Abstract: Durante el embarazo las arritmias pueden comprometer el pronóstico fetal y la estabilidad hemodinámica de la madre. La fibrilación auricular en el embarazo es infrecuente pero requiere un reconocimiento de manera precoz para prevenir el embolismo cerebral, las alteraciones circulatorias, el aborto y el parto prematuro. El manejo médico no sólo se circunscribe a controlar el ritmo o la frecuencia cardiaca, o en la paciente inestable a realizar cardioversión eléctrica; es necesario también, de acuerdo al tiempo, realizar anticoagulación Some arrhythmias during pregnancy can compromise fetal outcome and hemodynamic stability of the mother. Atrial fibrillation in pregnancy is an infrequent arrhythmia that needs to be recognized at an early stage to prevent cerebral embolism, circulatory disorders, miscarriage and premature delivery. Medical management is not only limited to control the pace or heart rate, or the unstable patient to perform electrical cardioversion, it is also necessary, according to time perform anticoagulation.

Abstract: a mobile thrombus into the right auricle leads to a high risk of pulmonary embolism and its presence in a patient with auricular fibrillation during a acute myocardial infarction is uncommon. this is the case of a female patient presenting with acute myocardial infarction, auricular fibrillation and a mobile thrombus in right auricle with risk of pulmonary embolism, which disappeared after platelet, anticoagulant and anti-aggregating treatment.

Abstract: introduction. auricular fibrillation is the most frequent preoperative cardiac arrhytmia among the patients undergoing cardiovascular surgery. the medical treatment, the transthoracic or intracardiac cardioversion with shock of synchronized continuous current, the stimulation (pacemaker), the ablation with catheter and surgery, where the surgical methods, the catheter techniques for ablation and the mixed techniques are included, are among the treatment modalities. methods. a prospective and descriptive study of 25 patients operated on of mitral pathology with chronic auricular fibrillation was conducted. they underwent radiofrequency ablation during 60 s, with 150 w of energy and control of a temperature of 60 oc. the procedure started around the pulmonary veins, and they were later included. all the patients were postoperatively followed up for a year and the efficiency of the procedure was evaluated in success and failure. results. the best efficacy of the treatment was related to the lowest age of the patients. the longer the time of evolution of the auricular fibrillation, the higher the rhythm of failure of the procedure. the same happened with the size of the left auricle. it was observed that a year later, 76 % of the patients were in sinusal rhythm and none of the complications presented were related to the procedure itself. conclusions. it was concluded that the ablation with intraoperative radiofrequency during the surgical treatment of the mitral valve was an effective and safe procedure in the treatment of patients with chronic associated auricular fibrillation

Abstract: Auricular acupuncture has been utilized in the treatment of diseases for thousands of years. Dr. Paul Nogier firstly originated the concept of an inverted fetus map on the external ear. In the present study, the relationship between the auricular acupuncture and the vagal regulation has been reviewed. It has been shown that auricular acupuncture plays a role in vagal activity of autonomic functions of cardiovascular, respiratory, and gastrointestinal systems. Mechanism studies suggested that afferent projections from especially the auricular branch of the vagus nerve (ABVN) to the nucleus of the solitary tract (NTS) form the anatomical basis for the vagal regulation of auricular acupuncture. Therefore, we proposed the “auriculovagal afferent pathway” (AVAP): both the autonomic and the central nervous system could be modified by auricular vagal stimulation via projections from the ABVN to the NTS. Auricular acupuncture is also proposed to prevent neurodegenerative diseases via vagal regulation. There is a controversy on the specificity and the efficacy of auricular acupoints for treating diseases. More clinical RCT trials on auricular acupuncture and experimental studies on the mechanism of auricular acupuncture should be further investigated.

