Welcome to the Journal of Cardiothoracic Surgery
Vipin Zamvar, David P Taggart
Journal of Cardiothoracic Surgery , 2006, DOI: 10.1186/1749-8090-1-1
Abstract: There are many cardiothoracic journals in the printed form. Most of these are affiliated to national or international societies, and all are subscription-based, thus there is restricted access to the material published. The turnaround time for papers can also be quite long for some of these journals. JCTS will aim to provide a quick and efficient service to authors. Being open access, the material published will be freely available on the internet. All articles will be archived in major databases including PubMed Central. The term open access is self-explanatory. In practical terms, it means that anybody with access to the internet will be able to access and download material published in the JCTS.The main motivation for researchers and clinicians who write papers is the desire to advance the frontiers of knowledge in their speciality. What better medium than an open-access journal that allows anybody, anywhere unfettered access to the results of their work? However, alongside this altruistic aim is a desire to be recognised for their work in a manner that will also help advance their careers. Citation counts of their papers, and impact factors of journals in which they publish are two objective (even though not entirely free of controversy) markers widely used in the scientific community. Open access journals will not put any author at a disadvantage. In fact there is some evidence that open access articles have a greater chance of being cited [1]. For articles published in the JCTS, authors will be able to view the number of times their article has been accessed. There is evidence that the number of accesses in the weeks after publication can be a potentially useful marker of subsequent citation count [2].Authors also value open access, and consider this an important factor in deciding which journal to submit their work to [3].The future of scientific publication is definitely headed in the direction of open access. The internet has profoundly changed the way we com

Abstract: We thoroughly reviewed the existing literature with particular focus on the curriculum, salary, board certification and quality of life for cardiothoracic trainees. The United States of America and the Federal Republic of Germany each have different cardiothoracic surgery training programs with specific strengths and weaknesses which are compared and presented in this publication. The future of cardiothoracic surgery training will become affected by technological, demographic, economic and supply factors. Given current trends in training programs, creating an efficient training system would allow trainees to compete and grow in this constantly changing environment.Cardiothoracic surgeons must possess a wide variety of technical and professional competencies. With time, cardiac operations are becoming increasingly difficult given aging patient population with more co-morbidities and increasingly severe coronary artery disease. On the other hand, training in cardiothoracic surgery is increasingly being restricted by work hour limitations. There are recent trends to reshape cardiothoracic surgery training to make it more efficient and productive. In this regard, it is very intersting and useful to examine various training systems globally. We decided to compare cardiothoracic surgery training system in the United States with the training system in Germany. Germany has one of the best developed cardiothoracic surgery training systems in the world and at the same time differs enough from U.S. training system to be considered for such a comparison.Available literature regarding cardiothoracic surgery training in the United States and Germany was reviewed by cardiothoracic surgeons in training and trained cardiothoracic surgeons from U.S. and Germany. Up-to-date publications by American Board of Thoracic Surgery (ABTS) and Accreditation Council for Graduate Medical Education (ACGME) were reviewed. Information about cardiothoracic surgery training in U.S.A. and Germany were d

Abstract: Objective: Sleeping problems are among the issues most mentioned by patients after cardiothoracic surgery. These problems can have a negative effect on duration of the hospital stay and recovery. In the ward of our cardiothoracic surgery department, a study was initiated to assess the quality of sleep after cardiothoracic surgery. The primary objective was to investigate the effect of cardiothoracic surgery on the quality of sleep. The secondary objective was to investigate the quality of sleep. Correlations with perioperative factors and related issues such as the type of surgery and medication were sought. Methods: A consecutive prospective cohort study was initiated (N = 72). The study used validated questionnaires to assess sleep: the Pittsburgh Sleep Quality Index (PSQI), the Epworth Sleepiness Scale (ESS), the Verran Snyder-Halpern Sleep Scale (VSH) and the Factors Influencing Sleep Quality (FISQ). Results: The PSQI showed that the quality of sleep one month after surgery was inferior to the quality of sleep before surgery (p-value: 0.03). The efficiency of sleep (time spent in bed) was higher after surgery then before surgery (p-value: 0.01). The VSH showed increased impaired sleep on the third night after surgery. The most disruptive factors were not being comfortable in a hospital bed, pain and the noise of medical devices. Conclusions: The quality of sleep after cardiothoracic surgery is worse when compared with the preoperative situation. The chief influencing factors are discomfort in bed, pain and disturbance from medical devices. The use of pain medication does not improve the quality of sleep.
LA REGLA DE CRAMER A PARTIR DEL PRODUCTO GENERALIZADO EN n

Universitas Scientiarum , 2003,
