Abstract: Relata-se a ocorrência de resistência à mefloquina administrada na dose de 20mg/kg em 51 crian as com malária falciparum atendidas em centro de referência em Manaus, Brasil, no período de outubro a novembro de 1997. Todas as crian as foram avaliadas nos dias 3, 5, 7, 14, 21, 28 e 35 do tratamento, segundo critérios clínico e parasitológico. Foi encontrada uma incidência de resistência RIII de 5,9% (IC 95% variando de 1,5 a 17,2), a raz o cura/resistência calculada foi 20:1 e cura/gravidade 62:1. Os dados chamam a aten o para a importancia da resistência à mefloquina nesse grupo de crian as.

Abstract: As características clínicas da malária falciparum foram estudadas em 61 crian as com idade de 0 a 14 anos, atendidas em centro de referência em Manaus, entre outubro e dezembro de 1997. Os sintomas encontrados foram febre (98,4%), cefaléia (80,3%), calafrios (68,9%), sudorese (65,6%), mialgia (59%), náuseas (54,1%), lombalgia (49,2%), v mitos (49,2%), tosse (45,9%), artralgia (31,1%), diarréia (34,4%), dispnéia (8,2%), convuls es (8,2%) e tonturas (4,9%). Palidez cutaneo-mucosa e anemia foram observadas mais freqüentemente nas crian as menores de 5 anos. A anemia esteve asssociada aos maiores níveis de parasitemia. Cinqüenta e oito (91,5%) pacientes apresentaram malária n o complicada, 3 (4,9%) malária grave e a letalidade foi 1,6%.

Abstract: OBJETIVO: Avaliar as formas de tratamento empregadas e os principais aspectos relacionados à morbidade e à mortalidade dos ferimentos cardíacos.. MéTODOS: Estudo retrospectivo de 102 doentes com les o cardíaca, atendidos nos dois prontos socorros de Manaus (Pronto Socorro Municipal 28 de Agosto e Hospital Pronto Socorro Dr. Jo o Lúcio P. Machado) no período de janeiro de 1998 a junho de 2006. RESULTADOS: Dos 102 doentes, 95,1% eram homens; a média de idade foi 27 anos; ferimentos por arma branca representaram 81,4% dos casos, contra 18,6% por arma de fogo; cardiorrafia foi realizada em 98,1% dos casos. As camaras cardíacas atingidas foram: VD: 43,9% (36,2% isoladamente e 7,7% associada a outras camaras); VE: 37,2%; AD: 8,5% e AE: 10,4%, com mortalidades específicas de 21%, 23%, 22% e 45%, respectivamente. Les es de duas camaras associadas alcan aram mortalidade de 37,5%, sendo 20% para VD+AD, 100% para VD+VE e zero para VD+AE. O pulm o correspondeu a 33,7% de 89 les es associadas. Os tempos médios de cirurgia e de interna o foram de 121 minutos e 8,2 dias, respectivamente. Cerca de 22,5% complicaram representando 41 complica es. A mortalidade foi 28,4%. Les es grau IV e V corresponderam a 55% e 41% dos casos, com mortalidade específica de 26% e 15%, respectivamente. Todos os doentes com les o grau VI morreram. CONCLUS O: O ferimentos cardíacos por arma branca estiveram associados a menor mortalidade, as les es cardíacas grau IV estiveram associadas à maior mortalidade e um menor tempo operatório esteve associado à maior gravidade e mortalidade. OBJECTIVE: To evaluate the forms of treatment employed to heart injuries and the main aspects related to their morbidity and mortality. METHODS: A retrospective study of 102 patients with cardiac injuries treated in the two emergency rooms in Manaus (Dr. John Lucio Pereira Machado Emergency Hospital and August 28 County Emergency Hospital) from January 1998 to June 2006. RESULTS: Of the 102 patients, 95.1% were men; mean age was 27 years; stab wounds accounted for 81.4% of cases and gunshot wounds for 18.6%; cardiorrhaphy was performed in 98.1% of cases. The heart chambers affected were: Right Ventricle (RV): 43.9% (36.2% isolated and 7.7% associated with other chambers); Left Ventricle (LV): 37.2%; Right Atrium (RA): 8.5%; and Left Atrium (LA): 10.4%; specific mortalities were of 21%, 23%, 22% and 45%, respectively. Abstract: considering the dearth of information with regard to the nutritional state of young athletes, were identified the prevalence of iron deficiency anaemia and its association with nutritional indicators of adolescent athletes participating in the sport initiation program of the vila olímpica foundation in manaus -am. a sample was made up of 194 voluntary adolescent male athletes classified as beginners, who participated in several different sports. data collected was by verification of anthropometric measures (body weight, height, and skinfold thicknesses of triceps and subscapular skinfold), hemoglobin dosage utilizing a portable hemoglobinometer, hemocue direct calorimetric method, and socioeconomic instrument. in the diagnosis of the nutritional states, we used height-for-age- and body mass index for the age indicators, adjusted cut-off points proposed by the who. the results showed that 9.4 % of the adolescents suffered from malnutrition, 8.2% overweight, and 4.6 obesity. the prevalence of iron deficiency anaemia found in the samples was 41.7%, with predominance in the lowest socioeconomic class. although the prevalence of anaemia was high, the nutritional state of the adolescents did not influence these results, considering that the majority looked healthy. in view of these findings, we suggest that effective means be taken for nutritional education to minimize and/or control iron deficiency anaemia.