Abstract: Un trombo móvil en la aurícula derecha implica un riesgo elevado de embolismo pulmonar y la presencia del mismo en un paciente con fibrilación auricular en el curso de un infarto agudo de miocardio, es infrecuente. Se presentó una paciente con infarto agudo de miocardio, fibrilación auricular y trombo móvil en aurícula derecha con riesgo embólico pulmonar, que desapareció luego de tratamiento antiagregante plaquetario y anticoagulante. Abstract: La oreja, debido a su situación expuesta en la cabeza, es un órgano susceptible de sufrir múltiples traumas e incluso pérdida; así mismo, es asiento de neoplasias malignas, carcinomas y melanomas que igualmente finalizan en amputación. Fuera de estas razones, la ausencia congénita del pabellón auricular es la segunda malformación facial de origen congénito más frecuente después del labio y paladar fisurados. Es común por tanto para el cirujano plástico verse frente a pacientes que consultan solicitando la solución a su deformidad. El presente artículo trata de se alar el camino a seguir dividido en cuatro etapas de acuerdo a los principios definidos por el Dr. Radford Tanzer y perfeccionados a través de los a os por el Dr. Burt Brent, e ilustra el manejo de la reconstrucción en casos de microtia, amputación auricular y el uso de colgajos de fascia temporal en los casos en que la piel de la región mastoidea ha sido comprometida y no es posible usarla como elemento primario para la reconstrucción auricular. Due to its very exposed situation on both sides of the head, the auricle is very susceptible to suffer severe traumas, that will at the end cause its amputation; it′s too one of the most common sites to present malignant tumors, carcinoma and melanoma, that will also end in the surgical amputation of the ear. Congenital absence of the ear, microtia, is the second most common facial anomaly after cleft lip and palate. Not infrequently plastic surgeon confronts with patients seeking an answer for correction of their deformities. The present article tries to show a four steps method, according to the principles described first by Dr. Radford Tanzer and later improved by Dr. Burt Brent and illustrates how to manage the auricular reconstruction in cases of congenital microtia or traumatic loss of the auricle, as well as the use of the temporal fascia flap as a way to provide skin coverage, when the skin of the mastoid area has been compromised and it is not useful for auricular reconstruction.

Abstract: The great auricular nerve (GAN) is the largest branch of the superficial cervical plexus that winds around the posterior border of the sternocleidomastoid muscle, accompanied by the external jugular vein. Forty fetuses (right side: 40/80; left: 40/80) with gestational ages between 15 to 28 weeks were microdissected to document the anatomy of the GAN. The results obtained were classified as: i) Incidence and morphometry: GAN was present in 100% of the fetal specimens with average length on the right and left sides recorded as 12.65 ± 2.14 mm and 12.55 ± 2.82 mm respectively. ii) Course: GAN was located parallel to the transverse cervical nerve and the external jugular vein. Duplicate external jugular veins were observed in 5% (4/80) with GAN located anterior to one of the tributaries; iii) Branching Pattern: 16% (13/80) of specimens depicted a single branch. Duplicate branches in 67% (54/80) (referred to as Type I: anterior and posterior branches and 33% (26/80) referred to as Type II: anterior and posterior branches; iv) Variation: the transverse cervical nerve formed a communication with GAN, inferior to the parotid gland in 1%. The anatomical knowledge of the course, bifurcation pattern and variations of GAN may prevent complications during surgical procedures such as parotidectomies. El nervio auricular mayor (NAM) es el ramo más grande del plexo cervical superficial que gira alrededor del margen posterior del músculo esternocleidomastoideo, acompa ado de la vena yugular externa. Cuarenta fetos (lado derecho: 40/80; izquierdo: 40/80), con edades gestacionales de 15 a 28 semanas fueron microdisecados para describir la anatomía del NAM. Los resultados obtenidos se clasificaron en: i) Incidencia y morfometría: NAM estaba presente en el 100% de las muestras fetales con una longitud media de los lados derecho e izquierdo de 12,65±2,14mm y 12,55±2,82mm, respectivamente. ii) Curso: NAM se encuentra paralelo al nervio cervical transverso y la vena yugular externa. Duplicación de las venas yugulares externas se observaron en el 5% (4/80) con el NAM situado por delante de uno de los afluentes, iii) Patrón de ramificación: 16% (13/80) de las muestras presentaba una solo ramo. Ramos duplicados en el 67% (54/80) de Tipo I, ramos anterior y posterior y, el 33% (26/80) Tipo II, ramos anterior y posterior, y iv) Variación: el nervio cervical transverso formando una comunicación con NAM, inferior a la glándula parótida en el 1%. Abstract: Not available Sila elección está causada por factores ajenos a la voluntad del agente, la libertad y la responsabilidad moral parecen perder su base. Pero si la elección carece de causas, se convierte en un acto irracional y, con ello, irresponsable. La salida de este dilema consiste en advertir la importancia de las razones morales en la deliberación práctica. De acuerdo con la tesis central del presente trabajo, la sensibilidad hacia las razones morales es una condición necesaria de la libertad y la responsabilidad moral. La inconmensurabilidad de tales razones con las razones no morales concede a la elección el lugar central que demanda la responsabilidad moral, pero, al mismo tiempo, la mantiene vinculada a la racionalidad, evitando su conversión en un acto irresponsable. Si esto es así, la disminución (o pérdida) de la sensibilidad de un agente hacia las razones morales significa también la disminución (o pérdida) de su libertad y responsabilidad moral.
