Termoplastia br?nquica: relato do primeiro tratamento endoscópico de asma na América Latina

Abstract: bronchial thermoplasty is a new bronchoscopic procedure that delivers radiofrequency energy to the airway and potentially reduces the smooth muscle-mediated bronchoconstriction. we report the case of a 48-year-old man with persistent moderate asthma submitted to bronchial thermoplasty. the treatment increased the forced expiratory volume in one second, increased the number of symptom-free days, reduced the use of relief medications, and improved the juniper asthma quality of life scale score. in this patient, bronchial thermoplasty was well tolerated and safe. this was the first bronchial thermoplasty performed in latin america. at 12 months after the procedure, the results were encouraging in terms of its potential benefits in patients with difficult-to-control asthma.

Abstract: currently available treatments for asthma provide satisfactory control of the disease in most cases. however, a significant number of patients do not respond to such treatments (i.e., do not achieve effective symptom relief). one novel approach to treating asthma is bronchial thermoplasty, in which the airway smooth muscle is specifically and directly treated. this procedure delivers radiofrequency energy to the airways in order to reduce smooth muscle-mediated bronchoconstriction. in this article, we present the thermoplasty technique, summarizing the results of the major randomized clinical trials of the procedure, as well as discussing its mechanisms of action and potential adverse effects. we also propose strategies for the future clinical use of this new treatment.

Abstract: objective: to determine the prevalence of gastroesophageal reflux disease (gerd) and to evaluate its clinical presentation, as well as the esophageal function profile in patients with idiopathic pulmonary fibrosis (ipf). methods: in this prospective study, 28 consecutive patients with ipf underwent stationary esophageal manometry, 24-h esophageal ph-metry and pulmonary function tests. all patients also completed a symptom and quality of life in gerd questionnaire. results: in the study sample, the prevalence of gerd was 35.7%. the patients were then divided into two groups: gerd+ (abnormal ph-metry; n = 10) and gerd- (normal ph-metry; n = 18). in the gerd+ group, 77.7% of the patients presented at least one typical gerd symptom. the ph-metry results showed that 8 (80%) of the gerd+ group patients had abnormal supine reflux, and that the reflux was exclusively in the supine position in 5 (50%). in the gerd+ and gerd- groups, respectively, 5 (50.0%) and 7 (38.8%) of the patients presented a hypotensive lower esophageal sphincter, 7 (70.0%) and 10 (55.5%), respectively, presenting lower esophageal dysmotility. there were no significant differences between the groups regarding demographic characteristics, pulmonary function, clinical presentation or manometric findings. conclusions: the prevalence of gerd in the patients with ipf was high. however, the clinical and functional characteristics did not differ between the patients with gerd and those without.

Abstract: based on historiographical studies that since 1980?s have undertaken a broad review of the social history of labor in brazil, the article presents some sociological hypotheses about the permanence of structural features of slavery past in the process of building of the capitalist sociality in the country. this legacy includes a depreciated perception of manual work, a derogatory image of black and even national people as workers, an indifference of the elites toward poor majorities, and an extremely rigid social hierarchy. according to the author, this framework of structural inertia provided the general parameters of free labor?s reproduction in the beginning of capitalist order in brazil.

Abstract: this article joins the persistent (and still current) effort to decipher the riddle of brazil's equally persistent inequality. resuming the interpretation of modern brazil proposed by juarez brand?o lopes in the 1960s, the article proposes to revisit the "vargas era" and its historical meaning and scope, in light of the reproduction of inequalities over time. the author contends that "regulated citizenship" generated the expectation of social protection among brazilian workers, feeding the promise of citizens' integration, which was not fulfilled, while performing the task of finally (but not definitively) incorporating workers as artifices in the brazilian state-building process.

Abstract: this study analyzes the changes in economic structure and labor markets in the last 60 years in brazil that provided the basis for establishing patterns in the transition from school to work for young men and women born since 1948. data from population censuses beginning in 1970 and the national household sample surveys (pnads) beginning in 1976 were used to support the idea that brazil witnessed a developmentalist pattern in the social trajectories of young people marked by lesser importance of education in shaping their initial life opportunities, constructed in a highly unstable and poorly structured labor market. this pattern can be distinguished from another, which we will call a fordist transition pattern, typical of advanced capitalist countries and characterized by strong family and state control over the general work qualifications processes, in which the school plays a central role and serves as the principal element for social mobility and creation of life opportunities.

Abstract: one aspect the literature overlooks on the effects of labor regulation of labor market efficiency is the degree of the legislation's effectiveness, i.e., its actual enforcement in daily work relations. even the more sophisticated econometric studies (which take into account the effects of interaction between labor market regulatory institutions in explaining its dynamics) leave this central issue aside, namely enforcement versus non-enforcement of the law. keeping this issue in mind, we seek to answer the following question in this article: given that the effectiveness of labor legislation depends on the interaction between the overall sanctions and the probability of the employer getting caught breaking the law, and given that the law's effectiveness is a decisive aspect for the real measurement of a country's labor costs, to what extent is the brazilian labor inspection system designed to meet its objective, namely to enforce the law?

Abstract: in the academic debate, brazilian unionism has been characterized both as representative and as non-representative ? with defenders of both positions grounding their arguments in the same basic facts. taking issue with these interpretations, the article questions whether or not union membership is a suitable measure of representation. if, given brazil?s corporative union structure, unions do not need to recruit members, to what extent can membership serve as an adequate measurement of union representation? precisely what do union membership rates measure, and precisely what does it mean to "represent workers" in brazil? the article first offers a conceptual discussion of the relationship between union membership and representation. the thesis is that in brazil the former does not suffice as a measurement of the latter. but if the union membership rate is high, what is it measuring in any case? the second part of the text explores the empirical side of the question, through analysis of a 1994 survey conducted within four occupational categories. it becomes clear that membership rates are indicative of the degree to which the union makes its presence felt in the life of those it represents. but it also becomes clear that in and of itself this rate does not account for the complexity of the relations between the rank-and-file and union leaders. in short, the referent is blurry ? but it is there. the text seeks to lend it greater conceptual and empirical clarity.
