Abstract: Paciente sin antecedentes patológicos de interés que debuta con diplopía y ptosis palpebral izquierda como primera manifestación de metástasis hipofisarias de un cáncer de origen pulmonar ampliamente diseminado y no conocido previamente. El diagnóstico se hizo con pruebas de imagen específicas del área hipofisaria en donde se apreció una neoformación en silla turca, que invadía el seno cavernoso izquierdo con erosión del dorso selar, junto con la presencia de nódulos pulmonares y múltiples adenopatías, además del estudio histológico de las lesiones. Se detectó un hipogonadismo hipogonadotropo como única alteración hormonal debido a la infiltración de la hipófisis anterior por las metástasis óseas en la base del cráneo. El paciente, a pesar de recibir varias sesiones de radioterapia, sufrió un deterioro neurológico progresivo falleciendo a las pocas semanas. A patient, without significant previous medical history, who started with diplopia and left palpebral ptosis as the first manifestation of hypophysary metastases of a widely disseminated and previously undetected cancer of pulmonary origin. Together with a histological study of the lesions, the diagnosis was made with specific image tests of the hypophysary area, which detected a neoformation in the sella turca that was invading the left cavernous sinus with erosion of the verso of the sellar, together with the presence of pulmonary nodules and numerous adenopathies. An hypogonadotropic hypogonadism was detected as the only hormonal alteration due to the infiltration of the anterior hypophysis due to the osseous metastases at the base of the cranium. The patient, in spite of receiving several weeks of radiotherapy, suffered a progressive neurological deterioration and died after a few weeks.
Tumor de cavidad oral como primera manifestación de un adenocarcinoma gástrico metastásico

Abstract: La cavidad oral es un blanco raro pero ocasional para lesiones metastásicas. Las metástasis orales han sido descritas principalmente en cáncer de pulmón, mama y ri ón. En nuestro trabajo se describe un caso poco común de una metástasis en mucosa gingival proveniente de un carcinoma gástrico sin otros síntomas, fue diagnosticada y tratada como adenocarcinoma de glándulas salivares menores. El examen histopatológico de la biopsia reportó un adenocarcinoma pobremente diferenciado. En endoscopia digestiva superior realizada posteriormente se evidenció un tumor gástrico, cuya biopsia demostró la presencia de un adenocarcinoma. El paciente recibió tratamiento con quimioterapia pero falleció por falla renal. En conclusión, un carcinoma oculto gástrico puede presentarse con metástasis a la cavidad oral, pudiendo ser ésta la primera y única manifestación. In our work we describe an uncommon case of gingival mucosa metastases from a gastric carcinoma without other symptoms that was diagnosed and treated as a minor salivary gland adenocarcinoma. Histopathological examination of the lesion revealed a poor differentiated cell adenocarcinoma. A subsequent upper endoscopy revealed a gastric tumour; the biopsy practiced of the tumour reported the presence of one adenocarcinoma. The patient underwent to chemotherapy but died of renal failure. In conclusion, an occult carcinoma of the stomach may rarely be present how metastasize to the oral cavity even as a first and exclusive clinical manifestation.
Metástase cutanea como primeira manifesta??o de adenocarcinoma pulmonar

Abstract: we report the case of a 58-year-old male patient who was referred for oncology consultation due to an epigastric mass that had been growing rapidly for three months. diagnostic investigation revealed that the mass was a metastasis of stage iv lung adenocarcinoma. the patient received five cycles of chemotherapy with cisplatin and gemcitabine as a first-line treatment, which was interrupted due to major adverse events. although the pulmonary disease stabilized, the cutaneous disease progressed. the patient then received pemetrexed as a second-line chemotherapy, together with concurrent external radiotherapy, which was well tolerated. there was complete remission of the epigastric mass. however, the patient died three months after the treatment. here, we emphasize the importance of a multidisciplinary approach and of its role in individualizing the treatment.
Síndrome mascarada como primeira manifesta o de adenocarcinoma pulmonar: relato de caso e revis o de literatura  [cached]

Abstract: OBJETIVO: Os autores relatam um caso de paciente com síndrome mascarada como primeira manifesta o de adenocarcinoma pulmonar. MéTODOS: Paciente de 57 anos com história clínica de emba amento visual unilateral (olho direito) de seis meses de evolu o e diagnóstico inicial de uveíte recidivante com tratamento à base de corticóide tópico. Referida por permanência dos sintomas mesmo com tratamento. Ao exame ocular apresentava hiperemia conjuntival com inje o ciliar moderada e les o peripapilar na coróide. Solicitados os exames complementares. RESULTADOS: A paciente foi diagnosticada com adenocarcinoma pulmonar (les o primária). As altera es em fundo de olho foram sugestivas de les es metastáticas na coróide. CONCLUS ES: A incidência das metástases uveais varia de 0,7 a 12% dos pacientes com tumores pulmonares. O tratamento depende de características como tamanho da les o, número de metástases, acuidade visual, bilateralidade e estado geral do paciente. O envolvimento ocular pode ser a primeira manifesta o clínica de adenocarcinoma pulmonar, conforme observado neste caso.

