Barotrauma pulmonar no intra-operatório de procedimento cirúrgico oftalmológico: relato de caso

Abstract: background and objectives: nowadays, severe anesthetic complications caused by the improper use of mechanical ventilators are rare. however, technical details even in recent models can be a trap for the anesthesiologist and threaten patient safety. the objective of this report was to demonstrate the importance of a careful analysis of the device to be used, as well as to detect and treat intraoperative tension pneumothorax. case report: a 16-year old female patient, physical status asa i, underwent corneal conjunctival covering under general anesthesia. anesthesia was maintained with isoflurane and controlled mechanical ventilation. no abnormalities were observed during anesthesia. at the final phase of the surgery, after mobilizing the anesthesia device to start the awakening process, the patient developed hypoxia, hypertension and ventilatory difficulties. after removal of the sterile drapes from the surgical field, subcutaneous emphysema was evident in the face, neck and upper limb. the tracheal cannula, which contained blood, was changed. a chest x-ray confirmed the diagnosis of pneumothorax that was immediately drained. inspection of the equipment revealed the presence of a kink in the tubing connecting the inferior portion of the canister to the equipment itself caused by mobilization of the articulated arm, blocking the normal flow of gases and leading to pulmonary barotrauma. conclusions: the development of tension pneumothorax during general anesthesia with positive pressure ventilation should always be considered. several factors can contribute to the development of this condition, which should be considered when they are present during surgeries. the anesthesia equipment should be examined carefully to detect potential causes of anesthetic complications.
Influência da renda sobre as despesas com alimenta??o fora do domicílio, no Brasil, 2002-2003

Abstract: this study describes and evaluates the influence of income on the percentage of food expenditures away from home for brazilian families. food acquisition data from the national household budget survey conducted from 2002 to 2003 (pof 2002/2003) by the brazilian institute of geography and statistics (ibge) or national census bureau was used in the analysis. information on food-and-drink expenditures away from home was analyzed. the influence of income on the share of food purchased away from home in the household budget, adjusted for socio-demographic variables, was analyzed through elasticity coefficients estimated in multiple linear regression. food purchased away from home accounted for 21% of total food expenditures by brazilian households. a 10% increase in income increased the share of food purchased away from home by 3%. income elasticity was high, especially for the lowest income families. the results demonstrate an important influence of income on food expenditures away from home, and higher income is associated with a greater share of food purchased away from home.

Abstract: food choice in diet composition is a determinant of individual health status. currently, there are gradually fewer conditions involving food scarcity and more involving excess food. changes in income and relative prices generate measurable effects on the population's food intake patterns. economic models have significant explanatory power for food demand, and the interactions between consumption, income, and prices are usually expressed as elasticity. however, the construction of some studies shows important shortcomings, especially for public policy application. this conceptual article discusses the potential contribution of food demand studies, suggesting improvements in the structural design of such studies with the inclusion of current nutritional concepts for redirecting the nutritional transition from under-nutrition to healthy eating, avoiding the present trend towards epidemic obesity.

Abstract: environment noise pollution is common place today, at intolerable levels. in hospitals, technological developments have, as a consequence, potentially harmful noise levels. much of the hospital noise comes from inside, rather than outside, and the major source of such noise is the intensive care unit, for example equipment and hospital staff talk. our goal with the present study was to investigate the noise level present in the different hospital environments, within a 222 bed hospital located at the 18th health zone, pr. materials and methods: the study was carried out in march, 2005, during a period of 24 hours, in tem different sectors. case study: we checked environmental sound level by means of a model 1350 decibel meter. results: the sound level found in our study was of 63.7 db(a) in average, which exceeds the 45 db recommended by the brazilian association of technical standards (1987). conclusion: in the analyzed sectors, the sound level was considerably above the recommended maximum. the hospital staff should be aware of this noise level and its effects, so that they may act in a more efficient way in order to reduce this noise pollution; thus benefiting the professionals and patient recovery.

Abstract: Este artigo examina como políticas públicas afetam o comportamento das empresas com rela o à inova o. Através do método de pesquisa de estudos de caso, o trabalho identifica como as empresas, em dois setores industriais no Brasil, têm respondido aos esfor os governamentais para estimular sua capacita o tecnológica. S o comparadas empresas do setor de energia elétrica do Brasil e do setor de informática do polo industrial de Manaus. De forma geral, os resultados indicam que as políticas de inova o têm contribuído para aumentar o interesse das empresas por inova o. No entanto, esse interesse tem se mostrado maior por parte das empresas cujo controle de capital é estatal e/ou estrangeiro, do que pelas empresas de controle de capital privado nacional. Isso é particularmente relevante, pois as empresas de capital privado nacional atuam em mercados pouco regulamentados, em que o nível de competi o é crescente. O limitado interesse dessas empresas por inova o refor a a ideia de que políticas de inova o n o podem englobar apenas a es voltadas para o lado da oferta, nem podem ser dissociadas de políticas voltadas para o fortalecimento do ambiente institucional. This article examines how public policies to foster innovation affect companies' behavior. Based on the case study research method, the article identifies how firms operating in the Brazilian electric power and IT sectors of the Manaus industrial district have responded to public efforts to stimulate technological innovation in both sectors. In general, the results suggest that these policies have increased private interest for innovation. However, this interest is greater among state controlled and multinational corporations than among domestic private companies. This finding is particularly relevant because domestic companies operate in deregulated markets with increasing competitive pressures. The limited interest of domestic companies for innovation suggests that innovation policies should not be limited to supply side factors and cannot be dissociated from policies aimed at strengthening the institutional environment.
Per capita versus adult-equivalent estimates of calorie availability in household budget surveys

Abstract: this study aims to estimate an adult-equivalent scale for calorie requirements and to determine the differences between adult-equivalent and per capita measurements of calorie availability in the brazilian population. the study used data from the 2002-2003 brazilian household budget survey. the calorie requirement for a reference adult individual was based on the mean requirements for adult males and females (2,550kcal/day). the conversion factors were defined as the ratios between the calorie requirements for each age group and gender and that of the reference adult. the adult-equivalent calorie availability levels were higher than the per capita levels, with the largest differences in rural and low-income households. differences in household calorie availability varied from 22kcal/day (households with adults and an adolescent) to 428kcal/day (households with elderly individuals), thus showing that per capital measurements can underestimate the real calorie availability, since they overlook differences in household composition.

