Lepto tek lateral flow: un método para el diagnóstico rápido de la leptospirosis humana en Cuba

Abstract: the lepto tek lateral flow technique (lf) in the serological and rapid diagnosis of severe cases with clinical and epidemiological suspicion of human leptospirosis was introduced in and applied by the national laboratory of leptospirosis reference (nllr). conventional lab techniques (microagglutination mat and indirect hemagglutination ha ) were used in the confirmation of cases under study, 40 serum samples corresponding to 33 patients suspected of human leptospirosis were studied. of them, 21 samples proved to be positive by lf, accounting for 52.5 %. on comparatively applying the lf and ha techniques to 26 monosera and 7 pairs of serum, 11 and 5 positive cases were obtaind, respectively. the coincidence between them was 73 % on studying monosera and 71.0 on studying the pairs of serum. on applying mat to 9 suspected cases, 7 of them yielded positive. the most frequently found serogroups were: icterohaemorrhagiae and canicola. only 5 positive cases were detected by the 3 applied techniques. the coincidence between mat and lf techniques was 89 % and that between ha and mat was 100.0. the application of mat and ha offered reliability to the obtained results and it was evidenced the high sensitivity of lf. the importance of the use of matched sera for the accurate diagnosis of this disease was demonstrated.

Abstract: introduction: human leptospirosis requires rapid and early microbiological diagnosis since it is a common lethal disease worldwide. objectives: to increase the quality of microbiological diagnosis of this infection, to expand the knowledge on the circulation of groups of leptospiras in cuba and to show the benefits of an agglutination assay using cuban latex particles and of commercial immunochromatogenic systems lepto dipstick, lepto tek lateral flow, lepto tek dri dot and sd leptospira igm-igg. methods: this descriptive research used sera from positive and negative control cases to evaluate and measure the diagnostic value of rapid serological diagnosis systems with respect to the microagglutination method of reference (mat). all the techniques used in this report are described in the manual of operations and procedures of the leptospira lab in "pedro kourí" institute of tropical medicine. results: all the studied diagnosis systems exhibited acceptable values of sensitivity, specificity and agreement when compared to the international microagglutination method of reference with live microorganisms. the great selectivity (antigen reactivity) and the diagnostic reliability of the diagnostic systems were confirmed; particularly the mixed cuban-made latex, the lepto dipstick and the sd leptospira igm-igg. conclusions: the procedures used in this research work exceeded the traditional methods including the microagglutination method of reference in terms of easiness, rapidity, technical simplicity and level of performance, and all were useful for the screening of antibodies to leptospiras.
Confirmación microbiológica de 2 brotes emergentes de leptospirosis humana en Cuba Microbiological confirmation of two emergent outbreaks of human leptospirosis in Cuba  [cached]

Abstract: Clinical samples from 293 patients with suspicion of leptospirosis were studied for the microbiologic confirmation of these events, in October-December 2005, when there were 2 outbreaks in humans, in Cuba. Sera samples of patients in acute phase and during convalescence, as well as hemocultures performed before the beginning of the antibiotic therapy, were analyzed. Conventional techniques (passive hemagglutination test and serogroups hemagglutination), and advanced or fast diagnosis techniques (Lepto tek Dri-Dot, Lepto-Cuba, Latex-India, Lepto tek Lateral Flow) were used for serologic diagnosis. In outbreak I, 26 % of the studied cases were confirmed by serological tests (22/84), and 25 % (5/20) through hemocultures; whereas in outbreak II, 48 of the 162 studied cases (30 %) were serologically confirmed, and it was possible to obtain isolation of leptospires in 6 of the 27 processed cases (22 %). The main serovariants found by serology were canicola, ballum, icterohaemorrhagiae, and pomona. The rapid diagnosis methods were useful screening tools in the most severe cases or at pediatric ages. The two epidemiologic events were caused by pathogenic leptospires infection, which contributed to the adoption of hygienic-sanitary measures in both provinces.

