 ± 0.76 mmol/L. At the end of the experiment, most of trained rats in T presented MLSS at the speed of28 m/min, blood lactate concentration of 3.37 ± 0.68 mmol/L. Most TCr had MLSS at the speed of28 m/min, blood lactate concentration of 3.52 ± 0.69 mmol/L. We conclude that creatine supplementation was not the cause of the improvement in the aerobic capacity of rats in the tread-mill exercise.

Abstract: behavioral research in dentistry has created possibilities of knowing different patterns of interaction between dentist and patient as well as procedures to improve dental care. the aim of the present study was to analyze the behavior of two dentists and a five-year old child, during clinical dental treatment. dental treatment events were recorded and then described based on a functional analysis framework. behaviors were classified into categories and analyzed in terms of actions-antecedent and consequential events. results showed that dental treatment involves a bi-directional control of patient and dentists' behavior. instruction and distraction were the main management procedures of cooperative behavior. physical restraint and verbal punishment were often used to control uncooperative behaviors. the results showed that what leads a dentist to have such behavior might be closely related to how the patient behaves as well as how much expertise the dentist has on dental treatment for children.

Abstract: purpose: to determine the relationship between the length of ciliary processes as measured by ultrasound biomicroscopy and the duration, localization and severity of uveitis. methods: we analyzed in this prospective study, 58 individuals including patients with different stages of uveitis and normal patients (112 eyes including 18 normal), between august 2001 and august 2002, at the cleveland clinic foundation - ohio - usa. all patients were referred to ultrasound biomicroscopy by the uveitis department, after a complete ophthalmological examination. a 50 mhz-transducer ubm model 840 (zeiss-humphrey) was used under topical anesthesia and immersion technique. results: the most frequent etiology was idiophatic uveitis (27.8%). recurrent, aggressive and diffuse uveitis lead to significant damage of the ciliary processes. the most significant loss of the ciliary processes was found in the inferior quadrant and the highest in normal patients and in the temporal quadrant. hypotonic eyes have their ciliary processes reduced. conclusions: ultrasound biomicroscopy has proved to be useful in evaluating anatomical changes of ciliary processes in uveitis and in ocular hypotony, and provides a basis to, make recomendations regarding to baseline screening of uveitis patients. patients who are found to have atrophy of the ciliary processes may need a more aggressive treatment for any signs of inflammation to prevent further damage and eventual hypotony.

Abstract: the anxiety manifested by the child during the dentistry treatment constitutes a potentially harmful element to the accomplishment of the treatment. the aim of this study was to describe and analyse 10 mother's behavior during sequential sessions of their children's dental treatment. all sessions were recorded in videotape biped every 15 second interval, indicating the moments that the records of the behaviors would be carried out. a behavioral repertoire general plotting allowed to point common standards to all the participants and specific standards, larger anxiety indicators of some participants. proportionally, the invasive anesthesia procedure evoked escape behaviors indicators larger frequency and/or mothers dodge. the pediatric dentistry, careful with parents' behaviors, can adopt strategies that reduce parents' anxiety, increasing behaviors frequency children's collaborating with the treatment.
