An Experience of Short-Term Results of Laparoscopic Inguinal Hernioplasty Using 3D Mesh in a Developing Country  [PDF]

Abstract:
Background: The experience of short term results of laparoscopic inguinal hernia repair using 3D mesh in a developing country is reviewed. Methods: From January 2012 to February 2014, 53 patients underwent laparoscopic inguinal hernioplasty. A retrospective case series of 53 consecutive patients undergoing TEP/TAPP by a single surgical team was followed prospectively with a focused physical examination and interview. 4 out of 53 patients had recurrent hernia following open repairs and 49 had primary hernias. Data collected included operative time, intraoperative bleeding, intraoperative difficulties, immediate postoperative pain, chronic groin pain, recurrence, sensory disturbance, activity or occupational limitation and personal satisfaction. Results: All the patients were male aged 32 to 75 years with a mean age of 53.5 years. Mean operative time was 37.4 minutes; intraoperative dissection, blood loss were less; and immediate postoperative pain was negligible as assessed by VAS. There was no mortality or major morbidity. Mean follow-up was 12 months (2 to 18 months). Follow-up was completed by interview and physical examination. Hernia was not found to recur during the follow up period. Chronic pain occurred in 2 patients (3.7%), which was mild in nature. Ninety-seven percent of patients were satisfied with their repair and would or had recommended TEP/TAPP to others using 3D Mesh. Conclusions: Short-term results of TEP/TAPP hernia repair using 3D mesh demonstrated to be an effective and safe procedure with low prevalence of chronic pain that is generally of a mild, infrequent nature. It was also concurred that there is decrease in operative time. Manipulation of mesh was significantly reduced. Intraoperative bleeding and use of post operative analgesia was reduced considerably. There was no recurrence, however the cost of the mesh increased the overall cost of the procedure acting as a limiting factor in a developing country.

Abstract: O BJECTIVES: To observe the various histological types of lesions in order to evaluate the different sitesinvolved and correlate the lesions with tobacco use. SETTING: Dental Department of Jinnah PostgraduateMedical Centre Karachi. METHODS: Biopsies from 75 clinically diagnosed patients were examined afterstaining them with haemotoxylin and Eosin, Masson Trichrome, Periodic Acid Schiff reagent, Reticulin andGrocott Gomori stains. RESULTS: N 38 were males and 37 were females . Maximum number of patientssuffering from Leukoplakia were in their fifth and sixth decades with a slight male preponderance. Mixed habitswere found in majority of the patients with buccal mucosa being the dominant site. CONCLUSION: Everywhite lesion of the oral cavity must be thoroughly investigated as it could result from a simple hyperplasticepithelium upto a frank carcinoma.
PREPARATION OF INSTRUCTIONAL MATERIAL FOR DISTANCE TEACHER EDUCATION

Abstract: Distance education is institutional concept of education centered round self-directed learning by means of correspondence courses with an integrated element of communication technology and facilitated with tutorial sessions, seminars etc. to meet need for enlargement of teaching force, improved professional standards and modernization of teaching methods, distance learning system provides alternative ways of gaining established qualifications. The content of such courses cannot depart from the normal; the difference of learning experience arises from the presentation of the course materials and from the situation of students and, therefore, different pedagogic skills are required. The term instructional material is used for the specific items used in a lesson and delivered through various media formats such as video, audio, print and so on. This paper discusses the presentation of instructional material for distance teacher education.
Auto Teller Machine (ATM) Fraud – Case Study of a Commercial Bank in Pakistan

Abstract: ATM occupies an important position in the e-Banking portfolio. It has given the consumers a quality of life allowing them to access cash and other financial information. Its role in promoting, developing and expanding the concept of ‘Anytime Anywhere Anyplace” banking is undeniable. It offers a real convenience to those who are on the run in their everyday life, but at the same time, it also carries a big element of risk. In this paper we have investigated and demonstrated a mapping flaw (bug) in the ATM Controller (commonly known as financial middleware), which allows the ATM card holders of various banks to fraudulently withdraw cash from the ATMs of ACB Bank Limited. The flaw remained undetected for nearly 3 months. Since the breach has been thoroughly investigated, we, therefore, concluded that the banks’ internal control system had failed to detect the implantation of mapping bug which deprived the bank of more than 21 million Pakistani Rupees. In addition, lack of understanding of higher management on the systems & procedures supporting ATM Infrastructure played a significant role in developing the bug. Considering the nature of the fraud and the degree of losses incurred, this paper has recommended strong internal controls implementation over the payment system applications. A detailed review of fraud screening strategy is also recommended to ensure that the security tools are optimized for their particular product or service. Turnkey ATM solution has also been recommended for the ACB Bank Limited.
An Evaluation of Teaching Practice: Practicum

Abstract: Teaching practice is an important place in teacher training, it is the theoretical part of teacher training. The purpose of this study was to evaluate the teaching practice, for the purpose a 22 item questionnaire was developed and pilot tested, the internal consistency (Cronbach Alpha) value of the questionnaire was 0.73.Five research questions were made for the study. 650 student teachers throughout the country from 26 teacher training institutions were considered the sample of the study, by selecting 25 student teachers randomly from each institution. The data collected through questionnaire was coded and analyzed through SPSS XII and Stat Pac calculator in terms of frequency, percentage and chi square. The study finds that majority of the student teachers is not given demonstration lessons in all the subjects, duration of teaching practice is not appropriate to develop teaching skills, the method of evaluation in practical component is not appropriate, choice of the student teachers is not considered while assigning the lesson, proportion of theory and practice not appropriate. It is recommended that that the student teachers should be given demonstration lessons before practice teaching, the duration of the teaching practice may be increased, theory and practice should be made proportionate, the student teachers should be made responsible for what they do, this can only be achieved if their choice in the selection of lesson is also considered.
Effect of Storage on Physico-Chemical Composition and Sensory Properties of Mango (Mangifera indica L.) Variety Dosehari

