Abstract: A question that would have to worry more to the global justice is the huge inequality of health in the world. In this paper, I analyse the causes of the global inequality of health and the ethical arguments for and against the need to treat this inequality from the perspective of the global justice. After refusing the arguments against both of the libertarianism and the statism, and after showing the critics both to the approach to the language of human rights and to the poggean proposal to reduce the duties of global justice to negative moral duties, I defend the need to use a sufficiency approach to global justice based in positive moral duties to face the severity and the moral urgency of the global inequalities of health. Una de las cuestiones que debería preocupar más a la teoría de la justicia global es la enorme desigualdad de salud que hay en el mundo. En este artículo, se repasan las causas de la desigualdad global de salud y los argumentos éticos a favor y en contra de la necesidad de tratar dicha desigualdad desde la perspectiva de la justicia global. Tras rechazar los argumentos en contra tanto del libertarismo de derechas como del estatalismo, y tras exponer las críticas tanto al lenguaje de los derechos humanos como a la propuesta poggeana de reducir los deberes de la justicia global a deberes morales negativos, defiendo la necesidad de partir de un criterio de justicia global suficientista y basado en los deberes morales positivos para afrontar la gravedad y la urgencia moral que suponen las desigualdades globales de salud.

Abstract: Diversos estudios a nivel mundial se alan la inequidad en salud según categorías étnicas o asociado con el estado de inmigrante. La perspectiva intercultural se aplica, por lo tanto, en países y situaciones muy distintas, y los criterios para definir los grupos sociales enfocados tal como los planteamientos varían según el contexto local. Sin embargo, hay desafíos comunes que son relevantes en todas las situaciones donde el tema de salud y etnicidad se pone de manifiesto. Además, a diferencia de la noción "interculturalidad", los problemas claves que comprometen, por ejemplo, la atención sanitaria para poblaciones indígenas o inmigrantes no son recientes, sino la historia es rica en experiencias previas muy significativas para la actualidad. Desde una visión global e histórica, el presente artículo pone a consideración tres puntos claves: la necesidad de definir adecuadamente a los grupos enfocados para evitar estereotipos culturales y la creación de desigualdades nuevas, el reto de convertir etnicidad en una categoría "saludable" y la reflexión explícita de la dimensión ética y moral, basada en la ética médica codificada y los derechos humanos. Health inequalities are broadly documented for ethnic minority groups and immigrants worldwide. Intercultural perspectives in health are thus developed in very different places and situations. Both the criteria for defining the target groups as well as the ways the health problems of these groups are looked at, are shaped by the particular local context. However, some challenges are common to almost all situations where the topic of ethnic diversity and health is considered. And in contrast to the rather novel term, the issues at stake in "intercultural heath" are not new at all. Against this background, the present article brings into focus three essential points: the necessity of defining ethnicity adequately to avoid stereotyping and the creation of new inequalities; the challenge of converting ethnicity into a helpful and thus "healthy" category in the field of medicine and health; and the need for the integration of explicit and serious reflections on medical ethics and human rights that provide for the normative framework and moral orientation of health activities with indigenous, migrant and other particularly vulnerable groups
Conocimientos acerca del Riesgo Cardiovascular Global en profesionales del Sistema Nacional de Salud The knowledge of the national public health professionals about the Global Cardiovascular Risk

Abstract: Introducción: las enfermedades cardiovasculares siguen siendo las primeras causas de muerte en Cuba y el mundo. Objetivo: valorar el grado de conocimiento sobre el Riesgo Cardiovascular Global en profesionales de la salud. Métodos: estudio exploratorio de corte transversal. Se escogió como pilotaje parte del municipio de La Habana del Este, que cuenta con ocho policlínicos y un Profilactorio Nacional Obrero, que es un centro de salud que recibe anualmente una considerable cantidad de pacientes. El trabajo se ejecutó entre el 1ro. de marzo y hasta el 31 de junio 2010. Se encuestaron 102 profesionales entre especialistas en medicina general integral y licenciados en enfermería, que prestan sus servicios en policlínicos ubicados en el municipio Habana del Este y en el citado Profilactorio; se utilizó para ello una encuesta de diez preguntas dise ada para la investigación. Resultados: la mayoría de los profesionales desconocen lo que es el Riesgo Cardiovascular Global, su importancia, cómo detectarlo y algunas de las enfermedades contribuyentes tales como el síndrome metabólico, la diabetes mellitus y la enfermedad renal. Conclusiones: los profesionales encuestados, cuya mayoría son de la atención primaria de salud, tienen conocimientos insuficientes sobre el Riesgo Cardiovascular Global, de ahí que no están aptos para enfrentarlo. Introduction: cardiovascular diseases are the first cause of death in Cuba and the world. Objective: to characterize scope of the health professionals' knowledge on the Global Cardiovascular Risk. Methods: an exploratory cross-sectional study was conducted from March 1st to June 31st, 2010. One part of La Habana del Este municipality which covers eight policlinics and the National Worker`s prophylactory a health center receiving a significant number of patients yearly- was chosen for the pilot study. The survey was applied to 102 professionals, including General Integral Medicine specialists and Bachelors of Nursing working in polyclinics located in the municipality and in the prophylactory. To this end, a ten-question survey designed by the main author was used. Results: most of the participating professionals had poor knowledge about the Global Cardiovascular Risk, its importance, about the ways to detect it and some diseases such as metabolic syndrome, diabetes mellitus and the renal disease that may influence over this risk. Conclusions: it is concluded that the surveyed professionals, most of them from the primary health care, lack the adequate knowledge to face the global cardiovascular risk.
Salud para la defensa: enfoque conceptual global de la investigación médico-militar

