Clinical Application of Dynamic Gait Index-Brazilian Brief Version  [PDF]

Abstract: Introduction: In elderly, the progressive degenerations in the nervous system and vestibular system compromise the human body balance leading to a greater risk of falls. The Dynamic Gait Index is a protocol that predicts risks to falls in seniors. Objective: To analyze the results of the application of the Dynamic Gait Index (DGI)-Brazilian brief proposal and establish correlation with the original version. Methods: Study approved by the Ethics and Research Committee of Federal University of Sergipe (number 0197.0.107000.09). We analyzed 223 files of socially active elderly, female gender, aged ranged 60 to 85 years, mean 68.54 (±7.61) years since 2014. The volunteers were evaluated by the DGI-Brazil. For the statistical analysis, the Pearson Correlation Momentum Correlation Test with Spearman Correlation was used, with p ≤ 0.05 and r = 1.0. Results: Sixty-three (27.8%) of the volunteers presented an altered DGI scores. The statistical analysis indicated that Task 1 and Task 2 did not contribute to decrease the total score in the test. The worst performances occurred in Tasks 3, 5 and 6. There was founded a positive correlation between Tasks 1 and 2; 1 and 8; 3 and 4; 3 and 5; 4 and 5; 2 and 4; 2 and 7. Negative correlation occurred between Tasks 4 and 8, and 6 was not correlated with another Task. Conclusion: The DGI-Brazilian brief version presented an excellent correlation with the original proposal and may be useful in clinical practice as a tool to evidence future risks of falls in the elderly people.

Abstract: objective: to compare the accuracy of spm and visual analysis of brain spect in patients with mesial temporal lobe epilepsy (mtle). method: interictal and ictal spects of 22 patients with mtle were performed. visual analysis were performed in interictal (visual(inter)) and ictal (visual(ictal/inter)) studies. spm analysis consisted of comparing interictal (spm(inter)) and ictal spects (spm(ictal)) of each patient to control group and by comparing perfusion of temporal lobes in ictal and interictal studies among themselves (spm(ictal/inter)). results: for detection of the epileptogenic focus, the sensitivities were as follows: visual(inter)=68%; visual(ictal/inter)=100%; spm(inter)=45%; spm(ictal)=64% and spm(ictal/inter)=77%. spm was able to detect more areas of hyperperfusion and hypoperfusion. conclusion: spm did not improve the sensitivity to detect epileptogenic focus. however, spm detected different regions of hypoperfusion and hyperperfusion and is therefore a helpful tool for better understand pathophysiology of seizures in mtle.
Microsatellite Instability in Head and Neck Squamous Cell Carcinoma: A Study of a Brazilian Population

Abstract: Squamous cell carcinoma (SCC) is the sixth most common solid tumor in the world. Apart from known risk factors for head and neck SCC (HNSCC), there is a lack of information about genetic susceptibility regions that may play pivotal roles in the tumorigenesis of these tumors. Therefore, we have aimed to analyze the presence of genetic instability in microsatellite markers distributed in the genome. Microsatellite instability (MSI) was found in 6 HNSCC patients, among which only one was detected by the D17S250 marker, whereas the other 5 occurrences (13.5%) were detected by the D3S1611 marker. No instability was found at markers D5S346, D10S197, D11S922, and D11S988. MSI detected by D3S1611 marker was present in 3 (14.3%) moderately differentiated tumors and in 2 (25.0%) poorly differentiated tumors, but no statistical significance was found. Genotypic frequencies for all markers showed no statistically significant distribution alteration, neither were they related to differentiation grade or patient age. Marker D3S1611 is located in the MLH1 gene, which is part of the mismatch repair system (MMR), helping to maintain genomic stability. We have found a higher rate of D3S1611 MSI in older patients, suggesting that this marker may be affected by aging processes in the DNA repair machinery. 1. Introduction Head and neck cancer is a significant cause of mortality and morbidity worldwide, presenting approximately 600,000 new cases yearly [1], whereas tumors of the oral cavity contribute with 389,000 new cases per year, with a mortality rate of 50% [2]. Squamous cell carcinoma (SCC) is the most common histological variant, comprising 90% of all cases [3]. SCC is the sixth most common solid tumor in the world [4], found preferentially in 50–70-year-old individuals [5]. Nonetheless, some studies point towards a frequency shift towards younger ages, with several cases in men with less than 40 years of age [6]. The epidemiology of head and neck SCC (HNSCC) shows tobacco, alcohol, and Human Papilloma Virus (HPV) infection as the most important risk factors, as well as genetic susceptibility [7–11]. Therefore, the search for genetic markers has increased significantly over recent years. Microsatellite regions are genomic 1–5?bp tandem repeats, generally noncoding, and the CA 2?bp repeat is the most common form [12]. These regions may present instability, especially caused by replication errors introduced by DNA polymerase, a phenomenon known as microsatellite instability (MSI) [13]. Obtaining lactic acid by descontinuous fermentation using different fermentative media

