Interven??o fonoaudiológica na síndrome de kabuki: relato de caso

Abstract: background: this study aimed at describing the language intervention process in a kabuki syndrome case, emphasizing the language therapy. procedures: this is a report of a six and a half year-old girl, diagnosed with kabuki syndrome, who attended language therapy for three years at a clinical school. the documental analysis of the medical register form was made, concerning interview evaluation and language therapeutic reports, examination and multidisciplinary evaluations. when the child was three years and five months old, she went through language evaluation and showed typical characteristics of kabuki syndrome: cardiac alterations, pneumonia, recurrent secretory otitis media, orthopedic alterations and convulsion crises. for the language evaluation, she presented a reduced period of hearing and visual attention, impaired oral comprehension, communication through only a few gestures and sounds and also behavioral problems. results: the therapeutic process aimed to promote language development emphasizing social interaction through ludic activities, and a familiar and scholar follow-up. an improvement was observed for comprehension and oral language, social interaction and attention steadiness. conclusion: this study allowed the trajectory disclosure of language development concerning the kabuki syndrome, from the diagnosis to the language therapy. it was observed that the therapy was effective concerning communication abilities and social interaction. these data may contribute to clarify some aspects in the literature towards the language therapy for the cited syndrome and also suggest clinical interventions for these cases.

Abstract: purpose: characterize and compare the communicative profile of children with disturbances of autistic specter with two different speakers. method: nine subjects took part in the research, among them four children of the masculine gender, between five years old and eight months and eleven years old, with disturbances of the autistic specter and five speech and language therapists. we carried out an analysis of the communicative profile respective to these children with two different speakers (proper child's speech and language and unknown therapist). for the analysis of the information we used the protocol of pragmatica and statistical treatment of the results (p < 0.05; tests of wilcoxon's signalized posts). results: we raised the number of the communicative acts, the mean through those acts was expressed (vocal, verbal and signal) and also the communicative functions. as a rule, the number of the communicative acts expressed by subjects as with proper child's therapists (5,10 acts/min) as with unknown therapist (4,93 acts/min) was not statistically significant. it was observed in the situation of interaction with proper therapist that the mean value for the percentage of occurrence of verbal communicative mean (31.50%) and the average of more interactive functions (33.25%) are larger than unknown therapist. conclusion: the different speakers, studied in this research, do not have influence on the communicative profile of these children. from that, we suggest new studies, with inclusion of other speakers, in order to settle which interactive variables interfere statistically in the children's communicative profile of the autistic specter intention to contribute to the elaboration of better therapeutic offers for these people.

Abstract: purpose: to investigate the communication initiatives among children with autistic spectrum disorders and their mothers, employing a pragmatic analysis. method: five children with autistic spectrum disorders, both genders, ages ranging between five and 12-year and their mothers. the control group comprised 5 children with normal development, paired with children with autistic spectrum disorders, according to gender and age and their respective mothers, totaling 20 participants. for the characterization of the sample, an information form and a scale for the assessment of autistic behavior were employed. each dyad was filmed for 30 minutes during ludic interactions and analyzed according to the pragmatic protocol. this instrument evaluates communication initiatives, means and communicative functions. data were analyzed statistically (p < 0.05; non-parametric tests of wilcoxon and mann-whitney were employed). results: statistically significant differences were observed between the communicative initiatives of children with autistic spectrum disorders and their mothers, when compared to the control group. the dyads which involved children with autistic spectrum disorders showed few communication initiatives and used predominantly gestures and less interactive vocalization. conclusion: such data turned easier the description of the fundamental aspects for the comprehension of the clinical manifestations in the studied population and also for elaborating intervention programs aiming at improving the quality of life related to the children and their respective families.

