Evaluation of laboratory markers of progression of HIV disease to death

Abstract: introduction: one of the important current problems in hiv/aids infection is the establishment of epidemiological and laboratorial prognostic parameters during patient follow-up. this study aimed at analyzing the evolution of laboratory tests: cd4 lymphocyte count, viral load, hemoglobin (hb), aspartate aminotransferase (ast), alanine aminotransferase (alt), and the epidemiological variables sex and age as prognostic factors for survival in progression to death among aids patients. methods: a retrospective study was conducted using analysis of medical records, and prospective 24-month follow-up of patients with hiv/ aids attended at the president vargas hospital outpatient clinic, a reference center in hiv/ aids attendance in the state of maranh?o, brazil. the study analyzed patients aged 10 to 60 years old, who manifested aids and who were not using antiretroviral therapy or had used it for less than 5 years. the chi-square test was used for statistical analysis. results: the sample included 100 patients - 57 were current outpatients, and 43 had died. the variables viral load (p=0.726), alt (p=0.314), sex (p=0.687), and age (p=0.742) were analyzed, and no evidence of association between them and worst prognosis was observed. conclusions: a significant relation was verified between low hb levels (p=0.000) and cd4 (p=0.000) and shorter survival.

Abstract: A ciência da Administra o relegou por muitos anos a responsabilidade de avaliar, discutir e criar ferramentas voltadas às organiza es do chamado Terceiro Setor, deixando que as demais Ciências Sociais dominassem a temática. A partir da década de 90, pesquisadores da Administra o voltaram a dedicar-se à análise dos fatores que influenciam a evolu o da gest o no Terceiro Setor. Neste estudo deu-se aten o especial ao histórico que explica as diversas fases de liga o das Organiza es N o Governamentais com a Igreja, com o Estado, e por último com o Setor Empresarial. Foram analisados os aspectos de gest o do Instituto Socioambiental árvore, através do enfoque qualitativo, utilizando método do estudo de caso que se caracteriza por ter como objeto uma unidade que se analisa profundamente. Quanto aos fins, a pesquisa foi exploratória e aplicada. Quanto aos meios, a pesquisa foi bibliográfica e documental. Foi possível descrever características fundamentais da gest o ligadas ao pensamento sistêmico; aprendizado organizacional; cultura de inova o; lideran a e constancia de propósitos; orienta o por processos e informa es; vis o de futuro; gera o de valor; valoriza o das pessoas; conhecimento sobre o cliente e o mercado; desenvolvimento de parcerias; e responsabilidade social. Concluiu-se em análise genérica a organiza o em estudo mostrou-se conectada com a evolu o da gest o no Terceiro Setor, principalmente pela gama de relacionamento através de redes sociais, e principalmente pelo seu envolvimento com organismos de fomento a excelência de gest o.
Recovery of silver residues from dental amalgam

Abstract: dental amalgam residues are probably the most important chemical residues generated from clinical dental practice because of the presence of heavy metals among its constituents, mainly mercury and silver. objective: the purpose of this study was to develop an alternative method for the recovery of silver residues from dental amalgam. material and methods: the residue generated after vacuum distillation of dental amalgam for the separation of mercury was initially diluted with 32.5% hno3, followed by precipitation with 20% nacl. sequentially, under constant heating and agitation with naoh and sucrose, the sample was reduced to metallic silver. however, the processing time was too long, which turned this procedure not viable. in another sequence of experiments, the dilution was accomplished with concentrated hno3 at 90oc, followed by precipitation with 20% nacl. after washing, the pellet was diluted with concentrated nh4oh, water and more nacl in order to facilitate the reaction with the reducer. results: ascorbic acid was efficiently used as reducer, allowing a fast reduction, thus making the procedure viable. conclusions: the proposed methodology is of easy application and does not require sophisticated equipment or expensive reagents.
Effect of short-term ornithine alpha-ketoglutarate pretreatment on intestinal ischemia-reperfusion in rats

Abstract: purpose: to investigate the effects of preventive enteral administration of ornithine alpha-ketoglutarate (okg) in an ischemia-reperfusion rat model. methods: sixty rats were randomized into five groups (g1-g5, n = 12). each group was divided into two subgroups (n = 6) and treated with calcium carbonate (caca) or okg by gavage. thirty minutes later, the animals were anesthetized with xylazine 15mg + ketamine 1mg ip and subjected to laparotomy. g1-g3 rats served as controls. rats in groups g4 and g5 were subjected to ischemia for 30 minutes. ischemia was achieved by clamping the small intestine and its mesentery, delimiting a segment of bowel 5 cm long and 5 cm apart from the ileocecal valve. in addition, g5 rats underwent reperfusion for 30 minutes. blood samples were collected at the end of the laparotomy (g1), after 30 minutes (g2, g4) and 60 minutes (g3, g5) to determine concentrations of metabolites (pyruvate, lactate), creatine phosphokinase (cpk), thiobarbituric acid reactive substances (tbars) and glutathione (gsh). results: there was a significant decrease in tissue pyruvate and lactate and plasma cpk levels in okg-treated rats at the end of reperfusion period. gsh levels did not change significantly in ischemia and reperfusion groups. however, tbars levels increased significantly (p<0.05) in tissue samples in okg-treated rats subjected to ischemia for 30 minutes. conclusion: short-term pretreatment with okg before induction of i/r decreases tissue damage, increases pyruvate utilization for energy production in the krebs cycle and does not attenuate the oxidative stress in this animal model.
Tissue response to a membrane of demineralized bovine cortical bone implanted in the subcutaneous tissue of rats

