Use of extratransference interpretation in psychoanalytic psychotherapy  [PDF]

Abstract: Background. In psychoanalytic psychotherapy, the transference analysis takes the central position of the work. The work in the extratransference sphere and experience in a professional practice with extratransference interventions have not been reported much in the literature. Extratransference sphere includes less transferring relation to a psychotherapist, transference to other objects, or may refer to the external reality rather than the psychic reality or fantasy. Case report. We pointed out extratransference interventions. We demonstrated an application of a genetic interpretation and reconstruction, too, which could restore and establish the connections between the past and the present, in order to understand the influences of the current reality and the past, and helping us, further, to resolve the infantile conflicts. Conclusion. Interpretation of extratransference situations takes an important part of an analytical work and it is an essential category of the interpretation. Analytic understanding should include transference and extratransference spheres, fantasy and reality, past and present. Working with neurotic patterns and character resistance needs an optimal choice of intervention in the given moment of the analytic process. Extratransference interventions are an essential category of intervention, irreplaceable for its effectiveness in the analytic process.

Abstract: introduction: while adolescence in essence means growth, depression on the other hand would by definition be the opposing "movement": a tendency towards languor, abatement, inhibition, immobility, prostration, zero, sometimes even towards nothing. development: in an attempt to articulate these two concepts and the notions of history, structure, and juncture, the author underscores the importance of an appropriate psychotherapeutic management of adolescent depression. to begin, he takes into account the fact that this disorder is multidetermined and in consequence, the existence of various vertexes of possible interventions. this paper focuses on psychoanalytic psychotherapy, one of several possible psychotherapeutic modalities available in this context. he discusses some of the generalities of psychoanalytic psychotherapy and then goes on to examine three themes not sufficiently explored in the literature: the initial interviews, the indications, and finally, what is known as bifocal therapy. conclusion: to conclude, he invites the reader to ponder on the importance of the therapeutic encounter with the depressed adolescent and on the transcendence of analyzing and giving meaning to her depression
Review of the extratransference intervention practice in the psychoanalytic psychotherapy

Abstract: The transference analysis takes the central position of the work in psychoanalytic psychotherapy. The work in extratransference sphere and experience in practical work with extratransference interventions are not often reported in expert literature. Extratransference sphere includes less transferring relation to the psychotherapist, transference on the other objects, or may refer to the external reality rather than the psychic reality or fantasy. In our illustration, we gave emphasis to extratransference interventions. Application of genetic interpretation and reconstruction were demonstrated as well, which can restore and establish connections between past and present, in order to understand influences of the past and current reality, and helping us to resolve infantile conflicts. Abstract: Introducción: En este artículo se realiza una reflexión conceptual sobre aquello que es común a cualquier tipo de psicoterapia. Objetivo: Proponer elementos básicos para pensar la psicoterapia. Método: Análisis teórico de diferentes conceptos: cuidar, pensar, felicidad, nosotros, frontera, pasado, voluntad y conversación. Conclusión: Con la proliferación de abordajes terapéuticos, es preciso pensar en un corpus teórico que sirva de base para el ejercicio de cualquier terapia. Introduction: In this article, a conceptual reflection is made on those things common to any kind of psychotherapy. Objective: We propose some basic elements of thinking regarding psychotherapy. Method: A theoretical analysis of different concepts: to look after, thinking, happiness, we, frontier, past, will and conversation. Conclusion: With the proliferation of therapeutic approaches, it is necessary to consider a theoretical corpus that may provide us with the foundations for the exercise of any therapy.
A Follow-up Study in Psychoanalytic Psychotherapy of Couples

Abstract: During 11 years we have been working on a research involving intervention with families seeking treatment in a counseling center-school and whose complaints are focused on their children. Using psychoanalytic psychotherapy with the couples, we have tried to achieve symptom remission in the children. The purpose of this study is to reflect on the maintenance, over time, of the psychological changes obtained in this kind of clinical practice, which encompasses both couples and families. Based on clinical-qualitative methodology and case study, two cases have been examined, with emphasis on the follow-up sessions for two couples who were undergoing psychoanalytic couple’s therapy for thirty-six and thirty months, respectively, with two additional follow-up sessions after the therapy was finished. The collection of clinical material to be analyzed according to the psychoanalytic references was made using excerpts from the session written notes taken by the therapist herself immediately after the sessions were over. As a result, this kind of intervention proved effective in clarifying latent marital conflicts and in fostering a healthy environment, one that could contribute to the emotional development of the child.

