Tehran University of Medical Sciences: Participation in Twenty Five Percent of Pediatric Science Production in Iran  [PDF]

Abstract: Objective: Scientific articles are indicators of research interests and efforts in every country. The aim of our study was to compare the scientific output between universities of medical sciences in Iran on pediatrics in the Pubmed database.Methods: In a descriptive study, we searched in Pubmed in order to assess the Iranian pediatrics publications with these words: "(Pediatric [*MeSH] OR Neonate [MeSH] OR Newborn[MeSH] OR Infant [MeSH] OR Child [MeSH] OR Children [MeSH] OR Adolescent[MeSH] OR Adolescence [MeSH])" AND (Iran [*AD] OR Iranian [AD] OR Persian [AD]). The identified articles were published May 1957 to December 2007 and we selected all articles from universities of medical sciences in Iran. We extracted year of publication, affiliation of authors and impact factor from hard copy of articles.Findings: From 819 papers published, 565 were authored by members of universities of medical sciences. Tehran University of Medical Sciences with 142 articles (17.3% of all, 25.1% from publications of universities of medical sciences) held the first place, Shiraz University of Medical Sciences with 116 (14.2% of all, 20.5% from publications of universities of medical sciences) the second place and Shaheed Beheshti University of Medical Sciences with 88 (10.7% of all, 15.5% from publications of universities of medical sciences) the third place. Regarding Impact Factors (IF) Tehran, Shaheed Beheshti and Shiraz Universities of Medical Sciences with 153, 93 and 81 were in the first, second and third place, respectively.Conclusion: Tehran University of Medical Sciences had twenty five percent of pediatric science production in Iran. This university had published higher number of papers on pediatrics among universities of medical sciences in Iran.
The study of Nursing Staff Participation in Strategic Decision Making in Tehran University of Medical Sciences Shariati Hospital

Abstract: Background and Aim: The main goal of each hospital - like all health care sectors - is protecting and increasing the health level in orsig country. The important role of human resources in hospitals is appropriate strategic decisions making for fast environmental changes and shows the need of staff participation. Nurses as a one of the biggest parts of hospital staff need to participate much more in strategic decision making in hospitals. This article is aimed to measure the level of nurses' participation in strategic decisions making.Materials and Methods: This research is descriptive - analytic. Total nurses of Shariati hospital were 577 nurses of which 175 nurses were selected and studied.The gathering information instrument was questionnaire adapted from Dustdar thesis. This questionnaire contained 35,5 - choice questions set by Likert basis. Project's data was analyzed with SPSS software in % 95 confidence levels and statistical specifications like mean, standard error,correlation and non parametric tests were used.Results: According to the results obtained, there are meaningful relation between staff participation in strategic decision making and: marriage situation (PV=0.01), Decision Directing (PV=0.0), culture of participation (PV=0.00), organization maturity (PV=0.00) and risk tolerance (PV=0.01) Conclusions: It has been observed that nurse participation in strategic decision making in Shariati hospital is in "low" position (participation mean score: 28.52 from 100). So an administrator effort for spreading culture of participation to owner's process is something that should be paid attention more than before.
Organizational Behavior of Employees of Tehran University of Medical Sciences

Abstract: Organizational behaviors are commonly acknowledged as fundamentals of organizational life that strongly influence both formal and informal organizational processes, interpersonal relationships, work environments, and pay and promotion policies. The current study aims to investigate political behavior tendencies among employees of Tehran University of Medical Sciences (TUMS).This cross-sectional, descriptive and analytical study was conducted on 810 TUMS employees at the headquarters of the Tehran University of Medical Sciences, Iran during 2010-2011. The research tool for data collection was a researcher-tailored questionnaire on political behaviors. The validity of the questionnaire was confirmed by seven management professors, and its reliability was tested by a pilot study using test-retest method which yielded a Cronbach’s alpha coefficient of 0.71. The respondents were asked to fill the questionnaire and express their perceptions and tendencies to engage in organizational behaviors. The collected data was read to and analyzed by IBM SPSS environment and correlation analytical methods.Overall, 729 respondents filled and returned the questionnaire yielding a response rate of 90%. Most of the respondents indicated that they had no tendency to engage in political behavior. Moreover, we found that there was a significant correlation between sex, higher education degrees, tenure and the employees’ tendency to engage in political behavior.The participants were not overtly political because of their personal belief, ethical values, and personal characters. Non-political and overtly political employees are both prejudicial for all organizations. Therefore, it seems that the medium rate of good political behavior is vital and prevalent in Iranian organizations.
Factors Affecting Organizational Centralization in Hospitals Affiliated with Tehran University of Medical Sciences