Abstract: Lactic acid has multiple uses in several industries such as food, cereal derivates, beverage, cosmetic, chemical and pharmaceutical. Due to its wide applicability the process to obtain lactic acid is one of the most studied processes. The aim of this study was to produce lactic acid using fermentation of cassava meal (residue from cleaning the flour mill) and cassava starch (amilacious fractions of tuberous root raw materials) previously hydrolyzed and supplemented. The fermentation of both cassava meal and cassava starch was carried out using a solution at 18% (m/v), previously hydrolyzed with thermostable alpha amylase (Termamyl 120L) and amyloglucosidase (AMG 300L); supplemented with yeast extract and peptone. The microorganism, Lactobacillus casei, was inoculated under the following process conditions: pH 6.4; at 37°C and agitation at 100 rpm for 96 hours. The process was periodically surveyed in order to analyze the concentration of lactic acid; concentration of reducing sugars; pH; biomass and cellular feasibility. The analysis of the results permits to conclude that both cassava meal and cassava starch are promising raw materials for obtaining lactic acid by fermentative media.
Influence of Bacillus Stearothermophilus generations in the production of its spores

Abstract: The validation of sterilization processes in food and pharmaceutical industries is a major tool for quality assurance, making the products safe, effective and reliable. Biological indicators, formed by spores of Bacillus stearothermophilus microorganisms considered at higher thermal resistance, are used to evaluate and monitor the physical parameters of a cycle of steam sterilization. In this way this study aimed to cultivate and characterize the microorganism Bacillus stearothermophilus generations, assessing the influence of these generations in the parameters of resistance, formation and concentration of its spores to be used in the production of biological indicators. The experiments were conducted cultivating the 1st, 2nd and 3rd generations of B. stearothermophilus in suitable culture media for sporulation, in Roux bottles, for a period of 15 days of incubation. During these 15 days, the sporulation process was evaluated by microscopy, according to Wirtz-Conklin's method, every 24 hours of incubation. The results showed that the generations do influence the sporulation process, indicating that the 3rd generation is the most suitable for the production of formed spores, in concentration and thermal resistance appropriate characteristics to the needs of a biological indicator to efficiently validate and monitor steam sterilization cycles.

Abstract: Clean-Test self-contained biological indicators are used to monitor steam sterilization cycles. Each biological indicator has a minimum population of 105 or 106 UFC bacterial spores of Geobacillus stearothermophilus ATCC 7953. The purpose of this work was to study the behavior of Clean-Test self-contained biological indicator used to monitor and validate steam sterilization systems, in relation to the incubation time (24, 48 hours and 7 days) and the performance of microbial resistance at different conditions of time, pressure and temperature. The analyses were performed starting with the evaluation of lots of biological indicators by determining the initial population (N0), determination of D value (decimal reduction time), determination of survival and death times for the microorganisms. Then was studied the resistance performance of the biological indicator Clean-Test at different exposure times, temperatures and pressure, as well as the evaluation and analysis of the relationship between incubation time and growth rate for periods of 24, and 48 hours 7 days. The behavior of biological indicator when the temperature and pressure were increased was inversely proportional to D value (microbial resistance). The lot behavior of positive biological indicators by the end of seven days of incubation indicated minimal results of 97%, satisfactory acceptability which is acceptable according to ANSI/AAMI/ISO 11138-1:(2006)because the lots of biological indicators tested met its requirements.