Abstract: Se efectuó un ensayo clinicoterapéutico en todos los pacientes con psoriasis, atendidos en el Policlínico Docente “30 de Noviembre” de Santiago de Cuba, desde octubre del 2005 hasta igual mes del 2006, para evaluar la efectividad de la psicoterapia autógena. Los integrantes de la casuística fueron asignados aleatoriamente a uno de 2 grupos: los tratados con psicoterapia autógena asociada al tratamiento convencional (grupo de estudio) y los que recibieron tratamiento convencional (grupo control). Los datos obtenidos evidenciaron que al inicio del plan terapéutico todos los pacientes tenían síntomas de psoriasis en brotes, pero finalmente la terapia combinada resultó más efectiva. A clinical therapeutical trial was carried out in all patients with psoriasis, assisted at "30 de Noviembre" Teaching Polyclinic from Santiago de Cuba , from October, 2005 to October, 2006, to evaluate the effectiveness of the autogenous psychotherapy. The members of the case material were assigned at random to one of 2 groups: those treated with autogenous psychotherapy associated to the conventional treatment (study group) and those that received conventional treatment (control group). The obtained data evidenced that at the beginning of the therapeutic plan all the patients had psoriasis symptoms in outbreaks, but finally the combined therapy was more effective.

Abstract: O objetivo do artigo é refletir sobre a possibilidade de forma o de uma alian a de trabalho entre psicoterapia psicanalítica e outras práticas psicoterápicas no atendimento a pacientes com psicopatologias graves. Apresentamos o caso clínico de um adolescente borderline que, em fun o do surto psicótico, tem seu tratamento reformulado, passando a contar com um acompanhante terapêutico. Por fim, desenvolvemos algumas considera es sobre os ganhos psicoterapêuticos de uma postura inovadora e interdisciplinar, bem como sobre as particularidades de cada atendimento clínico. The purpose of this paper is to take into account the possibility of developing a work partnership between psychoanalytical psychotherapy and other psychotherapeutical procedures, aiming at the treatment of patients suffering from serious psychopathologies. A borderline teenager patient is presented as a case, in which a psychotic episode has led to a reformulation of the treatment, including therapeutic accompaniment. The goal of this partnership is to combine clinical efforts that can bring more quality to the patient’s mental and social life. Finally, we focus on the psychotherapeutical benefits resulting from an innovative, interdisciplinary approach and some particularities of the clinical case.

Abstract: Introducción: La psicoterapia de apoyo es la modalidad terapéutica más utilizada en psiquiatría. Objetivo: El presente trabajo busca explicar los principios básicos teóricos y técnicos que sustentan su práctica. Método: Revisión bibliográfica en la cual se definen los términos, filosofía, indicaciones y alcances de la psicoterapia de apoyo. Desarrollo: Se presentan las estrategias técnicas fundamentales en este tipo de psicoterapia, como lo son la formulación de caso, el encuadre, la escucha terapéutica, la alianza terapéutica, el ambiente facilitador, el manejo de la transferencia y la interpretación en la psicoterapia de apoyo, aportando ejemplos clínicos. Introduction: Support psychotherapy (ST) is the most used treatment modality in psychiatry. Objective: The present work explains the basic theoretical and technical principles that support the practice of ST. Method: A bibliographical review is carried out, in which the terms, philosophy, indications and scope of support psychotherapy is defined. Development: The fundamental technical strategies in this psychotherapy type are presented, such as case formulation, setting, therapeutic listening, therapeutic alliance, the facilitating atmosphere, transference management and interpretation in ST. Clinical examples are given.

Abstract: Cualquiera que sea la forma que revista, la psicoterapia es común a la humanidad entera. Hoy día, en el mundo occidental, centenares de psicoterapias están en el mercado. Buena parte, si no todas, se pueden atribuir al contagio freudiano vaticinado por el mismo padre del psicoan álisis. Aunque Freud creyó haber fundado una psicoterapia científica, nunca postuló que su invento fuera la única terapia posible. La cientificidad del psicoanálisis ha sido severamente cuestionada en las últimas décadas. No obstante, no existe criterio alguno para decir qué sería una psicoterapia científica. La diversidad de las etnoterapias, tanto las de la antigüedad como las contemporáneas en la sociedad moderna y en las premodernas actuales, es innegable. En lugar de repudiarlas a todas como precientíficas -ya que no existe criterio de demarcación- lo razonable sería estudiarlas para intentar explicitar su lógica interna. Regardless of its facade, psychotherapy is common to the entire humankind. In the Western word market, there are now hundreds of therapies available. A good number of them, if not all, may be due to the Freudian contagion, predicted by the father of psychoanalysis himself. Even though Freud believed he had created a scientific psychotherapy, he never pretended his invention was the only possible therapy. The scientific nature of psychoanalysis has been severely questioned in the last decades. However, there are no criteria about how a scientific psychotherapy should be. The diversity of ethnotherapies, both ancient, modern and in current premodern society, in undeniable. Instead of repudiating them as prescientific -as there is no standard of demarcation- it would be reasonable to study these therapies, in order to clarify their innner logic.
