Laboratories Performance after Outsourcing in the Hospitals of Shahid Beheshti University of Medical Sciences

Abstract: Background and Objective: Nowadays, downsizing the government to have aneffective and flexible organization is considered to be government’s top priority inthe world and outsourcing is one of the ways to achieve this goal. Accordingly,Shahid Beheshti University of Medical Sciences has delegated some of itshospitals' duties to the private sectors. The present study has been carried out toassess the performance of outsourced clinical laboratories.Material and Methods: This Descriptive Evaluation study was carried out during6 month (May 2011 till February 2012). The data was collected by twoquestionnaires with close-ended questions and one with open-ended questions, andanalyzed by Wilcoxon, using SPSS software.Results: There is an improvement in outsourcing laboratory services. Forexample, increase in the number of tests (32%) and in the type of tests (37%).Consequently, increase of hospital income (51%). The number of personnel isdecreased and their responsibility and behavior are improved.Conclusion: Overall, it seems that outsourcing laboratory program could achieveits major goals such as: Absorbing non-governmental resources in bothadministrative and financial aspects, omitting extra expenditure, acquiringbenefits, and upgrading productivity of laboratories.Keywords: Outsourcing; Laboratory; Performance
A Hybrid Course for Probability and Statistics for Engineers: An E-Readiness Study at Shahid Beheshti University  [cached]

Abstract: Probability and Statistics for Engineers covers verities of subjects in the set theory, the combinatory analysis, probability, statistics, and (in some universities) the stochastic processes. Since, course receives only 3 credits it has to be thought 3 hours/week. This overloading content along with time limitation make course as a challenging and difficult one for students. Also, many instructors, including the first author, found the course very challenging to teach. Two popular on-site and e-learning training systems do not provide any appropriate solution. This article suggests a hybrid training system, which combines some elements of both training systems to reduce the disadvantages of both systems. Readiness of such hybrid course is measured by preparedness of students for online activities. The readiness study at Shahid Beheshti University shows that Internet skills, self-directed learning, learner attitude toward e-learning, e-mail skills, and software ability of students are factors which are significantly affect readiness of students.
"A Study of the Characteristrics of Hypokalemic Paralysis in Patient Hospitalized in Shahid Beheshti Hospital During 1990 –2000"

Abstract: Hypokalemic disease is a disorder in which the amount of serum’ s potassium declines and the patient faces limbs weakness. In this disease, eye muscle and respiratory muscles rarely become involved and death may follow due to respiratory paralysis and heart conducting disturbances. Since this disease is treatable and progressive weakness can be prevented, therefore in the study conducted we attempted to compare between the causes and epidemiological characteristics and start of clinical manifestation and the characteristics of this diseases with what had been presented in classical text and foreign sources in order to take effective measure for timely diagnosis. This was a descriptive prospective study. The records of hypocalemically paralyzed patients hospitalized in shahid Beheshti Hospital of Kashan during 1990 to 2000 were used to collect the data such as general condition of the patient, systematic and neurologic examination, serum potasium level, ABG, and paramedical measures like ECG were extracted and used for analysis. 27 patients were identified as Hypokalemic afflicted to paralysis and all were sporadic cases. The starting age for weakness attack was mostly in the second to fourth decade of life. The mean age for attack was 29 years . 96.3 percent were male and the rest were female. The level of potasium in serum of these patients during the weakness attack was mostly within 1 to 3.5 meq/L and the most changes in ECG was related to decrease in height at T and appearance of U- waves. 11 patients have more than 3 attacks (40.7 percent ), 10 patients faced all-attacks (37.3 percent) and 6 patients experienced 2 times attack (22 percent).Considering that hypokalemic paralysis disorder is a treatable disease and immediate diagnosis and treatment reduces cardiac and muscular problems, it is necessary for the physican who treat patients referring with flaccid paralysis to keep in mind four limbs of this disease and use diagnostic measures.
Study on Performance of Infectious Waste Sterilizing Set in Kashan Shahid Beheshti Hospital and Determination of its Optimum Operating Condition

Abstract: Background and Objectives; Autoclaving is one of the methods which sterilizes infectious solid wastes. Since variety of parameters such as temperature, time, and pressure influence autoclave performance, this study was carried out to evaluate the parameters and set optimum condition for the autoclave apparatus applied in Shahid Beheshti Hospital in Kashan.Materials and Methods: In this descriptive-analytical study, the performance of subjected autoclave was surveyed based on biological index and through setting 144 tests. Variables were packaging type in two groups (open and wrapped), loading type in three groups (light, medium and heavy), and four temperature-time features in fixed pressure equal to 101 kpa. Biological index was ATCC 7953 which contained Stearotermophilus Geobacillus spores. Finally obtained results were analyzed by Chi-Square test.Results: The results of statistical test showed that there isnt any meaningful relation between packaging type of waste, system loading, and efficiency of sterilization(P>0.05),while meaningful relation was found between system performance and variety states of temperature-time feature(P<0.05), illustrating temperature and time effects in fixed pressure on sterilization of solid waste.Conclusion: Based on the results, the best autoclave operational condition for sterilizing infectious solid wastes are: temperature-time equal to 10 min-140°C and 15 min-134°C in fixed pressure of 101 kpa, respectively. It was also revealed that temperature-time condition suggested by manufactory, i.e. 20min-121°C, is not sufficient for complete sterilization of solid waste.
Job Satisfaction and its Contributing Factors in Female Faculty Members of Shahid Beheshti University of Medical Sciences  [cached]