Abstract: purpose: to evaluate the adaptation to hearing aids by users attended by the brazilian public health system (sus), and to propose educational actions, based on the users' needs. methods: descriptive quantitative research, which evaluated 32 adults with hearing loss during the adaptation of hearing aids provided by the unified health system (sus). two structured questionnaires with closed questions were applied, and the data received descriptive analysis. results: moderate limitations and restrictions were observed, as well as reduced time of daily use, limiting the benefit of amplification. regarding use and handling, it was observed that 70.8% put on and take off the device by themselves, 79.2% take it off to bathe, 87.5% do not perform daily hygiene, and 37.5% have allergies or pain attributed to the use of the mold. conclusion: users present difficulties in the use and handling of hearing aids, which negatively influence their adaptation to it. actions to promote hearing health are necessary, with the aim to minimize these difficulties and facilitate hearing aid adaptation.
Screening for inborn errors of metabolism among newborns with metabolic disturbance and/or neurological manifestations without determined cause

Abstract: context: inborn errors of metabolism are hereditary affections resulting from incompetence in enzymatic reactions of intermediary metabolism. at present, several hundred hereditary metabolic disturbances are known, many of which correspond to severe life-threatening disorders. objective: the early detection of carriers has motivated the screening for these disturbances among newborns at the neonatal unit of hospital s?o paulo, in an attempt to initiate support treatment, when available, before clinical manifestations become evident. design: prospective study of risk patients. setting: laboratory for inborn errors of metabolism at the center for medical genetics of the departments of pediatrics and morphology of universidade federal de s?o paulo/escola paulista de medicina.newborn care unit at a tertiary care hospital. participants: 101 children admitted into the neonatal unit were included in this study by presenting hypoglycemia, metabolic acidosis, jaundice, difficulty in gaining weight, diarrhea, vomiting, hepato- and/or splenomegaly, cataracts, apnea, convulsions, hypo- or hypertonia. diagnostic tests: tests routinely utilized, performed for qualitative research of abnormal substances excreted in the urine in situations of metabolic disorder. results: children were included in the study mainly because of presenting hypoglycemia, jaundice and metabolic acidosis. sixty-four newborns presented at least one positive test result. most of the positivity was due to transitory metabolic alterations of the newborn, such as the case of transitory neonatal tyrosinemia, presented by 29 patients. nine infants were referred to the center for medical genetics of universidade federal de s?o paulo for continuation of the diagnostic investigation. for three of them, the tests applied permitted us to formulate a diagnostic hypothesis of mucopolysaccharidosis, tyrosinemia type i and non-ketotic hyperglycinemia, respectively. conclusions: the high positivity observed in the tests re

Abstract: O objetivo deste artigo é analisar a ética do cuidado de enfermagem no tratamento de pacientes oncológicos. Optou-se pela pesquisa bibliográfica de abordagem qualitativa cuja metodologia foi dividida em etapas. Na primeira etapa, foi realizado um levantamento de artigos publicados, no período de 2000 a 2009, em periódicos do Scielo, utilizando os descritores: saúde/doen a, enfermagem, ética do cuidado e oncologia. Esse levantamento culminou com a sele o de 28 artigos, 21 dos quais foram eleitos posteriormente para leitura aprofundada, assim como para utiliza o no estudo. Na segunda etapa, foram explorados dados do campo, com a transcri o e análise de depoimentos de profissionais da enfermagem sobre a concep o de cuidado bem como de dados sobre pacientes oncológicos, acompanhados no período de 2007 e 2008. A pesquisa e suas exigências protocolares, sob o número CAAE - 0147.0.213.000-10, foi aprovada pelo Comitê de ética da PUC Minas em 31 de agosto de 2010. O referencial teórico foi composto por autores que discutem o cuidado na perspectiva filosófica e teológica - Boff (2001, 2005 e 2006), Efken (2005) - e autores da área da saúde oncológica, entre eles, Carvalho et al. (2002), Popim & Boemer (2005), Visentin, Labronici & Lenardt (2007) e Araújo et al. (2009). Os resultados evidenciaram o cuidado na enfermagem como princípio ético essencial, assim como a necessidade de investimentos em políticas públicas de saúde no Brasil, que efetivem o trabalho de preven o, educa o e cura do cancer. Palavras-Chave: Saúde/Doen a; Enfermagem; ética do cuidado; Oncologia. Abstract The objective of this article is to analyze the nursing care in the treatment of oncology patients. We have opted for bibliographical research of qualitative nature, a methodology divided into stages. In the first stage we gathered articles in a publication called Scielo from 2000 to 2009, which described: health/illness, nursing, ethics of care, and oncology. Through this process the selection of twenty-eight articles was accomplished, however only twenty-one articles were utilized in the production of this study. In the second stage field data were explored, we transcribed and analyzed the statements of nursing professionals on the conception of care and on oncology patients overseen by them in the period of 2007 to 2008. The research protocol and its requirements under the number CAAE - 0147.0.213.000-10, was approved by the Ethics Committee of PUC Minas on August 31, 2010.