Abstract: Se introduce el término de salud para la defensa. Se exponen los puntos de vista personales del redimensionamiento y perfeccionamiento de la investigación y desarrollo y de la labor temática de investigación y desarrollo de los Servicios Médicos de las Fuerzas Armadas Revolucionarias (FAR). Dentro del concepto de salud para la defensa se incluye la optimización, mediante un dise o apropiado del estudio de las causas médicas que pudieran incidir en la no continuidad del período de vida profesionalmente útil del cuadro militar en su respectivo puesto de trabajo de acuerdo con su especialidad. El segundo aspecto lo constituye la utilización de las investigaciones de corte científico-pedagógico dirigidas a la labor de perfeccionamiento de la preparación de los oficiales médicos y otro personal participante para su desempe o en situaciones de tiempo de paz y tiempo de guerra. Se plantea que dentro del concepto de salud para la defensa tiene cabida la investigación por métodos científicos de las estructuras y procedimientos operacionales concebidos para el funcionamiento de los Servicios Médicos de las FAR en escenarios de guerra y/o catástrofes naturales, en los cuales las estructuras de los escenarios de tiempo de paz resultaren obsoletos o impracticables. The health for defense term is introduced. The personal points of view of the redimensioning and improvement of research and development and of the thematic work of research and development of the Medical Services of the Revolutionary Armed Forces (RAF) are dealt with. Within the concept of health for defense, it is included the optimization by an appropiate design of the study of the medical causes that may inlfuence on the non-continuity of the professionaly useful period of life of the military cadre in his job, according to his specialty. The second aspect referes to the utilization of the scientific and pedagogical research directed to the improvement of the training of the medical officers and other participating personnel for their performance in time of peace and war. It is stated that the research by scientific methods of the structures and operational procedures created for the functioning of the medical services of the Revolutionary Armed Forces in war and/or natural disasters, where the structures of the peace time settings may be obsolete or impracticable, may be included in the concept of health for defense.
El desarrollo de la economía global y su impacto sobre las políticas de salud  [cached]

Abstract: Se describe y explica el impacto sobre la salud humana, a escala mundial, del desarrollo reciente de la economía, en relación con la globalización. Se hace una revisión de la literatura actual de autores sobre el tema. Se presentan en acápites separados para examen los siguientes componentes: enfoque económico, enfoque social, enfoque político y enfoque de salud. Se expone la posición cubana al respecto. Se analizan como consecuencias de la globalización neoliberal de la economía: la pobreza, la inequidad, el deterioro del tejido social, la corrupción, la exclusión, las migraciones, el terrorismo, la limitación de soberanía, el ALCA. Se concluye que la denuncia, como instrumento de lucha en la batalla de ideas, es el primer elemento útil para conseguir revertir la situación crítica que se aprecia, y que el humanismo y la solidaridad son valores insuperables de la lucha actual contra el neoliberalismo salvaje The impact of the recent development of economy on human health at the world level in relation to globalization is described and explained. A review of the current literature on this topic is made. The following components are presented in separate paragraph to be examined: economic approach, social approach, political approach and health approach. The Cuban position on this regard is exposed. Poverty, inequity, the deterioration of the social tissue, corruption, exclusion, migrations, terrorism, the limitation of sovereignty and the F.T.A.A are analyzed as consequences of the neoliberal globalization of economy. It is concluded that the denounce as an instrument in the struggle of ideas is the first useful element to revert the present critical situation and that humanism and solidarity are insuperable values of the fight against the wild neoliberalism

Abstract: En este artículo se ilustra, con la reciente reforma al sistema mexicano de salud, el potencial del conocimiento en el dise o e implantación de las políticas públicas. En primer lugar se discute la relación entre conocimiento y salud. En una segunda parte se describen los esfuerzos que se llevaron a cabo en México para generar evidencias que eventualmente nutrieron el dise o e implantación de las políticas locales de salud. En seguida se analizan los contenidos de la reforma del sistema mexicano de salud y su concepto rector, La democratización de la atención a la salud. El artículo concluye con una discusión de las lecciones globales de esta experiencia de reforma. This paper illustrates, using as an example the recent reform of the Mexican health system, the potential of knowledge in the design and implementation of public policies. In the first part the relationship between knowledge and health is described. In part two, the efforts in Mexico to generate evidence that would eventually nourish the design and implementation of health policies are discussed. In the following sections the content and the guiding concept of the reform, the democratization of health, are analyzed. The paper concludes with the discussion of the main global lessons of this reform experience.