Abstract: Background and Aim: Organizational centralization, an important concept/system in management, relates to hierarchy of authority and degree of participation in decision-making. In this study we aimed to determine the extent of organizational centralization in hospitals affiliated with Tehran University of Medical Sciences (TUMS), Tehran, Iran and identify factors affecting the organizational centralization at the level of executive managers.Methods and Materials: All the managers and head nurses of 13 TUMS hospitals (26 subjects) were included in this cross-sectional study in 2005. Data, collected using questionnaires, were analyzed using the following tests: t-test, ANOVA, Pearson and Spearman, the software being the SPSS. The extent of organizational centralization was categorized into centralized, semi-centralized, and uncentralized.Results: Five of the 5 variables studied were statistically significantly associated with organizational centralization. They were gender (p=0.001), organizational position (p=0.005), attending management courses (p=0.001), and financial burden of decisions (p=0/03) in the hospital managers group, and possibility of controlling the decision (p=0.014) in the head-nurses group. Conclusion: The average organizational centralization in the hospitals was 75.38%; it was 76.3% and 73.38% for hospital managers and head-nurses, respectively. On the whole, the management system of TUMS hospitals is a semi-centralized system.

Abstract: The management of medical waste is of great importance due to its potential environmental hazards and public health risks. In the past, medical waste was often mixed with municipal solid waste and disposed in residential waste landfills or improper treatment facilities in Iran. In recent years, many efforts have been made by environmental regulatory agencies and waste generators to better managing the wastes from healthcare facilities. This study was carried in 12 educational hospitals of Tehran University of Medical Sciences. The goals of this study were to characterize solid wastes generated in healthcare hospitals, to report the current status of medical waste management and to provide a framework for the safe management of these wastes at the considered hospitals. The methodology was descriptive, cross-sectional and consisted of the use of surveys and interviews with the authorities of the healthcare facilities and with personnel involved in the management of the wastes. The results showed that medical wastes generated in hospitals were extremely heterogeneous in composition. 42% of wastes were collected in containers and plastic bags. In 75% of hospitals, the stay-time in storage sites was about 12-24h. 92% of medical wastes of hospitals were collected by covered-trucks. In 46% of hospitals, transferring of medical wastes to temporary stations was done manually. The average of waste generation rates in the hospitals was estimated to be 4.42kg/bed/day.
The Amount and Causes Deductions of Bills in Tehran University of Medical Sciences Hospitals

Abstract: Background and Aim: Annually, large amounts of fees that paid by hospitals will not be reimbursed as deductions by health insurances and takes irreparable financial losses to hospitals. The purpose of this study was to determine the amount of deductions imposed on hospital bills of Tehran University of Medical Sciences and their causes related to documentation. Materials and Methods: The present research was a cross-sectional and descriptive study performed in year 2009 on educational hospitals of Tehran University of Medical Sciences. All deductions reports related to Medical Services and Social Insurance related to year 2008 was collected from Income Unit of hospitals. The amount of deduction of each hospital was extracted and organized in the form of comparative tables. Data was analysed by descriptive statistics and Excel application. Then, the amount, type and causes of annually deduction of each hospital was determined. Results: Most deductions imposed on inpatient bills have been related to the tests, appliances, medicine, residency, surgeon commission, and anesthesia and for outpatient bills have been related to visit, tests and medicine which most of them have been created due to documentation deficiencies. Conclusion: Most of deductions are due to unfamiliarity of care staff with documentation requirements of insurance organizations. Therefore it is necessary to use a multi-aspect mechanism including education of documentation principles to staff, supervision on record control in the Medical Record Unit and establishment of a committee by university for related activities.