Abstract: skin metastases are infrequent, generally of late appearance and of very bad prognosis. they are the first symptom of cancer in 0.8% of cases. the prognosis of patients with skin metastases is worst than that for other localizations. the most frequent origin of skin metastases are lung cancer in men and breast cancer in women, followed by the gastrointestinal neoplasms. we present the case of a 63 year old patient with a cutaneous lesion that histopathology showed to be an adenocarcinoma. the workup study led to the diagnosis of metastatic gastric cancer.

Abstract: carcinomatous lymphangitis accounts for approximately 8% of all cases of metastatic pulmonary tumors. the most common primary sites are breast, lung, stomach, prostate and pancreas. we describe herein the case of a 42-year-old woman in whom the first manifestation of an ovarian adenocarcinoma was carcinomatous lymphangitis of the lung, an unusual presentation of the disease.

Abstract: A carcinomatose leptomeníngea é uma complica o neurológica de alguns tumores sistêmicos caracterizada por invas o multifocal das leptomeninges por células neoplásicas. Estima-se que 5% de todos os pacientes com cancer apresentem carcinomatose leptomeníngea. As manifesta es clínicas s o heterogêneas e caracterizam-se por sinais e sintomas relacionados a comprometimento de múltiplas áreas do sistema nervoso, particularmente nervos cranianos e raízes nervosas. O diagnóstico é baseado nos achados clínicos, exame do líquido cefalorraqueano (LCR) e exames de neuroimagem. O estudo mais informativo para o diagnóstico de metástase meníngea é o exame do LCR através do qual o achado de células neoplásicas é definitivo para o diagnóstico. O intuito deste trabalho é realizar revis o da literatura a partir de descri o de um caso clínico, de um homem de 42 anos de idade, em que a primeira manifesta o de neoplasia pulmonar consistiu de sintomas e sinais sugestivos de comprometimento neoplásico das leptomeninges.
Metástasis pulmonar miliar como primera manifestación de carcinoma de tiroides bien diferenciado en ni os

Abstract: Objetivo: Presentar el caso de una adolescente con manifestaciones de afectación pulmonar como motivo de consulta, debido a una metástasis pulmonar miliar de carcinoma (Ca) papilar de tiroides. Caso Clínico: Se trata de una adolescente femenina de 13 a os de edad quien presenta tos seca, disnea de esfuerzo, palpitaciones, dolor precordial, y pérdida de peso de 15 días de evolución. Al examen clínico se evidencia aumento de volumen de la región anterior del cuello a predominio izquierdo, con adenopatía cervical palpable, acrocianosis y cianosis peribucal. Hematología completa, función tiroidea y electrocardiograma normales. La gasometría arterial demuestra una hipoxemia severa. En Rx. de tórax se observa infiltrado miliar micro y macronodular bilateral. En ultrasonido impresionan adenopatías de ubicación pretraqueal y paracarotideas. Tomografía de cuello revela imagen altamente sugestiva de malignidad en lóbulo tiroideo izquierdo y tomografía toraco-abdominal muestra infiltrado pulmonar bilateral micronodular sugestivo de lesiones metastásicas. Estudio histopatológico de ganglio latero cervical izquierdo reporta metástasis de Ca papilar tiroideo a predominio de estructuras foliculares. Se hace diagnóstico de carcinoma papilar bien diferenciado de tiroides con metástasis a región pulmonar, se realiza tiroidectomía total y control de T4L y TSH a los 30 y 60 días post cirugía, con valores normales de T4L y ligeramente aumentados de TSH (7 uU/ml), planteándose metástasis pulmonares funcionantes. Se indica terapia ablativa con I131 presentando evolución clínica estable, pero con cifras de 300 mg/dL de tiroglobulina a los seis meses, por lo que se planifica otra sesión con radioyodo. A propósito de este caso, el segundo en cinco a os en la Unidad de Endocrinología del IAHULA, se lleva a cabo una revisión de la literatura sobre esta entidad nosológica. Conclusiones: En ni os, el Ca papilar de tiroides puede ponerse en evidencia por las manifestaciones clínicas debidas a la afectación pulmonar, con un pronóstico reservado, sobre todo si se trata de metástasis pulmonares de tipo miliar. Objective: To present the case of a female adolescent admitted with findings of pulmonary illness due to miliar pulmonary metastases of thyroid cancer. Clinical Case: A girl of 13 years old with non productive cough, dyspnea, palpitations, precordial pain and decrease of weigh, since fifteen days before she consulted at the pediatric emergency. Abstract: purpose: to describe a case of masquerade syndrome as the first manifestation of lung adenocarcinoma. methods: a 57- year-old female was referred with a 6-month history of blurred vision in her right eye. she was treated for relapsing uveitis but signs and symptons remained despite treatment with topical steroids. on physical examination she had conjuctival hyperemia, moderate cilliary injection and choroidal peripapillary lesion. ancillary examinations were requested. results: a lung adenocacirnoma was diagnosed as the primary lesion. the choroidal peripapillary lesion in her right eye was consistent with the appearance of a choroidal metastatic lesion. conclusions: the incidence of uveal metastasis ranges from 0.7 to 12% in patients with pulmonary tumors. treatment depends on characteristics such as size of the lesion, number of metastases, visual acuity, bilaterality and the patient′s general health. as confirmed by this case, ocular involvement may be the first clinical manifestation of lung adenocarcinoma.