Abstract: Introducción: la leptospirosis humana necesita de un diagnóstico microbiológico rápido y oportuno por ser una enfermedad letal y frecuente en todo el mundo. Objetivo: incrementar la calidad del diagnóstico microbiológico de esta infección, ampliar el conocimiento sobre la circulación de serogrupos de leptospiras en Cuba y demostrar la utilidad de un sistema de aglutinación con partículas de látex cubano y los sistemas inmunocromatogénicos comerciales LEPTO Dipstick, Lepto Tek Lateral Flow, Lepto Tek Dri Dot y SD Leptospira IgM-IgG. Métodos: en esta investigación descriptiva se utilizaron sueros de casos controles positivos y negativos para evaluar y medir el valor diagnóstico de los sistemas serológicos rápidos con respecto al método de referencia de microaglutinación (MAT). Todas las técnicas utilizadas en este reporte aparecen descritas en el Manual de Operaciones y Procederes del Laboratorio de Leptospiras, del Instituto de Medicina Tropical "Pedro Kourí". Resultados: todos los sistemas estudiados presentaron aceptables valores de sensibilidad, especificidad y concordancia en comparación con el método de referencia internacional de microaglutinación con microrganismos vivos. Se constató la amplia selectividad (reactividad antigénica) y la fiabilidad diagnóstica de los sistemas, se destacan en particular el látex mezclado de producción nacional, el LEPTO Dipstick y el SD Leptospira IgM-IgG. Conclusiones: ninguno de los procedimientos utilizados fue superado en cuanto a su sencillez, rapidez, simplicidad técnica y grado de ejecución al comparárseles con los métodos tradicionales, incluido el de referencia, y todos resultaron útiles en la pesquisa de anticuerpos frente a leptospiras. Abstract: Se estudiaron casos graves y grupos de riesgo, todos sospechosos de leptospirosis humana, mediante el uso de los estuches comerciales de diagnóstico rápido de lepto tek dri dot y lepto tek dip stick, en el Laboratorio Nacional de Referencia de Leptospiras (LNRL) del Instituto de Medicina Tropical "Pedro Kourí" (IPK). Los estuches novedosos se compararon experimentalmente con las técnicas serológicas convencionales hemaglutinación indirecta (HA) y microaglutinación (MAT). El estudio fue realizado en el período de enero a mayo de 2002. De los 40 pacientes graves sospechosos estudiados, 20 fueron positivos por lepto tek dip stick y 10 por HA. Por lepto tek dri dot y MAT, 20 y 9, respectivamente, fueron positivos. Los principales serogrupos encontrados por MAT fueron: Sejroe y Canicola. La coincidencia entre las técnicas de lepto tek dip stick y HA fue del 72,5 y entre el lepto dri dot y la MAT fue del 70 %. El índice de coincidencia entre el lepto tek dip stick y lepto dri dot fue del 80 % y entre MAT y HA fue del 85,0. Este mismo parámetro, medido entre lepto tek dip stick y MAT, fue del 85,0 y entre lepto tek dri dot y HA fue del 70,0. Con la introducción de los estuches lepto tek dri dot y lepto tek dip stick fueron detectados, en los casos positivos de leptospirosis humana, anticuerpos específicos contra leptospira. De los 485 monosueros procedentes de 424 pacientes de grupo de riesgo, 19 fueron positivos y 44 reactivos por HA. Al emplear MAT, 64 y 172, respectivamente, fueron positivos y reactivos. Lepto tek dri dot detectó 134 casos positivos. El índice de coincidencia entre MAT y lepto tek dri dot fue del 59 %, para un total de 251 casos coincidentes. La introducción de la tecnología de avanzada lepto tek dri dot permitió brindar un resultado rápido y de forma inmediata para la confirmación final de cada caso en estudio. Abstract: serious cases and risk groups suspected of having human leptospirosis were studied bymeans of rapid diagnosis kits known as lepto tek dri dot and lepto tek dip stick in the national reference laboratory of leptospira of “pedro kourí” tropical medicine institute (ipk). the new kits were experimentally compared with the conventional serological techniques such as indirect hemagglutination (ha) and microagglutination (mat). the study was conducted from january to may, 2002. of 40 suspected patients in critical condition, 20 were positive according to lepto tek dip stick and 10 were positive according to ha results. twenty and nine patients were positive by lepto tek dri dot and mat test respectively. the main serogroups found by mat were seijroe and canicola. the coincidence index between lepto tek dip stick and ha was 72,5% whereas that of lepto tek dri dot and mat was 70%. on the other hand, the coincidence index between the two new techniques was 80 % whereas that of the conventional methods was 85 %. this same parameter measured between lepto tek dip stick and mat was 85 %, and between lepto tek dri dot and ha was 70 %. the introduction of kits lepto tek dri dot and lepto tek dip stick made it possible to detect specific antibodies to leptospira in human leptospirosis-positive cases. of 485 monosera from 424 patients of the risk group, 19 were positive and 44 reactive by ha test. mat revealed 64 positive and 172 reactive monosera. lepto tek dri dot detected 134 positive cases. in this regard, the coincidence index between mat and lepto tek dri dot was 59 % for a total number of 251 coincident cases. the implementation of the state-of-the-art technology known as lepto tek dri dot allowed achieving immediately a rapid result for the final confirmation of each case under study.