Abstract: Background and Aim: Internal Evaluation is a process of providing evaluative-descriptive information based on the value and significance of educational objectives, operations and results to direct decisions, responses, information and research processes. Materials and Methods: In this research, studying the opinions of faculty members, students, and graduates, the educational system of Medical Laboratory Sciences Department at Tehran University of Medical Sciences (TUMS) was evaluated in the year 2010. The aim the study was to remodel and adjust the educational system with the society's needs and to promote the educational quality. The employed method was a cross-sectional, descriptive one and consisted of 10 procedural steps in eight sections dealing with the evaluated factors such as faculty members and students, and the satisfaction expressed by students. Results: The results of the study showed that the average of the eight investigated sections was 70.6% for the Department of Medical Laboratory Sciences, which can be interpreted as desirable. Conclusion: Internal evaluation can be considered as one of the best methods of improving the quality of education in the faculty of Allied Health Sciences School

Abstract: Background: Evaluation is a systematic process for collecting, analyzing and interpretation of the obtained information for the purpose of investigating how many the objectives in mind can be achieved. Education in an academic educational system means bringing about proper and increasing changes that affect the outcome of this system, that is affecting the students, and the aim of such evaluation was promotion of the quality of an education process. Methods: In this study, Department of Pediatrics Faculty of Medicine, Tehran University of Medical Sciences Tehran University of Medical Sciences, benefiting from opinion poll of university lectures, student and graduates have evaluated their system of educational in the years of 2003- 2004 and their aim was to remodel and adjust educational system with the society’s needs and promotion of the quality of such educations. The employed method was cross sectional and descriptive and analytical on the basis of 10 procedural steps and with in 8 sections dealing with evaluated factors. Such as faculty board members, management and organizational capability, students, manpower and logistic affairs, educational environments research work centers, health and therapeutic sections, educational equipment, research equipment, laboratory and diagnosis centers, educational courses and programs, teaching and learning process as well as satisfaction expressed by students. Findings: the results of the research showed that general average of 8 investigating sections in 2003 and 11 investigating sections in 2004, based on SWOTs model were respectively %53.3, %75.3 that may well be interpreted as a desirable research work. So 80% graduates of this group were satisfaction from their educational programs. Conclusion: Internal evaluation is the best indicator showing how much we should go to achieve certain aims, analyzes quality of the activity if such a system and by which we achieve logical and routine results.
Zinc Deficiency in First Year Female Students of Tehran University of Medical Sciences

Abstract: "nObjective: Zinc deficiency is an important health problem in developing countries. The aim of this study was diagnosis of zinc deficiency among young female students of Tehran University of Medical Sciences."nMaterials and methods: This cross sectional study evaluated 420 medical and paramedical students of Tehran University in 2005. All of them were female and in the first year of their education. Serum concentrations of zinc were measured by enzymatic method students in whom had sufficient criteria to enroll in the study. Zinc concentration more than 85 μ/dl was considered as normal value. Concentration less than 50 μ/dl was defined as severe zinc deficiency. Mild and moderate deficiencies were described as zinc levels between 50 to 85 μ/dl. Analysis was performed by t-test, chi-square and ANOVA using SPSS software. "nResults: Zinc deficiency was found in 7.1% of the participants. In women with body weights between 50.1-60 kg, zinc deficiency was less than patients with body weight of ≤ 50 Kg (p=0.04). In zinc deficient group acne was observed more frequently (p=0.01). Also anemic participants (Hb<12) had lower zinc level than non anemic women (Hb≥12) (p=0.001)."nConclusion: This study found severe and mild to moderate zinc deficiency in 1.4% and 5.7% of female students, respectively. Zinc deficiency is common in anemic students. Zinc supplementation is recommended in anemic patients and in cases of confirmed zinc deficiency.
Quality of Work Life in Nursing Employees of Tehran University of Medical Sciences Hospitals

Abstract: Background & Objective: Administrators have to pay attention to nurses as an important group among hospitals employees in order to have better patient outcomes. Recently, nurses' quality of work life is a new concept that attention to it can improve organizational efficiency. This research is to investigate the quality of work life of nurses at Tehran University of Medical Sciences Hospitals in 2005-2006.Methods & Materials: This is a cross-sectional study. The research was conducted among 349 nursing employees. Data were gathered using a questionnaire consisted of 30 elements of QWL. The respondents were asked to determine their overall quality of work life. Validity of the questionnaire was evaluated using content validity and reliability was determined using test-retest method. Statistical analysis was carried out using SPSS statistical software package.Results: The results showed that the majority of nurses were dissatisfied with environmental health and safety at work, monetary compensation and job welfare. There was significant correlation between the executive position of nurses, years of experience and their salary (P<0.05).Conclusion: The study findings showed that nurses had a poor quality of work life. This indicates that majority of employees are unsatisfied with the most aspects of their quality of work life.
