Infec??o pelo HIV entre gestantes atendidas nos centros de testagem e aconselhamento em Aids

Abstract: objective: to estimate hiv prevalence and identify high-risk sexual behavior for infection in pregnant women who were given prenatal assistance. methods: cross-sectional study based on attendance records of 8,002 pregnant women (25% of all municipalities) who lived in 27 municipalities in southern brazil in 2003 and had anti-hiv tests done in a testing and counseling center that performed prenatal assistance. sociodemographic and behavioral data were gathered, as well as syphilis and hiv test results, during the individual counseling sessions registered in the data bank of the sistema de informa??es dos centros de testagem e aconselhamento (information system on testing and counseling centers). women who sought the centers for confirmation of previous serology or were referred to this service due to the presence of aids symptoms were excluded from the data base. results: a total of 0.5% of all the pregnant women analyzed (ci 95%=0.3;0.6) were hiv positive. the only variable associated with hiv seropositivity was schooling. the majority of them were basically exposed through unprotected sexual intercourse with the only partner they had a steady relationship with. younger pregnant women who were single, unemployed and had lower level of education constituted the group with highest exposure. conclusions: the sistema de informa??es dos centros de testagem e aconselhamento turned out to be useful for the epidemiological surveillance of hiv infection and high-risk behavior among pregnant women and could also be useful as regards other populations.

Abstract: Este artigo analisa alguns dados de uma pesquisa realizada no ano de 1998, nos três CTAs localizados no município do Rio de Janeiro. A metodologia utilizou dados quantitativos e qualitativos. Analisamos os dados de produ o dos três servi os relativos ao ano de 1997, incluindo a soroprevalência ao HIV e dois questionários aplicados em 1998, voluntariamente respondidos pela clientela. Entre as conclus es podemos destacar: a) necessidade de se ampliar a divulga o desses servi os para as pessoas de baixa escolaridade e/ou baixa renda, uma vez que as pessoas incluídas nesse grupo obtiveram taxas de soropositividade mais altas e outros estudos as consideram mais vulneráveis às DST/HIV; b) necessidade de se estender a experiência dos CTAs quanto ao aconselhamento e testagem anti-HIV para outros servi os públicos de saúde; c) indicar que a prática do aconselhamento deve levar em conta os riscos individuais e coletivos às DST/HIV, incluindo as diferen as culturais entre os gêneros e as várias formas de relacionamentos existentes, impossíveis de serem normatizadas por orienta es gerais.

Abstract: O Centro de Testagem e Aconselhamento (CTA) foi instituído para atuar no diagnóstico e preven o de infec es sexualmente transmissíveis (IST) e constitui-se em importante fonte de dados epidemiológicos. Com o objetivo de identificar o perfil epidemiológico dos usuários do CTA no município de Alfenas-MG foi realizado um estudo transversal, descritivo e exploratório, no período de agosto de 2007 a junho de 2008. Foram utilizados dados secundários dos usuários cadastrados noservi o, que foram armazenados e analisados por meio do Programa Epi-info vers o 6.04. No total, foram realizados 162 atendimentos, com predomínio de pessoas do sexo feminino (53,1%) e média de idade de 35,9 anos. As taxas de positividade para o HIV, hepatites B e C e sífilis foram 1,9%, 14,4%, 5,9% e 1,9%, respectivamente. O principal motivo de procura pelo CTA foi a exposi o à situa o de risco (43,8%). O estudo evidenciou que, por meio das a es do CTA, épossível monitorar o status sorológico, o perfil sociodemográfico e os comportamentos relacionados ao HIV/Aids e às outras IST na clientela atendida, o que contribui para o alcance dos objetivos relacionados a preven o e controle dessas doen as.

Abstract: Con la intención de identifi car algunos de los desafíos que los Centros de Testaje y Aconsejamiento (CTA), organización tecnológica para la prevención de la DST/Aids en el Brasil, respondiendo al propósito de facilitar el diagnóstico temprano de la infección de HIV, este artículo apunta su estructuración en el Brasil y describe algunas características actuales de estos servicios en el estado de Sao Paulo, basado en el análisis de documentación y la literatura específi ca. Se observa que los CTA han alterado su perfi l, no sólo debido a los cambios de la epidemia, pero también debido a posibilidades de servicios, lo que puede comprometer su efectividad.

Abstract: this article analyzes the incorporation of counseling practice into anti-hiv testing service as the result of a social discursive construction of aids in the context of late modernity and based on a proposal by the brazilian std/aids national program. the main analytical references were the global influences in democratization process of modern practices and discourses contextualized by fairclough. other references were the evolution of preventive discourses broadcasted since the infection was first identified, and disciplinary power and confession technologies. counseling includes major themes such as 'technologization', reflexivity, and dialogue. in this practice a game of ambiguities can be identified between the democratic and coercive systems which conjugate liberating and disciplinary modes. understanding counseling from the vantage point of this ambiguity is to deal with issues still little investigated. it is necessary to understand counseling practice from the user's point of view, inasmuch as he/she is the end recipient of this practice.

