Surgical treatment of mitral valve insufficiency by valve repair

Abstract: OBJECTIVE: To analyze the short term results of mitral valve repair in a consecutive series of mitral insufficiency patients from different ethiologies. METHODS: A retrospective study was made of 86 patients with mitral insufficiency operated on between May 1992 and May 2001 for mitral valve repair. Mitral insufficiency was severe in 77 patients and moderate in 9. The functional class of the patients was I in 4, II in 48, III in 29 and IV in 5 patients. The etiology was rheumatic in 47 (54.6%) cases. RESULTS: Mitral valve repair was performed by only one procedure on the mitral valve in 6 patients, two procedures in 29, and three or more procedures in 51 (59.3%) patients. Mitral annuloplasty was performedin 81 patients, with the Braile posterior pericardial ring being the mostcommonly used (87.2%). Hospital mortality was 3.5%. There was improvement in the funcional class in 79 (91.8%) patients. Mitral valve function was normal in 80 (93%) patients and moderate mitral insufficiency occurred in 6 patients. CONCLUSION: Mitral valve repair can be performed with low mortality (3.5%) and high probability of valve function recovery (93%). It should be the procedure of choice in patients with mitral insufficiency.

Abstract: A deficiência de vitamina A constitui um problema de saúde pública no Nordeste brasileiro. O objetivo deste estudo foi determinar a prevalência da deficiência de vitamina A e os fatores associados em pré-escolares de Teresina, Piauí, Brasil. Os níveis de retinol sérico foram determinados por HPLC, e foram investigadas as características sócio-econ micas e demográficas de 631 crian as com idade de 36 a 83 meses. Investigou-se a associa o entre os níveis de retinol e as variáveis de interesse por análise de regress o linear uni e multivariada. O nível médio de retinol foi de 1,21mmol/L (IC95%: 1,17-1,25μmol/L), independente do sexo (p = 0,259). A hipovitaminose A (retinol < 0,69μmol/L) foi observada em 15,4% das crian as (IC95%: 12,7-18,4), com tendência à diminui o com o avan o da idade; 29% das crian as (IC95%: 25,2-32,4) tinham valores aceitáveis de retinol, mas n o adequados (0,70 a 1,04μmol/L). Encontrou-se associa o positiva entre níveis de retinol e idade, renda per capita, suplementa o prévia com vitamina A e escolaridade materna. A prevalência de hipovitaminose A representa um problema moderado de saúde pública, ressaltando a importancia das estratégias de combate a essa carência na regi o.
Epidemiologia dos acidentes ofídicos notificados pelo Centro de Assistência e Informa??o Toxicológica de Campina Grande (Ceatox-CG), Paraíba

Abstract: accidents caused by venomous animals are still a public health problem in brazil. although the production and distribution of antivenom is controlled, several regions have difficulties in reporting this type of accident. considering such fact, a cross-sectional study was carried out using indirect files on venom accidents that occurred in campina grande, along with 80 small towns comprising the region, from january to december 2005. the main objective was to outline a better epidemiological and clinical profile of these cases. all patients admitted to the poisoning information center of campina grande (ceatox-cg) had their injuries medically diagnosed as injuries caused by venomous snakes. data collection was based on the use of a reporting form by the national reporting system - sinan (sistema nacional de notifica??o de agravo do ministério da saúde). data analysis consisted of descriptive statistics using microsoft excel 2003. a number of 737 out of 1,443 patients who received care at the medical center had their cases related to venomous animals, and 277 of the accidents had been caused by venomous and non- venomous snakes. most accidents involved men, from 10 to 29 years of age, including peasants in rural areas, from may to november 2005. most of the cases (71.5%) involved the bothrops genus and upper and lower limbs, especially feet. as to severity, most of the cases were classified as mild, caused by the bothrops genus, and only one patient died. the average length of time spent in providing care to the victims was over 12 hours in severe cases caused by bothrops and crotalus. our results agree with the national epidemiological profile of snakebites, which are more frequent amongst male peasants aged from 10 to 49, affecting especially lower limbs, caused most often by the bothrops genus.

Abstract: background: lichen sclerosus is characterized by a collagenous hyaline/homogeneous zone at upper dermis, which remains undefined morphologically and biologically. our previous investigation showed that there are several changes at the extracellular matrix hyaline zone, such as accumulation of collagenic proteins (types i and iii) and sulfated proteoglycans/glycosaminoglycans. objectives: decorin and chondroitim sulfate (sulfated proteoglycans/glycosaminoglycans) immunoexpressions were the present investigation′s aim, emphasizing the hyaline zone related changes. methods: thirty one vulvar ls untreated clinical lesions were biopsed and evaluated histologically according to hewitt?s gradation and by immunohistochemical methods. results were compared with ones of the control group, which was formed by cutaneous fragments from vulvoperineal corrective surgeries. results: we could demonstrate that decorin and chondroitin sulfate were present at the hyaline zone in different moments of matrix modulation. in all hewitt stages chondroitin sulfate prevailed at the extracellular matrix in cases with a compact aspect of the hyaline zone while decorin was only seen in areas of less compactness. conclusion: this proteoglycans/glycosaminoglycans synthesis sequence suggests that decorin may be a possible initial marker/indicator for vulvar ls. we suppose either that chondroitin sulfate is possibly a factor that limit matricial changes extension till middle dermis level.

