Abstract: Aim. Self-report diaries are a low-cost method of measuring community participation but may be inaccurate, while the “gold standard,” observation is time consuming and costly. This study aimed to investigate the feasibility and validity of a global positioning system (GPS) for measuring outings after stroke. Design. Cross-sectional cohort study. Methods. Twenty ambulant people with stroke wore a GPS device and kept a diary for 7 days, and 18 were observed for half a day. We recorded recruitment rate, user perceptions, and data extraction time. GPS data were analysed against Google maps. Percent exact agreement (PEA) with observation was calculated for GPS and diary. Results. Of 23 eligible participants, 20 consented (mean 3.6 years after stroke). GPS data recovery was high (87%). Some participants had difficulty operating the on/off switch and reading the small screen. Data extraction took an average of 5 hours per participant. PEA with observation was high for number of outings (GPS 94%; diary 89%) but lower for purpose of outings (GPS 71%; diary 82%). Conclusions. The GPS device and diary were both feasible and valid for measuring outings after stroke. Simultaneous use of GPS and diaries is recommended for comprehensive analysis of outings. 1. Introduction Community participation is an objective indicator of the success and outcome of stroke rehabilitation [1]. The “amount” and “type” of community participation, such as outings, can be quantified, providing valuable information about changes over time due to intervention. While the measurement of participation is increasingly common in health research, the measurement process remains challenging. The criterion standard method is direct observation, which is objective but time-consuming and costly. In addition, the process of observation may influence a person’s behaviour. Self-report diaries are a simple and cheap method of measuring physical activity. Yet diaries produce unreliable data and are burdensome for participants, especially those with impaired cognitive abilities [2]. When retrospective interviews about events were compared with daily diary recordings of events, fewer recall errors were seen in the diary [3]. Diaries capture minor events and produce higher “incidence” rates, especially for healthy unimpaired populations; however, diaries can also underreport events and lead to lower compliance by people with disabling conditions when compared to retrospective interviews [3]. Abstract: several studies about knowledge transfer in multinational corporations have found that the process is influenced by factors such as absorptive capacity, tacit knowledge and power relations, all of which impact knowledge sharing strategies between corporate headquarters and foreign subsidiaries. a multiple case study of brazilian subsidiaries of three multinational corporations using in-depth interviews, based on a conceptual model consisting of four propositions, was conducted to identify factors linked to the knowledge transfer process and to assess their influence on that transfer. the first proposition tries to assess explicit knowledge, primarily through the use of it tools; the second analyzes the role of the subsidiary within the corporate network and how it influences the degree of knowledge sharing. the third assesses the influence of subsidiary absorptive capacity and the fourth analyzes the impact of worker exchange programs on knowledge sharing between headquarters and foreign subsidiaries. study results confirm the four propositions and enable the identification of relationships between factors, especially explicit knowledge and worker expatriation as complementary factors in knowledge transfer strategies.

Mycotic bovine nasal granuloma

Abstract: A case of mycotic bovine nasal granuloma in a 10 year-old Jersey cow, produced by Drechslera halodes is presented. Histopathological sections showed abundant hyaline and pigmented extra and intracellular fungal structures together with a polymorphic cellular granuloma formed by neutrophils, lymphocytes, plasmocytes, histiocytes and giant cells of the Langhans type. It is the first case of mycotic bovine nasal granuloma recognized in Uruguay although this disease seems to be frequent according to the opinion of veterinarian specialists. Another similar clinical case also in a Jersey cow from the same dairy house with an intense cellular infiltrate rich in eosinophils without granulomatous image, together with extracellular hyaline and fuliginous fungal forms, is also referred for comparative purposes. Geotrichum sp. was isolated. The need of an early diagnosis and treatment of the disease is stressed.

The endocanabinnoid system and diabetes - critical analyses of studies conducted with rimonabant

Abstract: Rimonabant is the first CB1 receptor inhibitor available in the Brazilian market. This new drug has been approved for the treatment of obese or overweight patients associated with cardiovascular risk factors. This way, it is important to emphasize that the Rimonabant drug has not been approved for the treatment of patients with isolated cardiovascular risks, but rather for the treatment of excess weight. It is questioned whether the effect of Rimonabant regarding cardiovascular risk factors (lowering blood pressure, promoting lipid profile improvement, and reduction of glycated hemoglobin) is related to weight loss, or whether risk factor improvement is superior to other anti-obesity medications or to weight loss based on life style change.Studies show that Rimonabant-driven weight loss is superior compared to placebo. However, to date, there are no studies which compare Rimonabant to other anti-obesity drugs or intensive life style change, such as the Diabetes Prevention Programme (DPP). The DPP compared the effects of the use of metformin to the change in life style and it was shown that the latter resulted in greater weight loss and cardiovascular risk factor improvement.Next, the effects of Rimonabant over each cardiovascular risk will be discussed.Rimonabant has been announced as the drug to result in blood pressure reduction. The study to show the most relevant blood pressure drop was the RIO-Lipids, which evinced a 2.1-mmHg in systolic BP in the Rimonabant group, and a 1.7-mmHg drop upon the subtraction of the placebo effect [1]. Although statistically relevant, the little clinical significance of this value leads to the conclusion that the effect of this drug on blood pressure is null.Regarding lipids in multivariate analyses, there seems to be a non-weight-dependent percentage of improvement. Abstract: Obviously, the juridical act based on which a person works for and under the authority of another person in exchange of a salary can be only an individual employment contract, as the Labour Code specifies. So, in this case we are talking about a contract which should be governed by the rules of this matter as the common civil law stipulates, including the freedom of negotiation and concluding of this legal act. Unfortunately, on the present Romanian labour market the labour contract is transformed into a contract of adhesion, imposed by the law. This reality, which can’t be ignored, distorts the legal nature of the labour contract and the free will principle which must be respected during the negotiation and conclusion of the individual employment contract.
