Radiocirurgia estereotáxica no tratamento de malforma??es arteriovenosas cerebrais

Abstract: since march 1983 we are attempting for the first time in brazil to develop improvements on stereotactic radiosurgery techniques. we have been using the multifocal irradiation method attaching to this purpose the barcia iv stereotactic frame to a 4 mv linear accelerator. we show details about the centralisation technique and the security patterns in our system. from october 1983 to november 1986, 25 patients with arteriovenous malformations of the brain were submitted to a radiosurgery treatment in our service. good results obtained on the follow-up from one to three years are analysed.

Abstract: Analisamos 53 casos de malforma es arteriovenosas (MAV) admitidos no Servi o de agosto de 1986 a mar o de 1998. A distribui o por sexo foi masculino 1,9:1 feminino, com faixa etária média de 32,5±11,5 anos. A apresenta o clínica inicial incluiu, em ordem de frequência, hemorragia intracraniana, sinais neurológicos focais, cefaléia, epilepsia, e coma profundo agudo. As MAV supratentoriais tiveram a seguinte distribui o: 10 temporais, 9 parietais, 6 frontais, 3 occipitais, 3 fronto-parietais, 4 corpo caloso/pericalosas, 1 têmporo-occipital, 1 têmporo-parieto-occipital e 6 profundas. As MAV de acordo com a classifica o de Spetzler-Martin, distribuiram-se em: grau II 17 casos (32%), grau III 21 casos (39%) e grau IV 10 casos (19%). Em 3 pacientes (5,6%) havia presen a concomitante de aneurisma intracraniano. Trinta e sete pacientes foram operados, 12 dos quais, com emboliza o parcial prévia. Em somente um caso a ressec o cirúrgica foi parcial. Onze pacientes foram somente embolizados, sendo a emboliza o parcial em 3 casos. Dois pacientes n o foram embolizados e nem operados (1 caso devido a pneumonite intersticial por AIDS e outro por problemas administrativos). Três casos foram encaminhados para radiocirurgia. Houve somente um óbito por embolia pulmonar aguda (1,8%). Concluímos que a abordagem multidisciplinar individualizada, envolvendo as especialidades de neurorradiologia intervencionista, neurologia e neurocirurgia, oferece a possiblidade de indicar a terapia mais adequada para cada caso, com bons resultados.

Abstract: radiosurgery is the precise radiation of a known intracranial target with a high dose of energy, sparing the adjacent nervous tissue. technological advances in the construction of linear accelerators, stereotactic instruments and in computer sciences made this technique easier to perform and affordable. the main indications for radiosurgery are inoperable cerebral vascular malformations, vestibular and other cranial schwannomas, skull base meningiomas, deep seated gliomas and cerebral metastases. more recently, the development of fraccionated stereotactic radiotherapy increased the spectrum of indications to bigger lesions and to those adjacent to critical nervous structures. we present our initial experience in the treatment of 31 patients. an adequate control of the neoplastic lesions was obtained and the adequate time of observation is still needed to evaluate the results in arteriovenous malformations.
Doses extracranianas em pacientes submetidos a radiocirurgia estereotáxica para tumores cerebrais

Abstract: objective: to estimate extracranial doses on eyes, thyroid, chest and pelvis in patients submitted to radiosurgery with 6 mv linear accelerator. materials and methods: the present study evaluated 11 patients, 7 of them with primary, and 4 with secondary brain tumors. in the latter group, 2 patients had two lesions. thermoluminescent dosimeters were utilized to estimate the extracranial dose. radiosurgery cones ranges between 1.50 and 3.75 cm and doses between 1300 and 2000 cgy. results: mean patients' age was 52 years, and 63.6% of them were women and 36.4%, men. lesion locations were the following: right acoustic nerve (1), frontal (2), parietal (5), right occipital (1), cerebellum (2) and parasagittal (2). mean received doses were the following: 5.1 cgy between the eyes; 4.8 cgy in the right eye; 6.5 cgy in the left eye; 4.2 cgy in the thyroid; 1.65 cgy in the chest; and 0.45 cgy in the pelvis. conclusion: the results demonstrate that that although the eye doses do not exceed the tolerance limits for occurrence of lens opacity, it is important that the risks associated with radiation doses are taken into consideration by radiotherapists in the planning of cranial radiosurgery procedures.

Abstract: fifty-three patients with arteriovenous malformation (avm) were studied. the mean age at the time of diagnosis was 32.5±11.5 years (range 3 to 56 years); the sex ratio was male 1.9:1 female. the most commmon clinical presentation was intracranial hemorrhage, followed by focal neurological signs, headache, epilepsy, and acute deep coma. fourty-three (82%) out of the total number of avm were supratentorial and had the following distribution: 10 temporal lobe, 9 pariental lobe, 6 frontal lobe, 3 occipital lobe, 3 frontotemporal, 4 corpus callosum/pericalosal, 1 temporo-occipital, 1 temporoparieto-occipital, 6 deep-located (3 thalamic, 3 basal ganglia). spetzler-martin grade ii was seen in 17 cases (32%), grade iii in 21 cases (39%), and grade iv in 10 cases (19%). avm-associated aneurysms were detected in 3 patients (5.6%). all three cases were middle artery aneurysms ipsilateral to avm. twelve (32%) out of 37 operated patients were submitted to embolization prior to surgery. avm was totally removed in all cases except one. embolization without posterior surgery was used in 11 patients; in 3 of them, embolization was partial. two patients were neither embolized nor operated on. three patients were submitted to radiosurgery because they were poor candidates to embolization and/or surgery. mortality rate was 1.8%. all patients returned to their normal life function after a period of recovery. associated or isolated therapies should be individualized to each patient with avm and is likely to provide results better than the expected outcome without any treatment.

Abstract: the association of intracerebral vascular malformations and primary cerebral neoplasm is rare. the most commonly found vascular malformation with neoplasm is intracranial arterial aneurysm. we describe two cases of vascular malformations associated with primary cerebral neoplasms, with histologic and immunohistochemical studies.

Abstract: Mediante una serie de medidas y cálculos Monte Carlo se han determinado las características dosimétricas y los espectros de los fotoneutrones que se producen en torno a un acelerador lineal para radioterapia de 18 MV. Las medidas se realizaron con dosímetros termoluminiscentes TLD 600 y TLD 700 que se expusieron desnudos y emparedados con Cd, así como dentro de una esfera de parafina y dentro de esferas Bonner.Measurements and Monte Carlo calculations has been utilized to determine the dosimetric features as well as the neutron spectra of photoneutrons produced around an 18 MV linear accelerator for radiotherapy. Measurementes were carried out with bare and Cd covered thermolumiscent dosimeters, TLD600 and TLD700, as well as inside a paraffine moderator. TLD pairs were also utilized as thermal neutrons inside a Bonner sphere spectrometer.