Abstract: the treatment of persistent bone defects has encouraged the search for proper techniques or bone substitutes. in dentistry, a common problem in the treatment of periodontal bone defects is the growth of tissues within the lesion, such as the junctional epithelium, which impair regeneration of these tissues. guided tissue regeneration (gtr), based on the separation of the tissues by means of membranes or barriers, was developed in an attempt to improve periodontal regeneration. the aim of this study was to histologically evaluate the tissue response to a membrane of demineralized bovine cortical bone implanted in the subcutaneous tissue of rats. the study periods were 1, 3, 7, 15, 30 and 60 days after implantation. analysis of the histological sections demonstrated a moderate to intense inflammatory response at 1 and 3 days, moderate at 7 and 15 days, and almost absent at 30 and 60 days. resorption of the membrane began 15 days after implantation, and at 60 days only remnants could be detected in some animals. we concluded that the demineralized bovine cortical bone membrane was well tolerated by the tissues and is completely resorbed after 30-60 days by mononuclear cells and multinucleated giant cells, which disappear upon completion of the process.

Abstract: objective: the purpose of this study was to determine the place of stay at postoperative and to verify medical-surgical complications that would justify admission to the intensive care unit, including death. methods: cross-over, prospective, open study that evaluated 120 patients who were submitted to primary bariatric surgery by video laparoscopy from may 2007 to april 2008 in a tertiary hospital. the aldrete kroulik index was used for release from the post-anesthesia recovery room and to define where the patient should be routinely referred for postoperative. results: among the 120 patients, 83 were women and 37 men with a mean age ranging from 35.4 ± 10.5 years (18 to 66 years), body mass index 45.6 ± 10.5. the time between hospital admission and start of surgery was 140.7 ± 81.8 minutes, surgery time was 105 ± 28.6 minutes, time of post-anesthesia recovery room was between 125 ± 38 minutes and length of hospital stay was 47.7 ± 12.4 hours, with 100% of the patients walking in 24 hours. the aldrete and kroulik index in the post-anesthesia recovery room achieved scores of 10 to 120 minutes in all patients, with a 100% survival . conclusion: using the aldrete and kroulik index in the post-anesthesia of gastric bypass by video laparoscopy in primary bariatric surgery, no patient was admitted in intensive care unit and no major complication was observed.

Abstract: This article approaches the development of journalism as well as the journalists work in Brazil as they are seen in Nelson Rodrigues memoirs, which were published as a book in 1993, untitled A Menina sem Estrela. In the first part, there is a short division of the history of journalism in periods. Then there comes a discussion of the way this activity is presented in the authors drama. The following part deals with the theory of fiction that support Rodrigues texts. Finally all attention is concentrated on the way journalism and journalists are presented in his memoirs.
Associations between rehabilitants’ functional limitations and conclusions made by a multidisciplinary team during vocational rehabilitation evaluation: Turku ICF Study  [PDF]

Abstract: Purpose: To investigate associations between functional limitations experienced by rehabilitants and multidisciplinary team’s recommendations during vocational rehabilitation evaluation. Methods: The descriptions of functional limitations were retrospectively identified for 32 patients with chronic musculoskeletal disorders. Vocational rehabilitation evaluation was conducted by a multidisciplinary team in an outpatient clinic of a university hospital. The descriptions were converted to ICF second-level categories. Square contingency coefficient Phi (φ) was calculated to describe the associations between ICF-categories appearing ≥10 times in the study sample and the recommendations given by a professional team. Results: In the study sample, 84 different ICF second-level categories were identified (average 18 codes/subject, range 9 - 25). Of them, 18 categories were observed for at least in 10 rehabilitants. Three statistically significant correlations were observed between these 18 categories and recommendations given by a rehabilitation team. Of them, positive association of moderate strength was found between joint mobility and muscle power functions and recommendation for retirement. Difficulties in walking showed weak negative correlation with recommendation for vocational rehabilitation. Conclusions: During vocational rehabilitation evaluation, the final recommendations given by rehabilitation professionals were only weakly correlated with functional limitations experienced by rehabilitants.

Abstract: the aim of this study was to compare the selection criteria based on the average daily gain (adg) and days to achieve 160 kg (d160) on pre-weaning of guzerat cattle. database on weight gains provided by the brazilian association of zebu cattle breeders (abcz) was used. the estimated genetic parameters and breeding values was obtained by the restricted maximum likelihood method, using a uni-trait animal model, with equations of mixed models. the model included the fixed effect of contemporary group, genetic additive direct and permanent environment, as random effects, beyond the residual error. the means for d160 and adg were 270.5 days and 642.3 g, respectively. spearman's correlations for pre-weaning adg and d160 (d160p), and adg and d160p corrected for contemporary group effect (d160pc), were equal to 0.92 and 0.94, respectively. the selection for d160p would favor sires that produced superior and less variable progenies. the correction of d160 to the contemporary group effect made this trait more efficient to identify more uniform sires. sires classification changed according to the adopted selection criterion, adg or d160p, mainly at the end points, where animals should be selected and rejected.