Abstract: the aim of this research was to describe the language intervention process of two brothers with pervasive developmental disorders, through a longitudinal clinical case study. two brothers - one nine and the other 11 years old - participated in the study. subjects had diagnosis of autism (case 1) and pervasive developmental disorder-not otherwise specified (case 2), respectively. as a procedure for data collection and analysis, a longitudinal study was carried out through a follow up of the cases during the four years of language therapy. the procedure included filmed therapy sessions, documental analysis of information contained in the patients' records regarding anamnesis, assessments and therapeutic reports, and multidisciplinary evaluations and exams. in both cases there was an improvement on visual contact, social interaction, vocabulary and symbolic play. for case 1, there was an increase from 2.0 to 6.2 communicative acts per minute; for case 2, an increase from 3.5 to 8.0 acts. both subjects demonstrated predominance of the verbal communicative mean and greater variety of communicative functions. other factors influenced these results, such as the intellectual deficiency, the family dynamics, the conflicts on the relationship between the brothers and the school environment in which they were placed. the study confirmed the relevance of the speech-language pathologist in interventions of pervasive developmental disorders, along with multidisciplinary teams, for the discussion of diagnoses and of the most appropriate procedures. longitudinal studies can contribute for more detailed and reliable analysis of therapeutic intervention in these cases, in order to clarify gaps existent in literature and to support the role of clinical speech-language pathologists.
Terapia de linguagem no espectro autístico: a interferência do ambiente terapêutico

Abstract: purpose: to analyze the functional communicative profile of children and adolescents with autistic spectrum disorders in two different language therapy environments which differ from each other on the physical environment. methods: participants were ten subjects with autistic spectrum disorders, six male and four female, with ages varying from 4 to 13 years. for data gathering, eight 30-minute individual language therapy sessions were videotaped: four sessions in regular therapy settings (common room) interspersed with four sessions in a specific environment setting (nic room), for one month. results were registered on the pragmatic protocol, and statistical analysis was carried out. results: no significant differences were found between the pragmatic profile presented by the subjects in the common room and in the nic room. conclusion: the physical environment studied did not significantly influence the functional communication profile of the autistic subjects, even though there were individual tendencies to present better performance in one room or another.

