Apolipoprotein and lipid abnormalities in chronic liver failure

Abstract: Total serum lipids, as well as apolipoproteins A-I (apo A-I) and B (apo B), were determined in 74 patients with chronic liver failure without cholestasis and in 82 normal subjects. The VLDL, LDL and HDL lipid fractions were reduced in the liver failure group by 36%, 24% and 46%, respectively (P<0.001). Apolipoproteins A-I and B were also reduced by 26% and 25%, respectively (P<0.001). However, the reduction of HDL cholesterol (HDLc) was more pronounced than that of apo A-I and the HDLc:apo A-I ratio was significantly lower in the liver failure group. After separating these patients into groups with plasma albumin lower than 3.0, between 3.0 and 3.5, and higher than 3.5 g/dl, the HDLc:apo A-I ratio was proportional to plasma albumin, but the correlation was not statistically significant. When these patients were separated by the Child classification of liver function, there was a correlation between the HDLc:apo A-I ratio and liver function. The differences in the HDLc:apo A-I ratio between the Child groups B and C, and A and C were statistically significant (P<0.05). We conclude that there is a more pronounced reduction in HDL cholesterol than in apo A-I in liver failure patients. Therefore, the HDLc:apo A-I ratio is a marker of liver function, probably because there is a decreased lecithin-cholesterol acyltransferase production by the diseased liver
Effect of antipyrine on the gastric emptying of liquid in rats

Abstract: antipyrine (at) and dipyrone (dp) delay gastric emptying (ge) in rats. the objective of the present study was to assess the effects of intravenous (iv) and intracerebroventricular (icv) administration of at and dp on the ge of liquid by rats. ge was assessed in male wistar rats (5-10 in each group) 10 min after the icv or iv drug injection by measuring percent gastric retention (%gr) of a saline test meal labeled with phenol red 10 min after administration by gavage. the at iv group was significantly higher (64.4 ± 2.6%) compared to control (33.4 ± 1.5%) but did not differ from the dp group (54.3 ± 3.8%). after icv administration of at, %gr (34.2 ± 2%) did not differ from control (32.6 ± 1.9%), but was significantly higher after dp (54.5 ± 2.3%). subdiaphragmatic vagotomy significantly reduced %gr in the at group (30.2 ± 0.7%) compared to the sham group, but was significantly higher than in the controls (23.0 ± 0.5%). in the animals treated with at iv, baclofen significantly reduced %gr (28.3 ± 2.4%) compared to vehicle-treated animals (55.2 ± 3.2%). the same occurred in the animals treated iv with vehicle and icv with baclofen. although vagotomy and baclofen reduced %gr per se, the reduction was twice more marked in the animals treated with at. the results suggest that at administered iv, but not icv, delays ge of liquid in rats with the participation, at least in part, of the vagus nerve and that this phenomenon is blocked by the activation of gabab receptors in the central nervous system.

Role of ACTH receptor in adrenocortical tumor formation

Abstract: adrenocorticotrophin (acth) is the major regulatory hormone of steroid synthesis and secretion by adrenocortical cells. the actions of acth are mediated by its specific membrane receptor (acth-r). the human acth-r gene was recently cloned, allowing systematic determination of its sequence, expression and function in adrenal tumorigenesis. the presence of oncogenic mutations of the acth-r gene in adrenocortical tumors has been reported. direct sequencing of the entire coding region of the acth-r gene of sporadic adrenocortical adenomas and carcinomas did not reveal constitutive activating mutations, indicating that this mechanism is not frequent in human adrenocortical tumorigenesis. recent studies demonstrated allelic loss of the acth-r gene in a subset of sporadic adrenocortical tumors using a psti polymorphism located in the promoter region of the acth-r gene. loss of heterozygosity of the acth-r was analyzed in 20 informative patients with a variety of benign and malignant adrenocortical tumors. three of them showed loss of heterozygosity of the acth-r gene. in addition, northern blot experiments demonstrated reduced expression of acth-r mrna in these three tumors with loss of heterozygosity, suggesting the functional significance of this finding at the transcriptional level. deletion of the acth-r gene seems to be involved in a subset of human adrenocortical tumors, contributing to cellular dedifferentiation.

Abstract: two problems inspired by paul nahin's biography of oliver heaviside, the home illumination problem and the determination of the leakage point in an undersea telegraphic cable are presented, discussed and solved in the framework of the applied electricity of the victorian age. the two problems are concrete and at the same time relatively simple illustrations of the intricate interconnections between physics and the world of practical affairs. basic elements of the theory of direct current circuits, as for example ohm's law, and high school algebra are the only tools we need to discuss them in class, at the high school level or at the fundamental university physics one.

Abstract: purpose: this article discusses the evolution of university hospitals, their characteristics, functioning and financing and their role in medical education and research. methods: data come from a workshop organized by the world health organization on university hospitals in 22 countries. results: the main finding is that these institutions are increasingly inadequate to the reality of the health sector in which they operate and to the challenges posed by the profound changes currently taking place in the health system of most countries. conclusion: a reform is needed, and the article presents a few possible strategies for reforming these hospitals.
