Abstract: Traumatic diaphragmatic rupture (TDR) is very rare in the pediatric age group. Because of its rarity and its coexistence with more injuries, the diagnosis is often delayed. Very little has been written about this condition in the pediatric age group. TDR, while uncommon, should be considered in cases of blunt thoracic trauma. All patients should undergo meticulous examination preoperatively. The clinical presentation and importance of making an accurate diagnosis and surgery is highlighted. We report a case of secondary spontaneous traumatic left-sided diaphragmatic rupture in a child that was managed by delayed surgical repair.

Abstract: The mobile communication is nowadays one of the basic needs of humanity. It is essential to the flourishing of human beings. Considering this reality, the need to use its mobile phone is become more important and diversified. The subscribers of the various mobile telephone operators are increasingly demanding. This situation poses the problems of the cover mobile network to the operators and leads them to opt for several solutions and investments. The mobile operators in order to satisfy their customers use a policy of pushing the limits of network coverage in time and space for festive moments in targeted zones. Thus, we have conducted a study on the topic: study of a kit of GSM radio site for event-driven movable coverage. This work is applied to GSM (Global system mobile) network of the operator Orange-Ci, leader of mobile telephony in Ivory Coast. We thus proceeded under investigation initially of the various aspects of the ordinary sites (motionless radio site) which are already deployed with Orange-Ci in order to impregnate us infrastructures and equipment used. This study revealed us that a radio site comprises 4 parts: infrastructures, installations and energy equipments, installations and radio equipments, and installations and equipment of transmission. After the first analysis, we made a study of the movable site. The study of the movable site enabled us to see the various possible solutions to fulfill the basic functions of a movable radio site. After analysis we retained that our radio site will be built on a truck on which a mast of 25 m maximum length for the antennas will be embarked, it will be fed by a generator also embarked on the truck and the solution of transmission selected is the transmission by satellite more precisely technology VSAT. We choose the various equipments (radio, transmission, energy) according to features which we defined to constitute the kit of movable radio site.

Abstract: Objective: Provide the place of ENT diseases at the Reference Health Center of the Commune V of Bamako. Materials and Method: A cross-sectional study using ENT medical assessment was carried out from January 2017 to December 2017 at the Reference Health Center of Commune V of Bamako’s District. Results: 1911 patients were included and that number represented 15.46% of all non-obstetrics medical consultations in the health center. The sex ratio was 0.83 for women (54.7%). The average age was 22 years old. Otologic diseases were 66.4% of the cases followed by nasal sinus cases (16.6%). Infectious and inflammatory diseases were 74.1% and were the main diagnosed diseases. The difficulties were mainly from technical platform. Conclusion: The diversity of these pathologies required a strong development or reinforcement of human and material resources capacities in of these structures for proximity care management.

Abstract: Introduction: There has been limited experience with Open Heart Surgeries (OHS) in Sub-Saharan Africa. In west Africa especially in Mali, most fledgling centers are unable to overcome the myriad of challenges encountered in establishing OHS though there is a high prevalence of surgically correctable heart diseases. The aim of this paper is to review our initial experience of our first cases in developing OHS program and discuss the challenges and prospects that need to be overcome to further develop it. Methods: A total of 6 patients who underwent OHS during the first “cardiac mission” in July 2016 were included in this retrospective study. The medical records of the patients were examined and data on age, sex, diagnosis, EuroSCORE, type of surgery, cardiopulmonary bypass details, complications and length of hospital stay were extracted. Results: Six patients with a male to female ratio of 1, ages ranging between 12 and 35 years (mean of 22.5 ± 12 years) were studied. The mean of EuroSCORE was 6 ± 41. Pericardial patch closure of isolated atrial septal defect was performed in one patient. One patient had mitral valve repair for rheumatic mitral regurgitation consisting of chordal shortening with a tricuspid valvuloplasty. Three patients had mitral valve replacement with tricuspid valvuloplasty. Four patients had mitral valve replacement. Sixty-day mortality?was 0%. Conclusion: Safe conduct of open heart surgery in Mali Hospital setting is feasible. Grant financial aid is required for rapid growth of Open-Heart Surgery in this part of Sub-Saharan Africa.

Abstract: Primitive cerebral lymphoma (PCL) is a rare entity often poorly known. They are defined as extra nodal tumor interesting the CNS, brain and eye, and this in the absence of systemic lymphoma disease and brain metastases. The favoring factor is established immunosuppression which is explained by an increase in the incidence related to HIV. The basis of treatment is chemotherapy with consolidation radiotherapy. We report the experience of the National Institute of Oncology in Rabat in management of this disease.