Abstract: objective: to characterize and analyze technological profiles of voluntary hiv counseling and testing centers in brazil. methods: a structured self-completion questionnaire with 78 questions was used. this questionnaire was answered by 320 (83.6%) of the 383 brazilian centers, in 2006. responses that characterized the services' technological profile were analyzed using k-means clustering technique. associations between the profiles described and the municipal contexts were analyzed using the chi-square and residue analysis for proportions, and anova and bonferroni for means. results: centers showed significant deficiencies to guarantee adequate services. a total of four technological profiles were identified. the "care" profile (21.6%) predominated among the services instituted before 1993, in areas with high aids incidence and in large cities. the "prevention" profile (30.0%), prevalent between 1994 and 1998, was the type that best complies with the ministry of health's norms, with better readiness and productivity indicators. the "care and prevention" profile (26.9%), included in the aids services, predominated between 1999 and 2002, and developed the most comprehensive set of activities, including std treatment. the "testing" profile (21.6%) was the most precarious, found where the epidemic is most recent and with a lower number of people tested. conclusions: counseling and testing centers constitute a set of heterogeneous services. in addition, service implementation guidelines have not been completely incorporated in brazil, thus having and influence on low resolution and productivity indicators and also the inadequate development of prevention activities.

Abstract: this article analyzes data from a research study conducted in 1998 at three testing and counseling centers, in the municipality of rio de janeiro. the methodology used was both quantitative and qualitative for data collection and analysis. the research team analyzed data from the files of cases assisted at the centers in 1997, including hiv seroprevalence, and two questionnaires that were applied in 1998, answered voluntarily by the clients. among the conclusions we can emphasize: a) the need to promote the availability of these free and voluntary testing and counseling centers to segments of the population with less schooling and lower income levels, since a higher percentage of low income clients and with lower levels of education, were among clients with positive hiv tests results. in addition, other studies have pointed out that these clients are more vulnerable to other std as well as hiv; b) the need to extend the experiences of the testing and counseling centers to others public health services; c) to indicate that the counseling practices must consider individual and collective risks to std and hiv, including cultural differences between genders and the many possible ways of relationships that cannot be normalized by general orientations.

Abstract: the centros de apoio familiar e aconselhamento parental (cafap, center for family support and parental counseling) correspond, in the portuguese reality of portuguese services, to a category of services designed to support families with at-risk children and youth. however, these services tend to operate in many different ways, still without legal and technical framing. in the present article, one aims to review the cafap's objectives and present a global organization model for these services, so they can operate as effective support resources for the activities of the courts, the child protection services and the community in the protection and promotion of families', children's and youth's well-being. with the purpose of creating common practices and methodologies, the proposed model offers a basic framework for the development of future research and evaluation efforts in regard to the efficacy of the services provided.

Abstract: Este trabalho apresenta a avalia o das a es de aconselhamento que se efetuam nos servi os do Sistema único de Saúde (SUS) nos seguintes contextos institucionais: Centro de Testagem e Aconselhamento (CTA), Servi o de Assistência Especializada em HIV/Aids (SAE) e servi os de saúde que atendem indivíduos portadores de doen as sexualmente transmissíveis (DST). A metodologia empregada foi a abordagem qualitativa para avalia o. A investiga o ocorreu no período de mar o a setembro de 1997. Foram avaliados quatro CTAs, cinco servi os de assistência a indivíduos com DST e um SAE - todos reconhecidos pela Coordena o Nacional de DST-Aids como centros de treinamento. No presente artigo, analisa-se a prática de aconselhamento desenvolvida nesses servi os, comparando-a com as diretrizes preconizadas pela Coordena o Nacional de DST/Aids (CN DST/Aids): a) exercício de acolhimento; b) escuta ativa; c) comunica o competente; d) avalia o de riscos e reflex o conjunta sobre alternativas para novos hábitos de preven o; e) orienta o sobre os aspectos clínicos e do tratamento (com vistas à ades o e melhoria da qualidade de vida). Ao término do artigo, s o sugeridos alguns redirecionamentos considerados necessários para uma realiza o mais efetiva dessa prática.

Abstract: this study analyzes the user profile of hiv testing and counseling centers in santa catarina state, brazil, in 2005, and factors associated with hiv infection. the methodology employed a retrospective, cross-sectional study of individuals who attended testing and counseling centers. data from 22,846 interviews were analyzed (64.7% women and 35.3% men). hiv prevalence was 2.0% in women and 5.6% in men. statistical analysis used bivariate and multivariate poisson regression by gender. according to the poisson regression, factors associated with hiv+ status were age bracket, schooling, marital status, professional situation, population group, steady partner's risk status, condom use, and reason for not using condoms with steady partner; for men, the independent variables were age bracket, schooling, type of partner, population group, steady partner's risk status, condom use, and reason for not using condoms with steady partner. hiv+ patterns differed between men and women, so that customized preventive approaches are needed.