Abstract: OBJETIVOS: investigar as iniciativas de comunica o na intera o entre crian as com distúrbios do espectro autístico (DEA) e suas m es, por meio de uma análise pragmática. MéTODO: cinco crian as com DEA, de ambos os gêneros, idades entre cinco e 12 anos juntamente com suas m es. O grupo controle constou de cinco crian as com desenvolvimento normal, pareadas com as crian as com DEA de acordo com gênero e idade e suas respectivas m es, totalizando 20 participantes. Para a caracteriza o da amostra foram utilizadas uma Ficha informativa e a Escala de Avalia o de Tra os Autísticos. Cada díade foi filmada por 30 minutos durante as intera es lúdicas, analisadas a partir do Protocolo de Pragmática. Este instrumento avalia as iniciativas de comunica o, meios e fun es comunicativas expressas. Os dados foram tratados estatisticamente (p < 0,05; foram utilizados os testes n o-paramétricos de Wilcoxon e de Mann-Whitney). RESULTADOS: foram observadas diferen as estatisticamente significantes entre as iniciativas comunicativas das crian as com DEA e suas m es, quando comparadas ao grupo controle. As díades que envolveram crian as com DEA iniciaram menos a comunica o e utilizaram predominantemente gestos e vocaliza es menos interativas. CONCLUS ES: estes dados possibilitaram descrever aspectos fundamentais para compreens o das manifesta es clínicas da popula o estudada e para a elabora o de programas de interven o que visem melhorar a qualidade de vida da crian a e sua família. Abstract: TEMA: este trabalho teve como objetivo descrever o processo de interven o fonoaudiológica em um caso de síndrome de Kabuki, com ênfase na terapia de linguagem. PROCEDIMENTOS: trata-se de relato de caso de uma crian a de seis anos e seis meses de idade, gênero feminino, com diagnóstico de síndrome de Kabuki, que realizou terapia fonoaudiológica durante três anos em uma clínica-escola. Foi realizada análise documental dos dados do prontuário relativos à anamnese, avalia o e relatórios terapêuticos fonoaudiológicos, exames e avalia es multidisciplinares. Aos três anos e cinco meses, a crian a passou por avalia o fonoaudiológica e apresentou, como histórico, características típicas da síndrome de Kabuki: altera o cardíaca, episódios de pneumonia, otite média secretora recorrente, altera es ortopédicas e crises convulsivas. Na avalia o fonoaudiológica, apresentou tempo de aten o auditiva e visual reduzido, dificuldade de compreens o oral, comunica o por meio de poucos gestos e vocaliza es e problemas de comportamento. RESULTADOS: o processo terapêutico teve como objetivo principal promover o desenvolvimento da linguagem com ênfase na intera o social por meio de atividades lúdicas, orienta o familiar e escolar. Foi observada melhora na compreens o e emiss o oral, no contato social e manuten o da aten o. CONCLUS O: este estudo permitiu divulgar o percurso de atua o fonoaudiológica na síndrome de Kabuki, do processo diagnóstico à terapia de linguagem. Verificou-se que a terapia foi eficaz no que se refere às habilidades comunicativas e de intera o social. Esses dados podem contribuir para elucidar lacunas existentes na literatura acerca da terapia fonoaudiológica na síndrome em quest o e subsidiar interven es clínicas nesses casos. Abstract: OBJETIVO: caracterizar e comparar o perfil comunicativo de crian as com distúrbios do espectro autístico com dois diferentes interlocutores. MéTODO: participaram da pesquisa nove sujeitos, sendo quatro crian as do gênero masculino, com faixa etária variando entre cinco anos e oito meses e onze anos, com distúrbios do espectro autístico e cinco terapeutas de linguagem. Foram realizadas análises do perfil comunicativo dessas crian as com os dois diferentes interlocutores (terapeuta de linguagem da própria crian a e terapeuta desconhecida). Para análise dos dados foi empregado o Protocolo de Pragmática e tratamento estatístico dos resultados (p < 0,05; Teste dos Postos Sinalizados de Wilcoxon). RESULTADOS: foram levantados o número de atos comunicativos, o meio pelo qual esses atos foram expressos (vocal, verbal e gestual) e as fun es comunicativas. Em média, o número de atos comunicativos expressos pelos sujeitos tanto com os terapeutas das próprias crian as (5,10 atos/min) como com a terapeuta desconhecida (4,93 atos/min) n o foi estatisticamente significante. Observou-se na situa o de intera o com o próprio terapeuta que a média do percentual da ocorrência do meio comunicativo verbal (31,50%) e a média de fun es mais interativas (33,25%) s o maiores do que com terapeuta desconhecida. CONCLUS O: os diferentes interlocutores, estudados nesta pesquisa, n o tiveram uma influência no perfil comunicativo destas crian as. Deste modo, sugerem-se novos estudos, com inclus o de outros interlocutores, a fim de estabelecer quais variáveis interacionais interferem estatisticamente no perfil comunicativo de crian as do espectro autístico, com o intuito de contribuir para a elabora o de melhores propostas terapêuticas para esta popula o. Abstract: this study aimed to estimate the dog replacement rate in an area endemic for visceral leishmaniasis, in which slaughter of seropositive animals was indicated as a control measure, and to evaluate the reasons why new animals were or were not acquired. the animals were replaced in 44.5% of the cases, and this was done mainly because of the need for a companion or guard dog. the main reason for not replacing the dog was fear of visceral leishmaniasis.

Abstract: post-kala-azar dermal leishmaniasis (pkdl) is rarely reported in south america. in spite of the fact that there are many reports about the association of visceral leishmaniasis and aids, pkdl is very uncommon in hiv-positive patients, and so far only four cases have been documented in the literature. we present another case with unusual clinicopathological aspects. the patient, a 28-year-old male, from salvador, bahia (an endemic area) presented with clinical manifestations of visceral leishmaniasis three years after the diagnosis of aids. during treatment for visceral leishmaniasis he developed disseminated miliary papules. microscopically, the skin biopsy showed a "saw-tooth" pattern with a lichenoid mononuclear infiltrate simulating lichen planus. the histopathological diagnosis was achieved through the finding of amastigotes. the authors discuss the clinicopathological aspects of this case based on a review of the specific literature.