Abstract: In this paper, it is proposed to apply the Dempster-Shafer Theory (DST) or the theory of evidence to map vegetation, aquatic and mineral surfaces with a view to detecting potential areas of observation of outcrops of geological formations (rocks, breastplates, regolith, etc.). The proposed approach consists in aggregating information by using the DST. From pretreated Aster satellite images (geo-referencing, geometric correction and resampling at 15 m), new channels were produced by determining the spectral indices NDVI, MNDWI and NDBaI. Then, the DST formalism was modeled and generated under the MATLAB software, an image segmented into six classes including three absolute classes (E,V,M) and three classes of confusion ({E,V}, {M,V}, {E,M}). The control on the land, based on geographic coordinates of pixels of different classes on said image, has made it possible to make a concordant interpretation thereof. Our contribution lies in taking into account imperfections (inaccuracies and uncertainties) related to source information by using mass functions based on a simple support model (two focal elements: the discernment framework and the potential set of belonging of the pixel to be classified) with a normal law for the good management of these.

Abstract: The aim of this work is to present a theoretical study of external magnetic field effect on a bifacial silicon solar cell’s electrical parameters (peak power, fill factor and load resistance) using the J-V and P-V characteristics. After the resolution of the magneto transport equation and continuity equation of excess minority carriers in the base of the bifacial silicon solar cell under multispectral illumination, the photo-current density and the photovoltage are determined and the J-V and P-V curves are plotted. Using simultaneously the J-V and P-V curves, we determine, according to magnetic field intensity, the peak photocurrent density, the peak photovoltage, the peak electric power, the fill factor and the load resistance at the peak power point. The numerical data show that the solar cell’s peak power decreases with magnetic field intensity while the fill factor and the load resistance increase.

Abstract: The Software Defined Network (SDN) is a concept based on a decoupling between the control plan and the data plan of a network. Thus, the network becomes programmable and can be coupled to the business applications of the users. The study that is discussed in this article looks at load planning and balancing in distributed controllers. To do this, a model and theoretical methods of performance evaluation related to appropriate software tools, to predict and control the quality of service offered to users is exposed. This paper exposed also a distributed architecture of controllers and then a module based on an adaptive load balancing algorithm that is fault tolerant and fluctuates controller loads. The experiments show a significant gain in efficiency of our solution.

Abstract: In this paper, we are presenting a new vector order, a solution to the open problem of the generalization of mathematical morphology to multicomponent images and multidimensional data. This approach uses the paradigm of P–order. Its primary principle consists, first in partitioning the multi-component image in the attribute space by a classification method in different numbers of classes, and then the vector attributes are ordered within each class (intra-order-class). And finally the classes themselves are ordered in turn from their barycenter (inter-class order). Thus, two attribute vectors (or colors) whatever, belonging to the vector image can be compared. Provided with this relation of order, vectors attributes of a multivariate image define a complete lattice ingredient necessary for the definition of the various morphological operators. In fact, this method creates a strong close similarity between vectors in order to move towards an order of the same principle as defined in the set of real numbers. The more the number of classes increases, the more the colors of the same class are similar and therefore the absolute adaptive referent tends to be optimal. On the other hand, the more the class number decreases or equals two, the more our approach tends towards the hybrid order developed previously. The proposed order has been implemented on different morphological operators through different multicomponent images. The fundamental robustness of our approach and that relating to noise have been tested. The results on the gradient, Laplacian and Median filter operators show the performance of our new order.
Soil Subgrade’s Characterization and Classification of Thies (Senegal, West Africa) on a Radius of 2.5 Kilometers along Five Roads  [PDF]

Abstract: This article explains the results of a study conducted on the characterizations of subgrade soils in the region of Thies. The road platforms are mainly composed of a background soil, which is generally overlapped by a surface layer that plays two roles. Firstly, it protects the soil structure, ensures the leveling, and facilitates the movement of vehicles. Secondly, it brings harmony in the mechanistic characteristics of the materials that compose the soil while improving the long-term life force. The methodology consisted in taking samples of subgrade soil along the roads all over the region of Thies in a 5 km diameter span. The identification tests allowed the Thies-Tivaoune, Thies-Khombole and Thies-Noto axes are characterized by tight sands, poorly graded size. While Thies Pout-axis is characteristic of severe solid particle size and spread well graded and serious to spread and well graded particle size. Finally the Thies-Montrolland axis is characterized by severe to very tight particle size and graduated to spread and serious and well graded particle size. The specific gravity values found Proctor test shows the presence of sand, sandy laterite and laterite. In the target area, polished soils of the A-3 type according to the AASHTO classification system are the most represented with 60%, followed by the A-2-6 type 25%, and the A-2-4 type with 9%, which are typical of gravel, clay, and silty sands. Soils of the A-1-b type (2%) typical of roc fragments, sands and clay are also represented. Polished sands of the A-3 type have a better efficiency on road infrastructures than other types of soil listed above. Finally, we’ve also noted the presence of soils of the A-2-7 and A-4 types with the low percentage of 2%. Subgrade soils of class S4 are the most represented with 58%, followed by those of class S5 with 42%. Samples of the Thies-Montrolland road have a claylike plasticity (CL or CH group), while those of the Thies-Pout road belong to the ML or OL and CL or OL groups with a tendency mostly directed to the CL or OL group. All these results confirm the very nature of soils on the two roads and put the light on the presence of lateritic materials with certain plasticity.