Abstract: Se realizó un estudio descriptivo de corte transversal con el objetivo de determinar algunos factores de riesgo aterogénicos en la población perteneciente al consultorio médico 3 del Policlínico Docente "Dr. Mario Mu oz Monroy", situado en la localidad de Guanabo, municipio Habana del Este, durante el período comprendido entre el 2de marzo al 1ro de diciembre del a o 2008, cuyo propósito general fue realizar un enfoque de riesgo individual (Riesgo Cardiovascular Global) en esta comunidad a partir de algunos factores de riesgo aterogénico identificados, tales como Hipertensión Arterial, Obesidad, Diabetes Mellitus y Tabaquismo, entre otros. La muestra estuvo conformada por los adultos mayores de 20 a os que ascendieron a 325 personas. Los principales resultados obtenidos muestran mayor frecuencia de hábito de fumar, hipertensión arterial y Diabetes mellitus, predominaron los hábitos alimentarios inadecuados,sobre todo, en el sexo masculino. Se pudo determinar que 22.8% de los pacientes presentan riesgos alto y muy alto de morbimortalidad cardiovascular en los próximos 5 a 10 a os y que una gran parte de las personas necesita intervención sanitaria para evitar o disminuir riesgos cardiovasculares en el futuro. A descriptive study of transverse estructure was carried out with the objective of determining some atherogenic risk factors in the particular population of the consulting # 3 of the Dr. Mario Munoz Monroy Teaching Policlinic, situated in the Guanabo health locality of the Habana del Este Municipality, during the period between the 2nd of March to the 1st of December, 2008, with a general proposition to carry out a focus on individual risk (Global Cardiovascular Risk) in this community with respect to some identified atherogenic risk factors such as Arterial Hypertension, Obesity, Diabetes Mellitus and Tabaquism among others. The sample was made up of adults above the age 20 years which came to about 325 persons. The principal results obtained show a higher rate of smoking habit, arterial hipertensión and diabetes mellitus. Inadequate eating habits predominated especially in the masculine sex. It was deduced that 22.8% of the patients present high and very high risk of cardiovascular morbimortality in the next 5 to 10 years and that a very large part of the petients need health intervention so that they can avoid or reduce cardiovascular risks in the future.

Abstract: context: it begins by recognizing several areas for analysis of the global crisis in relation to health. it then focuses on the analysis of some aspects of the economic crisis, policy alternatives to mitigate the financial crisis and the identification of health indicators more sensitive to the vagaries of the economy. objective: to propose some basis for analyzing the financial crisis of 2008 and its effects on health in the world. methodology: it based on the analysis of the possible effects of the economic crisis on health after consider four essential axes: the conditions of life and health determinants, indicators of socio-demographic and epidemiological changes, the total public expenditure and spending on health and monitoring of policies to mitigate the crisis. conclusion: the first crisis of globalization of the present century will have serious health implications and should lead to rethinking the global economic system based on state control and regulation of the markets. to avoid adverse effects on the environment and health, it is proposed addressing the determinants of global influences, to stimulate public policies that invest in health, boost action in international health legislation, and finally, to establish and implement a crisis system monitoring and its effect on health.

Abstract: this article comes from the intense international pressure that follows a near-catastrophy, such as the human influenza a h1n1 epidemic, and the limited resources for confronting such events. the analysis covers prevailing 20th century trends in the international public health arena and the change-induced challenges brought on by globalization, the transition set in motion by what has been deemed the "new" international public health and an ever-increasing focus on global health, in the context of an international scenario of shifting risks and opportunities and a growing number of multinational players. global public health is defined as a public right, based on a new appreciation of the public, a new paradigm centered on human rights, and altruistic philosophy, politics, and ethics that undergird the changes in international public health on at least three fronts: redefining its theoretical foundation, improving world health, and renewing the international public health system, all of which is the byproduct of a new form of governance. a new world health system, directed by new global public institutions, would aim to make public health a global public right and face a variety of staggering challenges, such as working on public policy management on a global scale, renewing and democratizing the current global governing structure, and conquering the limits and weaknesses witnessed by international health.