Abstract: objective: to describe the regional and socio-economic distribution of consumption of added sugar in brazil in 2002/03, particularly products, sources of sugar and trends in the past 15 years. methods: the study used data from household budget surveys since the 1980s about the type and quantity of food and beverages bought by brazilian families. different indicators were analyzed: % of sugar calories over the total diet energy and caloric % of table sugar fractions and sugar added to processed food/ sugar calories of diet. results: in 2002/03, of the total energy available for consumption, 16.7% came from added sugar in all regional and socio-economic strata. the table sugar/ sugar added to processed food ratio was inversely proportional to increase in income. although this proportion fell in the past 15 years, sugar added to processed food doubled, especially in terms of consumption of soft drinks and cookies. conclusions: brazilians consume more sugar than the recommended levels determined by the who and the sources of consumption of sugar have changed significantly.
Efeitos de níveis crescentes de fibra em detergente neutro na dieta sobre a digest?o ruminal em bubalinos e bovinos

Abstract: four buffaloes and four cattle rumen fistulated were used with the objective to study the effects of different levels of neutral detergent fiber in the diet on the digestion characteristics. the in situ degradations of dry matter (dm), neutral detergent fiber (ndf) of the coastcross hay and wheat meal, dm of the corn ground and soybean meal were evaluated. the digestibilities of dm, crude protein and ndf of the diets were also evaluated using a cr2o3 as a marker. the animals were allotted to two in a 4x2 factorial arrangement with four increasing levels of ndf in the dm (54, 60, 66 and 72%) and two animal species (buffaloes and cattle). four twenty-nine days subperiods were used, with 13 days for diet adaptation. the buffaloes showed higher ruminal dm and ndf disappearance of the coast-cross hay and soybean meal dm than cattle, in most of the incubation times. on the other hand, the ruminal ndf disappearance of wheat meal was higher for the cattle only in the first hours. there was interaction between species and ndf levels for the dm digestibility coefficients. the ndf levels influenced the ndf digestibility coefficient and it was not noticed effects of species or ndf levels for the cp digestibility coefficient.
Evolu??o da disponibilidade domiciliar de alimentos no município de S?o Paulo no período de 1979 a 1999

Abstract: objective: food budget surveys data were analyzed in order to describe the evolution (1971-1999) of food and nutrient availability in s?o paulo, brazil. methods: the study uses data from two family budget surveys conducted by funda??o instituto de pesquisas econ?micas, carried out during the periods of 1971-1972 (2,380 households) and 1998-1999 (2.351 households) in the urban area of s?o paulo city. per capita daily food availability was estimated by calculating the fraction of all the food bought for the month attributed to each participant of this study divided by the number of days in the month. the food patterns of this population were characterized according to the caloric and nutrient distribution of the 18 food groups and nutrients obtained in the respective surveys. results: the results show increased consumption of meats, dairy, processed foods and chilled and frozen meals. on the other hand, the consumption of grains, fruits and vegetables decreased. the consumption of sugar, oils and fats reduced slightly but since the consumption of processed foods increased, it is not possible to say that diet quality improved. at the same time the consumption of proteins, fats and calcium increased while the consumption of carbohydrates, fiber, folate and vitamin c decreased. conclusion: the changes in the eating patterns of the population of s?o paulo are consistent with the growing prevalence of obesity and non-communicable chronic diseases observed in the city.
Efeitos de níveis crescentes de fibra em detergente neutro na dieta sobre a digest o ruminal em bubalinos e bovinos

Abstract: Quatro bubalinos e quatro bovinos adultos com fístulas ruminais foram utilizados com o objetivo de se estudar os efeitos de diferentes níveis de fibra em detergente neutro na dieta sobre as características digest o ruminal. Foram avaliadas as degradabilidades da matéria seca, fibra em detergente neutro do feno de coastcross e do farelo de trigo, matéria seca do milho em gr os moídos e do farelo de soja; digestibilidade com indicador (Cr2O3). Os animais foram delineados em dois quadrados latinos (4x4) com arranjo fatorial 4x2, sendo quatro níveis crescentes de FDN na MS (54, 60, 66 e 72%) e duas espécies (bubalinos e bovinos). Cada subperíodo compreendeu 29 dias, sendo 13 de adapta o. Os bubalinos apresentaram maior desaparecimento ruminal da MS e FDN do feno de coastcross e da MS do farelo de soja que os bovinos, na maioria dos tempos de incuba o. Por outro lado, o desaparecimento ruminal da FDN do farelo de trigo foi maior para os bovinos, somente nas primeiras horas. Para o coeficiente de digestibilidade da MS, houve intera o de espécies com níveis de FDN. O coeficiente de digestibilidade da FDN foi influenciado pelos níveis de FDN e, para o coeficiente de digestibilidade da PB, n o foram notados efeitos de espécies ou níveis de FDN.