Abstract: we report the occurrence of resistance to mefloquine 20mg/day in 51 children with falciparum malaria treated, at reference center of manaus, brazil, from october to december 1997. all children were evaluated at day 3, 5, 7, 14, 21, 28 and 35 of treatment. clinical and parasitological cure criteria were adopted. the incidence of riii mefloquine resistance was 5.9% (ic 95% 1.5-17.2). the cure/resistance proportion was 20:1 and cure/severity was 62:1. these findings suggest the importance of mefloquine resistance within this group of children.

Abstract: the clinical characteristics of falciparum malaria were studied among 61 children, aged 0 to 14 treated at a reference center in manaus, from october to december 1997. the symptoms observed were fever (98.4%), headache (80.3%), chills (68.9%), perspiration (65.6%), myalgia (59.0%), nausea (54.1%), lumbar pain (49.2%), vomiting (49.2%), cough (45.9%), arthralgia (31.1%), diarrhea (34.4%), dyspnea (8.2%), convulsions (8.2%) and dizziness (4.9%). pallor and anaemia were found more frequently in children under five years old. anaemia was associated with high levels of parasitaemia. fifty-eight (91.5%) patients had uncomplicated malaria, 3 (4.9%) had severe malaria and the lethality was 1.6%.

Abstract: Evidências científicas mostram que mudan as ambientais antrópicas aumentam riscos de exposi o a diversas doen as. Na Estratégia Saúde da Família - ESF, tarefas com claro enfoque ambiental s o prescritas indicando às equipes de profissionais que considerem esses aspectos em suas interven es. O objetivo desta pesquisa foi conhecer representa es e práticas de profissionais de Saúde da Família de Manaus (AM) sobre a quest o ambiental e sua interface com a saúde pública. Os dados foram coletados por meio de observa o participante e entrevistas semiestruturadas, e a análise qualitativa destes deu-se pela Análise de Conteúdo e Triangula o de Métodos. Resultados da pesquisa revelaram que a maioria dos profissionais n o compreende o ambiente de forma sistêmica, mesmo tendo declarado que os fatores ambientais têm grande influência sobre a saúde humana; enquanto interven es, as práticas educativas seguem metodologias tradicionais e s o centradas na culpabiliza o do indivíduo e na simples transmiss o de conhecimentos pontuais; o relacionamento dos profissionais com a comunidade resume-se ao atendimento individual e/ou coletivo. Concluiu-se que, para a ESF contribuir para o reordenamento do sistema, é fundamental o redirecionamento desse novo modelo de política de saúde para efetivar-se como prática social e ambiental. There are abundant scientific evidences showing that the increased risk of exposure to diseases is a consequence of anthropogenic environmental changes. In the Family Health Strategy, tasks with a clear environmental focus are prescribed, indicating to the professional teams that they should consider these aspects in their health practices. The objective of this research was to study representations and practices of Family Health Professionals of Manaus - State of Amazonas, Northern Brazil - about environmental issues and their interface with public health. Data were collected by means of participant observation and semi-structured interviews, and the qualitative analysis was carried out through Content Analysis and Methodological Triangulation. The results showed that most professionals do not understand the environment in a systemic way, even though they recognize the great impact that environmental factors have on human health; as interventions, the educational practices follow traditional methodologies and focus on blaming the individual and on the simple transmission of knowledge; the professionals' relationship with the community is limited to personal and/or collective care. Abstract: the occurrence of listeria sp. in cheese craft type coagulum marketed in the city of manaus was studied. in the period of march to may of 1998 58 samples of the product were collected at fairs and located markets in six areas of the city. in two cheese samples we identified two species of listeria: l. monocytogenes and l. innocua. the first species was found fobe pathogenic for the human being. the results demonstrated there to be risk of transmission of illnesses from the consumption of cheeses processed without the due hygiene care.

Abstract: objective: to describe the epidemiological status of leprosy in children living in a metropolitan area. methods: there were studied 474 leprosy cases detected in children under 15 living in the metropolitan area of the city of manaus, northern brazil, between 1998 and 2005. the endemic profile and quality of care services were assessed based on data from the notifiable diseases information system using epidemiological and operational indicators of the national leprosy elimination program. results: of all leprosy cases detected in the period studied, 10.4% were found in children under 15. the detection rates in this age group remained at hyperendemic levels between 1998 and 2003, and decreased from 2004 onwards but keeping on very high endemic levels. the most common clinical form was tuberculoid followed by dimorphous. paucibacillary forms were seen in 70.7% of the cases. the level of disability at the time of diagnosis was assessed in 94.7% of patients and 2.9% of them had physical disabilities. most children (99.4%) were treated with the multiple drug regimen recommended by theworld health organization. conclusions: despite its decrease, detection rate of leprosy in children in the city of manaus remains at a very high endemic level.