Abstract: objectives: to assess the sensitivity, specificity, reproducibility, and stability of five latex agglutination systems for detecting antibodies against leptospira in human and animal sera, by using the leptospira serotypes that are most widely prevalent in cuba. methods: we performed an analytic and descriptive study with 706 human sera (65 tested positive for antibodies against leptospira with microagglutination (mat) and hemagglutination (ha) techniques; 156 sera that tested negative with mat and ha); 485 sera from 424 patients who had clinical or epidemiologic signs of leptospirosis; and 29 animal sera (16 from equines, 6 from bovines, 5 from porcines, 1 from a canine, and 1 from an ovine). all of the samples were tested with five latex conjugates made from whole cells of leptospira interrogans, specifically the four serogroups that circulated most widely in cuba from 2002 to 2004. the cells obtained from cultured cell lines yielded four specific conjugates (latex-canicola, latex-icterohemorrhagiae, latex-pomona, and latex-sejroe), as well as one latex conjugate made from a combination of all four serogroups in equal quantities (latex-pool). in addition, samples were tested with the commercial latex agglutination lepto tek tri dot (biomeriuex, france) kit. the stability and reproducibility of the latex conjugates were assessed through monthly controls over a period of 6 months with positive and negative sera. results: of the systems that were assessed, the best combination of sensitivity and specificity was obtained with the latex-pool conjugate (93,8% and 90,4%, respectively). the best combination of positive and negative predictive values was seen with the latex-sejroe conjugate (90,9% and 95,8%), respectively), followed by the latex-pool conjugate (94.2% and 96.6%, respectively). the positive and negative predictive values of the lepto tek dri dot commercial system were 78.5% and 88.4%, respectively. among the 137 patient samples that tested positive for one o

Abstract: forty one serum specimens from critical patients who were suspected of having human leptospirosis were studied by using simultaneously convencional test lepto-dipstick and serogroup microagglutination test (mat) with live antigens. different reactivity levels were reached in 26 tested samples by lepto-dipstick. this assay and mat coincided in 85,4% of results. it was confirmed that lepto-dipstick detects antibodies to various leptospiral serovars and that the degree of intensity of the band staining corresponded with the level of detected antibodies by mat. the implementation of lepto-dipstick in the diagnosis of human leptospirosis allows providing a rapid response to the medical staff in charge of the studied cases. this proved that lepto-dipstick is a rapid method that is easy-to-read and interpret.

Abstract: Se estudiaron 41 sueros provenientes de pacientes en estado grave con sospechas de leptospirosis humana, mediante la prueba comercial Lepto dipstick y haciendo uso paralelamente de la microaglutinación de serogrupos con antígenos vivos (MAT). Se obtuvieron diferentes niveles de reactividad por el dipstick en 26 de las muestras investigadas. Este ensayo y la MAT coincidieron en 85,4 % de los resultados. Se comprobó que el Lepto dipstick reconoce anticuerpos contra diferentes serovares de leptospiras, y que el grado de intensidad de la tinción de las bandas se corresponde con el nivel de anticuerpos detectados por la MAT. La aplicación del Lepto dipstick al diagnóstico de la leptospirosis humana, permitió ofrecer una respuesta rápida al personal médico de asistencia de los casos estudiados. Esto demostró que se trata de un método rápido de realizar y de fácil lectura e interpretación.