Abstract: Faculty members, as significant human resources, can play a crucial role in enhancing output in different social establishments, including universities and educational systems, if they are satisfied with their job condition. Nowadays nearly half the members belong to female employees in different organizations such as universities and educational settings. The attitude of this number of employees, including female faculty members, is of special significance if the quality of work is to be enhanced in universities or other educational establishments. Bearing in mind this significance, the current study investigated job satisfaction among female faculty members of Shahid Beheshti University of Medical Sciences (SBMU) in 2008, Tehran. The assumption was that the satisfaction level of faculty members from their job could significantly enhance the quality of education and clinical care at different colleges and teaching hospitals of the university. In line with this assumption, as a descriptive research, the study investigated job satisfaction among female faculty members at SBMU in 2008. Altogether a total of 116 subjects, selected randomly, were asked to complete the questionnaires. The Minnesota questionnaire and the Personal and Managerial questionnaire were used to measure the satisfaction level of the participants from their career. The researchers referred to different colleges and hospitals of SBMU to ask the participants to fill out the questionnaires. Then, following the data collection procedure and questionnaire analysis, the data were subjected to numerous statistical tests such as t-test, One- way ANOVA and Multiple comparisons tests. Job satisfaction among female faculty members at SBMU, according to the results, was low to some extent. The main factors accounting for a rather low satisfaction score were limited welfare facilities, low salaries and unpaid arrears, improper work environment and limited promotion opportunities.
Adequacy of Data on Injuries’ External Causes for Coding Based on ICD-10 in Inpatient Medical Records and Emergency Wards of Shahid Beheshti Hospital in Kashan

Abstract: Background and Aim: Knowledge about the external causes of accidents is vital for their prevention, and insufficient data on external causes can reduce their usefulness. That is why, the adequacy of the sources of data is of great importance. The aim of this research is to study the adequacy of data regarding the injuries external causes for coding based on ICD-10 in inpatient medical records and emergency wards of Kashan Shahid Beheshti Hospital.Materials and Methods: This prospective descriptive cross-sectional research was done on 1000 patients in Kashan Shahid Beheshti Hospital in 1386 (2007). The data needed for coding were chosen from medical records based on ICD-10 rules and for the following groups: transportation accidents, poisoning, burns, lightning, assaults, respiratory threats, intentional self harm, contact with venomous animals, and exposure to animate and inanimate mechanical forces. Then, the data were put in a checklist and analyzed with SPSS and descriptive statistics. Results: The data required for the three main characters of the external causes of injuries were adequate in 740 files (47%); the figure for the fourth character of the external causes of transportation accidents was found to be 430 (88.7%). Figures for the place of occurrence and the type of subjects activities at the time of accidents were found to be sufficient in 109 cases (21.16%) and 50 cases (5%), respectively. Conclusion: Data on the eternal causes of injuries do not have the required adequacy. More evaluations are essential to enhance the adequacy of such data.

Abstract: A storke is the sudden onset of focal neurologic deficit from a vascular mechanism and is the 3 rd cause of death in developed countries. This study was carried out to evaluate the known risk factors of stroke and their effect on the onset of it in Shahid Behesti hospital in Kashan in 1998. The descriptive strategy of this study was conducted on 300 patients with CVA, that their illness was documented based on their clinical and CT-Scan findings. The informations about the patients was listed in an information list after hospitalization. Most cases were between 70-80 years old. 64% of cases had hypercholestrolemia, 15% diabetic history, 54% hypertension, 40.6% had positive history of smoking and 22.4 % had combined hyperlipidemia. Considering the high incidence of hypertension, hyperlipidemia and diabetes among the patient with CVA, and the high incidence of smoking as well, more attention is needed to reduce these risk factors in the society.
Role of The International Book Fair to Supply Books for Libraries of Tehran & Shahid Beheshti University of Medical Science  [PDF]

Abstract: Background and Aim: The international book fair is an opportunity for school libraries. This study was conducted to determine the role of the Tehran international book fair to provide books for school libraries.Materials and Methods: The study is a descriptive survey. Employees and authorities worked in libraries affiliated to Tehran and Beheshti Medical Sciences Universities consisted the study population. Sample size included 41 individuals(26 staff and 15 officials). For data analysis descriptive statistics by using one sample t- test, chi square, ANOVA and correlation were used.Results: The highest average score dedicated to choose books in school libraries was a scale of a 3/35± 0/89. With an average score of 3/06±0/07, the role of International book fair in supplying books for school libraries was moderate. Correlation test showed that problems of school libraries had the highest correlation(r=0/36) while the lowest correlation was related to the possibility of book supply(r =0/067).Conclusion: The international book fair does not have an effective role in supplying books for school libraries. It is necessary to use new approaches to supply books for academic libraries.
Long Term Advantages of Kelly Plication for Stress Urinary Incontinence Treatment in Shahid Beheshti Hospital of Isfahan University of Medical Science

Abstract: Background and ObjectiveUrinary incontinence is a common complain in women. There are some methods in modern and alternative medicine for treating this disorder. The aim of this study was to evaluate long term outcome of anterior colporrhaply with Kelly plication for treatment of stress urinary incontinence. MethodsThis descriptive-analytic study was done on 20 patients who underwent Kelly plication for stress urinary incontinence between July to November 2005 in Beheshti Hospital of Isfahan University of Medical Science. Primary data were collected before operation and 2 times after operation (4&24 months). Questionnaires were filled out to assess the outcome of the procedure. ResultsThe mean age of the patients was 42 years old. After 4 months of the surgery, %80 of the patients had no complains of SUI. Recurrence of post operative urinary incontinence after two years was 50 %. ConclusionThis study showed high recurrence rate of the disorder after two years.