Perfil lipídico e estado nutricional de adolescentes

Abstract: objective: to evaluate the association between obesity and dyslipidemia in adolescents from public and private schools in campina grande, pb, brazil. methods: cross-sectional study of 180 adolescents, aged between 14 and 17 years, enrolled in the public and private school system of campina grande-pb. the nutritional status was classified according to the percentiles of body mass index for gender and age. a blood sample was taken to evaluate the lipid profile (total cholesterol, hdl-cholesterol, ldl-cholesterol, and triglyceride levels). dyslipidemia was considered when at least one of the blood parameters exceeded reference limits. statistical analysis was performed with epi info 3.3 and stata 7.0. results: the prevalence of overweight was 14.4%, while 83.9% of students were normal and 1.7% presented low weight. the nutritional status, when stratified by gender and type of school, did not show statistically significant differences. dyslipidemia was present in 66.7% of students and inadequate levels of hdl-cholesterol in 56.7% of them. the tertiles of body mass index were associated with total cholesterol and the ldl-cholesterol fraction, even when stratified by gender and type of school. conclusions: considering the age range of the sample, the results for overweight and dyslipidemias are of concern. there is a need to implement more effective policies oriented towards these adolescents to prevent children and adolescents becoming obese adults with other chronic diseases.

Abstract: the aim of this study was to assess suicide risk and risk behavior in young people. a cross-sectional study in the urban area of pelotas, rio grande do sul state, brazil was performed using systematic sampling with young people between the ages of 18 to 24 years. we measured the risk of suicide (mini), behavior (yrbss), abuse/substance dependence (assist) and socioeconomic status (abep). data was analyzed using spss software. the sample consisted of 1,560 young people and the prevalence of suicide risk was 8.6%. suicide risk was associated with: having suffered an accident that required a visit to the emergency room (p = 0.011), fighting (p = 0.016), carrying a weapon (p = 0.001) and carrying a firearm (p < 0.001), substance abuse/dependence (p < 0.001), not having used a condom during the last sexual relationship (p = 0.025), not having a steady partner (p < 0.001) and having sex with five or more people (p = 0.018). young people that present risk behavior also represent a suicide risk.

Abstract: objective: to evaluate serum concentrations of retinol and beta-carotene of children in teresina, piauí, brazil, and to evaluate their anthropometric profile and consumption of food sources of vitamin a. methods: cross-sectional study of 135 children from 36 to 83 months of age who attended a public child day care center. nutritional status was evaluated by: biochemical (serum concentrations of retinol and beta-carotene), anthropometric (weight for height - w/h and height for age - h/a indexes), and dietary (frequency of consumption of food sources of vitamin a) methods. results: the prevalence of vitamin a deficiency (vad) was 8.9% (95%ci: 4.7-15.0%). an association between previous supplementation with vitamin a and serum concentrations of retinol was observed, with a higher proportion of children with normal concentrations of retinol among those supplemented (p=0.025). there was a weak to moderate statistically significant correlation between the concentrations of retinol and beta-carotene, (p<0.021). the prevalence of low w/h and low h/a was respectively 1.9% (95%ci: 0.2-6.8%) and 9.7% (95%ci: 4.8-17.1%). the dietary evaluation showed low consumption of foods rich in vitamin a. conclusions: the high prevalence of vad combined to the prevalence of children with acceptable concentrations of retinol, low median concentrations of beta-carotene, low h/a, and inadequate ingestion of foods rich in vitamin a, reinforce the need to improve health and nutrition education in this population. based on these results, we recommend the ingestion of foods rich in vitamin a, as an important policy to deal with this nutritional problem. efforts are also necessary to maintain food fortification and supplementation programs.
Plexopatia lombar após histerectomia abdominal: relato de caso

Abstract: background and objectives: very often, neurological injuries diagnosed postoperatively are attributed to the anesthesia, but they can also be secondary to the surgery itself. the aim of this report was to present the case of a patient who underwent a pelvic surgery and developed lumbar plexopathy, and to discuss the possible causes. case report: a woman patient, 38 years old, 58 kg, 1.63 m, physical status asa i, was admitted to undergo a total abdominal hysterectomy for treatment of uterine leiomyomatosis. initial monitoring included pulse oxymetry, non-invasive blood pressure, electrocardioscope, and urine output. after venous cannulation in the left upper limb with an 18g catheter, cefazoline (2 g), dypirone (2 g), dexamethasone (10 mg), and metochlopramide (10 mg) were administered. with the patient in left lateral decubitus, a 16g tuohy needle was inserted in the l3-l4 space, in the median line, for the epidural block. fifteen milliliters of 0.75% ropivacaine and 2 mg of morphine were administered, followed by placement of the catheter for postoperative analgesia. the surgery was uneventful, and the patient's cardiovascular parameters were stable. eight hours after the procedure, at the postanesthetic follow-up, the patient could not walk and presented monoparesis in the left lower limb. the hypothesis of radicular syndrome was ruled out after clinical and radiological evaluation. since symptoms did not resolve, an electroneuromyography was done 30 days later, and was compatible with lumbar plexopathy, which was possibly caused by trauma. conclusions: the anesthesiologist must be aware of the postoperative neurological complications and should be part of the efforts to diagnose their causes, of the treatment, and its evolution.
