Abstract: Presentamos un caso de liquen simple crónico vulvar izquierdo, en una paciente con diagnóstico psiquiátrico de trastorno bipolar, tratado repetidamente como micosis. Se realizó biopsia que confirmó el diagnóstico y se trató exitosamente con antihistamínicos orales y corticoides tópicos A case of chronic simple liquen (Vidal′s Liquen) compromising the left vulvar area is presented. A lack of response to several topical previous treatments motivated vulvar biopsy that confirmed the diagnosis. The relationship with anxiety and/or depression is stressed. The treatment with antihistaminics and potent topical steroids yielded good results

Abstract: Se actualizaron algunos aspectos del liquen escleroso y atrófico (LEA) y se presentaron los resultados del estudio clínico-epidemiológico de 12 pacientes con LEA extragenital. Todos los casos revisados pertenecían a la raza blanca y la mayor incidencia se encontró en mujeres mayores de 40 a os. La localización de las lesiones fue más frecuente en el tórax. Some aspects on lichen sclerosus et atrophicus (LSA) were updated and the results of the clinicoepidemiological study conducted among 12 patients with extragenital lichen sclerosus et atrophicus were presented. Al l the reviewed cases corresponded to white individuals and the highest incidence was observed in women over 40. The most frequent localization of the injuries was the thorax.

Abstract: Paciente de 60 a os de edad que acude a consulta por prurito vulvar intenso de más menos 3 a os de evolución, que no resuelve con tratamiento y que ha notado cambios de coloración de la piel y aplanamiento de los pliegues cutáneos de los labios mayores con pápulas ulceradas y fétidas. Se realiza biopsia que da como resultado liquen esclerotrófico. Se comienza tratamiento con estrógenos conjugados y antihistamínicos pero se observa escasa mejoría, por lo que se decide como tratamiento final la vulvectomía simple con resolución de los síntomas The case of a 60-year-old female patient that seeked medical assistance due to intense vulvar pruritus is presented. She did not resolve with the treatment and she began to notice changes of the color of the skin and flattening of the cutaneous folds of labia majora with ulcerated and fetid papules. According to the result of the biopsy, she had lichen sclerosis et atrophicus. Treatment with conjugate estrogens and antihistaminics was started, but little improvement was observed. It was finally decided to perform simple vulvectomy with resolution of the symptoms.

Abstract: El liquen escleroso (LE) es una enfermedad inflamatoria, crónica y benigna. De etiología desconocida, se ha vinculado con diversos factores autoinmunológicos, genéticos, hormonales, infecciosos y traumáticos. Hay dos variedades clínicas de esta enfermedad: genital y extra-genital. La localización más frecuente en ambos sexos es la genital. Se presenta un caso clínico de LE extra-genital, con lesiones cutáneas poco frecuentes, ampollares y hemorrágicas, que coexistían con placas atróficas de larga evolución, no diagnosticadas previamente. Lichen sclerosus is an inflammatory disease, benign, chronic, of unknown aetiology. It has been linked with many factors such as autoimmune, genetic, hormonal and infectious diseases, as well as trauma. There are two clinical types of this disease: genital and extra-genital. The most frequent in both sexes is the genital form. We present a rare case of extra-genital localization with bullous and haemorrhagic manifestations that co-existed with late atrophic plaques, previously undiagnosed.

Abstract: background: lichen sclerosus is characterized by a collagenous hyaline/homogeneous zone at upper dermis, which remains undefined morphologically and biologically. our previous investigation showed that there are several changes at the extracellular matrix hyaline zone, such as accumulation of collagenic proteins (types i and iii) and sulfated proteoglycans/glycosaminoglycans. objectives: decorin and chondroitim sulfate (sulfated proteoglycans/glycosaminoglycans) immunoexpressions were the present investigation′s aim, emphasizing the hyaline zone related changes. methods: thirty one vulvar ls untreated clinical lesions were biopsed and evaluated histologically according to hewitt?s gradation and by immunohistochemical methods. results were compared with ones of the control group, which was formed by cutaneous fragments from vulvoperineal corrective surgeries. results: we could demonstrate that decorin and chondroitin sulfate were present at the hyaline zone in different moments of matrix modulation. in all hewitt stages chondroitin sulfate prevailed at the extracellular matrix in cases with a compact aspect of the hyaline zone while decorin was only seen in areas of less compactness. conclusion: this proteoglycans/glycosaminoglycans synthesis sequence suggests that decorin may be a possible initial marker/indicator for vulvar ls. we suppose either that chondroitin sulfate is possibly a factor that limit matricial changes extension till middle dermis level.
TRATAMIENTO DE LA NEOPLASIA INTRA EPITELIAL VULVAR MEDIANTE VULVECTOMíA TEGUMENTARIA  [cached]

Abstract: Entre 1999 y 2005 ingresaron 12 pacientes con el diagnóstico de neoplasia intraepitelial de la vulva. La edad promedio fue 50 a os, con rango de 26 a 74 a os. El diagnóstico se hizo mediante biopsia dirigida por vulvoscopia. Se efectuó vulvectomía tegumentaria a 9 pacientes según técnica que se describe. 54,5% de las pacientes presentaba lesiones vulvares múltiples y 45,5% presentó una lesión al examen. En la biopsia, 63% de las lesiones presentaban signos sugerentes de infección por virus papiloma humano. 55,5% de las pacientes tratadas no presentó nuevas lesiones y 33% tuvo una o más recurrencias, 11% no asistió a control, no hubo complicaciones quirúrgicas ni mortalidad Between 1999 and 2005, 12 patients with the diagnosis of vulvar intraepithelial neoplasia were ingresses. The average age was 50 years (r: 26-74 years). The diagnosis was made by means of biopsy directed by vulvoscopy. In 9 women tegumental vulvectomy were performed according to technique that is described. 54.5% of the patients presented multiples vulvar lesions and 45.5% only one lesion at the examination. In biopsy, 63% of the lesion presented suggestive signs of infection by HPV. 55.5% of the treated patients did not present new lesions and 33% had one or more recurrences, 11% did not attend control, there were no surgical complications or mortality