Abstract: The effect of storage on physico-chemical changes such as weight loss (wt. loss), total soluble solids (TSS), titratable acidity (TA) and sensory evaluation such as skin color (SKC), flesh color (FLC), texture (TEX), taste (TAS) and flavor (FLA) of packaged Dosehari mango in cardboard carton of export quality, having one hole in each sidewall on four sides confronting each other, in order to maintain the modified atmosphere were recorded after an interval of 3 days at ambient temperature (32-35°C with 53.6-78.8% RH). The data obtained were statistically analyzed for Analysis of Variance (ANOVA) by using 2-Factorial Complete Randomized Design (CRD) and Duncan`s Multiple Range Test (DMRT) was applied to compare the mean values obtained. A significant effect of storage (P<0.05) on Dusheri variety of mango was observed and had an increasing trend of average percent wt. loss (0.00 to 36.1 %), TSS (10 to 25.27 %), and decreasing trend of percent TA (0.5% to 0.094 %) with an average mean of 15.67 %, 11.55 % and 0.28% respectively during 15 days of storage period. The SKC score was increased from 5.0 to 8.70, FLC (5.0 to 8.44), TEX (6.00 to 8.68), TAS (5.00 to 8.46) and FLA score was increased from 5.0 to 7.61 with an average means of 4.22, 4.01, 4.43, 4.32, and 3.84 respectively during 15 days of storage at ambient temperature. It was also observed that in general the SKC score had increased up to (6.91) at 6th day, FLC (5.73), TEX (6.83), TAS (6.07) and FLA (5.67) score had increased at 3rd day of storage and then gradual decreased to 1.25, 1.83, 1.35, 1.67 and 1.57 respectively at 15th days of storage, therefore, showing an increasing trend first and then significant decreased of SKC, FLC, TEX, TAS and FLA score respectively during storage. Whereas control showed higher percentage of wt. loss (19.88%), lower retention of TSS (9.43%) or TA (0.15%), and very low score of other quality parameters such as SKC (1.71), FLC (1.58), TEX (1.82), TAS (1.61) and FLA (1.47), respectively during 15 days of storage period.
CORRELATION BETWEEN MARKS OBTAINED BY STUDENTS IN ASSIGNMENTS AND FINAL EXAMINATION OF MASTER OF EDUCATION CONDUCTED BY ALLAMA IQBAL OPEN UNIVERSITY

Abstract: The main objective of the research was to find out the correlation between the marks obtained by the students of Allama Iqbal Open University in assignments and final examination of M.A (Education) M.Ed in Autumn 2006.For this purpose, the result gazette of M.A (Education) M.Ed was got from regional campus Allama Iqbal Open University, Bahawalpur. All the students who appeared from this region were taken as sample. The value of correlation in the subject, Elementary Education was 0.228, in Secondary Education -0.033, in Higher Education was 0.069 and in Teacher Education in Pakistan was 0.154.There were 16 students who got more than 80 marks in assignments but they were fail in the final examination because they did not get at least 40 marks in the final examination.
A STUDY OF STUDENT’S ATTITUDE TOWARDS DISTANCE TEACHER EDUCATION PROGRAMME IN PAKISTAN

Abstract: ABSTRACTThe present study was undertaken to measure the attitude of the students towards teacher training programmes through Distance Education offered by Allama Iqbal Open University (AIOU), Islamabad, Pakistan. To achieve the said end, the descriptive method of research was considered appropriate. The population of the study consisted of the B. Ed. Students of AIOU. Three hundred students from the Sahiwal region were selected as sample for the study. Questionnaire was used as a Research tool to gather the relevant data. Data was analysed in terms of percentage and mean score. It was found that majority of the students appreciated the overall input of the programme though a few eyebrows were raised concerning the assessment of the assignments.
COMPARATIVE STUDY OF STUDENT SUPPORT SERVICES OF AIOU AND UKOU

Abstract: ABSTRACT This paper attempts to compare the availability, quality, similarities and differences of student support services in Allama Iqbal Open University (AIOU) and United Kingdom Open University (UKOU) and also to identify and enlist the deficiencies that AIOU students are facing in the student support services. The study found out that student support services of AIOU are quantitatively developing rapidly on the lines of UKOU. Though the regional campuses of both the institutions have almost the same status in the provision of student support service yet the UKOU students have better services in the guidance and counseling, modern communication facilities and career guidance. Moreover, there also exists Open University student association in UKOU. The conclusions led to the recommendation that AIOU regional campuses may be made independent like UKOU, counseling and guidance cell might be established at every regional campus, modern communication facilities like toll free, auto answer may be provided at AIOU regional campuses.

Abstract: In 2004, Das, Saxena and Gulati proposed a dynamic ID-based remote user authentication scheme which has many advantage such as no verifier table, user freedom to choose and change password and so on.However the subsequent papers have shown that this scheme is completely insecure and vulnerable to many attacks. Since then many schemes with improvements to Das et al’s scheme has been proposed buteach has its pros and cons. Recently Yan-yan Wang et al. have proposed a scheme to overcome security weaknesses of Das et al.’s scheme. However this scheme too is vulnerable to various security attacks such as password guessing attack, masquerading attack, denial of service attack.
